The cystic teratoma is the most common germ cell tumors in reproductive age women. The malignant change of cystic teratoma is rare, about 1~2%, and mostly change to squ-amous cell carcinoma. We experienced 4 cases of squamous cell carcinoma arising in cystic teratoma and re- port our cases with a brief review of the literature.

3.Poor-risk Intracranial Aneurysm with Large Intracerebral Hematoma.

Analyses and studies have been made on 34 cases of the combined ICH, showing mass effect among 305 male and female ruptured intracranial aneurysmal patients who were admitted to Kyungpook National University Hospital from Mar. 1984, through Feb. 1990. The incidence of the large ICH was 11.1% with 2.1 to 1 ratio of male to female. The highest age incidence of the hematoma was in the 40 to 60 years bracket. The most frequent site of the aneurysm combined with the large ICH was at MCA aneurysm, 38.6%, followed by at A-com artery aneurysm, 4.8%. The most frequent location of the hematoma was in temporal lobe, 88.2%. The rate of combined large ICH was 20.3% in multiple aneurysmal bleeding patients who revealed higher incidence of the hematoma than the single bleeding patients. In amount of the hematoma, 30-40ml bracket ranked top occupying 32.4% of all cases. After surgical treatment, 29.4% patient was improved to the good and fair on discharge. The rate of mortality was 61.8%, which was mainly due to direct effect of hematoma and cerebral infarction caused by vasospasm.

4.Anterior Cervical Fixation in Spine Injury Patients Requiring Tracheostomy.

Because of the possibility of increased the risk of soft tissue or bone infection, surgeon have been somewhat reluctant to use the anterior cervical approach in spinal injury patients who had previously undergone tracheostomy. The authors investigated whether the use of a Caspar plate in such patients during anterior cervical fixation altered the risk of infection. We analyzed the mechanism and level of injury, operative procedures, post-operative course and infection in 12 cervical injury patients with previous tracheostomy. The procedure had been performed on the day of injury because of semicoma resulting from combined head injury in two cases, and because of multiple rib fracture with hemothorax, and old age with chronic obstructive lung disease in one case each; it had been performed between 1-6 days after injury because of high fever with pneumonia and atelectasis in eight cases. The interval between tracheostomy and anterior fixation was 5-20(average, 13) days. During clinical follow-up at 8-21 (average, 16) months, bone union without infection was observed in all patients. The authors concluded that in patients with cervical cord injury, tracheostomy did not increase the risk of infection in subsequent anterior cervical fixation using a Caspar plate.

5.Serum Neopterin in Head Injury.

Cell-mediated immunity frequently becomes impaired after major trauma. To investigate the immune response of the head injury, the authors measured the level of the serum neopterin, which represents the index of macrophage activation, in 53 head-injured patients. In the injured group the serum neopterin level was 5.4+/-3.1 nmol/l and slightly higher than that of the normal control group, 4.4+/-1.0 nmol/1. In the group of the patients with the Glasgow Coma Scale(GCS) 3-9, the level was significantly higher than the group GCS 10-15. The differences were not significantly related to the age or the presence of intracranial hematoma. It is conceivable that head injury will change the patient's immune response and it may be influenced by neural control.

6.Posterior Fixation and Fusion for Posteriorly Displaced Odontoid Process Fractures.

Several different methods of posterior wiring and fusion have been advocated for treatment of the odontoid process fractures, regardless of direction of displacement. Between 1983 and 190, 8 cases of type II odontoid process fracture underwent posterior wiring and fusion at Kyungpook National University Hospital. Among them, three cases were posteriorly displaced fractures. For these three cases, the authors modified wiring method and applied acrylic resin. The authors thought this procedure is not complex and a good method for posteriorly displaced type II fractures in which conventional posterior wiring may actually create a tration effect that further displaces the fractured odontoid into the spinal canal.

7.Gliomatosis Cerebri: Clinical Features and Prognosis.

