1.Analysis of health education effects on integrated program for controlling endemic fluorosis in Guizhou Province

Objective To evaluate the effects of implementing the health education in the Central Fund Program to control endemic fluorosis in Guizhou.Methods The samples were randomly surveyed to evaluate knowledge awareness in students and households as well as the habit formation after implementing the integrated program which mainly consisted of installing the improved stoves,supported by the Central Funds and health education in 5 counties.Results After health education,the rates of knowledge awareness in the students and the households were 94.80%(15 562/16 415)and 88.23%(4482/5080),respectively,and increased significantly compared with those before intervention[44.20%(26 364/59 645),22.81%(3082/13 510)],the difference being significant(χ2=13 324.05,6546.24,P<0.01).The rates of drying corn and chili with the coal fire were 5.61% (57/1016)and 5.41%(55/1016),respectively,and decreased significantly compared with those before intervention [77.41%(1076/1390),78.92%(1097/1390)],the difference being significant(χ2=1214.49,1270.92,P<0.01).The rates of washing corn and chili were 99.51%(1011/1016)and 94.59%(961/1016),respectively,and increased significantly compared with those before intervention[84.60%(1176/1390),76.55%(1064/1390)],the difference being significant(χ2=154.80,143.32,P<0.01).The rates of using the uncovered and unventilated iron stoves and table stoves were 4.71%(38/807)and 8.37%(60/717),respectively,and decreased significantly compared with those before intervention[29.99%(14 483/48 299),98.33%(95 070/96 685)],the difierence being significant(χ2=243.51,25 282.99,P<0.01).Conclusions Implementing the health education is the basis for the integrated measures for controlling the endemic fluorosis in the endemic regions.The consciousness and activity of the target people have been enhanced greatly.The good behaviors in the target people are forming,the expected goal is reached.
2.Study of the follow-up management strategies after installation of improved stoves to prevent coal-burning type endemic fluorosis in Guizhou province

Objective To study the follow-up management strategies after improving stoves for controlling coal-burning type endemic fluorosis in Guizhou and to provide a scientific basis for exploring the follow-up management measures.Methods In 2006 - 2009,three counties of Puding,Bijie and Liuzhi with improved stoves in 2005 and implemented follow-up management measures for subsequent three years were chosen,3 towns were chosen randomly in each chosen county,2 villages were selected randomly in each chosen town,10 households were investigated randomly in each chosen village,and 20 students were investigated of the knowledge of fluorosis control.The investigation included also the usage of the stoves,dehydration methods of grain and related conditions.Results ①The head of the family and student awareness of fluorosis control were 82.6%(743/900)and 91.7% (1650/1800),respectively,and 35.0% ( 385/1100 ) and 61.0% (6605/10 835 ),respectively,before the test,and the difference was statistically significant( x2 =33.04,1189.12,all P < 0.01 ).②The rates of chimney out of the house were 88.5%(69/78) and 100.0%(102/102),respectively,compared with those before the trial[74.4%(2125/2856) and 2.1%(104/4984),respectively],the difference was statistically significant(x2 =720.56,4295.38,all P < 0.01).The iron stove and the table stove's airtight utilization rates were 85.9%(67/78) and 100.0%(102/102),respectively.③The corn and the hot pepper's correct drying rates were 100.0%(180/180).Compared with those[27.2%(49/180) and 32.2%(58/180),respectively]before the trial,the difference was statistically significant(x2 =26.68,37.38,all P < 0.01 ).The corn and the hot pepper's washing rates before eating were 95.0%(57/60) and 98.3%(177/180),respectively.Compared with those[85.0%(153/180) and 77.8%(140/180)]before the trial,the difference was statistically significant(x2 =135.00,490.82,all P < 0.01 ).Conclusions The target population's awareness and knowledge of fluorosis prevention and related behavior are significantly elevated and enhanced after implementation of the three years post-management,which has reached the desired goal.The strategies of the follow-up management suits Guizhou province and widespread application is suggested.
3.Role of angiotensin-(1-7) in amino-acid-neurotransmitter-mediated blood pressure regulation in rat rostral ventrolateral medulla

