2. Clinical values of multi-marker combined detection for early diagnosis of infection in patients with liver cirrhosis

Objective To analyze the clinical values of combined detection of procalcitonin (PCT), interleukin-6 (IL-6), C-reaction protein (CRP), neutrophil CD64, white blood cell (WBC) count and neutrophil ratio for diagnosis of infection in patients with liver cirrhosis. Methods The hospitalized patients with liver cirrhosis were divided into the infection group and the non-infection group according to the bacterial culture and clinical symptoms. The serum levels of PCT and IL-6 were detected by Chemiluminescence immunoassay. The serum CRP was detected by biochemistry analyzer. The neutrophil CD64 was detected by flow cytometer, and WBC count and neutrophil ratio were detected by blood cell analyzer. The collected data were analyzed by logistic regression and receiver operating characteristic (ROC) curves. ResultsAll the markers in the infection group were higher than those in the non-infection group (P<0.01). The results of logistic regression analysis showed that PCT, IL-6 and neutrophil CD64 could predict the infection in patients with liver cirrhosis,with the odd ratio being 7.199 (95%CI, 2.180-23.771),1.010 (95%CI,1.002-1.017)and 2.312 (95%CI,1.485-3.600), respectively. However, CRP, WBC count and neutrophil ratio showed no predictive values. The ROC curves showed that the area under curves (AUC) of PCT, IL-6 and neutrophil CD64 were 0.791 (95%CI,0.727-0.856),0.762 (95%CI,0.693-0.832)and 0.884 (95%CI,0.835-0.933), respectively. The AUC of combined detection of the three markers was 0.932 (95%CI,0.897-0.967), with a diagnostic accuracy of 86.9%. Conclusion PCT, IL-6 and neutrophil CD64 can predict infection in patients with liver cirrhosis, and combined detection of the three markers can improve the diagnostic efficiency.
3.The relationship between DKK1 protein expression and lymph node invasion, and prognosis in patients with intrahepatic cholangiocarcinoma

Objective The objectives of this study were to investigate the relationship between DKK1 protein expression and lymph node invasion,and prognosis in patients with intrahepatic cholangiocarcinoma.Methods From January 2005 to December 2012,78 patients with intrahepatic cholangiocarcinoma were treated with endoscopic resection.Tissue microarray of intrahepatic cholangiocarcinoma and adjacent tissues were done and biochemical indexes were measured before operation.The patients were recruited every 3 months after operation.The Kaplan-Meier method was used to analyze the recurrence-free survival curve and the overall survival curve.The Cox proportional hazards regression model was used to analyze the multivariate analysis and to determine the factors that affect the disease-free survival(RFS)and overall survival(OS).Results There were significant differences in the expression of GGT,hepatic portal lymph node invasion,Child-Pugh grade,MMP9 and DKK1 protein in the negative and positive expression of intrahepatic cholangiocarcinoma(P < 0.05).The positive expression rate of DKK1 protein in intrahepatic cholangiocarcinoma was 35.90% (28/78),and the positive expression rate of DKK1 protein in hilar cholangiocarcinoma was 14.10% (11/78).RFS and OS in 78 cases of intrahepatic cholangiocarcinoma were 51.28% (40/78) and 50.00% (38/78),41.03 % (32/78) and 38.46% (30/ 78),25.64% (20/78) and 23.08% (18/78) after surgery for 1,3 and 5 years,respectively.The results from univariate and multivariate analysis showed that GGT,CA19-9,CEA,tumor size,DKK1 and hilar lymph node invasion were the prognostic factors of OS in patients with intrahepatic cholangiocarcinoma.CEA,tumor size,DKK1 and hilar lymph node involvement were prognostic factors for RFS in patients with intrahepatic cholangiocarcinoma.DKK1 protein expression positive and negative OS curve or RFS curve showed that 5 years after intrahepatic cholangiocarcinoma patients DKK1 protein expression positive and negative OS were 28.20% and 20.51%;RFS were 24.36% and 21.79%.Conclusion The expression of DKK1 protein is closely related to lymph node invasion in patients with intrahepatic cholangiocarcinoma.The expression of DKK1 protein is the prognostic factor of OS and RFS in patients with intrahepatic cholangiocarcinoma.
4.Treatment of thoracolumbar fractures with unilateral pedicle screw fixation through paraspinal approach.

