2.The Translocation of Nuclaer Factor Kappa B on Corneal Epithelial Cells after Ultraviolet B Irrradiation.

PURPOSE: To seek the role of nuclear factor kappa B (NF-kB) on the corneal epithelial cell death after ultraviolet (UV) irradiation. METHODS: Human corneal epithelial cells transfected by Simian Virus 40 were used in this study. UVB(312 nm) located at 10cm distance from bottom (0.6 mW/cm2 ) was irradiated for 10, 20, 30, and 40 seconds. To measure the cytotoxicity, 3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyl tetrazolium bromide (MTT) method was used. Translocation of NF-KB was examined by immunocytochemistry with anti NF-K B p65 antibody and Electrophoretic Mobility Shift Assay (EMSA). To confirm the role of NF-KB , sulfasalazine, a specific inhibitor of NF-KB (0.5 mmole), was pretreated for 30 minutes before irradiatrion, and cytotoxicity and translocation of NF-KB was evaluated. RESULTS: UV irradiation resulted in a significant decrease in viability of cultured human corneal epithelial cells, especially after 20 second duration. When HCECs were irradiated with UVB, the translocation of N F -KB was observed in immunocytochemistry. These translocation was peaked 2 hours after UV irradiation in EMSA. In HCECs pretreated with sulfasalazine, either the cellular death or the translocation of NF-KB was blocked. CONCLUSION: UV irradiation can translocate NF-KB on the cultured human corneal epithelial cells. The cellular death after UV irradiation was blocked by sulfasalazine, a potent inhibitor of translocation of NF-KB. These findings suggest that NF-KB plays an important role in cellular death after UV irradiation.

3.The Clinical Study of Meniscal Repair.

PURPOSE: To analyze and report the analyzed clinical results of meniscal repair. MATERIALS AND METHODS: Thirty-two cases were reviewed after arthroscopic meniscal repair. The meniscal repair was indicated by unstable tears in red-red or red-white zone with the tear size longer than 10mm. The location, pattern of the tear and associated injury were examined respectively, and the clinical results were evaluated with preoperative, postoperative Lysholm score and findings of physical examination. Physical examination was focused on tenderness, click, effusion and McMurray test. Also, a second look arthroscopy was performed in the case of bucket-handle tears and other types with recurring symptoms and positive physical findings. RESULTS: The number of medial meniscus tear was higher than lateral meniscus tear and the were no considerable differences in the number between the right and the left knees. The most common location of tear was the red-red zone tear in 30 cases, and the most common tear pattern was longitudinal tear in 22 cases. The number of bucket-handle tear was nine. The most common associated injury was ACL injury. Acording to clinical assessment, 29 out of 32 cases showed satisfactory results, and the results of 7 out of 9 bucket-handle tears were good. CONCLUSION: An analysis of the clinical results of meniscal repair has been carried out, showing that meniscal repair is regarded as a reliable treatment modality in meniscal tear with proper patient selection.

Adenomyomatosis of the gallbladder is Characterized by hyperplastic changes including overgrowth of the mucosa, thickening of the muscle wall, and intramural diverticula, crypts, or sinus tracts(Rokitaasky-Aschoff sinuses). The main diagnostic test for the detection of this disease is oral cholecystography but it's use is being decreased. Recently, Ultrasound, ERCP, and CT have been used for diagnosis. We present a report of case in whom ademomyomatosis of gallbladder was disgnosed on ultrasound and ERCP and confirmed by surgery. The essential feactures of ultrasound and ERCP diagnosis are discussed.

5.Primary Malignant Fibrous Histiocytoma of the Diaphragm: One Case Report.

A 25-year-old female patient was adimitted due to discomfort in the right chest and left epigastric area. Chest X-ray and chest CT showed a round 4x4x4cm sized mass located in the right lower lobe and abutted on the right diaphragm and metastatic lesions in the right lower lobe and right middle lobe. In exploratory thoracotomy, we discovered a mass originating from the right diaphragm which directly invaded the right lower lobe and metastatic subpleural lesions in the right lower lobe and right middle lobe. The mass was diagnosed as sarcoma by frozen section biopsy and we removed the mass by en bloc resection of the right diaphragm and right lower lobe. Metastatic subpleural lesions in the right lower lobe and right middle lobe were removed by wedge resection. The mass was malignant fibrous histiocytoma of the diaphragm in permanent pathologic examination and chemotherapy was done as adjuvant measure. We report one extremely rare case of malignant fibrous histiocytoma of the diaphragm with literature.

6.Insufficiency Fracture of Proximal Femur Shaft without Bisphosphonate Therapy: Report of Three Cases.

