3.A Case of Dirty Neck' of Atopics.

We report a case of Dirty Neck in a 27-year-old male suffering from severe atopic dermatitis. 'Dirty Neck', a reticulate pigmentation of the anterior and anterolateral aspects of the neck, is a specific finding in some subjects with atopic dermatitis. Two groups have been indentified. One is found in older patients with severe atopic dermatitis whose pigmentary lesions develop after puberty and show little seasonal variation. The other is found in younger patients with mild eczema who have a increase in pigment during the summer. Our patient showed a dark brown colored, linear, reticulate pigmentation of the neck, especially anterolateral aspeet regarded as the former group. More comprehensive and detailed clinical study is needed to evaluate the importanee of this entity in regard to as a minor clinical feature of Korean atopic dermatitis.

6.Effect of Calcium Channel Blocker on Gene Expression of Renin after lschemic Renal Injury.

OBJECTIVES: lschemic acute renal failure(ARF) is characterized by an abrupt and sustained decline in GFR within minutes to days after renal ischemia and not immediately reversed on restoration of renal blood flow. The typical delay of a few days to a few weeks suggests reversible parenchymal damage awaiting cell regeneration for functional recovery. Many potentially cell damaging factors, such as ATP depletion, plasma membrane phospholipid degradatian and superoxide-induced membrane damage, play a central part in ischemic injury. More recently, much attention has been focused on the role of calcium, especially ischemic cell injury and the possible therapeutic role of calcium channel blockers emerged from studies conducted several years ago. In the past, it was thought that activation of renin-angiotensin system plays a role in the pathogenesis of ARF. Now the role of angiotensin in human renal ischemia also appears to be controversial. The following study was done in order to investigate the effect of a calcium channel blocker, nifedipine, on gene expression of renin during acute ischemic renal injury. METHODS: The Sprague-Dawley rats were divided into 4 groups, group I(n=3) as the control, group II (n=3) as the sham operation group, group III(n=15) as the ischemic renal injury group without nifedipine pretreatment, and group IV(n=15) as the ischemic renal injury model by right nephrectomy and left renal artery clamping for 40 minutes with systemic nifedipine pretreatment(10mg/kg), 1n ischemic renal injury model(group III and IV), rats were further divided into three subgroups according to reperfusion time of 1,24,72 hours. The non-ischemic right kidney removed at the time of initial procedure served as paired control. Total renal RNA was extracted by Chomczynskis method and electrophoresis was done in a 1% agarose gel containing 2,2M formaldehyde. Northern was performed at 42degrees C with isotope labeled renin probe for 18 hours, Autoradiographs were obtained and quantitated by a densitometer measured at 530nm. RESULTS: 1) The expression of renin gene was markedly decreased after renal ischemia and slowly recovered to one half of the control level after 72 hours of reperfusion. 2) Renin gene expression pattern of ischemic renal injury with prior nifedipine treatment was similar to the ischemic group without nifedipine pretreatment. CONCLUSION: These findings suggest that the renin gene expression was markedly decreased after renal ischemia and slowly recovered. Systemic nifedipine pretreatment does not have a significant effect on gene expression pattern of renin in ischemic renal injury.

8.Regional Difference of Causative Pollen in Children with Allergic Rhinitis.

The aim of this study was to investigate role of common pollen in Korean school-aged children with allergic rhinitis (AR) in 5 provinces (Incheon Metropolitan City-Gyeonggi Province, Chungcheongbuk-do, Gwangju Metropolitan City, Busan Metropolitan City, and Jeju Special Self-Governing Province), using a questionnaire and skin prick test, and to assess the differences among the residential regions. Among the enrolled 14,678 total children, 1,641 (22.0%) had AR. The sensitization rate to pollen (38.7%) was the second highest among examined allergens and significant differences were in the sensitization rates to trees, weeds, and grasses among the 5 provinces (P < 0.05). The sensitization to trees (25.2%) was the highest common among the pollen types and significant differences also were observed in the sensitization rates to alder, birch, Japanese cedar, oak, and elm among the 5 provinces. The sensitization rate to weeds (19.9%) was the second highest and significant differences were observed in the sensitization rate to Japanese hop, mugwort, and ragweed among the 5 provinces. The sensitization rate to house dust mite was 86.8%, the highest among examined allergens and that to Dermatophagoides farinae exhibited regional differences (P = 0.003) but not to D. farinae (P = 0.584). The sensitization rate to mold (13.5%) was the highest in Jeju and lowest in Busan, and a statistically significant difference was detected among the 5 provinces. These results support that examined pollen allergens are strongly associated with residential region due to regional causative pollen differences among children with AR within Korea to investigate the main pollen allergens.

9.Epicanthoplasty Using Y-M Plasty.

Lots of Koreans have relatively thick skin, small palpebral fissures and unfolded eyelids with a prominent epicanthal fold. Various methods have been developed to correct epicanthal fold. However, excessive or hypertrophic scar can be occurred, especially in the medial canthal and nasal area. And the recurrence may restrict the application of these methods. We developed a new epicanthoplasty using Y-M plasty to correct the epicanthal folds without obvious scar. From February 1999 to August 2010, all patients underwent Y-M plasty for the correction of epicanthal folds. Y-shaped incision line was designed not extending to the nasal area and the orbicularis oculi muscles were resected with skin. The dog ear deformities were corrected and sutured using nylon 7-0. The epicanthal folds were completely corrected. Prominent scar and recurrence were not observed. Most of the patients are satisfied with the results. Mean preoperative ICD(intercanthal distance) was 38.84 mm and average 2.24 mm ICD was reduced after the operation. Epicanthoplasty using Y-M plasty is relatively safe, easy and rapid method to design and apply even in the beginners with low risk of scars and recurrences. So, the authors propose this new versatile technique in the case of minimal to moderate epicanthal folds.

10.A Case of an Adult-Onset Langerhans Cell Histiocytosis Involving the Temporal Bone.

Langerhans cell histiocytosis (LCH) is a rare disease characterized by clonal proliferation of Langerhans cells. The temporal bone is frequently involved in pediatric LCH cases, but there have been few reports of adult LCH with temporal bone involvement. We present a case of adult onset LCH affecting the temporal bone. The diagnosis was made by biopsy, based on histopathologic findings of Langerhans cells. Diagnostic and therapeutic considerations of LCH involving the temporal bone are discussed.