The present study was undertaken to investigate the linkage between angiotensin-(1-7) [Ang-(1-7)] and the release of amino acid neurotransmitters in the the rostral ventrolateral medulla (RVLM) by techniques of microinjection, microdialysis combined with high performance liquid chromatography (HPLC)-fluorescent detection. Unilateral microinjection of Ang-(1-7) into the RVLM of anesthetized rats produced an increase in mean arterial pressure (MAP) accompanied by an increased release of glutamate (Glu). In contrast, microinjection of Ang779, a selective antagonist of Ang-(1-7) receptor, caused a decrease in MAP with a decreased releaceof Glu and an increased release of glycine,taurine and r-aminobutyric acid.The pressor effect of Ang-(1-7)and the depressor effect of Ang779 were in part blocked by corresponding andtagonists of aminoacid receptors.These results suggest that the pressor effect of Ang-(1-7)in the RVLM may be partially due to an increased release of Glu,whereas the depressor effect of Ang779 may be partially attributed to a decreased release of Glu and an increased release of inhibitory amino acid neurotransmitters
4.Effects of pentachlorophenol on rat sertoli cells.

OBJECTIVETo detect the toxic effects of pentachlorophenol (PCP) on cultured rat Sertoli cells.
METHODSThe viability of Sertoli cells was detected and morphological examination was performed, followed by flow cytometric assay to evaluate the toxic effect of PCP on rat Sertoli cells.
RESULTSMTT assay showed that PCP induced a concentration- and time-dependent decrease in Sertoli cell viability. Flow cytometric assay revealed that the number of dead Sertoli cells grew along with increased exposure to PCP.
CONCLUSIONPCP, with obvious cytotoxic effects, can cause necrosis of Sertoli cells in vitro.

5.Preliminary clinical experience on radiofrequency catheter ablation of right-sided accessory pathway guided by Ensite-NavX navigation

Objective To explore the feasibility and methodology of radiofrequency catheter ablation (RFCA) guided by 3D navigation system (Ensite-NavX) for right atrioventricular accessory pathway.Method Thirty-three cases of right accessory pathway atrioventricular reentrant tachycardia including 16 cases in right free wall,3 in right middle septum,14 in right posterior septum; 23 cases of dominant accessory pathway and 10 cases of concealed were treated by RFCA guided by NavX navigation.NavX navigation modeling method or spatial localization method was exploited to locate target positioning.Result All patients were successfully ablated without serious complications.Among them,25 cases were operated without exposure to X-ray,7 patients were exposed for several seconds to verify catheter position,1 case in right free wall was ablated under X-ray combined with Swartz sheath ablation.Conclusion Nonfluoroscopy or less fluoroscopy RFCA for right atrioventricular accessory pathway with Ensite-NavX is safe and feasible,modeling or spatial orientation method are helpful to locate the ablation target positioning.
6.A study of proximal humerus fractures using close reduction and percutaneous minimally invasive fixation.

OBJECTIVETo investigate the clinical effects of close reduction and percutaneous minimally invasive fixation in the treatment of proximal humerus fractures.
METHODSFrom April 2008 to March 2010, 28 patients with proximal humerus fracture were treated with close reduction and percutaneous minimally invasive fixation. There were 21 males and 7 females, ranging in age from 22 to 78 years,with an average of 42.6 years. The mean time from suffering injuries to the operation was 1.7 d. Nineteen cases caused by falling down, 9 cases by traffic accident. The main clinical manifestation was swelling, pain and limited mobility of shoulders. According to Neer classification, two part fractures were in 17 cases and three part fractures in 11 cases. The locking proximal humerus plate was used to minimally fixation through deltoid muscle under acromion. The operating time,volume of blood loss, the length of incision and Constant-Murley assessment were applied to evaluate the therapeutic effects.
RESULTSThe mean operating time was 40 min, the mean blood loss was 110 ml, and the mean length of incision was about 5.6 cm. The postoperative X-ray showed excellent reduction and the plate and screws were successfully place. Twenty-eight patients were followed up for 6 to 24 months (averaged 14.2 months). The healing time ranged from 6 to 8 weeks and all incision was primarily healed. There were no cases with necrosis head humerus, 24 cases without omalgia, and 4 cases with o-malgia occasionally. All the patients can complete the daily life. The mean score of Constant-Murley assessment was 91.0 +/- 5.8, 24 cases got an excellent result, 3 good and 1 fair.
CONCLUSIONClose reduction and percutaneous minimally invasive fixation, not only can reduce surgical invasive, but also guarantee the early function activities. It has the advantages of less invasive, fixed well and less damage of blood circulation.

