1.13 Cases of Adrenal Tumor Treated by Adrenalectomy.

Adrenalectomy was performed in 13 cases of adrenal tumor admitted to the department of urology during the period from 1977 to 1988. The patients comprised 7 patients with Cushing's syndrome. 1 patient with primary aldosteronism and 5 patients with pheochromocytoma. All surgical approaches were provided transperitoneally including subcostal incision in 9 cases, midline abdominal incision in 3 cases and thoracoabdominal incision in 1 case. 2 cases of pheochromocytoma and 1 case of Cushirig's syndrome, of which masses weighed over 700g and had severe adhesion to the kidney, underwent either additional nephrectomy or partial nephrectomy along with adrenalectomy. Postoperative hospital stay ranged between 7 and 17 days. Hormonal assay done on 1 day to 1 week postoperatively revealed normal or subnormal value.

2.The Value of Transrectal Ultrasonography in Benign Prostatic Hyperplasia.

Transrectal ultrasonography was done in 14 healthy men and 52 patients with benign prostatic hyperplasia to demonstrate the pathomorphology of the prostate and to determine the preoperative prostatic size. We found that the transrectal ultrasonographic examination allowed for an accurate estimation of the prostatic size. The following results were obtained. 1. The average values of anterioposterior, transverse and longitudinal diameter were 2.45+/-0.49 cm. 4.12+/-0.69 cm and 3.39+/-0.42 cm in control group and 3.37+/-1.06 cm. 5.07+/-0.71 cm and 4.46+/-0.84 cm in BPH. 2. The average weight of the prostate was 20.07+/-5.15 gm in control group and 47.02+/-25.81 gm in BPH. 3. In patients treated with transurethral resection of the prostate the mean estimated weight was 36.0 gm while the mean actual resection weight was 11.4 gm The difference between these 2 values was 24.6 gm. 4. In patients treated with open prostatectomy the mean estimated weight was 78.8 gm, while the mean actual resection weight was 48.7gm. The difference between these 2 values was 30.1 gm.

4.Squamous Cell Carcinoma Arising From Chronic Osteomyelitis: A Report of One Case

Squamous cell carcinoma is a rare complication of chronic osteomyelitis. Squamous cell carcinoma of extremity campromises approximately 1 to 2% of all squamous cell carcinoma and of these 2.5 to 4.5% are secondary to chronic osteomyelitis. Biopsy of squamous cell carcinoma, which arises in the proliferating edge of the cutaneous ulcer and invades the bone, should include tissues from all portion of sites of ulcer including bone marrow spaces. But histological diagnosis may be difficult becasue of preexisiting and coexisisting metaplasia and pseudo-epitheliomatous hyperplasia. With prompt and aggressive surgical treatment, the prognosis for patient with squamous cell carcinoma secodary to chronic osteomyelitis is good. Routine regional lymphadenectomy at site of amputation seems unnecessary. This case report represets an unusual bone marrow involvement of squamous cell carcinoma which was complicated by femur fracture.

9.MR Imaging of Soft Tissue Hemangioma of the Extremity.

PURPOSE: To analyse the MR imaging characteristics of the soft tissue hemagiomas of the extremity. MATERIALS AND METHODS: We retrospectively reviewed histopathologically proven 18 cases of hemangiomas of the extremity including 15 cavernous hemangiomas, 2 venous hemangiomas and 1 capillary hemangioma. The lesions were frequent in female and low extremity. We analyzed the site, extent, margin, signal intensity, enhancement pattern, the homogeniety of internal architecture, and abnormal vessels near the lesion. RESULTS: The extensive lesions involving multiple muscle and tissue layers were more frequent than localized lesions (12/18). Twelve cases had ill defined margin. In most cases, the lesion was heterogeneous mass composed of nodular and septal components. The nodular component showed intermediate signal intensity in T1WI and hign signal intensity in T2WI. The setal component showed mixed inconstant signal intensity. One case of capillary hemangioma showed relatively homogeneous signal intensity than cavernous hemangioma and the main portion of 2 venous hemangiomas had mutilple tubular signal voids. At least some portion of the lesion was enhanced in all enhanced cases. CONCLUSION: The soft tissue hemangiomas of the extremity showed characteristic MR findings and MRI would be an useful method in the diagnosis of the soft tissue hemangiomas of the extremity.
