1.A Study Using Diffusion-Weighted MR Image in the Experimental Models with Diffusion Difference.

PURPOSE: To see the stability and error in the diffusion-weighted magnetic resonance (MR) imaging technique in the experimental models and to observe the signal intensities in the early cerebral lesions of the animal models. MATERIALS AND METHODS: Diffusion coefficients of acetone and distilled water were measured by diffusion-weighted MR image and were compared with actual values. Differentiation of diffusion from perfusion were done at the resin flow phantom. The signal intensities caused by early parenchymal changes were measured in normal, hypovolemic, and embolic, and dead animal models by using diffusion-weighted image and compared with pathoIogic finding and vital staining. RESULTS: Diffusion coefficients of acetone and distilled water were 4.48 x 10-3 and 2.72 x 10-3 which were very close to the actual values. Diffusion-weighted MR image obtained at flow phantom was not affected by flow (perfusion) at the 100-400 of b-factor range. Animal study done at that b-factor range revealed a significant signal difference between the left and right sides only at the embolic model induced by polyvinyl alchol particles (p<0.05). These changes were not detected in microscopic finding but could be identified in vital staining. CONCLUSION: Diffusion-weighted MR image can be used to detect early parenchymal change when the appropriate b-factor range was applied.

2.Laryngeal Schwannoma: A Case Report.

Laryngeal schwannoma is extremely rare. We report the CT and MRI findings of a case occurring in a 65-year-oldwoman, and describe the pathologic correlation. Pre-contrast CT scanning revealed a right supraglot-tic mass witha slightly hyperdense central part and a hypodense peripheral part. Post-contrast CT scanning re-vealed anenhanced hyperdense central part and a rim-like hypodense peripheral part. The density of the pe-ripheral part waslower than that of muscle. The mass showed homogeneous low signal intensity on T1-weighted MR images, homogeneoushigh signal intensity on T2-weighted MR images, and an enhanced high signal intensity central part and a lowsignal intensity peripheral part on gadolinium enhanced T1-weighted images. The enhanced central part correlatedwith Antoni A areas and the peripheral part, showing low attenuation, correlated with Antoni B areas.

3.A Clinical Analysis of Traumatic Diaphragmatic Rupture.

A clinical analysis was made of 18 cases of traumatic diaphragmatic rupture which had been treated during the 8 years from January 1989 to December 1997 at the Department of Surgery & Chest Surgery, Dae Rim Saint Mary's Hospital. The results are as follows: 1) The most common ages were 3rd and 4th decades (61.1%).There were 14 males and 4 females (M : F = 3.5 : 1) 2) The traumatic diaphragmatic ruptures were due to blunt trauma in 13 (72.2%) patients (left 10, right 3) and penetrating trauma in 5 (27.8%) patients (left 5). 3) The common symptoms were dyspnea (77.8%), abdominal pain (66.7%), and chest pain (55.6%). 4) 12 of the 18 patients were diagnosed before operation. Six patients were diagnosed during operation. 5) 15 of the 18 patients were operated on within 24 hours (83.3%). 6) The most common associated intraabdominal injured organ was spleen (44.4%). The most common herniated organ was colon (27.8%). 7) Most of the injuries ranged in size from 6 to 10 cm in the blunt trauma patients (53.8%) and less than 5 cm in penetrating trauma patients (100%). 8) The operations were done using a laparotomy alone in 11 patients, a thoracotomy in 5 patients, and a thoracoabdominal incision in 2 patients. 9) Postoperative complications were developed in 8 patients (44.4%). 10) The perioperative mortality was 11.1%, and the causes of deaths were hypovolemic shock and septic shock. In conculusion, traumatic diaphragmatic rupture without severe associated organ injury could reveal a good outcome, if early diagnosis and proper treatment was performed.

Background:
Peripheral nerve injury is a complication that can occur after general anesthesia. It significantly impairs the patient’s quality of life and may lead to permanent disability. Nerves in several areas can be damaged during the perioperative period, but it is very rare that numbness of the lower lip is caused after general anesthesia. Case: A 73-year-old man with diabetes mellitus underwent urological surgery under general anesthesia. The day after surgery, he complained of numbness on the right lower lip caused by a mental nerve injury. Diabetic mononeuropathy or neurapraxia related to mechanical compression was considered a possible cause. The symptoms resolved spontaneously after six weeks.
Conclusions
Mental nerve injury is a rare perioperative complication in surgical patients under general anesthesia. In this case, patients should be reassured and advised to avoid injuries to the mouth and lips. However, specific treatment is not required.

6.Quisqualis indica extract ameliorates low urinary tract symptoms in testosterone propionate-induced benign prostatic hyperplasia rats

Benign prostate hyperplasia (BPH) is a common disease in old-age males, accounting for approximately 77% of morbidity within the age range of 40 to 70 years. It has been shown that morbidity increases with social graying. Quisqualis indica linn (QI) has been used to treat inflammation, stomach pain, and digestion problems. In this study, we evaluated the symptom-regulating effects of QI extract on a testosterone-induced BPH rat model. After inducing BPH in rats using testosterone propionate (TP) injection, we assessed basal intraurethral pressure (IUP) and increments of IUP elicited by electrical field stimulation (5 V, 5, 10, or 20 Hz) or phenylephrine (Phe) (0.01, 0.03, 0.1 mg/kg IV). To induce BPH, 8-week-old rats were subjected to a daily subcutaneous TP (3 mg/kg) injection for 4 weeks. Finasteride (Fina) (10 mg/kg PO) was administered to the rats in the first treatment, while QI (150 mg/kg PO) was administered to those in the second group. Blood pressure was measured together with IUP, after which low urinary tract (LUT), ventral prostate (VP), testicle, and corpus spongiosum were isolated and weighed. Basal IUPs for the Fina- and QI-treated groups were 87.6 and 86.8%, respectively.LUT and VP organ weights in the QI group were lower than those in the Fina group. However, the QI group showed significantly reduced electrical stimulated or Phe-induced IUP increment compared to the Fina and BPH groups. These results proved that QI can be beneficial for BPH symptoms by inhibiting 5α-reductase and consequently decreasing prostate and releasing urinary pressure.
7.A Case of a Gastric Submucosal Lymphatic Cyst Associated with Early Gastric Cancer.

