1.A Diagnostic Significance of Transurethral and Transrectal Ultrasonography.

An ultrasonic scanner in the field of urology is described that can be used for transurethral as well as for transrectal scanning of the bladder and prostate. The bladder was visualized best by transurethral scanning, while the rectal approach was preferable for examination of the prostate. Transurethral ultrasonography was performed in 7 patients with bladder cancer. Transrectal ultrasonography was performed in 21 controls and 41 patients with prostatic disease. The following results were obtained: 1. Diagnostic accuracy of the staging of transurethral ultrasonography was 85.7% in bladder cancer. 2. The average values of maximum anterioposterior, superio-inferior and transverse diameter. obtained from ultrasonogram were 2.43+/-0.29cm, 3.27+/-0.41cm, 4.07+/-0.91cm in control group ;3.11+/-0.6lcm, 4.16+/-0.66 cm, 4.71+/-0.76cm in B.P.H.; 3.65+/-0.46cm, 4.88+/-0.23cm, 5.98+/-0.18cm in prostatic cancer. 3. Diagnostic accuracy of transrectal ultrasonography was 93% in B.P.H. and 80% in prostatic cancer. 4. Transurethral ultrasonography enabled us to determine the degree of tumor invasion of the bladder wall and was useful to help select appropriate therapy. Transrectal ultrasonography has proved especially in estimating prostatic size and in detecting pathology.

2.A Case of Ruptured Aneurysm of the Sinus of Valsalva into the Left Ventricle with Complete AV Block in Behcet's Syndrome.

Behcet's disease is an inflammatory condition of multiple organ systems in witch recurrent oral and genital ulcers are the most typical signs. Less common clinical features include cerebral vasculitis, arterial aneurysm, deep vein phlebitis, aseptic meningitis, and discrete bowel ulcers. The most serious complication of Behcet's disease is arterial involvement especially ruptured arterial aneurysm. The aneurysm of sinus of Valsalva in Behcet's disease is a rare condition. There is no report about it in Korea. We report a case of a aneurysm of the sinus of valsalva with Behcet's disease. The patient had complete AV block and a right coronary sinus of Valsalva aneurysm which ruptured into the left ventricle. The diagnosis was made with transesophageal echocardiography. The patient was implanted with permanent pacemaker for relief of congestive heart failure due to complete atrioventricular (AV) block. He discharged and he is still follow-up in outpatient clinic.

3.A Case of Idiopathic Hypertrophic Cranial Pachymeningitis Associated with Tolosa-Hunt Syndrome.

Idiopathic hypertrophic cranial pachymeningitis (IHCP) is a rare disease, which causes chronic progressive inflammation and thickening of the basal dura mater and produces multiple cranial neuropathies, headache, ataxia, and seizure. A 40 year-old man presented with a continuous, diffuse, dull headache with left periorbital pain, exophthalmos, ptosis and loss of visual perception. Brain MRI revealed thickened, highly enhanced pachymeninges in all supratentorial areas in association with the left cavernous sinus. A subsequent meningeal biopsy showed non-specific chronic inflammations of the pachymeninges. The patient dramatically responded to steroid therapy. To our knowledge, this is an extremely rare case of IHCP associated with Tolosa-Hunt syndrome. (J Korean Neurol Assoc 19(1):56~59, 2001

4.Ruptured Lumbar Disc in Patients Undergoing Manipulation of the Lumbar Spine.

Cauda equina syndrome(CES) and disc rupture have been implicated as a potential complication of spinal manipulation. A review of the literature from 1911 to 1996 received 61 reported cases of cauda equina syndrome or disc rupture in patients undergoing manipulation. This article presents four new cases, where a temporal association was found between the onset of cauda equina symptoms or disc rupture and lumbar manipulation. The type of manipulation administered and the relationship between the treatment and symptoms is reviewed. Our patients symptoms suggest that acute herniation was caused, or at least precipitated by spinal manipulation. In each of these cases the chiropractitioner failed to comprehend the nature of the problem and take appropriate action. As a consequence, the patient went untreated for several days. Many chiropractic therapists assumed the incidence of CES caused by manipulation to be approximately one in many millions of treatments. However, we assumed that CES or disc rupture is far more common than the literature would reflect. It is therefore essential that persons practicing or prescribing manipulation would be aware of these complications.

6.A Clinical Study on the Vaginal Delivery after Previous Cesarean Birth.

OBJECTIVE: The purpose of this study was to evaluate the outcome and safety of vaginal delivery after previous cesarean birth. METHODS: This study was based on 303 cases of delivery with previous cesarean birth at Masan, Fatima Hospital from May, 1997 to April, 1998. Among them, 62 cases had performed trial of labor. We had made a comparison between elective repeat section group and trial of labor group by analizing the frequency, successful rate, maternal morbidity, perinatal morbidity and mortality. RESULTS: Among 303 cases with previous cesarean birth, trial of labor was done in 62 cases(20.5%). Among trial of labor group, vaginal delivery was done in 54 cases (87.1%) and repeat section was done in 8 cases(12.9%). Indications for elective repea section before the onset of labor were refuse trial of labor(51.9%), request for tubal ligation(17.4%), and previous section > or =2(7.5%), etc. The successful rate of vaginal delivery according to indication for previous cesarean birth was 85.0%(17/20) in the cases of dystocia and 88.1%(37/42) in the cases except dystocia. The successful rate was not influenced by the indication for previous cesarean birth(P>0.05). There were no maternal death or uterine rupture in the cases of trial of labor. There were no significant difference between elective repeat section group and trial of labor group in maternal morbidity, perinatal morbidity and mortality(P>0.05). CONCLUSION: Under strict indications, vaginal delivery subsequent to cesarean birth may be safe, and can reduce the rate of cesarean section that was increased constantly.

7.A Case of Infected Urachal Sinus.

The urachus lies between the peritoneum and transversalis fascia and extend from the anterior dome of the bladder toward the umbilicus. The urachal sinus is opened at the umbilicus superiority and not connected with the bladder. This may become symptomatic at any age with an infected discharge. We experienced a case of infected urachal sinus in a 21 years old girl, so we report this case with review of literatures.

The incidence of Giant Condyloma Acuminata of the penis is unknown but considered rare. They are similar to condyloma acuminata, histologically and grossly similar to squamous cell ca. Most authorists consider that they get the real possibility of malignant change and they had been first described by Buschke in the German literatures in l896 as a variant of condyloma acuminata. A case of malignancy of the penis which has apparently transformed from Giant Condyloma Acuminata of the penis is herein presented with review of related literatures.