7.Clinical analysis of 29 cases with neuroendocrine neoplasm in the digestive system.

OBJECTIVETo investigate the diagnosis and treatment of neuroendocrine neoplasm (NEN) in the digestive system.
METHODSClinical data of 29 patients with NEN from January 2000 to December 2012 in The First Affiliated Hospital of Dalian Medical University were analyzed retrospectively and the prognosis was evaluated according to the new WHO classification.
RESULTSThere were 19 males and 10 females and the average age was 46.5 years. All the patients had no clinical manifestations of carcinoid syndrome, and they all received surgical treatment. Two cases were gastric neuroendocrine carcinoma(NEC), who received radical total gastrectomy and distal gastric resection respectively. Three cases had neoplasm in the duodenum, including 2 NEC and 1 neuroendocrine tumor(NET), and they all underwent Whipple's procedure. Two cases were small intestine NEC, who received partial small intestine resection. Three cases had neoplasm in the appendix, including 1 NEC treated with right hemicolectomy and 2 NET with appendectomy. One case was ascending colon NEC, who received right hemicolectomy. Eighteen cases had neoplasm in the rectum, including 4 NEC treated with low anterior resection and abdominoperineal resection respectively, and 14 cases of NET underwent low anterior resection, local resection, and endoscopic resection respectively. The 1- and 3- year survival rates of 13 NEC cases were 38.4% and 7.7% respectively. The 5-year survival rate of 16 NET cases was 81.3%.
CONCLUSIONSNEN of digestive system is located mainly in the rectum and the clinical symptom is unspecific. Radical resection of NEN is the preferred treatment. The prognosis of NEC is poor, and that of NET is better.

8.Magnetic resonance imaging of eosinophilic meningoencephalitis caused by Angiostrongylus cantonensis following eating freshwater snails.

BACKGROUNDAngiostrongyliasis cantonensis is a worldwide-existing parasitic disease. However, the relevant reports on its radiological appearances are limited. In this study, we investigated magnetic resonance imaging (MRI) features of eosinophilic meningoencephalitis in a group of consecutive patients caused by human infection with Angiostrongylus cantonensis after eating freshwater snails.
METHODSWe performed brain MR imaging on 74 patients with angiostrongyliasis cantonensis. The scanner was a 0.5T unit. For each patient MR pulse sequences of SE T1-weighted image (T1WI) and FSE T2-weighted image (T2WI) were used. After intravenous administration of gadolinium chelate (Gd-DTPA) repeated T1-weighted images were obtained. MRI features of the lesions in the brain and meninges were analyzed and recorded after observing initial and follow-up MR images. The classification of the types of angiostrongyliasis cantonensis infection was done on the basis of locations of the disorders.
RESULTSForty-one (55%) normal and 33 (45%) abnormal MRI appearances in the brain were found. According to locations of the disorders, the types of angiostrongyliasis cantonensis infection were determined as follows: seventeen cases of type meningitis, three of type myeloencephalitis, one of type neuritis and twelve of mixed type (eight of type ventriculitis and five of type pneumonitis were among them). In type meningitis, abnormal leptomeningeal enhancement was visualized. In type myeloencephalitis, lesions in the brain parenchyma may have iso- or slightly low signal intensity on T1WI and high signal intensity on T2WI. Enhanced nodules in various shapes were shown on gadolinium-enhanced T1WI, a few lesions appeared as crescent enhancements and some lesions did not reveal abnormal enhancement. Other than brain lesions, an enhanced nodule was seen in the cervical spinal cord in one patient. In type ventriculitis, brain ventricular enlargement was demonstrated. In type neuritis, a nodule and abnormal enhancement in the right optic nerve was revealed. In type pneumonitis, patchy ground-glass opacity and consolidative lesions at the periphery of the lungs were seen. Follow-up results indicated that most lesions in the brain could resolve in 2 to 8 weeks.
CONCLUSIONSAngiostrongyliasis cantonensis presented as both single type and mixed type. Nodular enhancing lesions in the brain and/or linear enhancement in the leptomeninges were the main findings, while crescent enhancement would be the characteristic sign of the disease on gadolinium-enhanced T1WI. Focal edematous changes without contrast enhancement in the brain could be seen on MRI in some cases.

