1.A Case of Eruptive Vellus Hair Cysts.

A 22-year-old-male patient has numerous brownish-black, soft follicular papules which compatible with acne are scattered on the chest, scapular regions, back and buttocks for 4 5 years duration. Biopsy specimen revealed cystic structures in the mid-dermis lined by squamous cell epithelium w4ich contained laminated keratinous material and varying number of vellus hairs, histopathologically, compatible with diagnosis of eruptive vellus hair cysts. 2.Two Cases of Pityriasis Rotunda Associated with Liver Cirrhosis.

Pityriasis rotunda is characterized by perfectly circular to oval brownish scaly patch on the trunk and extremities. This disease is considered to be a form of acquired ichtkyosis of unknown etiology and develops usually in association with certain systemic diseases, such as tuberculosis, malignant neoplasrn, leprosy, ovarian cyst and uterine myoma etc, We experienced 2 cases of pityriasis rotunda on the abdomen, chest and back associated with liver cirrhosis in 66 year old man 43 year old woman and clinical presentation was made with review of related literatures.

4.A Clinical Study on the Acetabular Fracture

Acetabular fracture is severe injury and is associated with other body injuries severely. It's complications are traumatic arthritis and avascular necrosis of femoral head which induce severe permanent disability and its treatment is equivocal. A clinical study was performed on 70 patients with 65 fresh fractures and 5 old fractures of the acetabulum, who were hospitalized and treated at Severance Hospital from January 1971 to December 1980 and following results were obtained. 1. The prevalent age distribution was between 20 and 50 years of age (75.7%), and the ratio between males and females was 2.2:1. The most common cause of injury was traffic accident (70.%). 2. 56 patients (80.0%) were associated injuries of other parts and the most common associated fracture was pelvic bone fracture and the most common associated soft tissue injury was urological injury. 3. A classification of acetabular fracture was induced from Judet and Letournel's anatomical classification; simple fracture was 45 cases (64.2%), associated fracture was 25 cases (35.8%) and most common hip dislocation was posterior dislocation (31.4%). 4. The methods of treatment were; closed management by using traction for 53 patients, surgical management by open reduction and internal fixation for 12 patients, and hip reconstructive surgery by total hip replacement(2 patients), cup arthroplasty (2 patients), acetabular roof formation by using iliac bone graft (1 patient). 5. The satisfactory result of conservative treatment was (57.8%), surgical treatment was (72.7%), hip reconstructive treatment was (80.0%). 6. The complications of the acetabular fracture were traumatic arthritis (36.1%), avascular necrosis (23.0%), myositis ossificans (6.6%), infection (3.3%). 7. Finally, the results of treatment depend largely on anatomical reduction, early joint motion.

6.A Case of Monilethrix.

Monilethrix is a rare familiar disorder of unknown cause, occurring in either sex, and ueually confined to the scalp. In most cases it is manifested in early childhood. lt is characterized by localized or widespread dystrophy of hair and follicles, the shafts showing regular variations in diameter producing alternate "nodes" and "internodes" which confer a striking beaded or moniliform appearance on the hair. We experienced a case of monilethrix, which was associated with keratosis pilaris and occurred in three generations. The patient's one sibling, mother, 4 uncles and 2 aunts of mother side and grandmother with her sister were all affected with a similar diaorder of hair.

8.13 Cases of Adrenal Tumor Treated by Adrenalectomy.

Adrenalectomy was performed in 13 cases of adrenal tumor admitted to the department of urology during the period from 1977 to 1988. The patients comprised 7 patients with Cushing's syndrome. 1 patient with primary aldosteronism and 5 patients with pheochromocytoma. All surgical approaches were provided transperitoneally including subcostal incision in 9 cases, midline abdominal incision in 3 cases and thoracoabdominal incision in 1 case. 2 cases of pheochromocytoma and 1 case of Cushirig's syndrome, of which masses weighed over 700g and had severe adhesion to the kidney, underwent either additional nephrectomy or partial nephrectomy along with adrenalectomy. Postoperative hospital stay ranged between 7 and 17 days. Hormonal assay done on 1 day to 1 week postoperatively revealed normal or subnormal value.