There is a growing interest in the factors related to insufficiency fractures. We are going to report three insufficiency fracture cases which are considered to be caused by osteoporosis, rheumatoid arthritis, steroid use and femoral shaft bowing among the patients not taking bisphosphonate. All cases are caused by low energy trauma and among these cases, one patient is being presented with a prodromal symptom and another patient complains of both prodromal symptoms and bilateral lesions.

7.The Evaluation of Contrast Sensitivity and Glare after LASIK Using Flying Spot Mode Laser According to Pupil Diameter and Refractive Error.

PURPOSE: To study the effect of LASIK using flying spot mode laser on contrast sensitivity and glare according to pupil diameter and preoperative refractive error. METHODS: Fifty seven eyes of 42 patients had LASIK using MEL 70 G-scan (Asclepion Meditec, Germany) to correct myopia ranging from -2.50 to -10.50 diopter (D). The contrast and glare sensitivity were measured preoperatively and 1 week, 1 month and 3 months after LASIK using CGT-1000 (Takagi, Japan). RESULTS: The mean preoperative spherical equivalent (SE) was -5.4+/-2.9D. The mean pupil diameter was 5.95+/-0.46 mm in scotopic condition. Contrast sensitivity was significantly reduced 1 month after LASIK, depending on pupil diameter and degree of preoperative myopia. However, values returned to preoperative figures thereafter (P<0.05). Glare sensitivity was improved at all frequencies 3 months after LASIK, and most significantly at high frequency (P=.031). CONCLUSIONS: LASIK using employing the flying spot mode laser can effectively accomplish refractive correction without reduction of preoperative contrast and glare sensitivity values.

9.Plasma proteomic analysis of patients with squamous cell carcinoma of the uterine cervix.

OBJECTIVE: To compare plasma protein expression between patients with squamous cell carcinoma (SCC) of the cervix and normal controls. METHODS: Plasma samples from patients with benign gynecological disease (normal cervix, n=6) and cervical cancer (SCC, n=6) were subjected to plasma proteomic analysis using two dimensional gel electrophoresis (2-DE) and matrix-assisted laser desorption/ionization mass spectroscopy (MALDI-MS). Western blotting and immunoturbidimetric assay were performed to validate the results of 2-DE. RESULTS: Eight proteins showed differential expression between controls and SCC patients; six (ceruloplasmin, complement C3, afamin precursor, alpha-1-B-glycoprotein, transferrin, alpha-fibrinogen precursor) were up-regulated, while two (chain A, crystal structure of antithrombin and apolipoprotein A-IV precursor) were down-regulated in the plasma of SCC patients. Western blotting analysis revealed significant elevation of ceruloplasmin, complement C3, afamin, and alpha-1-B-glycoprotein in the plasma of SCC patients in comparison to controls. Immunoturbidimetric assay of a larger group confirmed the results of 2-DE and Western blotting, and showed that ceruloplasmin and complement C3 were significantly elevated in the plasma of SCC patients in comparison with controls and patients with carcinoma in situ (CIS) of the uterine cervix. CONCLUSION: Plasma protein expression determined using 2-DE and MALDI-MS will give a chance to identify tumor-specific biomarkers for SCC of the cervix.

10.Diabetes Fact Sheet in Korea, 2016: An Appraisal of Current Status.

BACKGROUND: This report presents the recent prevalence and comorbidities related to diabetes in Korea by analyzing the nationally representative data. METHODS: Using data from the Korea National Health and Nutrition Examination Survey for 2013 to 2014, the percentages and the total number of subjects over the age of 30 years with diabetes and prediabetes were estimated and applied to the National Population Census in 2014. Diagnosis of diabetes was based on fasting plasma glucose (≥126 mg/dL), current taking of antidiabetic medication, history of previous diabetes, or glycosylated hemoglobin (HbA1c) ≥6.5%. Impaired fasting glucose (IFG) was defined by fasting plasma glucose in the range of 100 to 125 mg/dL among those without diabetes. RESULTS: About 4.8 million (13.7%) Korean adults (≥30 years old) had diabetes, and about 8.3 million (24.8%) Korean adults had IFG. However, 29.3% of the subjects with diabetes are not aware of their condition. Of the subjects with diabetes, 48.6% and 54.7% were obese and hypertensive, respectively, and 31.6% had hypercholesterolemia. Although most subjects with diabetes (89.1%) were under medical treatment, and mostly being treated with oral hypoglycemic agents (80.2%), 10.8% have remained untreated. With respect to overall glycemic control, 43.5% reached the target of HbA1c < 7%, whereas 23.3% reached the target when the standard was set to HbA1c < 6.5%, according to the Korean Diabetes Association guideline. CONCLUSION: Diabetes is a major public health threat in Korea, but a significant proportion of adults were not controlling their illness. We need comprehensive approaches to overcome the upcoming diabetes-related disease burden in Korea.
