2.Computerization of Microscope Slide Labelling.

The automation in surgical pathology, particularly in the reporting and encoding system using personal computers has been greatly improved in recent years, but the computerization of microscope slide labelling has not been improved. The authors have developed database program for slide labelling using FoxPro 2.5 and FoxBASE SCOUNIX 2.1.2: For I year during trial an effect has been put forward to simplify and organize the labelling work in routine surgical pathology. The program is now become easily applicable to the labelling work without disturbing the normal flow in a pathology laboratory. It is possible to get information concerning how many paraffin blocks and H&E slides have been made, as well as what kind of special stains have been requested for each case. The authors think that the computerization of labelling work in routine surgical pathology is a fairly easy task, and this should simplify the labelling work at a lower cost, diminish the workload of a typist or technician, and indirect information concerning the workload in a pathology laboratory can be calculated.

3.A Case of Granuloma Gluteale Infantum.

Granuloma gluteale infantum is a granulomatous lesion in the diaper area of infants. Athough usually seen in infants, it has been described in incontinent elderly patients. The causes of granuloma gluteale infantum are not clear, although child care equipment(ditergents1, diapers2, and starch3), corticosteroids2,4,5,6, candidial infection3 urine and feces7 have been postulated for its etiology. We report a case of granuloma gluteale infantum in a 5-month-old boy. He was healthy except for an asymtomatic 1.5×1.5cm sized, round, Rat-topped brownish and firm nodule on the perianal area, and two, 1×3cm and 1×6cm sized brownish plaques on the intertriginous area of the neck. The lesions were resistant to the treatment of systemic antibiotics. These nodules completely resolved in the three months after we tried to keep the diaper area clean without using any corticosteroids or other ointments and after changing from plastic diapers to cotton o A histological examination of the nodule on the perianal area showed mild hyperkeratosis, spongiosis, and focal infiltration of neutrophils in the epidermis. 4.Analysis of Surgical Blood Use in Operation at YUMC.

This study was performed to guide the effective utilization of blood and optimal blood ordering schedule for various elective surgeries, based on the analysis of 1,462 transfused surgical procedures and 5,933 blood units transfused during operation in the period of two years through January, 197 to December, 1988 at Yeungnam University Hospital. The frequency of transfusion, and mean transfused units were evaluated and recommended blood unit for each surgical procedure was proposed. We assure that the successful establishment of this guideline can lead to substantial monetary saving, reduced blood outdating, and a decreased blood bank workload with a more appropriate allocation of the technician's time and effort.

6.Significance of Low Serum Urea Nitrogen Concentrations.

The prevalence of low serum urea nitrogen concentrations (<6 mg/dL) in our patient population was 3.8% (452 per 11,911 determinations), representing 372 individual cases. Of these, 323 of the patients' charts were located, reviewed, and classified into disease groups. Malignant tumor was found in 25.1% of the 323 patients, the remaining patients without malignant tumor had various clinical conditions; pregnancy (19.5%), infectious diseases (12.1%), trauma (10.2%), cerebrovascular accident (6.2%), hepatopathy (5.3%), and miscellaneous conditions (21.6%). The prevalence in female and young aged patients under 5 years was considered due to the lower reference range in these groups than in that of female adult groups, and moreover pregnancy in another superimposing factor in the former group. In regards to the causes of this condition, our study corresponded well to the textbook cited etiology of the low serum nitrogen concentration, although malnutrition and hepatopathy were inverted in their incidental order. We postulated, through the present study, that the low serum urea nitrogen concentration is not pathognomonic findings to suggest specific disease or conditions, but rather to reflect patient's general conditions such as hydration, nutrition, or administration of certain drugs.

7.The Diagnostic Significance of AgNORs and MIB-1 Labelling Index in Atypical Meningioma.

