1.Clinical Study on Cast-Brace in Femoral Fractures

14 cases of femoral shaft fractures were treated with cast-brace for two years from June, 1974 to June, 1976 at the Department of Orthopedic Surgery, College of Medicine, Seoul National University Hospital. The results were as followings; 1. Of the 14 cases, eleven cases were male and three were female, and they were between 18 to 51 years of ages. 2. The fracture site was accounted that three of 14 cases were at the distal one third of the femur and the other eleven at the mid one third with four cases being comminuted. 3. 7 cases were treated with cast-brace initially, and the other 7 cases were secondarily treated, because of being complicated. 4. The average time of fracture healing was 14 weeks in the cases with cast-brace which is compared with 20 weeks of spica-cast control group in the same period. 5. One of 7 cases treated primarily developed shortening and angulation, which was corrected by open reduction and internal fixation.

4.Comparative Study of Absorbable Suture Material and Nonabsorbable Suture Material in Micro

The suture materials influence the success of microvascular anastomosis importantly. The purpose of this experiment is to observe the effect of absorbable suture material and nonabsorbable suture material in microvascular anastomosis. The study was carried out on one side of carotid arteries of fourty rats which were anesthetized intraperitoneally with urethane. The experimental procedures differed in two groups, Group A in which the carotid artery was sutured with 10-0 monofilament polyglycolic acid, Group B in which the carotid artery was sutured with 10-0 monofilament nylon. Gross and histological examination were used to compare the results of end to end suture above two suture material. The results were as follows. 1. Nylon had greater tensile strength and ease of handling than polyglycolic acid, but held knots poorly. 2. Polyglycolic acid had difficult handling and lesser tenslie strength than nylon, but held knots hardly. 3. Polyglycolic acid had high patency rate, some lesser tissue response than nylon.

6.A Clinical Study on Ligament Injuries of the Knee

The authors reviewed 73 cases of ligament injuries in 70 patients from October, 1972 to June, 1978 who were admitted and treated in the Orthopedic Department at Kyung Hee University Hospital. Of 70 patients, the sex distribution was 59 patients (84%) in male and 11 patients (16%) in female. The high incidence was in age group from 21 to 40 years, that was 46 patients (55%). Traffic accident was most common cause of ligament injuries that was 61 patients (87%). Methods of treatment were direct repair, reconstruction and conservative treatment. Degree of angle and distance were measured with stress radiogram. Degree of angle and distance were also compared with normal and abnormal knee joints. Eleven cases were treated conservatively, and 62 cases, operatively, of whlch direct repair performed in 56 cases and reconstructive surgery, in 6 casea, The summary were as follows: 1. The result was better in the group with conservative treatment, as compared with operative treatment. And conservative treatment was applied only in single ligament injuries. 2. In normal knee joint, degree of angle was distributed evenly in the range of 0° to 10°, and distance was in the range of 0 to 10 mm. 3. In stress radiograrn, the significance of ligament injuries was present in more than 10° and 10 mm. 4. The measurement of distance seemed to be more meaningful than degree of angle. 5. It is helpful to measure the degree of angle and distance in diagnosis of ligament injuries, though not absolute.

8.Paraplegia following Chemonucleolysis A Case Report and Discussion of Neurotoxicity.

Chymopapain was discovered by Jensen in 1941, and in 1963 Smith demonstrated the chondrolytic properties of chymopapain. Since then, many patients have been injected with this agent. Although there are evidences of neurotoxicity to chymopapain in animals, adverse effects in humans have rarely been reported. We present a case delayed onset of paraplegia after chymopapain chemonucleolysis and review the neurotoxicities to chymopapain.

9.Experimental Study for Measurement of Ureteral Compliance in Rat.

The compliance of upper urinary tract plays a main role in preservation of renal function against pressure overload. It consists of renal pelvic compliance and ureteral compliance. We measured ureteral compliance in rat with new experimental model applying pressure-flow study. Two catheters were placed in left renal pelvis. one for flow infusion, another for pressure monitoring. Pressure-flow study was done with ligation of uretero-vesical junction(UVJ) and subsequent pressure-flow study with ligation of uretero-pelvic junction(UPJ) in the same rat. Ureteral compliance was calculated from upper urinary tract compliance with UVJ ligation minus pelvic compliance with UPJ ligation. The data from seven rats were analyzed. Mean ureteral compliance (+/-standard deviation) was 0.0011+/-0.00034 ml/cmH2O and mean contribution of ureter for upper urinary tract compliance was 19%. Our new experimental model for measurement of ureteral compliance was simple and easy to perform. Results in this study will be used as baseline data for further investigation of uretera1 compliance in chronic partial ureteral obstruction.

10.Effect of Intracameral Mitomycin-C Irrigation on ERG in Rabbit.

Mitomycin-C(MMC) has been increasingly used as adjunct chemotherapy during glaucoma filtering surgery in order to increase the success rate, because of its ability to inhibit the proliferation of fibroblast. Although MMC demonstrates toxic effects in the anterior segment of the eye, few studies on its retinal toxicity have been reported. Therefore, this study was performed to investigate the retinal toxicity of MMC in the rabbit after intra cameral irrigation. After anesthesia, 1 ml of MMC(40 mg/ml) was intracamerally irrigated in right eye and 1ml of balanced salt solution(BSS) was used as control in the left eye. The electroretinogram was checked before, 2 and 7 days after intracameral MMC irrigation. The fundus examination and histopathology by light microscopy and transmission electron microscopy were performed at 7days after MMC irrigation. On electroretinogram, scotopic b-wave amplitude in MMC irrigated eye was significantly decreased 7 days after irrigation as compared with BSS irrigated eye. The retina in MMC irrigated eye was normal on the fundus findings and histopathology by light microscopy and transmission electron microscopy 7 days after irrigation. This findings suggest that intracameral irrigation of MMC may induce the functional disturbance rather than structural alteration of retina.
