3.Effects of smoking and alcohol drinking on arsenic metabolism of people exposed to different concentrations of arsenic in drinking water

Objective To explore the effects of smoking and alcohol drinking on arsenic metabolism of people exposed to different concentrations of arsenic in drinking water.Methods Residents in Shanxi exposed to different concentrations of arsenic in drinking water and age ≥ 18 years old adults were chosen as the subjects for this study in 2008,the subjects were divided into three groups according to the concentrations of arsenic in drinking water: high-arsenic exposure group (more than 0.05 mg/L),low-arsenic exposure group (between 0.01 and 0.05 mg/L) and control group(less than 0.01 mg/L),excluded recently had eaten seafood and had poisoning symptoms of chronic arsenic in drinking water in the crowd.Smoking and alcohol drinking habits were investigated by questionnaire.Arsenic species in the urine samples were detected with hydride generation atomic absorption spectroscopy.Total arsenic(tAs) was the sum of iAs%,MMA% and DMA%.iAs%,MMA% and DMA% were calculated as iAs/tAs,MMA/tAs and DMA/tAs,respectively.The first methylation ratio(FMR) and the secondary methylation ratio(SMR) were calculated as (MMA + DMA)/tAs and DMA/(MMA + DMA),respectively.Results Three hundred and ninety-five adults were chosen in this study.In the high exposure group the alcohol drinking and smoking subjects had higher MMA%(16.24%) but lower SMR(82.19% ) than the non-drinking and non-smoking subjects (12.16% and 86.13%,respectively).The differences of both MMA% and SMR were significant(P < 0.05 ).No significant difference was observed between the non-smoking/non-drinking subjects and the smoking or the drinking subjects(all P > 0.05 ).In the low exposure group there were higher MMA%( 13.86%,13.99%) lower DMA%(72.87%,77.76%)and lower SMR (83.48%,83.90% ) in those with smoking or drinking/smoking compared with the non-drinking and non-smoking subjects (11.83%,80.35% and 86.54%,respectively,all P <0.05 ).No significant difference was observed between drinkers and non-drinking/non-smoking subjects(P > 0.05).In the control group there were a higher MMA%( 17.27%,17.06%) lower DMA% (73.89%,72.29%) and lower SMR (81.48%,82.58% ) in those with smoking or drinking/smoking compared with the non-drinking and nonsmoking subjects( 11.52%,79.68% and 87.19%,respectively,all P < 0.05).No significant difference was observed between drinkers and the non-drinking/non-smoking subjects (all P > 0.05).ConclusionThe arsenic methylation capacity of people with drinking and smoking is poorer than that of non-drinking and non-smoking subjects after arsenic exposure.

Objective To compare and quantify the determinants in quantitative electroencephalogram(q-EEG) and heart rate variability power spectrum analysis(HRV-PSA) of ketamme(KTM) anesthesia for preschoolers. Methods Seventy four cases were selected and assigned into 3 groups named A(4-5 years), B(5-6 years), C(6-7 years), 22,28,24 cases in every group respectively. 5.Clinical Characteristics of Children with Moyamoya Disease

Objective To discuss the clinical characteristic and diagnosis of children with moyamoya disease(MMD).Method The clinical features of 4 children with MMD were analyzed.Results The early clinical spectrum in children with MMD was transiently ischemic attack,and presented with injured neuron manifestations after some seizures,such as paralysis,extremity sensory disturbance, seizure of epilepsy,language disorder,involuntary movement and psychotic symptoms.Magnetic resonance angiography(MRA) and DSA demonstrated multiple cerebral vessels occlusion or stenosis and moyamoya vessels,so MRA became the first choice for detec- ting MMD.Conclusions The clinical symptom and neuron symptom of children MMD may not be typical,and it is easily misdiagnosed.Its correct diagnosis depends on thorough physical examination,appropriate laboratory tests,and the general knowledge of this disease.
6.Immunophenotypic Features of NK/T Cell Lymphoma in Children and Its Association with Epstein-Barr Virus

Objective To study the immunophenotypic features of NK/T cell lymphoma in children and its association with Epstein-Barr virus (EBV) infection. Methods Five cases of children′s NK/T cell lymphoma were studied. CD45RO, CD3?, CD56, CD20, TIA-1 and granzyme B were detected by immunohistochemistry staining for investigating immunophenotype. The expression of EBV-latent membrane protein (LMP-1) were detected by immunohistochemistry. EBV-encoded RNA (EBER1/2) were detected by in situ hybridization (ISH). Results CD45RO, CD3?, TIA-1 and granzyme B were positive in 5 cases, CD56 was positive in 2 cases, while CD20 negative in 5 cases.EBER1/2 positive in 4 cases and LMP-1 positive in 3 cases.Conclusions NK/T cell lymphoma in children is strong associated with EBV infection,and EBV infection may play an important role in the pathogenesis of children NK/T cell lymphoma.

In gene manipulation, different selectable markers with various linkers are necessary. In order to get selectable markers directly, we constructed from pBlueScript SK(-) a series of particular plasmids, pSKsymKm, pSKsymBle, pSKsymEry, pSKsymHyg and pSKsymGm, each contains Kanamycin, Bleomycin, Erythromycin, Hygromycin or Gentamycin resistance cassette. By restriction enzyme digestion and gel extraction, any of five antibiotic resistance genes with specific ends can be conveniently obtained.
8.Cemented bipolar hemiarthroplasty with a novel cerclage cable technique for unstable intertrochanteric hip fractures in senile patients.

