BACKGROUND: Recent studies have shown an incomplete agreement with the previously proposed diagnostic criteria of Hanifin and Rajka for atopic dermatitis(AD). This could be due to ethnic differences in the clinical manifestations of AD. To make a diagnosis of AD with confidence, one has to depend on the history and clinical picture according to race. OBJECTIVES: The purpose of the present study was to find out the importance of individual symptoms and histories for the diagnosis of Korean AD by questionnaire. METHODS: The frequency of 25 questionnaire items was studied in 130 patients with typical AD (48 childhood, 82 adolescence/adult) compared to that in 198 control subjects (89 childhood, 109 adolescence/adult). RESULTS: Nine items(itchy with bodily heat, itchy when sweating, cholinergic urticaria, miliaria, irritation with fabrics, food reaction, pollen allergy, itch with stress) were shown to be of much diagnostic significance(p<0.001) in childhood AD,otherwise thirteen items(itchy with bodily heat, itchy when sweating, cholinergic urticaria, sense of fever, excessive sweating, miliaria, irritation with cosmetics, irritation with fabrics, irritation prone hands, itchy with woolen clothes, pollen allergy, itch with stress, dermographism) were in adolescent and adult, AD. CONCLUSION: We found some diagnostic significance of clinical histories in Korean AD patients by questionnaire. We propose that this accessible questionnaire would be an efficient and powerful method with which to obtain new and valid information on AD.

Although cystic teratoma is the most common benign tumor of the ovary, the association of sebaceous gland tumor with cystic teratoma is rare. We have recently experienced a case of sebaceous gland tumor, arising in the cystic teratoma of the ovary in a 78-year-old Korean woman. Histologically, the tumor was characterized by an organoid lobular architectures of the sebaceous glands which are exculsively composed of germinative and mature sebaceous cells. Although it is difficult to come to a valid conclusion due to the presence of atypical mitosis and necrosis, this tumor was regarded as benign from the viewpoint of preserved organoid structures, and absence of capsular invasion or metastasis.

Becker's nevus has been known as a sudden onset of hyperpigmentation on shoulder or arm in the second decade of life with slow peripheral extension and later development of hypertrichosis. We observed two cases of typical Becker's nevus which was confirmed by characteristicclinical and histopathological findings. One was 45-year-old male who has a patch of hyperpigmentation and hypertrichosis on left upper arm at birth, the other was 2()-year-old male who developed on right shoulder after sunburn which is a salient feature by Becker's initial description.

The posterior cruciate ligament in the stoutest ligamentous structure in the knee joint. Kennedy and Hawkins have confirmed its strenght to be about 2 times that of the anterior cruciate ligament or tibial collateral ligament. This would appear to offer the support that this ligament has a vital role to play in knee stabilization. But, in the past, non-surgical management was frequently elected either because of the belief that most kness with posterior cruciate insufficiency are asymptomatic or because it is difficult to access surgically. Recently, Hughston and Trickey reported good results after surgical repair of the rupture of the posterior cruciate ligament. And current trend is to repair it surgically. So, we analyzed the 36 cases of acute posterior cruciate ligament injuries, who were admitted and treated surgically in Severance Hospital between Jan. 1981 to Dec. 1985, and obtained the following results. 1. Single rupture of the posterior cruciate ligament largely results in good outcome. 2. When the rupture site is in the substane, we can obtain relatively good results with the augmentation using the medial head of the gastrocnemius. 3. When combined with medial or lateral compartment injuries, the chance of later instability and reconstruction is high.

PURPOSE: Over the past several decades, the survival rates of low birth weight(LBW; birth weight <2,500 g) infants have been improving. Many of these infants have had bronchial asthma(BA) at childhood and aldolescence. The aim of this study is to investigate whether LBW influenced the severity of BA in prealdolescent children and aldolescence by a retrospective case control study. METHODS: 15 LBW children(1,500 gDae-dong hospital as the case group were compared with 2 BA children to each case as the control group(total 30 children) on the basis of following criteria:the same age, he/she attended Dae-Dong hospital or local medical center after 8 years old, birth weight was more than 2,500 g. Asthma severity was modified from 2003 Guideline for management of childhood asthma at The Korean Academy of Pediatric Allergy and Respiratory Disease. PFT(Pulmonary function test) were measured using a portable spirometer. The value are expressed as percentage predicted using standard reference values. Total IgE and specific IgE Ab. were assessed by the PRIST method and MAST Allergy Test. Methacholine challenge test and PC20 was assessed by the Masterlab. RESULTS: LBW group had:a significantly more severe symptoms between 8 year-old and 18 year-old than control group.(P=0.015) A significantly more used to ICSs(Inhaled corticosteroids) and add-on therapy than control group.(P=0.009)(P=0.0001) There were no significant differences in the PFT and PC20 and specific IgE Ab. between LBW group and control group. But, total IgE level is higher in the control group than in the LBW group. CONCLUSION: These results suggest that LBW is a risk factor for the severity of BA. We suggest that infants with birth weight of less than 2,500 g have a more severe risk of asthma during prealdolescent children and aldolescence than who were birth weight of more than 2,500 g.
