2.A Case of Sick Sinus Syndrome Derived in Elderly Patient on Hemodialysis.

Sick sinus syndrome comprises a variety of conditions involving sinus node dysfunction and commonly affects elderly persons. Though sudden cardiac death accounts for as much as 15% of all cause mortality in uremia, requiring permanent cardiac pacing in end-stage renal disease hemodialysed patients are very few. Most cases of sick sinus syndrome are idiopathic, and the cause can be multifac- torial. A-V conducton defects were attributed to extensive metastatic calcifications, involving the cardiac skeleton, conservative to severe hyperparathyroidism and inadvertent use of calcitriol and calcium carbonate as phosphate binders. If optimal hemodialysis is provided, benefits of permanent pacing are equal in uremic or non uremic patients and pacemaker implantation should be instituted. We report a case of sick sinus syndrome derived in elderly patient on hemodialysis.

3.A Study of Ni-resistant bacteria isolated from gingival crevicular fluid on the patients wearing Ni-Cr alloy prosthesis (In terms of molecular biological aspects).

As a material of metal-ceramic prosthesis, nickel as a form of Ni-Cr alloy has been used for many dental prostheses in many cases. However, several problems in use of the alloy have been revealed (ex ; tissue stimulation, skin allergy, hypersensitivity, cytotoxicity and carcinogenecity). Little is known about nickel with respect to the relationship between Ni-prosthesis and gaining of Ni-resistance in oral microorganisms. The present study was undertaken to check wheather use of Ni-prosthesis leads to occurrence of Ni-resistant microorganisms. So this study may suggest the possible relationships between the oral microorganisms and nickel-resistance in oral enviroment. Bacteria were isolated from the gingival crevicular fluid on the patients wearing Ni-Cr prosthesis. The isolated bacteria were tested for their Ni-resistance in nickel containing media at different concentration from 3mM to 110mM. E. coli HB101 was used as control. The Ni-resistant bacteria were isolated and biochemically identified. The Ni-resistant bacteria were tested several biochemical, molecular-biological tests. Performed tests were ; measuring the growth curve, antibiotic test, growth ability test in liquid media, isolation of the chromosome and plasmid, digestion of DNA by restriction enzyme, electrophoresis of chromosome and plasmid DNA, identification of Ni-resistant genes by the DNA hybridization. The results were as follows: 1) The bacteria isolated from gingival crevicular fluid on the patients wearing Ni-Cr alloy prosthesis showed nickel-resistance. 2) The isolated microorganisms grew at nickel containing media of high concentrations (60mM-110mM). 3) Based on the biochemical tests, the isolated microorganisms were identified as Enterococcus faecalis(13 cases), Klebsiella pneumoniae(1 case) and Enterobacter gergoviae(1 case). 4) Enterococcus faecalis expressed not only nickel resistance but also the multi-drug resistanceto several antibiotics ; chloramphenicol, kanamicin, streptomycin, lincomycin, clindamycin. However all strain showed the sensitivity against the tetracycline. 5) DNA hybridization result suggest that there is no homology between the previousely known gene of nickel resistance in Klebsiella pneumoniae and chromosomal DNA Enterococcus faecalis.

4.ADHESION OF CANDIDA ALBICANS ISOLATES TO ACRYLIC RESIN IN RELATION TO SALIVARY GLYCOPROTEINS IN DENTURE STOMATITIS PATIENTS.

Adherence of Candida albicans(C. albicans) to the surface of a denture is believed to be an initial and essential step in the formation of denture-induced stomatitis. Previous studies have provided enormous infomation on the relationship between composition of palatine gland/parotid saliva and upper denture stomatitis. Relatively little information is available on the correlation between lower denture stomatitis and sublingual-submandibular(SLSM) saliva. The plaque samples were collected from the two sites(100mm2) on the inner surface of lower partial denture corresponding to the stomatitis and healthy region of the lower partial dentures of 12 denture stomatitis patients and 6 normal persons who wore lower partial dentures. The samples were plated to isolate C. albicans on a selective Saboraud's dextrose agar plate and the isolates were identified by germ tube test and gram staining. The subjects were divided into group I (stomatitis with C. albican), group II (lesion without C. albicans), group III (no lesion but C. albicans), and group IV (normal and healthy denture wearer). Individual SLSM saliva (20microgram of protein) was analyzed by SDS-PAGE(SDS-poly-acrylamide gel electrophoresis) with Coomassie brilliant blue and PAS(Periodic Acid Schiff) staining. The salivary proteins separated in the polyacryamide gels were subjected to immunoblot analysis using anti-lactoferrin, anti-sIgA, and anti-secretory component of sIgA. In this study using custom made acrylic denture resin beads(5mm in diameter) coated with stimulated individual SLSM saliva, the binding ability of individual C. albicans strains to the beads was observed. Levels of C. albicans adhered to the acrylic resin beads were determined by measuring the optical density of the bound C. albicans to the beads at 580nm. The results showed that a higher number of C. albicans was observed in the lesion site than health site. The saliva of group I contained more high molecular weight glycoprotein(mucin, MG1) as compared to group II, III, and IV. And lactoferrin and sIgA affected to the binding ability of C. albicans to acylic resin beads. Binding ability of individual C. albicans to the acrylic resin coated with respective individual saliva was found to be greater in group I than the other 3 groups. And when bound cells of C. albicans isolated from individual subject #2 to the saliva coated beads were used, binding ability of subject #2 saliva coated beads was founed to be greater than the other subjects. These results suggested that denture induced stomatitis is related to individual patient's salivary protein composition, especially MG-1. Future studies will be directed toward saliva examination of patients who have general disease and analysis of pellicles formed on prosthesis with respect to oral disease.

