5.Renal Streaky Artifact during Contrast-Enhanced Abdominal and Pelvic CT' Comparison of High versus Low Osmolality Contrast Media.

PURPOSE: Introduction of low osmolality contrast agent(LOCA) has allowed safer, more comfortable contrast-enhanced CT examination, but there has been significant increase in image degradation when evaluating the kidneys due to streaky artifact. The authors reviewed findings of contrast-enhanced abdominal and pelvic computed tomography(CT) to know the difference of renal streaky artifact between a high osmolality contrast agent (HOCA) and LOCA. MATERIALS AND METHODS: This study included two hundred contrast-enhanced CT in 200 patients, 100 performed with HOCA(meglumine ioglicate, 150ml) and 100 performed with LOCA (iopromide, 150ml). The severity of renal streaky artifact was compared between HOCA and LOCA groups. RESULTS: Of the scans performed with HOCA, 40 had no artifact, 52 had grade I artifact, 6 had grade II artifact, and 2 had grade III artifact Of the scans performed with LOCA, 23 had no artifact, 44 had grade I artifact, 29 had grade II artifact, and 4 had grade III artifact. There was significant difference in the degree of the streaky artifact depending upon the osmolality of the contrast media used(by {{{{x^2}}}}-test, P=.0001). CONCLUSION: The results of this study revealed a statistically significant increased incidence of artifacts and distortions of renal image with LOCA when compared with HOCA.

8.Experimental Study for Development of New Alloy for Domestic Total Knee Prosthesis

The total knee replacement is an excellent method for the treatment of osteoarthritis, rheumatoid arthritis, etc. of the knee joint. The tendency of the use of total knee prosthesis is increasing now in Korea. But domestic production of the prosthesis is not avaliable and all are imported from abroad. Moreover, these prosthesis are not well adapted to Korean people, and in occation, the custom-made prosthesis should be needed. So, domestic production of the prosthesis is required. For production of new prosthesis, in cooperation with KAIST(Korea Advanced Institute of Science and Technology), we developed the new alloy 15-57 for total knee prosthesis. We conducted an animal experimental study to confirm the biocompatibility of the new alloy. In this experiment, the Zimmer® company vitallium was compared with this new alloy as control study. The results are as follows: 1. Biocompatibility of the new alloy 15-57 is similar than that of Zimmer® company vitallium in histological study of metal-bone interspace area. 2. Surface corrosion of both metal was nearly absent. 3. In summary, the new alloy 15-57 is suggested as an acceptable metal for the production of total knee prosthesis.

9.Arthroscopic Adhesiolysis of Knee Ankylosis

Severe limitation of knee motion is a complication of operative procedures or trauma about the knee. Physical therapy and occasionally manipulation of knee with anesthesia may improve results when performed soon after the onset of motion limitation. When performed later, these have been proven less useful. This is a review of 15 patients who had severe limitation of knee motion following open operative procedures or trauma about knee and were treated by percutaneous release of adhesions under arthroscopic control. There were eleven males and four females, ranging in age from 23-67years(mean 38 years). The interval between the last open operative procedure or trauma and arthroscopic adhesiolysis ranged from three and half months to four years(mean 11.5 months). The interval between arthroscopic adhesiolysis and final follow-up evaluation ranged from 6 months to 3 years(mean 1.5 years). Average preoperative knee motion was 38.2 degrees. After arthroscopic adhesiolysis, maximum knee motion under anesthesia was 120.3 degrees and average final motion at follow-up was 101.2 degrees. The results of treatment of treatment of Tbc knee sequalae with secondary osteoarthritis and pyogenic arthritis were less favorable for motion improvement. Arthroscopic adhesiolysis seems to be a useful method of treatment in knee ankylosis. Morbidity was low, complications were few and the duration of hospital stay was short.

10.Clinical Significance of Ultrasonography for Detection of Hip Joint Effusion in Children: Analysis of the 23 Cases of Transient Synovitis of the Hip

Transient synovitis of the hip in children is a non-specific inflammatory and self-limited condition. It is also the most common cause of painful hip in children under ten years of age. Despite of the benign prognosis, there are many difficulties in distinguishing it from other diseases of the hip joint. It remains a common diagnostic problem for clinician because the clinical symptoms, physical findings, and conventional radiography is not pathognomonic of the condition. The authors paid attention to the increase of effusion in the affected hip and studied the value of the hip ultrasonography in 24 cases of transient synovitis from August 1985 to July 1987. The results are summerized as follows : 1. The ratio of male to female was 7 to 1, average age was 7.9 yrs, average hospitalization period was 4.5 days. 2. In simple X-ray studies, no bony change was detectable except for soft tissue signs in 68% of the cases. 3. Capsule-to-bone distance in sagittal ultrasonographic section revealed abnormal increase in 87.5% of the affected hip. 4. Average capsule-to-bone distance of affected hip joint was 7.50 mm, while that of the normal hip was 4.26 mm. 5. As shown in the above studies, ultrasonography can be considered good noninvasive technique in detection and follow-up of hip effusion. So, it is thought to be a valuable method in the diagnosis of transient synovitis of the hip in children.
