1.Effects of Inspiratory Pressure Preset on Alveolar Gas Exchange Using Anesthetic Ventilator.

The study was undertaken to determine the most adequate tidal volume when used volume preset ventilator during anesthesia. The thirty patients were received controlled mechanical ventilation with constant inspiratory pressure of 10 cmH2O and respiratory frequency of 12/minute. The results were as follows: 1) The PH was 7.39±0.01 and it is within normal limit. 2) The PaCO2 was 34.0±0.6 mmHg and it is a slightly hyperventilatory state. 3) The PaO2 was 228.0±8.2 mmHg. 4) The Buffer base was 20.7±0.3 mEq/L and it is a slightly buffer base deficient state. From the above results. We concluded that if patients were fully relaxed during general anesthesia, it is desirable to maintain the inspiratory pressure of anesthetic mechanical ventilator to 10 cmH2O for adequate alveolar ventilation.

2.Paraplegia following Chemonucleolysis A Case Report and Discussion of Neurotoxicity.

Chymopapain was discovered by Jensen in 1941, and in 1963 Smith demonstrated the chondrolytic properties of chymopapain. Since then, many patients have been injected with this agent. Although there are evidences of neurotoxicity to chymopapain in animals, adverse effects in humans have rarely been reported. We present a case delayed onset of paraplegia after chymopapain chemonucleolysis and review the neurotoxicities to chymopapain.

3.Ultrasonographic determination of fetal gender

Sonographic determination of fetal gender was attempted prospectively in most pregnancies of more than 26 weeks. We stuided 193 cases of pregnancies with ultrasound for recent 9 months from June 1984 to Feb. 1985 atdepartement of radiology, Soonchunhyang university, Soonchyunhyang chunan hospital, and analysed ultrasonographicfinding of fetal gender. The results were as follows; 1. Overall accuracy rate for fetal gender is 90%. 2.Accuracy rate for male fetus is 97.8%. 3. Accuracy rate for female fetus is 88.2%.

4.RISA (Radioiodinated Serum Albumin) Cisternographic Study in Diagnosis of the Hydrocephalus.

Cisternography is isotopic demonstration of CSF dynamics, CSF rate & pathways, by scintillation scanning of the head after intrathecal administration of RISA. We have investigated the cisternographic findings in the hydrocephalus patients in view of the diagnosis and prognostic criteria. Cisternography provides the etiology & classification of hydrocephalus and reliable indication of CSF shunt operation. We found correlation between prolonged ventricular retention, delayed migration, inadequate clearance of the isotope and favorable response to shunting.

5.Clinical Study on Psoriasis - 2 . Classification of Severity and Comparative Study by the Activity of Psoriasis.

In the study of 142 psoriatic patients who visited psoriasis clinic in Seoul Nstional University Hospital during recent five years, psoriasis was classified according to the severity based on the activity. The guideline of activity was as follows : 1. Mild: skin lesions stationary for the last one month. 2. Moderate : peripherally spreading plaque lesions with only occasionally small papules. 3. Severe : rapidly developing new lesions from the periphery of plaques or normal skin, or newly developing pustules. This study was done to evaluate the distribution and clinical features of psoriasis according to the activity. Among 142 patients of psoriasis, 84(59.2%) patients had mild psoriasis, 39(27. 4%) patients had moderate psoriasis, and 19(13.4%) patients were presented as severe psoriasis.

6.A Case of Apocrine Hidrocystoma of the Glans Penis.

Apocrine hidrocystoma is an uncommon benign tumor derived from the secretory segment of apocrine sweat glands and the cyst wall of this tumor consists of columnar to cuboidal cells with a decapitation secretion indicative of apocrine secretion. A 5-year-old boy had a translucent, flesh-colored cystic nodule, 5x2x2mm in size, on the glans penis near the urethral meatus for 4 years. Although apocrine hidrocystoma of the glans penis is very rare, characteristic histopathological features make it easy to diagnose as apocrine hidrocystoma of glans penis, which should be differentiated from the median raphe cyst of the penis.

7.The Clinical Courses of Parkinson's Disease.

The thirty-two patients of Parkinson's disease had been followed up at the Department of Neurology, Hanyang University Hospital from 1982 to 1985. The following results were obtained: 1. The age distributions at onset of illness are that the 6th decade in 41%, the 7th decade 35% in male and each of the 6th decade and 5th decade is 35% in female. 2. The mean of the duration of illness is 4.5 years and the mean duration of follow-up is 2.1 years. 3. The causes of Parkinson's diseases are idiopathic 44%, carbonmonoxide intoxication 31%, drugs 6%, and vascular disease 3% in sequence. 4. The common initial symptoms are slowness of movement 31%, tremor 31%, and rigidity 22%. 5. The slowness of movement and tremor are major observable symptoms in idiopathic origin and rigidity in carbomonoxide intoxication one. 6. The overall improvement rate on drug treatment is 75%. Various types of response fluctuation has been observed in 4 patients after 5-10 years of treatment.

8.A Case of Hyperimmunoglobulinemia E Syndrome.

The hyperimmunoglobulinemia E (Jobs) syndrome (HIES) is characterized by marked elevated levels of IgE, recurrent cutaneous and systemic staphylococcal infections, atopic-like dermatitis, and defective neutrophil chemotaxis. Three cases of HIES have been reported in Korea, but not in the dermatology literature. We report a case of HIES with cutaneous infections and MRSA (methicillin-resistant Staphylococcus aureus). A 15-month-old girl presented with intractable pruritic excoriated papular pustular skin lesions and multiple subcutaneous abscesses. Surgical drainage of the abscesses and a course of antibiotic treatment in addition to topical steroids for about 7 weeks resulted in a remarkable improvement.

9.MR Findings of Multiple Sclerosis in Spinal Cord.

PURPOSE: To analyze the MR imaging features of multiple sclerosis (MS) in spinal cord. MATERIALS AND METHODS: Nine MR (magnetic resonance) images of six patients with suspected MS were retrospectively evaluated in active phase (n=6) and inactive phase (n=3) before and after Gadolinium-DTPA administration. RESULTS: In all patients with clinically active phase, plaques of spinal cord appeared as high signal intensity on T2-weighted image with isointense cord swelling on Tl-weighted images. All lesions were enhanced on Gd-DTPA enhanced Tl-weighted images. The patterns of enhancement were nodular, circumferential rim-like, and segmental. On follow-up images in 3 patients who became clinically stable, all enhancing lesions disappeared. CONCLUSION: MR is a good modality in detection of spinal MS, and Gd-DTPA-enhanced MR is valuable in differentiating active MS from stable MS.