OBJECTIVETo observe the clinical result and assess clinical value of cemented bipolar hemiarthroplasty with a novel cerclage cable technique for treatment of unstable intertrochanteric hip fractures in senile patients.
METHODSForty-eight consecutive patients with unstable intertrochanteric fractures were treated in our hospital from March 2001 to March 2006 (Evans type III in 11 cases, Evans type IV in 25 cases and Evans type V in 22 cases). All the cases were evaluated by Zuckerman functional recovery score (FRS) and operative risk assessment software 1 (ORAS1), which were based on the patients' physical and laboratory examinations preoperatively. Seventeen cases (19 hips) were treated with cemented bipolar hemiarthroplasty. There were 5 male cases (5 hips) and 12 female cases (14 hips, including 2 patients who suffered from additional slight injuries and resulted in contralateral hip fracture and were treated with the same procedure 3 months after the first operation). The average age was 85 years (78-95 years). All the operations were carried out under general anesthesia, through Southern incision and lateral approach by the same orthopaedic surgeon. All prostheses consisted of Link SPII femoral stem and bipolar femoral head. All patients were followed up for more than 30 days.
RESULTSThe operative risks of all the 17 cases (19 hips) were calculated by ORAS1 preoperatively. The average preoperative FRS was 81.7 (80.7-82.7). The average predictive value of operative morbidity was 10% (7%-15%). The average predictive value of mortality was 2.97% (2.1%-3.2%). The average operation time was 1.5 hours. The average blood transfusion was 400 ml. There were no operative or anesthetic complications and no deaths within 30 days after operation. Sitting up was permitted 3 to 4 days after operation, and partial weight bearing was permitted 5 to 7 days after operation. Patients were allowed to walk with a walker 10 days after operation. The average FRS was 78.7 at 30 days postoperatively. No patient died during at least one year follow-up.
CONCLUSIONSAlthough the value of the technique of cemented bipolar hemiarthroplasty in the treatment of unstable intertrochanteric hip fracture is not widely recognized, we have consistently achieved satisfactory results with strict preoperative risk assessment, strict indication selection and systematic postoperative rehabilitation.

9.Influence of preoperative nicorandil application on coronary no-reflow in patients with acute anterior myocardial infarction

Objective:To explore influence of preoperative nicorandil application on coronary no-reflow during emer-gency percutaneous coronary intervention(PCI)in patients with acute anterior myocardial infarction(AAMI). Methods:A total of 116 AAMI patients were randomly and equally divided into routine treatment group and nic-orandil group(received nicorandil based on routine treatment).TIMI grade,incidence rate of no-reflow,ECG ST-segment elevation resolution(STR)and incidence rate of major adverse cardiovascular events(MACE)after PCI were compared between two groups.Results:Compared with routine treatment group after PCI,there were signifi-cant rise in percentage of infarct related artery(IRA)TIMI grade 3(77.6% vs.91.4%)and STR on 2h after PCI[(0.15 ± 0.05)mm vs.(0.18 ± 0.05)mm],and significant reductions in incidence rates of no-reflow(13.8% vs. 3.4%)and MACE(17.2% vs.5.2%)in nicorandil group after PCI,P<0.05 or <0.01. Conclusion:Preoperative nicorandil application can reduce incidence rate of no-reflow after PCI,improve myocardial perfusion and reduce in-cidence rate of major adverse cardiovascular events.
10.Study of mRNA expression of vascular endothelial growth factor-(A, C, D) genes and its effect on prognosis of breast cancer.

OBJECTIVETo study mRNA expression of VEGF-(A, C, D) genes in breast carcinoma and its effect on prognosis.
METHODSThe mRNA expression of VEGF-(A, C, D) genes in 61 cases of primary breast carcinoma and 29 cases of benign mammary lesions was detected by TaqMan real-time reverse transcription-polymerase chain reaction (real-time RT-PCR) technology. The correlation between mRNA expression and various clinicopathologic parameters and survival data was analyzed.
RESULTSThe level of mRNA expression of VEGF-(A, C) genes was significantly higher in primary breast carcinomas (2.79 +/- 1.31 and 3.33 +/- 0.88 respectively) than in benign mammary lesions (1.59 +/- 1.35 and 2.76 +/- 0.55 respectively, P = 0.000 and 0.002 respectively). The percentage of VEGF-D mRNA expression was 73.77% in breast carcinomas, as compared to 51.72% in benign mammary lesions (P = 0.038). However, there was no statistically significant difference in the mRNA expression levels of VEGF-D gene between primary breast carcinomas and benign mammary lesions (P = 0.683). On the other hand, the ratio of VEGF-D mRNA expression to VEGF-C mRNA expression was lower in lymph node-positive than in lymph node-negative cases. This also correlated with the occurrence of nodal metastasis by uni- and multivariate analysis (P(uni) = 0.046 and P(mult) = 0.062). High mRNA expression of VEGF-(A, C) genes was associated with poor disease-free survival (P = 0.030 and 0.044 respectively).
CONCLUSIONSThe expression of VEGF-(A, C, D) genes may play a role in the disease progression of breast carcinoma. The ratio of VEGF-D mRNA expression to VEGF-C mRNA expression correlates with the occurrence of lymph node metastasis in breast carcinoma. The mRNA expression of VEGF-(A, C) genes may serve as a useful prognostic indicator in breast cancer.
