1.Effectiveness of reminder system in decreasing catheter-associated urinary tract infections: A Meta-analysis

Objective To objectively evaluate the effectiveness of reminder system in reducing catheter-associated urinary tract infections.Methods Databases were electronically searched to collect the trials.According to the inclusion and exclusion criteria,the literatures were screened,the data were extracted and the methodological quality of the included studies was assessed.Then Meta-analysis was performed by using RevMan 5.0 software.Results A total of 10 literatures involving 5 662 participants were included.The results of Meta-analysis showed that the reminder system group was superior to the control group in decreasing the incidence rate of catheter-associated urinary tract infections,shortening the days of indwelling catheter,and there were statistical differences.Conclusions Reminder system can shorten the days of indwelling catheter,decrease catheter-associated urinary tract infections,
2.Influence about complications of gastrointestinal tract among ICU patients by three methods of nasal feeding pump

Objective To discuss the influence about three kinds of nasal feeding methods on ICU critical patients with gastrointestinal complications, thus providing guidance for ICU patients to choose reasonable way of nasal feeding pump injection. Methods 120 ICU patients were randomly assigned into three groups, which is the continuous 6-hour and intermittent 2-hour every three time pump group and continuous 24-hours pump group and intermittent 24-hour(continuous 2-hour and intermittent 1-hour every eight time units)pump group ,each group has 40 cases. To observe the the influence about three different way of nasogastric on ICU critical patients with gastrointestinal complications. Results The incidence of diarrhea and constipation among the three groups was not statistically significant (P>0.05) . The gastric retention rate of the continuous 24-hour pump group was significantly higher than that of the continuous 6-hour and intermittent 2-hour pump group and the intermittent 24-hour pump group, the differences were statistically significant (P < 0.05). There was no statistically significant difference between the continuous 6-hour and intermittent 2-hour pump group and the intermittent 24-hour pump group, (P > 0.05). Conclusions The continuous 24-hour pump group in gastric retention was slightly worse than the other two pump way. Compared with ICU class three a week and feeding times , the continuous 6-hour and intermittent 2-hour pump method is more suitable for ICU patients.
3.Effect of the improved continuous subglottic approach in patients with mechanical ventilation

Objective To investigate the clinical effect of the improved continuous subglottic approach in patients with mechanical ventilation. Methods A total of 90 ICU patients with mechanical ventilation , were divided into observation group and control group 1 and control group 2 by random digits table method,each group was 30 cases.The observation group used the improved continuous subglottic suction. The control group 1 used the way of continuous 24 h suction under glottis. The control group 2 used the way of every 2 h suction under glottis combined with subglottic. The incidence of ventilator-associated pneumonia (VAP), the rate of tube blockage, the amount of subglottic secretion and the damage of airway mucosa were observed. Results The incidence of 7 d VAP in the observation group was 3.33%(1/30), which was not significantly different from that in the control group 1 and the control group 2 (P>0.05), but it was lower than the incidence of VAP at home and abroad. The rate of plugging in the observation group was 6.67%(2/30), and that in the control group 1 was 23.33%(7/30), and the difference was statistically significant (χ2=4.320, P<0.05). The daily flow rate of subglottic secretion was (30.93 ± 8.01) ml/d in the observation group, and (19.75 ± 6.52) ml/d in the control group 2, the difference was statistically significant (t=7.542, P<0.05). The injury rate of airway mucosa in the observation group was 10.00%(3/30), and 33.33%(10/30) in the control group 1, the difference was statistically significant (χ2=4.812, P<0.05). Conclusions The improvement suction under glottis method can be widely applied in clinical practice,which can effectively reduce the incidence of VAP, subglottic tube plugging rate, fully drainage of subglottic secretions, the degree of airway mucosa damage, reduce nursing workload compared with intermittent subglottic.
4.Effect of multi position prone position latex pad on patients with severe acute respiratory distress syndrome undergoing prone position ventilation