5.Statistics on Outpatients, Inpatients and Operations in Urology Department of the University of Kyoung Pook (1958-1960).

Clinical and statistical observation was made on outpatients, inpatients and operations in Urology Department of the University of Kyoung Pook during the period from Jan. 1958 to Dec. 1960 and the results obtained were as follows: I) Clincal Statistics on Outpatients: 1) Total out-patients were 1.151 cases of which total. 1,211 were males and 240 were female. and the male to female ratio was 5:1.2) The period of outpatient attendance was peak from May to August.3) According to age grouping, the maximum attendance was from 21-3O years group, next 31-40 group then 11-20 group, and 21-30 years group formed 1/3 of total patients.4) The most frequent diseases were tuberculosis of urinary tract in 9.8% showing annual decrease according to number and percentage, cystitis in 6.5%, urolithiasis, hydrocele and prostatic tumor which showed annual increase.5) Stones of kidney and bladder showed slight increase.6) Neurosis showed annual increase. II) Clincal Statistics on Inpatients: The inpatients during 3 years were 151 cases, and this total comprised 10.6% of total outpatients.1) The patients admitted comprised 9.8% of male and 15% of female to total outpatients, and the male to female ratio was 3.3:1) The most frequent diseases were renal tuberculosis, urolithiasis, tuberculous epididymitis, hydrocele and urethral stricture3) Stones of kidney and bladder showed annual increase but ureteral stones showed no change.4) The patients admitted to hospital comprised the following percentage of the total attending as outpatients with the diseases indicated: stones of kidney and ureter in 82.6%, bladder tumor in 79.2%, kidney tumor in 83.3%, urethral stricture in 80% and renal tuberculosis in 40%.5) The number of admitted patient has gradually increased year by year. III) Clincal Statistics on Operations: The cases of total operation were 115 during 3 years and the cases of operation formed 2/5 of all cases in 1960 and nephrectomy was peak according to number and percentage showing annual decrease while uretero-ileo-cystoplasty. cystolithotomy and plastic surgery of urethra showed annual increase.

Metformin is a biguanide oral hypoglycemic agent commonly used in the treatment of diabetes mellitus. Metfofmin is excreated largely by the kidney and binds only negligibly to plasma proteins. Toxicity is usually seen when metformin is prescribed to patients with renal insufficiency. Lactic acidosis in diabetics on metformin therapy is rare but still associated with poor prognosis. For an early diagnosis, clinical symptoms of intoxication should be well known by physicians and patients. First-line therapy for correction of lactic acidosis and effective elimination of metformin is bicarbonate hemodialysis. We report a case of acute renal failure due to metformin-associated lactic acidosis in elderly type 2 diabetes mellitus patient and she was successfully treated by hemodialysis with bicarbonate buffer.

7.A case of jejunal angiodysplasia is diagnosed by capsule endoscopy in a patient with hypertrophic cardiomyopathy.

A 79-year-old woman with a known history of hypertrophic cardiomyopathy presented with severe anemia of unknown origin. She had also suffered from repeated episodes of upper gastrointestinal bleeding for the previous 2-3 years. Despite small bowel series and panendoscopic and angiographic studies, the origin of anemia remained undefined until a small multiple bleeding site was found during capsule endoscopy. The lesion proved to be angiodysplasia in jejunum. We report a case of jejunal angiodysplasia is diagnosed by capsule endoscopy in a patient with hypertrophic cardiomyopathy.

8.The change of bone formation according to magnetic intensity of magnet placed into titanium implant specimens.