OBJECTIVES: Gliomatosis cerebri is an uncommon primary brain tumor characterized by diffuse neoplastic proliferation of glial cells, with the preservation of the underlying cytoarchitecture. The aim of this study is to evaluate clinical features, outcome of surgical treatment and adjuvant therapy of gliomatosis cerebri. METHODS: Between Jan. 1990 and Dec. 2000, 12 patients were diagnosed with gliomatosis cerebri based on characteristic radiological and histological findings. The patients' age ranged from 18 to 77(mean 44) years and the male to female ratio was 7: 5. Nine patients underwent decompressive surgery and three, biopsy only. Postoperative radiation therapy was given in all cases except three. In addition to radiation therapy, four patients received chemotherapy. The mean duration of follow-up period was 18.8 months. RESULTS: The most common presenting symptom were seizure and motor weakness. The mean duration of symptom was 5.9 months. There was 5 bilateral lesions and tumor involved corpus callosum in 5, basal ganglia-thalamus in 4, and brain stem in 2. There was no operative mortality but four patients died during the follow-up. The mean survival period for 11 patients was 20.5 months from the time of diagnosis. In univariate analysis, the lesion involving corpus callosum, basal ganglia-thalamus and brain stem correlated significantly with the short length of survival(p<0.05). Also, postoperative radiation as a adjuvant therapy prolonged the patient's survival(p<0.05). CONCLUSIONS: In the management of gliomatosis cerebri patients, early detection by MR imaging, active management of increased intracranial pressure, decompressive surgical removal and postoperative adjuvant therapy such as radiation is thought to be a good treatment modality.

8.A Case of Intrathoracic Kidney in an Adult.

Intrathoracic kidney is a very rare congenital anomaly in development of the kidney. It was firstly described by Mikulics in 1922. More than 130 cases were reported in the world. Most of the patients have no clinical problem and discovered incidentally in chest X-ray. We have experienced a case of intrathoracic kidney in a 48-year-old female who was admitted due to recurrent calculous cholecystitis. Intrathoracic mass lesion was noted in the plain chest film and confirmed as intrathoracic kidney by chest computed tomography and intravenous pyleography. We report a case of intrathorcic kidney with brief review of literature.

9.Personality Change after Aneurysmal Surgery of Anterior Communicating Artery.

The neuropsychological consequences of ruptured intracranial aneurysms are being given increasing attention with the recent increase in survival by surgical repair of the aneurysms. The territory of anterior communicating artery(AcoA) seems to be particularly susceptible to neurobehavioral impairments. They include amnesia. Confabulation, and personality change. The neurobehavioral changes have been collectively referred to as the "AcoA syndrome". The study of amnesia and confabulation was extensive for several decades, but the personality change after anterior communicating artery aneurysm rupture seems to have been given less attention. The present study used the MMPI to assess the personalities of 15 AcoA aneurysmal patients and a control group of 19 patients with subarachnoid hemorrhage from aneurysms located at other sites. Overall, AcoA patients showed no remarkable differences in personality in comparison with the control group. However, 5 patients with abnormal MMPI profiles that have abnormally incre ased depression subscale score and an associated increased scores on psychopathic deviance, hysteria, psychasthenia and schizophrenia subscales were detected by cluster analysis, and 4 of them had causable focal lesions such as basal forebrain, ventromedial frontal lobe, and medial frontal lobes about interhemispheric fissure on brain CT. Also, a group with the worst initial clinical state, Hunt and Hess grade IV, developed more personality impairment than the group with Hunt and Hess grade I-II-III regardless of the aneurysmal location. They showed increased scores on depression, psychopathic deviance and schizophrenia subscales.

10.Aeromonas hydrophila Myonecrosis in an Immunocompetent Patient.

Aeromonas hydrophila is a facultative anaerobic Gram negative bacillus. It usually thrives in fresh and brackish water and primarily causes gastroenteritis in humans. Rarely aeromonads have been associated with extra-intestinal infections, such as skin and soft infection, meningitis, epidural abscess, endocarditis and pneumonia in immunocompromised hosts. Here we report Aeromonas hydrophila soft tissue infection occurring in a 53-year-old Korean woman without underlying disease who was successfully treated with proper antibiotics.