Objective:To investigate the effect of multi position prone position latex pad in patients with severe acute respiratory distress syndrome undergoing prone position ventilation, aiming to improve the effect of oxygenation therapy and reduce the complications associated with prone ventilation.Methods:A randomized controlled trial was used to select 80 patients with prone ventilation with severe acute respiratory distress syndrome who were admitted to the ICU of Yancheng First People ′s Hospital from February 2020 to February 2022 by convenient sampling method. The random number method was used to divide into experimental group and control group, with 40 cases in each group. The control group used soft pillow cushions for traditional prone ventilation, and the experimental group used homemade multi-site prone latex pads for prone ventilation. The effects of oxygenation therapy (oxygenation index and PaCO 2), the incidence of complications (pressure injury, artificial airway tube slippage, artificial airway tube bending) and nursing workload (length of turning over and total treatment time) were compared between the two groups. Results:After the intervention, the oxygenation index and PaCO 2 in the experimental group was (265.79 ± 21.15), (31.52 ± 3.43) mmHg (1 mmHg=0.133 kPa) respectively, and the control group was (177.43 ± 17.76), (39.73 ± 4.80) mmHg respectively and the differences between the two groups were statistically significant ( t=15.43, 10.17, both P<0.05). The incidence of pressure injury and artificial airway tube bending in the experimental group was 7.5% (3/40) and 0 respectively, and the control group was 37.5% (15/40) and 20.0% (8/40) respectively, and the differences between the two groups were statistically significant ( χ2=10.32, 8.89, both P<0.05). The incidence of artificial airway tube slippage in the experimental group and the control group was 0 and 7.5% (3/40) respectively, and there was no significant difference between the two groups ( χ2=3.12, P>0.05). The duration and total treatment duration of prone turning in the experimental group was (3.56 ± 0.53) min and (34.47 ± 3.72) h respectively, and the control group was (5.18 ± 0.61) min and (50.22 ± 4.60) h respectively, and the differences between the two groups were statistically significant ( t=6.25, 13.55, both P<0.05). Conclusions:The application of multi position prone position latex pad in patients with severe acute respiratory distress syndrome can significantly improve the oxygenation effect, reduce the incidence of complications and reduce the workload of nursing, which is worthy of clinical application.
5.Screening Key Genes of Ferroptosis in Atherosclerosis Based on GEO Database Bioinformatics and Experimental Validation

Objective To screen key genes for ferroptosis in atherosclerosis(AS)using bioinformatics methods and analyze the biological functions of key genes to gain an in-depth understanding of the pathogenesis of AS.Methods AS gene expression profile chip dataset GSE100927 was downloaded from the Gene Expression Omnibus(GEO)database.P<0.05 and|logFC>1|were used as the conditions to screen the differential genes of AS.These genes were intersected with ferroptosis gene dataset to screen out the genes related to AS ferroptosis.Gene ontology(GO)functional annotation and enrichment analysis of Kyoto Encyclopedia of Genes and Genomes(KEGG)were carried out.The STRING online analysis tool combined with Cytoscape visualization software was subsequently used to mine genes that play a key role in the biological process of AS,and the AS dataset GSE9874 was used as the verification set to verify the expression of key genes.Blood samples from 30 confirmed AS patients in the cardiovascular department of our hospital from January to March 2023 were collected as the experimental group,while blood samples from 20 healthy volunteers were collected as the control group.Sample RNA was extracted,and quantitative real time polymerase chain reaction(qRT-PCR)was used to verify the selected genes.Results A total of 10 differential genes related to ferroptosis were screened by bioinformatics method.GO functional enrichment analysis showed that differential genes were mainly involved in inflammation,apoptosis,oxidative stress and other biological processes,while KEGG pathway enrichment analysis showed that differential genes were mainly involved in ferroptosis,HIF-1 signaling pathway,and leukocyte migration pathway through endothelial cells.Seven key modules of gene construction were screened out through the protein interaction network,which were FTL,SLC40A1,CYBB,NCF2,HMOX1,DPP4 and ALOX5.GSE9874 was used for verification,and 5 key genes including ALOX5,DPP4,FTL,SLC40A1 and NCF2 were finally screened out.The expression detection of key genes in clinical samples by qRT-PCR showed that compared with the control group,the up-regulated genes in blood of AS group were DPP4(t=1.795,P=0.046),FTL(t=2.218,P=0.029)and SLC40A1(t=2.859,P=0.009),and the down-regulated genes were ALOX5(t=8.039,P<0.001)and NCF2(t=11.867,P<0.001),and the differences were significant.The experimental results were consistent with the results of bioinformatics analysis.Subgroup analysis showed that DPP4 expression in plaque group was higher than that in intima thickening group,and the difference was significant(t=2.843,P=0.036).Conclusion The key genes of ferroptosis screened by bioinformatics are AS,ALOX5,DPP4,FTL,SLC40A1 and NCF2,which may be potential targets for the diagnosis and treatment of AS,providing new ideas for the clinical diagnosis and treatment of AS.
