1.A Case of Bowenoid Papulosis Developing on the Genitalia and Temporal Area.

We report herein a case of bowenoid papulosis, which developed on the groin, the shaft of the penis and left parietal area, of scalp in a 31-year-old male. Clinical features present as brown to brown-black papules and verrucous plaques ranging from 2 cm to 12 cm in diameter. Electron microscopic examination of the specimen obtained from our patient fail to reveal viral particles.

2.A Case of Aneurysmal Benign Fibrous Histiocytoma.

Aneurysmal benign fibrous histiocytoma(ABFH) is a relatively rare variant and has a distinctive histopathological features, that is collections of capillaries, foci of hemorrhage, siderophages, and foamy macrophages surrounding cleft-like and cavernous blood filled spaces in the tumor. But, in spite af the distinctive features, it is not infrequently confused with malignant melanoma, angiomatoid malignant fibrous histiocytoma, and vascular tumors such as spindle cell hemangioendothelioma, nodular Kaposi's sarcoma and angiosarcoma. We report a case of ABFH developed on the left upper extremity of a 41-year-old man.

3.A Case of Pretibial Myxedema.

Pretibial myxedema is a condition in which there is loeal thickening of the skin by a mucin-like deposit; it is nearly always asosciated with ophthalmopathy and thyrotoxicosis, not infrequently becomes more pronounced after treatrnent of thyrotoxicosis. The precise cause of pretibial myxedema is not known, but it appears that IgG LATS represents an autoantibody against a thyroid antigen, retroorbital tiesue and tbe skin, so, pretibial myxedema is presumed to be the result of a local antigen-antibody tissue reaction. A 57-year-old man had the history of diabetes since 1964 and Graves disease since May 1980, he was treated with metimazole for 1 month, with improving thyrotoxicosis but developed the pretibial myxedema. The histologic findings showed considerable amount of mucin, especially hyaluronic acid with toluidin blue stain at PH 3.0. The lesions were improved by local application of 0.01 x fluocinolone acetonide ointment with occlusive dressing technique.

4.Effect of Delipidization on Binding of Hydrocortisone to Human Stratum Corneum.

Skin lipids are a major constituent of the epidermal barrier, impermeable to a great extent to many chemicals. The barrier cspacity of the skin must be diminished by its delipidization through extract of the fatty material. The objectives of this study were to determine the degree that lipids and proteins contribute to the binding of straturn corneum in percutaneous penetration of hydrocortisone following skin delipidization. The binding capacity of C-hydrocortisone partitioned to powdered stratum corneum without delipidization in 10 human subjects was 1,554.34+/-357.48 dpm, and when delipidization, was 2,234.68+/-761.12 dpm. The partitioning rate was 0.38% in powdered stratum corneum without delipidization in 10 subjects, and when with ddipidization, was 0.55%. The result that the binding capacity of hydrocortisone to stratum corneum with delipidization increased more than without delipidization are statistically significant (p<0.05).

6.A Case of Perforating Pseudoxanthoma Elasticum.

We report herein a case of perforating pseudoxanthoma elasticum in a 44-year-old female who was suffered from yellowish, crepelike, lax, redundant patches on her neck, axilla, antecubital area, groin, popliteal area bilaterally and serpiginous erythernatous plaqus in yellowish redundant patch of right axillary area. Asymptomatic erythematous papule developed on the previous biopsy site 20 days after taking biopsy. Histopathologically, short and gnarled abnormal elastic fibers, which are basophilic stained, are clumped in middle and lower dermis, and some fibers extend through epidermal channel to the surface of skin.

7.Clinical and Histopathological Observation on Benign Melanocytic Tumor and Malignant Melanoma ( 1974 ~ 1984 ).

This clinical and histopathological study was performed with 147 cases of benign mielanocytic tumors and 19 cases of malignant melanomas, which were obtained as surgical specimens from 1974 to 1984 at Department of Clinical Pathology, Catholic Medical College. The results were as follows: 1, In 89 cases of acquired benign melanocytic nevi, the average age of intradermal type(64 cases) was 34. 7 years and that of compound type(24 cases) was 24. 6 years. 2. In 30 cases of congenital nevus, nevus cells were present in the lower two thirds of reticular layer of the dermis in 93. 3% and in the subcutis as well in 3.3%. A case of giant congenital nevus with balloon cell appearance was found. 3. Of the 147 benign melanocytic tumors, a pigmented spindle cell nevus and a desmoplastic nevus were observed. 4. Blue nevi were 11 in number and excised from the face in 7, buttock in 2, shoulder in 1, upper arm in 1, and all were common type histopathologically. 5. Twelve malignant melanornas which were possible to be re-examined histopathologically were composed of 5 nodular type, 3 acral lentiginous type, 1 superficial spreading type and 3 metastatic malignant melanoma.

10.Osteolysis around Screw in Cementless Total Knee Replacement.

Screw fixation of the tibial component offers advantages in initial fixation in cementless total knee replacement. But the high incidence of screw related osteolysis was reported. The purpose of this study is to evaluate clinical results of cementless total knee replacement and to look for radiographic changes at the screw-bone interface. From January 1988 to December 1991, primary cementless total knee replacements with Miller Galante I (Zimmer, Warsaw. IN) were performed to 53 knees at Kyung Hee university hospital. Among them, 21 cases which could be followed-up for more than 4 years were studied retrospectively about the clinical and radiographic results. The mean follow-up period was 5.5 years(ranged from 4.2 years to 8 years). At the last follow-up period, knee scores of Hospital for Special Surgery were improved from mean 56 points to 90 points and the range of motion from 72 degrees to 110 degrees. In the last follow-up radiographs, osteolysis around screw was classified as linear(type I ), cystic(type II ) and cavitary(type III ) according to the width of the lucency around screws. Among 21 cases, radiographic findings of osteolysis around screw were detected in 10 cases but not in 11 cases. But clinical results were similar between these two groups. Among the total 84 screws(4 screws in each case), 21 screws(25%) showed screw related osteolysis typed as I in 13 screws(15.4%), II in 4 screws(4.8%) and III in 4 screws(4.8%). Among the 13 cases that followed-up more than 6 years, 20 screws(38%) showed screw related osteolysis typed as in 11 screws(21.2%), I in 5 screws(9.6%) and II in 4 screws(7.8%). The most frequently involved site of screw was anteromedial(33.3%). In conclusion, after mean 5.5 year follow-up, the clinical results were satisfactory but the development of osteolysis around screw might be an indicator of the implant failure.
