1.A study on cyclic AMP in alveolar bone treated by orthodontic forces.

Tooth movement by orthodontic force is based upon alveolar bone resorption at compression site and bone formation at tension site of tooth. The function of cyclic AMP is to participate not only in initial action of bone cells by mechanical forces but also in the continuous cellular response leading, to bone remodeling. This experiment was performed to clarify the role of cyclic AMP in bone remodeling by mechanical forces. The orthodontic forces of about 80 gm and 100 gm were applied to the right canines of maxillary and mandibular bone, respectively, in cats, treated for periods of time ranging from one hour to 28 days. Alveolar bones were obtained from compression and tension sites . surrounding tipping maxillary and mandibular canines as well as from contralateral control sites. The samples were extracted, boiled and homogenized, and the supernatants were assayed for cyclic AMP by a radioimmunoassay mothod. The results were as follows: 1. The orthodontic movement of canines was increased to the end of experimental period and the action of orthodontic forces on tooth movement was more effective in maxillary canine. 2. The cyclic AMP levels of alveolar bones in compression and tension sites initially decreased, then increased and remained elevated to the end of experiment. The differences of the cyclic AMP levels between treated sites and non-treated sites were gradually increased. 3. The cyclic AMP levels in treated sites of mandibular alveolar bone was higher than that of maxillary alveolar bone.

3.Cancer Development in the Remained Rectum after Subtotal Colectomy in a Familial Adenomatous Polyposis Patient.

Familial adenomatous polyposis (FAP) is an hereditary autosomal dominant disease characterized by development of hundreds to thousands of adenomatous polyps in the colon and rectum. The common symptoms are bloody stool, diarrhea, and abdominal pain. The average age at onset of symptoms is 33 years. Because of inevitable progression to malignancy, it is necessary to remove the entire colonic and rectal mucosa. Current surgical options are total proctocolectomy with permanent ileostomy, trans-abdominal colectomy with ileorectal anastomosis (IRA), and restorative proctocolectomy with ileal pouch-anal anastomosis (IPAA). Recently IPAA can give optimum control of colorectal polyposis in FAP patients with an acceptable incidence of postoperative complications and satisfactory functional results. We experienced one case of FAP who had malignacy in the remained rectum after subtotal colectomy. IPAA was done and the result was satisfactory.

4.A study of the effects of multibanded orthodontic appliances on the oral Lactobacilli.

A study of the effects of multibanded orthodontic appliances on the oral lactobacilli was conducted on 41 multibanded group and 41 caries free group. The following conclusions were obtained. 1. In multibanded group, the average count of lactobacilli per milli-liter of saliva showed about 219,300 in male and 254,100 in female. In caries free group, the mean count of lactobacilli per milli-liter of saliva showed about 66,700 in male and 68,500 in female, Therefore, the average of lactobacilli in multibanded group was much higher than that of the caries free group in both sexes. 2. The average of all the types of lactobacilli colonies (Disk-type, Bell-type, Y-type) in multibanded group was much more than that of caries free group. 3. Multibanded orthodontic appliances were found to sufficiently alter oral environment to significantly affect disk-types and total counts in both sexes and bell-type in female, at 1% level of confidence. But changes in bell-type and y-type, in male, were considered to be significant at 5% level of confidence.

5.A Case of Male Urethral Diverticulum with Giant Calculi.

We present one case report of anterior urethral diverticulum in a male with giant calculi. The patient was a 56-year-old male with the complaints of base-ball sized palpable scrotal mass and post-void dribbling for 15-16 years. The diverticulum was filled with multiple whitish-yellow stones. The largest one was 8X7X6cm in size.

6.A Case with Severe Bone Disease Due to Primary Hyperparathyroidism Caused by Parathyroid Chief Cell Hyperplasia.

The routine screening of patients for hypercalcemia has increased the incidence of hyperpa-rathyroidism, But, Primary hyperparathyroidism is relatively rare disease in korea. Especially, primary hyperparathyroidism caused by parathyroid chief cell hyperplasia has not been reported. The numbers of patients with bone disease, renal stones, other severe complication has declined, but the numbers of patients with psychiatric and neuromuscular disturbance or with asymptomatic disease has increased. We experienced a case with severe bone diseases due to primary hyperparathyroidism caused by parathyroid chief cell hyprepiasia which was confirmed through the mearsurement of serum calcium and parathyroid hormone, neck CT scanning and surgical exploration and which was managed by total parathyroidectomy with autotransplantation.

7.A Study on the Seropositivity of HBsAg among Biennial Health Examinees: A Nation-wide Multicenter Survey.

