1.A Clinical Study on Ligament Injuries of the Knee

The authors reviewed 73 cases of ligament injuries in 70 patients from October, 1972 to June, 1978 who were admitted and treated in the Orthopedic Department at Kyung Hee University Hospital. Of 70 patients, the sex distribution was 59 patients (84%) in male and 11 patients (16%) in female. The high incidence was in age group from 21 to 40 years, that was 46 patients (55%). Traffic accident was most common cause of ligament injuries that was 61 patients (87%). Methods of treatment were direct repair, reconstruction and conservative treatment. Degree of angle and distance were measured with stress radiogram. Degree of angle and distance were also compared with normal and abnormal knee joints. Eleven cases were treated conservatively, and 62 cases, operatively, of whlch direct repair performed in 56 cases and reconstructive surgery, in 6 casea, The summary were as follows: 1. The result was better in the group with conservative treatment, as compared with operative treatment. And conservative treatment was applied only in single ligament injuries. 2. In normal knee joint, degree of angle was distributed evenly in the range of 0° to 10°, and distance was in the range of 0 to 10 mm. 3. In stress radiograrn, the significance of ligament injuries was present in more than 10° and 10 mm. 4. The measurement of distance seemed to be more meaningful than degree of angle. 5. It is helpful to measure the degree of angle and distance in diagnosis of ligament injuries, though not absolute.

2.Three Cases of Fever Unknown Origin with Lymphoproliferative Features and a Unique Pattern of 18-FDG Uptake on the Fusion PET/CT.

Evaluation of a fever of unknown origin (FUO) is complex. Recently, PET scanning has been approved for screening in FUO evaluation. We treated three cases of FUO associated with increased FDG uptake in the bone marrow of the femur and tibia on the fusion PET/CT; all three had the same pattern of uptake. Bone marrow biopsies revealed mature lymphocyte and histiocyte infiltration and myxoid changes in one case, and cortical bone involvement in another case. The cases were all young females who had fever with neutropenia and relative lymphocytosis that lasted for several weeks and then remitted spontaneously. Even though the results of the studies were not diagnostic, the unique uptake pattern on PET/CT and the histology might be related to the cause of the illness and should be studied further to assess the association with classic FUO.

3.A Clinical Study on the Regional Ejection Fraction and Regional Wall Motion In Acute Myocardial Infarction.

Regional left ventricular dysfunction is common in patients with coronary artery disease and accurate analysis of regional dysfunction is of particular interest. For the quantitative assessment of regional dysfunction, we measured regional ejection fraction by radial sector division method in 19 patients with acute myocardial infarction and 13 normal controls who had multigated blood pool scan. And two independent observer analyzed regional wall motion using 5 point grading system in 18 patients undergoing radionuclide ventricular cineangiography. The results obtained were as follows : 1) Regional wall motion scores for the gated blood pool study agreed completely in 72 of 108 segments (66.7%) and agreed within 1 grade in 88 of 108 segments(81.5%) and agreement rate is lowest in the septal area. 2) Global left ventricular ejection fraction was 63.2+/-4.2% in normal controls, 36.6+/-6.8% in extensive anterior wall infarction group and 52.6+/-9.7% in inferior wall infarction group. The value of extensive anterior wall infarction group was significantly lower than that of inferior wall infarction group(p<0.005). 3) Regional left ventricular ejection fraction by radial sector division method in normal control group were as follows : area 1 ; 56.5+/-6.7%, area 2 : 77.9+/-4.8%, area 3 ;84.3+/-5.5%, area 4 : 76.8+/-6.6%, area 5 ; 84.7+/-7.6%, area 6 ; 85.9+/-11.2%, area 7 ; 75.5+/-12.3%, area 8 ; 74.9+/-14.0%, area 9 ; 75.5+/-8.8%, area 10 ; 54.2+/-11.0%, 11 ; 34.5+/-16.3, area 12 ; 37.1+/-18.0%. 4) Mean regional ejection fraction in 7 patients with anterior wall infarction showed significantly lower values in area 4 to area 8, and in area 2 to area 5 in case of inferior wall infarction group. 5) We thought that regional ejection fraction obtained by radial sector division method is valuable index for the management and evaluation of patients with coronary artery disease.

4.Overview of the Unites States Air Force Suicide Prevention Program and Implications for Korean Armed Forces.

Suicide is a major public health problem among Korean military personnel. As multiple factors and their interactions are related to suicide, multicomponent intervention might be useful for reducing suicide and suicide-related outcomes. The United States Air Force Suicide Prevention Program (AFSPP) launched in 1996 is a good example of an evidence-based multifaceted program focusing on leadership involvement and whole community-level efforts. For early identification and management of an at-risk population, AFSPP aims to encourage help-seeking and reduce stigma by changing the community's social norms regarding mental health and suicide. The present study reviewed the key elements of this successful intervention and made a proposal for improving the suicide prevention strategy in the Korean armed forces. This review might be useful for establishing a comprehensive intervention in local communities and organizations outside of the military.

7.Four Cases of Brunner's Gland Adenoma.

Brunners gland adenoma is a benign epithelial tumor of the duodenum originating from submucosal Brunners gland. This is an extremely rare entity that account for only 10.6% of benign duodenal tumor, which are themselves relatively rare, representing 0.008% of all surgical and autopsy specimens. The clinical manifestation are nonspecific gastrointestinal complaints, such as bloating or epigastric pain, and the tumor gives rise to melena or anemia, due to the ulceration or erosion of the tumor. The diagnosis is usually made by radiologic studies and gastroduodenal endoscopy which can also provide definitive treatment. The aim of treatment is complete removal of the lesion and exclude malignancy. We report on 4 cases of Brunners gland adenoma which was confirmed by operation or endoscopic polypectomy.

