2.The Surgical Treatment of Osteoporotic Vertebral Collapse Caused by Minor Trauma.

With an aging population, osteoporotic vertebral collapse is an increasingly common condition. This compression fractures has been considered a benign entity, quite responsive to conservative treatment. In a rare patients, however, a major neurologic complication and painful kyphosis despite conservative treatment can develop. Therefore, the purpose of this present study is to analyze the surgical results of 14 patients with severe back pain, an increasing kyphosis and neurologic deficits caused hy osteoporotic vertebral collapse following minor trauma, who were treated surgically. Presenting signs and symptoms included severe back pain with progression of kyphosis in 6 patients and increasing neural deficit in 8 patients. Of 14 patients, eight patients had an intravertebral cleft sign (vacuum sign). Indications for surgery included increasing kyphotic deformity, intractable pain, or increasing neurologic deficit. There was no correlation between intravertebral cleft sign and neurologic deficit. However, patients who had intravertebral cleft sign had not well respond to conservative treatment. As treatments, combined anterior and posterior fusion in 8, anterior fusion in 4, posterior instrumentation, and wide decompressive laminectomry in one patient, respectively, were carried out. The final correction of the deformity averaged 0.3 degrees. Therefore. correction of kyphosis was not favorably maintained because of variable surgical methods, and sinking of graft bone or instrumentation into the osteoporotic vertebral bodies. However, pain was reduced significantly in all patients. In addition neurological symptoms improved in 7 patients. One patient underwent reoperation with nnterior inierbody tusion together with anterior instrument because of an increasing kyphosis, neurologic. iymptoms and scvcre hack pain following wide decompressive laminectomy. There was no complication relatecl to instruments. The authors strongly helieved that surgical intervention has highly satisfactory results in patients who have intravertehral cleft sign with persistent back pain despite conservative treatment, and proressive or persistent neurologic deficits following osteoporotic vertebral collapse.

Mucocele of the appendix is uncomrnon and rarely diagnosed preoperatively. The malignant rountpart-i.e. mucinous cystadenocarcinoma--has the same grross appearance and many micro scopic features in cornmon wilhe the benign form. It rnay be associated with ovarian mucinous cystadenoma of strikingly similar microscopic appearence. A serious complication is a rupture of the mucoeele resulting in pseudomyxoma peritonei. Each case of benign and malignant mucocelr of the appendix falsely diagnosecl as an oovarian tumor before laparotxumy are presented wilh a brief rieview of the literatures.

4.Reference Values for the Ambulatory Blood Pressure in Normotensive Korean Young Men.

BACKGROUND: To obtain reference values for the ambulatory blood pressure, 30 normotensive Korean young males, aged 22-28 years(mean age: 25 years) were investigated. METHODS: The ambulatory blood pressure was recorded with SpaceLabs Model 90207 monitor over 24 hours at 20 minutes intervals during day-time(6 am-10pm) and at 30 minutes intervals during night-time (10 pm-6 am). RESULTS: The ambulatory blood pressure of the 30 subjects averaged 119/74mmHg over 24 hours, 122/78 mmHg during day-time and 112/68 mmHg during night-time. The upper limit of normal ambulatory blood pressure was suggested as the value of mean+/-2SD which revealed 131/84mmHg for 24 hours, 134/90 mmHg for day-time and 126/80 mmHg for night-time. CONCLUSION: Reference values for the ambulatory blood pressure, in normotensive Korean young men, were obtained by SpaceLab model 90207 monitor.

5.A case of Ovarian Metastasis from Carcinoma of the Gallbladder: a rare Krukenberg Tumor.

A wide variety of cancers metastasize to the ovaries. In a majority of instances the primary site is the gastrointestinal tract, breast, or other gynecologic organs. The best known tumor of this type is signet-ring cell adenocarcinoma. The gallbladder and bile duct are rare sources of these metastases. The authors have had an experience of a case that was presented of Krukenberg tumor metastatic from the gallbladder and report the case with brief review of literature.

