1.A Case of Syringoma Limited to the Vulva.

Vulva syringomas are rare in reviewing literature. In our case, a 29-year-old had multiple papular lesions involving both sides of the vulva. Microscopically the lesion showed dilated cystic sweat ducts some of which have small comma like tails resembling tadpoles. It is the purpose of this report to present a rare case of syringoma which was limited to the vulva without involvement of other sites.

2.Left Atrial Enlargement: Echocardiographic Assessment of Electrocardiographic Criteria.

A comparison betwen electrocardiographic and echocardiographic criteria of Lt. atrial enlargement was made in 40 cases who demonstrated left atrial enlargement either electrocardiographically or echocardiographically. 1) There were 20 cases with Lt. atrial enlargement by electrocardiographic criteria and echocardiographic criteria and 30 cases by echocardiographic criteria 2) Of 40 subjects, 19 cases were found to satisfy both electrocardiographic and echocardiographic critera of Lt. atrial enlargement at the same time. 3) 10 cases were consistent only with electrocardiographic criteria of Lt. atrial enlargement, but not with echocardiographic criteria 4) On the other hand, 11 cases of Lt. atrial enlargement consistent with echocardiographic criteria did not meet the electrocardiographic criteria 5) The respective predictive indices of electrocardiographic and echocardiographic criteria of Lt. atrial enlargement were: Electrocardiographic criteria; positive: 65.52%, Negative: 79.9%. 4.Supratentorial Surgery Complicated by Cerebellar Hemorrhage: Report of Three Cases.

Among the complications of intracranial surgery, postoperative hemorrhage remote from the operative site is rare, especially when it occurs in the cerebellum after surgery involving the supratentorial region. We report three cases of cerebellar hemorrhage after supratentorial surgery : 1) A 66-year-old man with subdural hygroma in both frontotemporal areas ; 2) A 56-year-old man with chronic subdural hematoma in the left frontotemporal area ; 3) A 39-year-old woman with aneurysms at the right middle cerebral artery bifurcation and basilar tip. The possible etiologies of cerebellar hemorrhage in the three cases presented here were reviewed. We speculate that when the patient is in the supine position with head rotated and extended, the displacement of the cerebellum causes stretching of the superior vermian veins and their tributaries, resulting in tearing of these vessels, and that a sudden decrease in intracranial pressure may accelerate this process. To prevent this complication, meticulous perioperative management of the patient is essential.

5.A Case of Childhood Systemic Lupus Erythematosus Showing Erythema Multiforme-like Eruption.

We present a case of childhood systemic lupus erythematosus(SLE) with renal disease in a 10-year-old girl with multiple, scattered, erythema multiforme-like eruptions on the face, trunk, forearms and lower legs. Serologic abnormalities included hypocomplementemia, high ANA & anti DNA titers, and LE cell test was positive. The histologic finding of the lesion showed leukocytoclastic vasculitis and direct immunofluorescent test revealed linear deposit of IgM at the dermoepiderrnal junction. Electron microscopy showed glomerular subepithelial depasition of eletron dense material in the kidney.

6.Ultrastructural Changes of Hair Follicles Induced by Cyclophosphamide in the Rat.

Authors studied the effects of cyciophosphamide, a potent inhibitor of nucleoprotein synthesis, to ivestigate the morphologic evidence of destructive actions to the hair. follicles. Sprague-Dawley rats were received 4 mg/kg of eyclophosphamide for 1 to 6 weeks, intraperitoneally, and examined light and electron microscopically. Light microscopically, distortion and constriction of the hair shafts, diminished diameter of the hair bulbs, and atrophy of. the hair matri.x were developed from 1 week. which were more prominent in second weeks and they were progressed after that time. Hairs were frequently fractured due to constriction of the hair shaf ts. Electron microscopically, cells of the hair pulp were decreased in number, and cells of the hair matrix were atrophied, Degenerative changes of the cellular organelles participating in grovth. and development of the hairs were noted, such as disordered formation of tricholyaline granules, diffuse atrophy and increased electron density of the inner root sheath, and loss of the glycogen and intercellular edema of the outer root sheath, but basal cells of the matrix showed minor changes relatively. From the above results, cyclophosphamide may specifically alter the matrix cells of the hair follicles and induces anagen hair losses, which may be reversible at a small dosage when the drug is discontinued, because basal cells of the matrix ahow rninor changes.

7.Tinea Corporis Occurring in One ramily Caused by Trichophyton verrucosum.

We report a case of tinea corporis occurring in one family caused by Trichophyton verrucosum. Father, mother and a son in a family had well defined scaly erythematous patches on the forearm, cheek and postauricular area. Histopathologically fungal hyphae were found in the horny layer of the epidermis in PAS stained sections. In mycological study, charact,eristic feature of T. verrucosum were found. The possible source of the infections was the cattle that they raised in their house. After eight weeks administration of ketoconazole, the lesions were healed leaving slight brownish discoloration.

10.Epidemiology of Low Back Pain.

STUDY DESIGN: The same contents of questionnaire about the low back pain(LBP) were conducted among the Korean and the Japanese hospital workers. OBJECTIVES: To compare the prevalence of LBP and the correlates of occupation and care-seeking. SUMMARY OF LITERATUR REVIEW: Previous epidemiological researches about LBP were usually performed within a country, Korean researches were for the Korean population and Japanese for the Japanese. MATERIAL & METHODS: The history, the cause, the frequency, the severity, and the method of treatment of LBP were compared between the Korean (N=336) and the Japanese (N=428) hospital workers RESULTS: The prevalence of past LBP history were higher in Korean (80%) than in Japanese case(68%), but the percentage of care-seeking (Japan: 64% & Korea: 49%) and lost workdays (9% & 7%) were inverted. The prevalence was the highest in nurse occupation (73% & 89%), and most of these cases reported the cause of LBP as work-related (72% & 71%), which was much higher than the other occupations (34% & 39%). Among the care-seeking (Japan: N=186, Korea: N=131), the percentage of hospital visit was higher in Japanese (43%) than in Korean case (24%). The other methods of care included pharmacy, massage, finger-pressure therapy, acupuncture and moxa, and these were similarly prefered by two countries. CONCLUSION: Epidemiology of LBP was compared between Japanese and Korean hospital worktors. Further study will be needed to compare the actual epidemiology of an LBP.(232 words)