OBJECTIVE: The primary objective of this study was to estimate the prevalence of HBsAg-positives in the late 1990's among Korean adults. In addition, we evaluated the association of age, a residential area, a vaccination rate, a family history of chronic liver diseases and a past history of acute liver disease with the seropositivity of HBsAg, and estimated the prevalence of chronic HBV infection by follow-up for 6 month or more. METHODS: A total of 10 areas, six metropolitan and four small cities, were selected. In each cities, one health screening center was selected for recruitment of study subjects. The study subjects were enrolled from a general health examination program that is provided by medical insurance companies. Questionnaires on various risk factors were administered to the study subjects. Sera was drawn and tested for HBsAg by radioimmunoassay. HBeAg and ALT were also tested for those of HBsAg positive. The HBsAg positives was retest for HBsAg 6 months later. RESULTS: Among the study subjects (n= 1816), the seroprevalence of HBsAg was 5.5% (95% CI= 4.5%-6.6%), 7.4% in men (95% CI= 5.8-9.4) and 3.6% in women (95% CI= 2.5-5.0). A past history of acute liver disease and a family history of chronic liver diseases was shown to be risk factors for HBsAg positivity. Among the 31 HBsAg-positives, negative seroconversion rate was estimated to be 3.2 %, Thus, prevalence of chronic HBV infection was estimated to be 5.3% (95% CI= 3.7-6.6). CONCLUSION: In this study, the HBsAg seroprevalence rate was lower than that of the other studies in 1980's, particularly in young adult and women. Considering the public health importance of liver cancer and chronic liver diseases, the further effort is needed to prevent and reduce the HBV infection.

8.An Appropriateness Evaluation of Cesarean Section, Cholecystectomy, and Admission in Pediatric Pneumonia.

The aim of this study was to evaluate the appropriateness of some kinds of surgery and admission, such as cesarean section (C/S), cholecystectomy, and pediatric pneumonia. For appropriateness evaluation, we ourselves developed some criteria, which were included in the category of explicit and linear criteria, with the assistance of specialists of relevant clinical field. The evaluation of appropriateness was performed by two family physicians. The major findings were as follows: 1. For ceserean section, 77.6% of deliveries were determined to be 'appropriate', but the level of appropriateness was not significantly different among hospitals between hospital groups by size. The most frequent indication of C/S was repeated operation, followed by cephalopelvic disproportion(CPD). The labor trials for vaginal delivery among repeated C/S and CPD cases were performed in 24.5% of pertinent deliveries. 2. About 73.8% of cholecystectomy cases was appropriate to one of the surgical indications, without significant differences among hospitals. Of surgical indications, 'sufficiently frequent and intense symptom recurrence' was the most frequent, and 'confirmed acute cholecystitis' was the second. 3. Of children admitted due to pneumonia, only 57.4% of cases satisfied admission criteria, and the level of appropriateness of admission was different among hospitals. The common reasons for admission were 'failure to initial treatment', 'suspected bacterial pnermonia', 'young infant', etc. We could find that there were differences of quality among hospitals in some procedures, especially in the pediatric pneumonia and labor trial before C/S, which suggested that the implementation of quality assurance activities would be necessary in this country. In this study, we used some simple and primitive research tools and the numbers of subjects and tracer procedures were limited. So advanced studies with plentiful subjects and more representative diseases or procedures should be tried.

9.Effects of host modulation by nonsteroidal anti-inflammatory drugs on periodontal disease: a systematic review and meta-analysis.

PURPOSE: Nonsteroidal anti-inflammatory drugs that prohibit biosynthesis of arachidonic acid metabolites have been considered potent host modulation agents. The aim of this review was to determine the effect of nonsteroidal anti-inflammatory drugs adjunctive with nonsurgical periodontal treatment in patients with periodontal disease. MATERIALS AND METHODS: Three electronic databases were searched to identify relevant studies. The methodological quality and mean differences of the change in clinical attachment level and probing depth were analyzed according to Cochrane review methods. RESULTS: Twelve studies were included in the methodological assessment and nine studies were suitable for inclusion in the meta-analysis. The mean difference in the clinical attachment level gain did not differ significantly between the nonsteroidal anti-inflammatory drugs and control groups at any observation time. The highest mean difference in clinical attachment level gain was 0.30 mm at 4 weeks (95% confidence interval = -0.37 to 0.97). There was a significant mean difference in the probing depth reduction, of 0.34 mm (95% confidence interval = 0.29 to 0.40) at 6 weeks. CONCLUSION: Therefore, nonsteroidal anti-inflammatory drugs have additional therapeutic effect when administrated with nonsurgical periodontal treatment.

10.3 Cases of Megameatus Intact Prepuce(MIP): Pyramid Procedure.

Megameatus intact prepuce (MIP) is an unusual variant of hypospadias and comprised 3 percent of all hypospadias. Embryologically, maldevelopment of glandular epithelial infolding would appear to be the abnormal process responsible for MIP formation. We have performed the pyramid procedures in 3 patients seen with the MIP variant during 14 months and achieved good results. The pyramid procedure optimizes a safe and simple dissection of the megameatus-urethra, and allows for subsequent reduction and caliber of both with excellent functional and cosmetic results.