There is no definite histological criteria which can predict the biologic behavior of meningiomas, although resectability is the most important factor in terms of recurrence. For grading meningiomas, various factors have been studied, such as hypercellularity, nuclear pleomorphism, small cells with high N/C ratio, prominent nucleoli (PN), frequent mitosis, loss of architecture, focal necrosis (FN). We investigated 116 meningiomas to evaluate the correlation between the factors and the proliferative activity using AgNORs and MIB-1 labelling index (LI). They were divided into 3 groups: Group A includes meningiomas with none of the factors; group B with one of the factors; group C with two or more factors. MIB-1 LI was correlated with each factor, but AgNORs was not. There was a statistical difference among group A (<1.28%), B (2.7%) and C (5.1%) (p<0.05) using MIB-1 LI. FN was the most frequently associated with other factors, and it had the highest MIB-1 LI (6.31%). MIB-1 LI of group B was 5.1 2.3%. In group B, the most frequent combination was FN and PN, and it showed the highest MIB-1 LI (5.74%). This study indicates that FN and PN are important for diagnosis of atypical meningioma, and MIB-1 LI appears to be a useful method for estimating the proliferative activity of meningiomas, and 5% or more of MIB-1 LI could help in making a diagnosis of atypical meningioma.

8.Diagnostic Significance of Clinical History of Atopic Dermatitis in Koreans by Questionnaire.

BACKGROUND: Recent studies have shown an incomplete agreement with the previously proposed diagnostic criteria of Hanifin and Rajka for atopic dermatitis(AD). This could be due to ethnic differences in the clinical manifestations of AD. To make a diagnosis of AD with confidence, one has to depend on the history and clinical picture according to race. OBJECTIVES: The purpose of the present study was to find out the importance of individual symptoms and histories for the diagnosis of Korean AD by questionnaire. METHODS: The frequency of 25 questionnaire items was studied in 130 patients with typical AD (48 childhood, 82 adolescence/adult) compared to that in 198 control subjects (89 childhood, 109 adolescence/adult). RESULTS: Nine items(itchy with bodily heat, itchy when sweating, cholinergic urticaria, miliaria, irritation with fabrics, food reaction, pollen allergy, itch with stress) were shown to be of much diagnostic significance(p<0.001) in childhood AD,otherwise thirteen items(itchy with bodily heat, itchy when sweating, cholinergic urticaria, sense of fever, excessive sweating, miliaria, irritation with cosmetics, irritation with fabrics, irritation prone hands, itchy with woolen clothes, pollen allergy, itch with stress, dermographism) were in adolescent and adult, AD. CONCLUSION: We found some diagnostic significance of clinical histories in Korean AD patients by questionnaire. We propose that this accessible questionnaire would be an efficient and powerful method with which to obtain new and valid information on AD.

9.Sebaceous Gland Tumor Arising in Cystic Teratoma of the Ovary: An unique form of monodermal teratoma.

Although cystic teratoma is the most common benign tumor of the ovary, the association of sebaceous gland tumor with cystic teratoma is rare. We have recently experienced a case of sebaceous gland tumor, arising in the cystic teratoma of the ovary in a 78-year-old Korean woman. Histologically, the tumor was characterized by an organoid lobular architectures of the sebaceous glands which are exculsively composed of germinative and mature sebaceous cells. Although it is difficult to come to a valid conclusion due to the presence of atypical mitosis and necrosis, this tumor was regarded as benign from the viewpoint of preserved organoid structures, and absence of capsular invasion or metastasis.

10.A Case of Mycosis Fungoides following Multiple Open Fractures.

We report a case of mycosis fungoides in a 30 year-old man who had been treated for multiple open fractures after a traffic accident. Histopathologic findings showed epidermotrophism wiih hyperchromatic haloed lymphocytes and atypical lymphocytic infiltration in the upper dermis. The infiltrated cells were positively stained with CD45Ro, but not with CD20. He had no evidence of extracutaneous involvement by laboratory studies, Skin lesions improved markedly after treatment with systemic PUVA therapy combined with topical corticosteroids for 3 months.