OBJECTIVETo evaluate the feasibility and efficacy of unilateral pedicle screw fixation in treating thoracolumbar fractures through paraspinal approach.
METHODSFrom January 2006 to January 2009,21 patients with single level thoracolumbar fracture without neurological symptoms were treated with unilateral pedicle screw fixation through paraspinal approach. There were 14 males and 7 females,aged from 21 to 65 years old with a mean of 36.4 years. The duration from injury to operation ranged from 6 h to 5 d with an average of 3 d. According to the classification of Denis fracture, compression fractures happedned in 12 cases and burst fractures happened in 9 cases,including 1 case with T5 fracture, 2 cases with T7 fracture, 2 cases with T10 fracture, 3 cases with T11 fracture, 8 cases with T12 fracture, and 5 cases with L1 fracture. Based on the Flankel grade, all patients were classified as grade E. Anterior vertebral body height ratio, sagittal Cobb angle, condition of internal fixation failure, visual analogue score (VAS) were evaluated.
RESULTSAll patients were followed up from 12 to 36 months with an average of 20.5 months. No internal fixation failure was found. Anterior vertebral body height ratios at preoperative 3 days after operation and last follow-up were 54.3 +/- 2.8, 92.9 +/- 1.5, 93.8 +/- 1.7, respectively;sagittal Cobb angle at the three timepoints were (27.8 +/- 2.5) degrees, (5.3 +/- 0.8) degrees, (6.3 +/- 1.4) degrees, respectively; the difference was statistical significant (P < 0.05). VAS was (1.2 +/- 0.4) points at last follow-up and had obviously improved (P < 0.05).
CONCLUSIONTreatment of thoracolumbar fractures with unilateral pedicle screw fixation through paraspinal approach is safe with the advantages of micro-trauma and less blood loss,which can not only completely retain the posterior spinal complex structure, reinforce the spinal stability, raise the reductional quality, but also improve the strength of fixation and the distribution of stress force.

5.Molecular characterization of a HMG-CoA reductase gene from a rare and endangered medicinal plant, Dendrobium officinale.

The 3-hydroxy-3-methylglutaryl-CoA reductase (HMGR) catalyzes the conversion of HMG-CoA to mevalonate in mavalonic acid pathway, which is the first committed step for isoprenoid biosynthesis in plants. However, it still remains unclear whether HGMR gene plays a role in the isoprenoid biosynthesis in Dendrobium officinale, an endangered epiphytic orchid species. In the present study, a HMGR encoding gene, designed as DoHMGR1 (GenBank accession JX272632), was identified from D. officinale using the reverse transcription polymerase chain reaction (RT-PCR) and rapid amplification of cDNA ends (RACE) methods, for the first time. The full length cDNA of DoHMGR1 was 2 071 bp in length and encoded a 562-aa protein with a molecular weight of 59.73 kD and an isoelectric point (pI) of 6.18. The deduced DoHMGR1 protein, like other HMGR proteins, constituted four conserved domains (63-561, 147-551, 268-383 and 124-541) and two transmembrane motifs (42-64 and 85-107). Multiple sequence alignment and phylogenetic analyses demonstrated that DoHMGR1 had high identity (67%-89%) to a number of HMGR genes from various plants and was closely related to Vanda hybrid cultivar, rice and maize monocots. Real time quantitative PCR (qPCR) analysis revealed that DoHMGR1 was expressed in the three included organs. The transcripts were the most abundant in the roots with 2.13 fold over that in the leaves, followed by that in the stems with 1.98 fold. Molecular characterization of DoHMGR1 will be useful for further functional elucidation of the gene involving in isoprenoid biosynthesis pathway in D. officinale.

