1.A Case of Proliferation Trichilemmal Cyst.

Proliferating trichilemmal cyst is a fascinating follicular neoplasm. Prior to its recognition, the tumor was misinterpreted as squamous cell carcinoma both clinically and histologically. The incidence of the lesion is rare and we can not find this case in the Korean literature. Authors have found a large, lobulated, hemorrhagic progressively growing tumor on the scalp of a 66 year-old woman. By its appearance it was hard to think the lesion was a benign tumor. Histological examination confirmed the diagnosis of proliferating trichilemmal cyst. Local excision and skin graft brought about complete healing of the lesion.

2.Clinicopathological Comparison of Papulonecrotic Tuberculid and Pityriasis Lichenoides et Varioliformis Acuta.

Papulonecrotic tuberculid (PNT) and Pityriasis lichenoides et varioliformis acuta (PLEVA) have been in controversy in their pathogeneses and entity itselves. Authors reviewed litera,tures of the two dermatoses with observation of nine casea of PNT from 1979 to 1981 and evaluated whether PNT could be also classified as PLEVA. The results were a,s follows: 1. By the review of literatures PNT and PLEVA showed great similarities in their clinical aspects including shape of cutaneous lesions, course, favorite age and subjecive symptoms. Histopathological overlappings were also found. 2. Of the nine cases of PNT authors observed, seven cases were histopathologically PLEVA and four cases could be thought PLEVA also clinically. But way six cases were treated by anti-tuberculous drugs. 3. The incidence of other tuberculous signs and results of tuberculin skin tests were all significant in the nine cases above. With the above observations it seems that PNT is one of the causes of PLEVA and thus could be a kind of PLEVA.

3.A Case of Eccrine Poroepithelioma.

A single pedunculated slowly growing tumor was found on the right thigh of a 49 year-old woman. The tumor cells were largely those of eccrine poroma but the degree of the extension of the tumor cell masses, which occupied most of the dermis, were like that of invasive basal cell epithelioma. In addition there were areas where the cells show considerable atypia and dyskeratosis. These histopathological findings were compatible with eccrine poroepithelioma first reported by Mishima and Morioka(1969). The histologic differences between eccrine poroma, eccrine poroepithelioma and eccrine porocarcinoma were discussed.

5.Effects of Ischemic Preconditioning on the Left Ventricular Function, Myosin Isozyme, Myofibrillar ATPase Activity and Ultrastructure of the Rabbit Heart.

BACKGROUND: Single or multiple episodes of brief period of ischemia and reperfusion(ischemic preconditioning, IP) have been shown to limit infarct size after a subsequent longer period of ischemia. A considerable number of possible mechanisms has been proposed, however, controversies still remain. Accordingly, we evaluated the effect of four cycles of 5 minutes ischemia and 5 minutes reperfusion(IP) followed by subsequent 30 minutes ischemia(ISCH) and 60 minutes reperfusion using isolated Langendorff-Perfused rabbit hearts. Methods and RESULTS: After a 50-minute recovery phase, parameters of the left ventricular function(LVF) including left ventricular developed pressure(LVDP), contractility and the heart rate were recorded, and ultrastructure was examined. Myosin ATPase activity was determined by measurement of inorganic phosphorus and isozymes of the myosin heavy chain were examined by polyacrylamide gel electrophoresis containing pyrophosphate buffer. The ISCH hearts showed severe to irreversible change of the cardiac myocytes homogenously in contrast to the IP hearts in which changes were not homogenous and irreversible injury was only focal. However, parameters of the LVF were not significantly different between the IP and the ISHC hearts during reperfusion. Myosin ATPase activities were also not significantly different(0.67+/-0.123 micromol/mg protein/h in the IP hearts, 0.56+/-0.172 micromol/mg protein/h in the ISCH hearts, and 0.76+/-0.239 micromol/mg protein/h in the control hearts). Band patterns of the myofibrillar proteins, separated by sodium ddodecyl sulfate-polyacrylamide gel electrophoresis, revealed no differences between the IP, ISCH and the control hearts. Myosin heavy chains in the IP and the ISCH hearts were separated into 3 isozymes, V1,V2and V3in pyrophosphate gel electrophoresis in contrast that the control hearts revealed two isozymes, V1and V2. However, there were no differences in the protein composition and electrophoretic motility between the IP and the ISCH hearts. CONCLUSION: These results indicate that IP could not attenuate the changes in LVF, myosin ATPase activity and myosin isozymes on reperfusion, however, it could attenuate the ultrastructural changes of the cardiac myocytes.