PURPOSE: The purposes of this investigation were to discover the possibility of clinical application in the areas of dental implants and bone grafts by investigating the bone formation histologically around specimen which was depending on the intensity of magnetic field of neodymium magnet inside of the specimens. MATERIAL AND METHOD: 1. Measurement of magnetic intensity - placed the magnet inside of the specimen, and measured the intensity of magnetic field around the 1st thread and 3rd thread of specimen 20 times by using a Gaussmeter(Kanetec Co., Japan). 2. Surgical Procedure - Male rabbit was anesthetised by constant amount of Ketamine (0.25ml/kg) and Rompun (0.25ml/kg). After incising the flat part of tibia, and planted the specimens of titanium implant, control group was stitched without magnet, while experimental groups were placed a magnedisc 500(Aichi Steel Co., Japan) or magnedisc 800(Aichi Steel Co., Japan) into it, fixed by pattern resin and stitched. 3. Management after the surgery. - In order to prevent it from the infection of bacteria and for antiinflammation, Gentamycin and Ketopro were injected during 1 week from operation day, and dressed with potadine. 4. Preparation of histomorphometric analysis - At 2, 4 and 8 weeks after the surgery, the animals were sacrificed by excessed Ketamine, and then, specimens were obtained including the operated part and some parts of tibia, and fixed it to 10% of PBS buffer solution. After embedding specimens in Technovit 1200 and B.P solution, made a H-E stain. Samples width was 75micrometer. In histological findings through the optical microscope and using Kappa image base program(Olympus Co. Japan), the bone contact ratio and bone area ratio of each parts of specimens were measured and analyzed. 5. Statistical analysis Statistical analysis was accomplished with Mann Whitney U-test. RESULTS AND CONCLUSION: 1. In histomorphometricl findings, increased new bone formation was shown in both control and experimental groups through the experiment performed for 2, 4, and 8 weeks. After 4 weeks, more osteoblasts and osteoclasts with significant bone remodeling were shown in experimental groups. 2. In histomorphometric analysis, the bone contact ratios were 38.5% for experimental group 1, 29.5% for experimental group 2 and 11.9% for control group. Experimental groups were higher than control group(p<0.05)(Fig. 6, Table IV). The bone area ratios were 60.9% for experimental group 2, 46.4% for experimental group 1 and 36.0 % for control group. There was no significantly ststistical difference between experimental groups and control group(p<0.05)(Fig. 8, Table VII) 3. In comparision of the bone contact ratios at each measurement sites according to magnetic ntensity, experimental group 2(5.6mT) was higher than control group at the 1st thread (p<0.05) and experimental group 1(1.8mT) was higher than control group at the 3rd thread(p<0.05)(Fig. 7, Table V, VI). 4. In comparision of the bone area ratios at each measurement sites according to magnetic intensity, experimental group 2(5.6mT) was higher than control group and experimental group 1(4.0mT) at the 1st thread(p<0.1) and experimental group 2(4.4mT) was higher than experimental group 1(1.8mT) at the 3rd thread(p<0.1)(Fig. 9, Table IX, X). Experiment group 2 was largest, followed by experiment group1 and control group at the 3rd thread of implant. There was a significant difference at the 1st thread of control group and experiment group 2. and at 1st thread and 3rd thread of experiment group 1 and 2, and not at control group experiment group 1.(p<0.1)

9.Inadvertent subdural block in epidural anesthesia: A case confirmed by radiographic contrast material.

Malposition of catheter in the subdural space is a possible complication during attempted epidural anesthesia. We report here an inadvertent subdural block confirmed by the radiographic contrast materiaL Epidural anesthesia was planned for a full-term gravida and epidural catheter was inserted about 3 cm cranially at the level of L1-2. After injection of 19.5 ml local anesthetic, blood pressure was 70/50 mmHg. Fluid was rapidly infused and intravenous ephedrine 10 mg was given. After then, vital signs were well maintained and twin babies were delivered without events. Twentyfive minutes later, she complained chest tightness and became tachypneic. Ketamine 50 mg was given and endotracheal intubation was performed with ease. She obeyed command, but could not open her eyes for some duration. Self respiration returned after 110 minutes and minute ventilation was 6 liter/minute at that time. 160 minutes later, extubation was done and radiological examination was performed using contrast materiaL The cathter tip was positioned in anterior subdural space at T12 level.

10.Cytotoxicity of Benzalkonium Chloride on the Corneal Epithelial Cell of Rabbit.

Authors investigated the functional and morphological cytotoxicity of benzalkonium chloride(BAC) used clinically on the cultured corneal epithelial cell of rabbit in vitro. Corneal epithelial cells containing radioactive 51Cr were exposured for 5 minute, 10 minute, 30 minute and 60 minute to BAC 0.001%, 0.005%, 0.01%, 0.05%, 0.1%, and phosphated buffer solution(control). Cell injury assay was performed; % 51Cr released(cell lysis) and % cell detachment(cell dysfunction), and documentary photographs were taken with transmission electron microscopy(TEM). Epithelial cell lysis was severe at over BAC 0.05% after 5 minutes exposure, and cell dysfunction was severe at BAC 0.005% after 30 min exposure. The higher the concentration and the longer the duration of BAC exposure time, cell lysis and dysfunction of corneal epithelial cell were increased significantly(p<.05). In histological finding, the epithelial cell was injured with the disruption of plasma membrane, dialtated cistern form of rough endoplasmic reticulum and Golgi complex at BAC 0.001% after 5 min exposure. The nuclear damage of epithelial cell appeared at BAC 0.01% after 30 minutes exposure or at BAC 0.1% after 10 minutes exposure. As results, the clinical dose of BAC, ranged from 0.004% to 0.002% should be induced a particulary toxic effect, we should be carefully use the BAC, especially when using frequently or using for long duration.