9.Evaluation of Hematopoietic Abnormalities and Apoptosis in Myelodysplastic Syndromes Using Long Term Bone Marrow Culture System and In Situ Nick End Labeling Method.

OBJECTIVES: The myelodysplastic syndromes (MDS) are a group of acquired clonal hematopoietic disorders characterized by peripheral cytopenias and a hypercellular or normocellular dysplastic bone marrow. The mechanisms responsible for development of MDS are not known. We performed this study to evaluate the hematopoietic abnormalities and apoptosis in MDS. METHODS: Long-term bone marrow culture (LTBMC) was performed for colony assays, cobblestone area assay, stromal morphologic changes from 7 patients with MDS and 7 normal controls. In situ nick end labeling (ISNEL) method was performed for detection of apoptosis from LTBMC in 7 patients with MDS and 7 normal controls. ISNEL method also performed in bone marrow cell bloc samples in 36 patients with MI3S. RESULTS: Viability of nonadherent cells from LTBMC of patients with MDS was not decreased compared with normal controls at 1 week, but significantly decreased at 2 and 3 weeks compared with normal controls (P<0.0001). Formation of the cobblestone areas from patients with MDS was slightly decreased compared with normal controls at 1st week, but significantly decreased at 2nd and 3rd weeks compared with normal controls (P<0.0001). Slightly decreased compared with normal controls at 1 week, but significantly decreased at 2 and 3 weeks compared with normal controls (P<0.0001). Stromal layers produced in LTBMC of normal controls and 1 patient with MDS were detected at 1 week and were formed confluent stroma from 3 weeks, but another patients with MDS who did not form a confluent stroma. Patients with MDS had significantly lower colony forming unit granulocyte-macrophage (CFU-GM) compared with normal controls at 1 (P<0.01) and 2 weeks (P<0.001) of LTBMC. Two weeks of LTBMC resulted more profound inhibition of CFU-GM formation than 1 week (P<0.0001). Apoptotic cell death was absent in adherent and non adherent cells from normal controls at 1 and 2 weeks, but massive apoptotic cell death was found in adherent and non adherent cells from patients with MDS at 1 and 2 weeks and the degree of apoptosis was profound at 2 weeks compared with 1 week. Among the 36 patients, fifteen patients demonstrated varying degrees of apoptosis positive cells, 4 having low, 8 intermediates, and 3 high scores. Remaining 21 patients showed absent apoptosis or only occasional positive cells. CONCLUSION: Hematopoietic abnormalities such as a failure of differentiation are caused by the stromal defects and the biologic basis of the apparent paradox of peripheral cytopenias in the face of hypercellular (or normocellular) marrow is related by intramedullary apoptotic cell death of the stromal and hematopoietic cells.

10.p27Kip1 Expression and Apoptotic Index in Prostatic Adenocarcinoma.

p27kip1, a cyclin dependent kinase inhibitor, has been recognized as a negative regulator of cell cycle. To investigate the role of p27kip1 on progression of cancer and apoptotic pathway, we analyzed p27kip1 expression using immunohistochemical stain in 40 cases of prostatic adenocarcinoma and apoptotic index by TUNEL method in 30 cases of prostatic adenocarinoma. Both were correlated with Gleason grade and Gleason score. Loss of p27kip1 expression was more frequent in prostatic adenocarcinomas of higher score (Gleason score 7 to 10) (60.7%) than in those of lower score (Gleason score 4 to 6) (33.3%) (p<0.05). The value of mean apoptotic index of carcinoma was 1.13+/-0.26, 1.80+/-0.91, 2.06+/-0.79, and 2.12+/-0.82 in grade 2, 3, 4, and 5, respectively, and was positively correlated with grade of carcinoma (p<0.05). Mean apoptotic index of higher Gleason score (score 7 to 10; 2.05+/-0.63) was also significantly increased than in lower Gleason score (score 4 to 6; 1.34+/-0.39) (p<0.05). Mean apoptotic index in cases with and without p27kip1 expression was 1.92+/-0.86 and 1.89+/-0.81, respectively (p>0.05). These results suggest that loss of p27kip1 expression and increased apoptotic index may be the morphologic markers to predict the behavior of prostatic adenocaricnoma. The role of p27kip1 on apoptotic pathway seems to be meager in this study and needs further study.