6.Attenuated Ischemic Injury to Human Liver during Hepatectomy by Simple In Situ Hypothermia.

BACKGROUND/AIMS: To assess the preventive effect of simple in situ cooling on ischemic injury in human livers. METHODS: The study consisted of 22 patients who were to undergo liver resection (right lobectomy, n=6, left lobectomy, n=3, posterior segmentectomy, n=1, and extended cholecystectomy, n=1, in each group). All but 2 had normal remnant livers and all patients were randomised to undergo either warm ischemia or in situ cooling (n=11 in each group). RESULTS: The mean (SD) occlusion time was 53.7 (5.8) minutes for the warm ischemia group and 52.7(3.1) for the in situ cooling group. After in situ cooling, the state of the liver as indicated by serum alanine aminotransferase activity (ALT) and prothrombin time had improved substantially. Mean (SD) ALT activity was 483 (192) U/L in the warm ischemia group compared with 288 (157) in the in situ cooling group (p<0.02) on the first postoperative day. The respective figures for prothrombin time (%) were 58(23) compared with 77 (13), (p < 0.05). CONCLUSION: In situ cooling lessened the amount of ischemic damage done to the liver during hepatectomy compared to treatment with that of warm ischemia.

7.Intraosseous Epidermal Cyst of the Distal Phalanx: A Case Report.

An intraosseous epidermal cyst is a rare benign cystic lesion. It is thought to result from congenital factors or trauma and can lead to bone destruction because the cyst develops at the soft tissue around the bone. Radiological findings of intraosseous epidermal cysts are a well-defined radiolucent lesion, with cortical expansion. It is important to differentiate an intraosseous epidermal cyst with other disease developed at distal phalanx because its clinical and radiological findings are similar. We report two rare cases of intraosseous epidermal cysts that developed at the distal phalanx.

8.The Effects of Tracheal Intubation with McCoy or Macintosh Laryngoscope on Blood Pressure, Heart Rate.

BACKGROUND: The aim of this study was to compare the cardiovascular changes followed by laryngoscopy with the McCoy laryngoscope blade with those followed by laryngoscopy with the Macintosh laryngoscope blade. METHODS: Forty eight patients were randomly divided into two groups. Following induction with fentanyl 2 mcg/kg and thiopental 5 mg/kg, and muscle relaxation with vecuronium 0.1 mg/kg, the vocal cords were visualized with either the McCoy or the Macintosh laryngoscope blade, then tracheal intubation was performed. Heart rate and arterial blood pressure were measured just before and after laryngoscopy, and 1, 3 and 5 min later. RESULTS: There was a significant increase in both heart rate and arterial blood pressure after tracheal intubation using the Macintosh laryngoscope. Also, use of the McCoy blade resulted in a significant increase in both heart rate and arterial blood pressure. CONCLUSIONS: There was no significant difference on arterial pressure and heart rate to laryngoscopy and tracheal intubation with either the McCoy blade or the Macintosh.

9.A Case of the Forme Fruste Choledochal Cyst.

Forme fruste choledochal cyst (FFCC) is one of the diverse types of choledochal cyst with little or no dilatation of the extrahepatic bile duct (EHBD). It is considered that FFCC has to do with the pancreatobiliary malunion (PBM). In children, 3 to 6 millimeters of EHBD is assumed to be normal. Even though there is no clear-cut definition, FFCC is likely to be associated with bile duct dilatation less than 10 millimeters. Almost all cases have PBMs and symptoms of the pancreatitis or cholangitis. We experienced a case of FFCC in a 4-year-old boy. His EHBD measures 10mm diameter. He had symptoms of pancreatitis and elevated hepatic transaminases. The pancreatobiliary common channel was 28 millimeters. He underwent EHBD resection and Roux-en-Y hepaticojejunostomy and was discharged with no specific complications.

10.Two Cases of Clothing Dermatitis.