6.The Changes of Tear Flow and Tear Film Stability after Antiglaucoma Drug Instillation.

PURPOSE: To investigate the changes in tear flow and tear filim stability in patients with long-term instillation of antiglaucoma drug. METHODS AND OBJECTS: We measured Schirmer test score and tear film break-up time (BUT) in long-term antiglaucoma drug instilling 60 patients (120eyes) who had been diagnosed with open angle glaucoma and in randomly selected 60 normal subjects (120eyes) from March 2000 to February 2001 at Chosun University Hospital. Obtained results were retrospectively analyzed with the age progression and sex of subjects, duration of instillation, number and type of drugs to find out correlations between these factors. RESULTS: Average Schirmer test score and BUT were measured considerably lower in antiglaucomat drug instilled group compared to normal-subjects group (P<0.05, student t-test). Both measurements considerably decreased as age of patients and drug instillation time increased (P<0.05, ANOVA test). But number and type of instilled drugs had no considerable correlation to the obtained results (P>0.05, ANOVA test). CONCLUSIONS: It should be predicted that the patients who had instilled antiglaucoma drug for a long time and who are also of old age may develop dry eye syndrome and eye surface problem. So special care should be taken with glaucoma treatment in such cases.

8.Treatment of Infected Nonunion of the Femur with Marked Shortening by Compression and Gradual Distraction at the Nonunion Site: A Report of 2 cases.

The authors report two cases of successful reconstruction in infected nonunion of the femur involving marked shortening by compression and gradual distraction at the nonunion site using Ilizarov external fixator. At first, infection was controlled by radical excision & the administration of systemic antibiotics for four weeks and we also started compression at the nonunion site 3 days after operation. Autogenous iliac bone graft was added to strengthen the site of new bone formation lastly. In this report, the role of the compression seems to provide a good environment for distraction osteogenesis by focal necrosis and triggering inflammation. Compression and gradual distraction may be one of the treatment mordalities for nonunion of a long bone with massive bone loss or shortening, even infected.

9.Apolipoprotein E Gene Polymorphism and Plasma Lipid Levels in Male Korean Patients with Chronic Renal Failure.

Accelerated atherosclerosis is not only a frequent complication but also the most common cause of death in patients with chronic renal failure (CRF). Although mechanisms are unclear, disorder of lipid metabolism may be a major factor. Since apolipo-protein (apo) E is known to play a major regulatory role in lipid metabolism, we evaluated apo E genotype in 72 male patients with CRF and compared with that in 194 rnale normal controls. In addition, we measured plasma lipid and apolipoprotein concentrations and evaluated them according to apo E genotype in patients and controls. Apo E genotype was determined with the INNO-LiPA Apo E kit (Innogenetics, Belgium), which is based on reverse hybridization. The results are as follows ; 1) The distribution of the three major apo E alleles in patients with CRF ( e 2: 6.2%, e 3: 80.6%, e 4: 13.2%) was not different from that in controls ( e 2: 4.1%, e 3: 87.6%, e 4: 8.3%). 2) In patients with CRF, total cholesterol, lowdensity lipoprotein (LDL) and high-density lipoprotein (HDL) levels were significantly lower and the triglyceride and lipoprotein (a) levels were significantly higher than those in controls. 3) In controls, E 4/3 group had significantly lower levels of HDL than E 3/3 and E 3/2 groups. In patients with CRF, E 4/3 group had significantly higher levels of total cholesterol and apo B lipoprotein than E3/2 group. In conclusion, although there was no significant difference in the apo E genotype frequencies between male patients with CRF and controls, apo E polymorphism may play an important role in the determination of individual differences in plasma lipids in male patients with CRF.

10.Minimal Change Disease and Focal Segmental Glomerulosclerosis in Identical Twin Brothers.

The simultaneous occurrence of primary glomerulonephritis in identical twins has been rarely reported previously. It has suggested that genetic factors may play an important role in the pathogenesis of primary glomerulonephritis. We describe a pair of 17-year-old identical twin brothers with asymptomatic proteinuria, one with histologically proven minimal change disease and the other with focal segmental glomerulosclerosis. HLA typing in twin brothers revealed an identical phenotype consisting of A25, A33, B44, B54, Cwl, Cw7, DR7 and DRB1. To our knowledge, this is the first case of glomerulonephritis in identical twins in Korea.
