1.Diagnosis and test for diabetic kidney disease.

Diabetic kidney disease, as one of the important diabetic complication, developed in 20% to 40% of patients with diabetes and is now the most common cause of end-stage renal disease. Although it has been recommended that annual screening of renal function including microalbuminuria in diabetic patients, many patients are currently under-diagnosed state. Early recognition of diabetic renal complication has a pivotal role in the management of diabetic patients for improvement of patient's prognosis. The detection of microalbuminuria is particularly important as a marker of early diabetic kidney disease, and is related with an elevated cardiovascular complications. Like other chronic renal disease, diabetic kidney disease has characteristic to show a progressive decline in renal function, but significantly increased cardiovascular mortality even in the early stage of diabetic kidney disease. Therefore, more aggressive trials for detection of the presence of diabetic kidney disease and comorbid cardiovascular disease and management for cardiovascular risk factor reduction and adequate therapeutic intervention for slowing the progression of renal disease is essential to proper management for patients with diabetic kidney disease.

3.A review on mathematical models for estimating indoor radon concentrations.

Radiation from natural sources is one of causes of the environmental diseases. Radon is the leading environmental cause of lung cancer next to smoking. To investigate the relationship between indoor radon concentrations and lung cancer, researchers must be able to estimate an individual’s cumulative level of indoor radon exposure and to do so, one must first be able to assess indoor radon concentrations. In this article, we outline factors affecting indoor radon concentrations and review related mathematical models based on the mass balance equation and the differential equations. Furthermore, we suggest the necessities of applying time-dependent functions for indoor radon concentrations and developing stochastic models.

4.The Role of Visfatin in Diabetic Nephropathy

As a result of the energy overload in obesity, insulin resistance, type 2 diabetes, dyslipidemia, hypertension, and atherosclerosis develop, which together comprise the metabolic syndrome. Although the kidney becomes a victim of hyperglycemia in diabetes mellitus, recent work has shown that the abnormalities of lipid and glucose metabolism in the kidney are similarly important to those in adipose tissue. Interestingly, obesity triggers the release of adipokines such as leptin, resistin, and visfatin, and these can then be associated with the progression of diabetic nephropathy and other vascular complications. These adipokines, which are also synthesized in the kidney, appear to have an important role in renal injury associated with insulin resistance. Our studies found that visfatin is not only a surrogate marker of systemic inflammation in type 2 diabetic patients but is also up-regulated in diabetic kidney through the uptake of glucose into renal cells, which leads to the activation of the intracellular insulin signaling pathway and pro-inflammatory mechanisms. However, we also observed a beneficial effect of visfatin administration to type 2 diabetic mice. Visfatin injection improved diabetic nephropathy in vivo, in contrast to our previous in vitro study of cultured renal mesangial cells. These results suggest the possibility of multiple cross-talk between adipose tissue and kidney in the metabolic syndrome, particularly in diabetic nephropathy. Further study should be undertaken to understand the role of adipose tissue and kidney as major organs in the metabolic syndrome.

5.Measuring Workload of Home Visit Care Activities Using Relative Values.

OBJECTIVES: The purpose of this study was to measure the workload of home visit care activities and their relative values. This study examined also factors that affect the workload of home visit care activities. METHODS: The participants of this study were 126 homehelpers of 50 home visit care agencies at the 2nd Longterm Care Insurance Demonstration Project. The workload of home visit care activities was divided into total work and four dimensions ; physical efforts, mental efforts, stress and time. Home visit care activities consisted of four categories with 24 items. We used magnitude estimation method to measure their relative values of the four dimensions. The participants answered the relative values of each activities based on the reference service. We used the activity for supporting their elderly's evacuation as the reference service. RESULTS: Most of the respondents were over 40 years old female. They consumed most their time supporting elderly's going out. They consumed their highest physical, mental efforts, and stress for activities of coping with emergency situation. The Pearson correlation coefficients showed significant relationships between workload and each dimensions. This study showed that all four dimensions are statistically significant predictors of workload of home visit care activities. Also, we found that the home-helper's career affects the workload of home visit care activities. CONCLUSIONS: The workload of home visit care activities could be explained by physical efforts, mental efforts, stress and time.

6.Glutathione S-Transferase M1 Status and Breast Cancer Risk: A Meta-Analysis.

It is not yet clear whether Glutathione S-transferase M1 (GSTM1) polymorphisms affect the risk of breast cancer. The aim of this study is to provide a comprehensive meta-analysis of all the available, published case-control studies on the extent of the possible association between GSTM1 polymorphisms and susceptibility to breast cancer. Twenty case-control studies on GSTM1 and breast cancer were identified using both PUBMED and a manual search. Meta-analysis was conducted by the Peto method. Subgroup analyses were undertaken, in order to explore the relationship between effect sizes and the study characteristics. The overall odds ratio (OR) was found to be 1.06 (95% CI, 0.99-1.14). The OR for post-menopausal women with GSTM1 deficiency was determined to be 1.19 (95% CI, 1.05-1.34). In populations with a low frequency of GSTM1 deficiency, a greater increase was observed (OR, 1.20; 95% CI, 1.08-1.34). Furthermore, the highest associations were found in post-menopausal women with a low frequency of GSTM1 deficiency (OR, 1.44; 95% CI, 1.20-1.73). The fact that GSTM1 deficiency is not rare in the general population implies that the attributable risk for breast cancer could be sizable. Further studies focusing on the structure of haplotype blocks of GSTM1 are required in order to find a specific haplotype with a predisposing breast cancer susceptibility allele.