A lymphatic cyst of the stomach is rare benign tumor which is composed of thin-walled lymphatic space lined by benign-appearing endothelial cells. It rarely manifests clinical symptoms, so it is found incidentally in most cases. Recently, a case of submucosal lymphatic cyst associated with early gastric cancer was experienced. An endoscopy revealed a slightly elevated, edematous lesion adjacent to the malignant ulcer. This case is herein reported with a brief review of related literature.

8.In Vitro IL-1, TNF and IL-6 Production of Peripheral Blood Mononuclear Cells Stimulated with Different Hemodiaysis Membranes.

OBJECTIVES: In order to evaluate the role of these cytokines in biological response induced by blood interaction with hemodialysis membranes. METHODS: We have investigated the IL-1, TNF and IL-6 concentrations in the supernatant of 24-hours cultured peripheral blood mononuclear cells (PBMC) without(spontaneous group) or with broken cuprophan or P1VMA membranes in 9 chronic hemodialyzed patients and 8 healthy controls. The blood samples were drawn before dialysis using following criteria: (a) in last dialytic treatment with PMMA membranes(HDEl), (b) after two weeks of dialytic treatment wih cuprophan membranes(HDE2). RESULTS: In the both of patient group(HDE1 and HDE2) and controls production of IL-l, TNF and IL-6 of PBMC stimulated with cuprophan or PMMA membrane particles was increased compared to those of spontaneous group. IL-1 production of HDE1 stimulated PMMA(99.31 +/- 30.15fmol/ml) was significantly higher compared to that of cuprophan(48.43 +/- 11.29fmol/ml), TNF production of HDE2 with cuprophan(114.86 +/- 38.5lfmoVml) was significantly high compared to that of spontaneous group(52.42 +/- 29.94fmol/ml). IL-6 production of HDE2(646.70 +/- 103.84fmol/ml) was significantly high compared to that of spontaneous group(385.88 +/- 87.03fmoVml). Comparing cytokine production of PBMC, there was a significant correlation between IL-1 and IL-6(r=0.78), IL-1 and TNF(r=0.78) and TNF and IL-6(r=0,76). CONCLUSION: Our results show that the interaction of cuprophan or PMMA membranes with blood increase the production of IL-1, TNF and IL-6. We suggest that in patients undergoing routine hemodialysis PBMC are primed by exposure to chronic stimulation.

9.A Case of Intrathoracic Kidney in an Adult.

Intrathoracic kidney is a very rare congenital anomaly in development of the kidney. It was firstly described by Mikulics in 1922. More than 130 cases were reported in the world. Most of the patients have no clinical problem and discovered incidentally in chest X-ray. We have experienced a case of intrathoracic kidney in a 48-year-old female who was admitted due to recurrent calculous cholecystitis. Intrathoracic mass lesion was noted in the plain chest film and confirmed as intrathoracic kidney by chest computed tomography and intravenous pyleography. We report a case of intrathorcic kidney with brief review of literature.

10.Effects of Ovariectomy and Estrogen Replacement on Rat Bladder Physiology and Morphology.

PURPOSES: An overactive bladder is highly prevalent in middle-aged woman, especially during the postmenopausal period. We evaluated the relationship between detrusor overactivity and postmenopausal state and the effects of estrogen replacement for detrusor overactivity caused by the ovariectomy in rat. MATERIAL AND METHODS: Thirty female Sprague Dawley rats were divided into three groups: the control, the ovariectomy group and the estrogen replacement group after the ovariectomy. The ovariectomy and estrogen replacement groups had the bilateral ovariectomies at 12 weeks of age. After 2 weeks, the control and ovariectomy groups were injected weekly with saline, while the estrogen replacement group was injected with estradiol benzoate (500 migrogram/kg) weekly. After 12 weeks, the voiding frequency study was recorded and a cystometrogram was performed while awake. The harvested bladders were used in the carbachol-induced detrusor muscle contraction study and the distribution of estrogen, M2 and M3 muscarinic receptors in the rats' bladder. RESULTS: The ovariectomy group voided more frequently than the others (p=0.005). During the awake cystometrogram, the detrusor characteristics of the ovariectomy group were a higher mean intervoiding pressure and smaller bladder capacity than the others (p=0.000). There was no significant statistical difference between the control and estrogen replacement group. The other cystometric parameters were not statistically different either. The detrusor muscle contraction study showed no difference between three groups. There was a significant difference in the distribution of M2 and M3 receptors of bladder mucosa between the control and ovariectomy group. CONCLUSIONS: This study suggested that the detrusor overactivity resulted from increased connective tissue ratio and decreased M2, M3 receptor in the bladder mucosa. Moreover, estrogen replacement in the postmenopausal state had the effect of reversing the physiological and morphological changes caused by an estrogen deficiency in the bladder.
