1.Cervical Prevertebral Hematoma - a Rare Complication of Acupuncture Therapy: A Case Report.

A 36-year old man with neck pain developed an acute cervical prevertebral hematoma after acupuncture therapy at an oriental medicine hospital. MR imaging demonstrated a fluid collection, and this suggested a diagnosis of retropharyngeal hematoma, and the patient was managed conservatively. We report here on a case of a retropharyngeal hematoma following acupuncture therapy and we review the relevant literature.

2.The Toxic and Morphologic Effects of Mitomycin-C, 5-FU and Genistein on Rabbit Corneal Endothelium.

PURPOSE: We determined the toxic and morphologic effects of the anti-proliferative drugs, mitomycin-C (MMC), 5-fluorouracil (5-FU) and genistein on rabbit corneal endothelium. METHODS: After intramuscular anesthesia, each drug of different concentrations (MMC at 0.05, 0.1, and 0.2 mg/ml; 5-FU at 5, 10, and 50 mg/ml; and genistein at 0.013, 0.027, and 0.054 mg/ml) was perfused into the anterior chamber of 54 white rabbits (108 eyes). The same amount of balanced salt solution was perfused into control eyes. The corneal thickness was measured before perfusion and 15 min, 30 min, 45 min, 1 h, and 24 h after perfusion. Corneal samples were prepared at 24 h after perfusion to determine the changes in corneal thickness and to observe morphologic changes of corneal endothelium under scanning electron microscope (SEM). RESULTS: A significant increase in corneal thickness was observed. Destruction of corneal endothelial cell structure was seen under scanning electron microscope at 24 h after perfusion with MMC at 0.2 mg/ml for 1, 3, and 5 min, and at 0.1 mg/ml for 5 min; and 5-FU at 50 mg/ml for 5 min into the anterior chamber. However, no significant difference was seen in corneal thickness or in corneal endothelial morphology at 24 h after perfusion with genistein. CONCLUSIONS: To avoid morphologic changes of the cornea, we recommend the anterior chamber perfusion of MMC at 0.1 mg/ml between 1 and 2 min, 5-FU at 10 mg/ml between 3 and 5 min, and genistein at 0.027 mg/ml for 5 min. Genistein at low concentrations showed no morphologic change in the cornea, suggesting the possible clinical use with safety.

5.The Measurement of Size of Human Extraocular Muscles and their Changes in Thyroid Associated Ophthalmopathy in Korea.

Enlargement of the extraocular muscles may cause diplopia and exophthalmos. The most common cause of the exophthalmos is thyroid associated ophthalmopathy(TAO), followed by inflammatory diseases and orbital tumors. TAO has the classical features of proptosis and lid retraction. However, the diagnosis is frequently confused with other eye diseases due to various symptoms and signs. Therefore, measurement of the size of extraocular muscles in both normal and TAO patients will make diagnosis and treatment plan easier in TAO and other muscular diseases of the orbit. For the clinical application, we measured the size of muscle with and without magnification and the results were compared. Normal mean values of cross sectional length and thickness of the extraocular muscles measured by CT scan showed 8.83x 3.60 in the inferior rectus, 9.20x3.20 in the superior muscle group, 9.45x 3.48 in the medial rectus and 9.65x3.23 mm in the lateral rectus muscle with no sex differences. All four muscles showed statistically significant enlargement in TAO and the thickness showed greater increase than the length. Superior muscle group(superior rectus and levator muscle)was the most frequently involved in TAO in both sexes. There was no significant statistical difference in the methods of measurement.

6.The Significance of Amniotic Fluid Index at Admission as a Predictor of Latency Period in the Patients with Preterm Premature Rupture of the Membranes.

OBJECTIVES: The purpose of this study was to determine the clinical significance of residual amniotic fluid volume, as measured by the amniotic fluid index(AFI), on the prediction of latency period and perinatal outcomes in patients with preterm premature rupture of the membranes(PPROM). METHODS: Study population consisted of 103 singleton pregnancies with PPROM between 24 and 34 weeks' gestation. Amniotic fluid index was determined using transabdominal ultrasound at admission. Latency period was defined as time interval in hours between admission and delivery. All medical records of mothers and neonates were reviewed. Spearman's rho rank correlation, receiver-operator characteristic(ROC) curve analysis, survival analysis and Cox's proportional hazard model were used for statistical analysis. RESULTS: There was a statistically significant correlation between latency period and AFI at admission. ROC curve analysis showed that AFI at admission had a significant predictive value in the prediction of latency period < 48 hours. Survival analysis demonstrated that AFI < 5.0 was strongly associated with the likelihood of shorter latency period, Cox's proportional hazard model indicated that AFI < 5.0 was a significant independent predictor for the occurrence of latency period < 48 hours even after adjustment of other independent variables. The patients with AFI <5.0 had a higher rate of cesarean section due to fetal distress than those with AFI > 5.0 but the incidence of chorioamnionitis was not significantly different between two groups. Comparing the perinatal mortality and neonatal morbidity including infectious morbidity between the neonates born to mothers with AFI <5.0 and those with AFI > 5.0, there was no significance after adjustment of gestational age at birth. CONCLUSION: AFI at admission has a significant correlation with latency period and predictive value in prediction of latency period in the patients with PPROM. AFI <5.0 is a independent predictor for the shorter latency period.

8.Experience of Extracorporeal Shock Wave Lithotripsy (ESWL) with EDAP-LT01: A Report of 660 Cases.

PURPOSE: Since ESWL had been developed, many experiences of the treatments to urinary stones with various type of lithotriptors were reported. We evaluated the efficacy of EDAP-LT01 piezoelectric lithotriptor according to the size, location of the stone, number of the treatment session, adjuvant therapy and cause of failure. MATERIALS AND METHODS: We retrospectively analyzed the data of 660 cases who had been treated with ESWL. Among the patients, the renal stones were 122, the ureteral stones were 452, and combined with renal and ureteral stones were 43 cases. The mean size of stones was 13mm (range: 4-38mm). ESWL was performed to all the patients with urinary stones except complete staghorn calculi. No anesthesia was performed except in one child (5 years old). RESULTS: Overall success rate was 94.7%, and there were close correlations between the success rate and the size. The mean number of sessions was 2.4. There was no significant complication, but 35 cases were failed by ESWL monotherapy, and auxiliary procedures were done in 21 cases. Efficiency quotient was 0.59. CONCLUSIONS: The ESWL with EDAP-LT01 seems to be effective and safe to treat the urinary stones of various location and size, even compared to the lithotriptors of other type. We also propose that ESWL monotherapy with this machine is efficient for the treatment of the partial staghorn calculi and stones larger than 30mm.

9.Perifolliculitis Capitis Abscedens et Suffodiens Successfully Treated with Isotretinoin.

Perifolliculitis capitis abscedens et suffodiens is a rare chronic recalcitrant follicular disorder which clinically presents itself as pulstules, nodules, intercommunicating abscess and sinuses that leave atrophic, hypertrophic or keloidal scars. Although the etiology of this of condition is unknown, its association with acne conglobata and hidradenitis suppurativa, collectively termed the follicular occlusion triad, suggest a cammon basic pathogenic mechanism of follicular retention. The therapeutic problem is reflected in the variety of therapies that have been tried with varying degrees of success. We report a case of a 34-year-old man who showed a patch of alopecia on the vertex of his scalp. Clinically, the scalp showed multiple soft fluctuant elevated nodules with nonscarring alopecic patches. We performed a treatment with isotretinoin(13-cis-retinoic acid) during 6 months. The patients clinical appearance was much improved and local recurrence and hair loss have not been observed to date.