8.Risk Factors for Cause-specific Mortality of Very-Low-Birth-Weight Infants in the Korean Neonatal Network.

This study attempted to assess the risk factors for mortality of very-low-birth-weight (VLBW) infants in the neonatal intensive care unit (NICU, n=2,386). Using data from the Korean Neonatal Network, we investigated infants with birth weights <1,500 g and gestational ages (GAs) of 22-31 weeks born between January 2013 and June 2014. Cases were defined as death at NICU discharge. Controls were randomly selected from live VLBW infants and frequency matched to case subjects by GA. Relevant variables were compared between the cases (n=236) and controls (n=236) by Cox proportional hazards regression to determine their associations with cause-specific mortality (cardiorespiratory, neurologic, infection, gastrointestinal, and others). In a Cox regression analysis, cardiorespiratory death were associated with a foreign mother (hazard ratio, HR, 4.33; 95% confidence interval, CI, 2.08-9.02), multiple gestation (HR, 1.65; 95% CI, 1.07-2.54), small for gestational age (HR, 2.06; 95% CI, 1.25-3.41), male gender (HR, 1.69; 95% CI, 1.10-2.60), Apgar score < or =3 at 5 min (HR, 1.97; 95% CI, 1.18-3.31), and delivery room resuscitation (HR, 2.60; 95% CI, 1.53-4.40). An Apgar score < or =3 at 5 min was also associated with neurological death (HR, 2.95; 95% CI, 1.29-6.73). Death due to neonatal infection was associated with outborn delivery (HR, 5.09; 95% CI, 1.46-17.74). Antenatal steroid and preterm premature rupture of membranes reduced risk of cardiorespiratory death (HR, 0.43; 95% CI, 0.27-0.67) and gastrointestinal death (HR, 0.30; 95% CI, 0.13-0.70), respectively. In conclusion, foreign mother, multiple gestation, small gestation age, male gender, Apgar score < or =3 at 5 min, and resuscitation in the delivery room are associated with cardiorespiratory mortality of VLBW infants in NICU. An Apgar score < or =3 at 5 min and outborn status are associated with neurological and infection mortality, respectively.

9.Risk Factors of the High-Risk Pregnancy and Association with Particulate Matter

Purpose:
High-risk pregnancy, resulting in major complications, such as abortion, gestational hypertension, and preterm labor, are on the rise, requiring intervention. Increased concentration of particulate matter that is 10 μg per cubic meter or less in diameter (PM10) due to increased air pollution and industrialization has recently been noted as a risk factor for health problems, and prior studies have reported that PM10 is related to abortion, gestational hypertension, and preterm labor.
Methods:
This study used data from the Korean medical panel between 2008 and 2016 to identify 879 women with normal pregnancies and 301 women diagnosed with high-risk pregnancies. The association between high-risk pregnancy and PM10 was analyzed using the annual average regional concentration during the same period by the Korea Environment Corporation.
Results:
At 35 years of age or older, the risk of abortion (p<0.001; 95% confidence interval [CI], 1.43–2.71) and preterm labor increased (p<0.05; 95% CI, 0.33–0.90). Married pregnant women were at a high risk of preterm labor (p<0.05; 95% CI, 0.25–0.98), and the prevalence of gestational hypertension was high in the metropolitan area (p<0.05; 95% CI, 12.09–25.93). On comparing the average annual concentration of particulate matter according to the level of atmospheric ventilation of the Korea Environment Corporation using a cutoff of ≤50 μg/m3, the probability of preterm labor was 2.2 times higher in groups exposed to particulate matter concentration of ≥50 μg/m3 (p<0.01; 95% CI, 1.49–3.25).
Conclusion
PM10 was related to preterm labor, and the risk of preterm labor was high if the average annual concentration of PM10 in the individual’s residential area exceeded 50 μg/m3.
10.Measurement and modeling of indoor radon concentrations in residential buildings.

Radon, the primary constituent of natural radiation, is the second leading environmental cause of lung cancer after smoking. To confirm a relationship between indoor radon exposure and lung cancer, estimating cumulative levels of exposure to indoor radon for an individual or population is necessary. This study sought to develop a model for estimate indoor radon concentrations in Korea. Especially, our model and method may have wider application to other residences, not to specific site, and can be used in situations where actual measurements for input variables are lacking. In order to develop a model, indoor radon concentrations were measured at 196 ground floor residences using passive alpha-track detectors between January and April 2016. The arithmetic mean (AM) and geometric mean (GM) means of indoor radon concentrations were 117.86±72.03 and 95.13±2.02 Bq/m³, respectively. Questionnaires were administered to assess the characteristics of each residence, the environment around the measuring equipment, and lifestyles of the residents. Also, national data on indoor radon concentrations at 7643 detached houses for 2011-2014 were reviewed to determine radon concentrations in the soil, and meteorological data on temperature and wind speed were utilized to approximate ventilation rates. The estimated ventilation rates and radon exhalation rates from the soil varied from 0.18 to 0.98/hr (AM, 0.59±0.17/hr) and 326.33 to 1392.77 Bq/m²/hr (AM, 777.45±257.39; GM, 735.67±1.40 Bq/m²/hr), respectively. With these results, the developed model was applied to estimate indoor radon concentrations for 157 residences (80% of all 196 residences), which were randomly sampled. The results were in better agreement for Gyeonggi and Seoul than for other regions of Korea. Overall, the actual and estimated radon concentrations were in better agreement, except for a few low-concentration residences.