9.Changes of activated circulating endothelial cells and survivin in patients with non-small cell lung cancer after antiangiogenesis therapy.

BACKGROUNDAlthough antiangiogenesis therapy plays an important role in anti-neoplastic treatment with its recognized efficacy and slight adverse effect, there is no prospective clinical trial to define ideal markers for predicting efficacy of antiangiogenic therapy. This study was undertaken to investigate the changes of activated circulating endothelial cells (aCECs) and survivin after anti-angiogenesis therapy and their significance in predicting the efficacy of the therapy.
METHODSPatients of non-small cell lung cancer (NSCLC) treated with chemotherapy with or without Endostar were observed. The amount of activated CECs was detected by flow cytometry, and the expression of survivin mRNA was determined by real-time polymerase chain reaction (PCR).
RESULTSAfter treatment, the amount of activated CECs decreased significantly in clinical benefit cases (P = 0.021 in chemotherapy alone, P = 0.001 in chemotherapy plus Endostar), increased in disease progressive cases (P = 0.015 in chemotherapy alone, but P = 0.293 in chemotherapy with Endotatar). After therapy, the expression of survivin mRNA decreased in clinical benefit cases (P = 0.001) and increased in disease progressive cases (P = 0.018). A positive correlation was found between activated CECs and survivin in the chemotherapy group pre- and post-therapy (P = 0.001 and 0.021, respectively), but only in the chemotherapy with Endostar group pre-therapy (P = 0.030) rather than post-therapy. A positive correlation was found between the decreased activated CECs after therapy and time to progression (TTP) (r = 0.322, P = 0.012); a negative correlation was found between the amount of survivin mRNA in serum post-therapy and TTP (r = -0.291, P = 0.048).
CONCLUSIONSActivated CECs and survivin may be ideal markers forecasting efficacy and prognosis of NSCLC. The former can reflect more sensitively antiangiogenic efficacy and the latter is more sensitive to shrinkage or swelling of tumors. Their combination can evaluate more accurately the efficacy of antiangiogenic therapy of NSCLC.

10.Clinical application of uvulopalatopharyngoplasty with resection of the musculus uvulae and preservation of uvulae mucosa.

OBJECTIVE:
To explore the effect of modified uvulopalatopharyngoplasty with partial resection of the musculus uvulae and preservation of the mucosa of uvulae for obstructive sleep apnea hypopnea syndrome (OSAHS).
METHOD:
Thirty-six patients with OSAHS were enrolled and determined by physical examination, Muller's Test, and cephalometry whose obstructive plane were in oropharynx. One group (n = 18) only received uvulopalatopharyngoplasty with uvulae preservation, while the other group (n = 18) received uvulopalatopharyngoplasty with partial resection of the musculus uvulae and preservation of the mucosa of uvulae. Polysomnography (PSG), Epworth Sleepiness Scale (ESS) and Level of Snore were measured one day before operation and seix months after operation.
RESULT:
The apnea hypopnea index (AHI), lowest SaO2 (LSaO2), ESS and Level of Snore have changed in both groups (P < 0.01) after surgery in both groups. There were no statistically significance (t= --0.301, P > 0.05) in validity ratio between two groups. Postoperative Level of Snore were significantly different between two groups while AHI, LSaO2, ESS were not significantly different between two groups.
CONCLUSION
Uvulopalatopharyngoplasty with partial resection of the musculus uvulae and preservation of uvulae mucosa is an effective surgical method for patients with OSAHS which obstructive plane is in oropharynx. It can decrease not only the level of snore but also the limited ratio of shrink in uvulae after routine surgery.