8.Evaluation of Residual Myocardial Ischemia in the Infarct Zone by Dipyridamole Stress SPECT 99mTc-MIBI Myocardial Scan.

In the recovery phase of acute myocardial infarction, residual ischemia is an important predictor of future cardiac event. To quantitatively evaluate the residual ischemia in the infarct zone and to see whether it can be predicted by particular coronary angiographic characteristics, dipyridamole stress SPECT 99mTc-MIBI myocardial scan was used in 31 patients with uncomplicated acute myocardial infarction. Quantitative assessment of residual ischemia in the infarct zone was made by the ratio of reversible defect in the polar display of SPECT MIBI scan to the total polar map area. Residual ischemia in the infarct zone in these patients does not correlate with patency, collateral circulation or jeopardy score of the infarct related artery. PTCA was performed in 7 patients with significant reversible defect and suitable anatomical lesion to evaluate the reversibility by revascularization. Following the PTCA mean reversible defect decreased significantly from 10.1+/-2.2% to 1.3+/-1.3%(P<0.05). In 10 patients with multivessel disease, 7(54%) out of 13 remote sites showed significant reversible defect showing limited usefulness in the detection of concomitant remote lesion in multivessel disease. Thus dipyridamole stress 99m Tc-MIBI myocardial scan is useful in the evaluation of residual ischemia in the infarct zone, and helpful in the decision making and assessing the result of revascularization.

9.Percutaneous Transluminal Coronary Angioplasty(PTCA) in Patients with Coronary Artery Disease.

BACKGROUND: To evaluate the efficacy, complication and restenosis rate of percutaneous transluminal coronary angioplasty(PTCA), we analized the results of PTCA for 277 cases(308 lesions) in 240 patients admitted to Seoul National University Hospital under the diagnosis of stable angina, unstable angina, acute myocardial infarction(AMI)or postinfarction angina between April 1986 and October 1991. METHODS: We performed PTCA with over-the-guidewire technique and followed up for 2-42 months(mean 16.2 months). To evaluate the clinical efficacy, patients without AMI and unstable angina underwent pre- and post-PTCA exercise test. RESULTS: Successful PTCA's were performed in 273 out of 308 lesions(88.6%). The success rate of PTCA for left anterior descending artery(LAD), left circumflex artery(LCX) and right coronary artery(RCA) was 88.5%(177/200), 88.9%(40/44) and 87.5%(56/64), respectively. Simultaneous PTCA for multiple coronary arteries were successfnlly perfomed in 26 out of 27 cases(96.3%). Success rate for individual coronary artery subtypes according to ACC/AHA PTCA subcommitte was 99.1% for type A lesions and 78.2% for type B lesions. Total exercise duration by treadmill test increased from 6.74+/-3.24min to 9.70+/-2.85min after PTCA(p<30.01). There were non-fatal acute coronary occlusions in 8 patients(3.3%), but there was no death related to PTCA nor emergency coronary artery bypass graft(CABG). During the follow-up period of 2 to 42months(mean : 16.2months), restenosis was suspected clinically in 90 patients, and was confirmed in 43 patients with coronary angiography. Among them, we performed second PTCA in 31 patients, and third PTCA in 2 patients. Seventy nine percent of restenosis developed within 6 months after PTCA. CONCLUSION: We conclude that PTCA was an effective and safe treatment modality for patients with coronary artery disease, especially when performed in patients with type A or B lesions and some of selected type C lesions.

10.MARGINAL FIT OF GLASS INFILTRATED ALUMINA CORE FABRICATED FROM ALUMINA TAPES.

The purpose of this study was to compare the marginal fit of all ceramic crowns prepared from alumina slip casting, which is consistent with the conventional In-ceram system, and those fabricated from alumina tapes which is currently under development in an effort to alleviate complexities involved in the forming procedure of the In-ceram crown core. All ceramic crowns, made of In-ceram(slip casting) and alumina tapes(Doctor blade casting), were prepared with 90degreeand 135degreeshoulder margins. The crowns were cemented with a glass ionomer cement and embeded in epoxy resin. The embedded crowns were sectioned faciolingually and mesiodistally and marginal discrepancies and marginal gaps were measured under the Measurescope MM II. The measurements were analyzed using Wilcoxon rank sum test and Kruskal-Wallis test and the results were as follows: 1. In the case of 90degreeshoulder margin, the combined marginal discrepancies and marginal gaps were 78.3 micrometerm and 44.4 micrometer respectively, for the all ceramic crowns fabricated using the alumina tapes. In comparison, the values were 65 micrometer and 25.5 micrometer for the In-ceram crowns. For the marginal gaps a statistical difference existed (p<0.05) but no significant difference was observed for the marginal discrepancy (p>0.05). 2. In the case of 135degreeshoulder margin, the combined marginal discrepancy and marginal gaps were 82.1 micrometer and 40.2 micrometer respectively, for the all ceramic crowns formed with the tapes. As compared with the marginal discrepancy and gaps of the 90degreeshoulder margin in the fabricated from the alumina tapes, no significant statistical differencies were discerned in both cases (p>0.05). 3. There was no statistically significant difference in the fits among four locations around the margins of the all ceramic crowns fabricated using the alumina tapes. The results obtained in this study showed that the marginal fits of the glass infiltrated alumina cores fabricated from the alumina tapes are slightly higher value than those prepared using the In-ceram but the difference is within a clinically acceptable range.