OBJECTIVETo observe the clinical result and assess clinical value of cemented bipolar hemiarthroplasty with a novel cerclage cable technique for treatment of unstable intertrochanteric hip fractures in senile patients.
METHODSForty-eight consecutive patients with unstable intertrochanteric fractures were treated in our hospital from March 2001 to March 2006 (Evans type III in 11 cases, Evans type IV in 25 cases and Evans type V in 22 cases). All the cases were evaluated by Zuckerman functional recovery score (FRS) and operative risk assessment software 1 (ORAS1), which were based on the patients' physical and laboratory examinations preoperatively. Seventeen cases (19 hips) were treated with cemented bipolar hemiarthroplasty. There were 5 male cases (5 hips) and 12 female cases (14 hips, including 2 patients who suffered from additional slight injuries and resulted in contralateral hip fracture and were treated with the same procedure 3 months after the first operation). The average age was 85 years (78-95 years). All the operations were carried out under general anesthesia, through Southern incision and lateral approach by the same orthopaedic surgeon. All prostheses consisted of Link SPII femoral stem and bipolar femoral head. All patients were followed up for more than 30 days.
RESULTSThe operative risks of all the 17 cases (19 hips) were calculated by ORAS1 preoperatively. The average preoperative FRS was 81.7 (80.7-82.7). The average predictive value of operative morbidity was 10% (7%-15%). The average predictive value of mortality was 2.97% (2.1%-3.2%). The average operation time was 1.5 hours. The average blood transfusion was 400 ml. There were no operative or anesthetic complications and no deaths within 30 days after operation. Sitting up was permitted 3 to 4 days after operation, and partial weight bearing was permitted 5 to 7 days after operation. Patients were allowed to walk with a walker 10 days after operation. The average FRS was 78.7 at 30 days postoperatively. No patient died during at least one year follow-up.
CONCLUSIONSAlthough the value of the technique of cemented bipolar hemiarthroplasty in the treatment of unstable intertrochanteric hip fracture is not widely recognized, we have consistently achieved satisfactory results with strict preoperative risk assessment, strict indication selection and systematic postoperative rehabilitation.

9.Clinical effect of circumcision stapler in the treatment of phimosis and redundant prepuce.

OBJECTIVETo observe the clinical effect and safety of circumcision stapler in the treatment of phimosis and redundant prepuce.
METHODSWe treated 120 patients with redundant prepuce or phimosis using circumcision stapler and another 60 by conventional dorsal-incision circumcision. We observed intraoperative blood loss, operation time, postoperative pain, wound healing time, cosmetic appearance of the penis, and postoperative complications and compared them between the two groups of patients.
RESULTSStapler circumcision showed obvious advantages over the conventional method in intraoperative blood loss ([2. 3 ± 1. 3] vs [15.6 ± 2.9] ml), operation time ([7.1 ± 1.4] vs [22.6 ± 4.6] min), wound healing time ([12.0 ± 2.9] as [16.3 ± 3. 1] d), postoperative pain score (1. 9 ± 1. 3 vs 5. 2 ± 1. 7), incision edema, and cosmetic appearance of the penis (all P <0. 05). Besides, stapler circumcision exempted the patients from stitch-removal pain. However, the incidence rate of postoperative local ecchymosis was significantly higher in the circumcision stapler group than in the conventional circumcision group (20. 8% vs 8. 3% , P <0. 05).
CONCLUSIONCircumcision stapler, with its advantages of easier manipulation, shorter operation time, better cosmetic penile appearance, less pain, and fewer complications, is superior to conventional circumcision in the treatment of phimosis and redundant prepuce.

10.Observation on therapeutic effect of acupoint injection desensitization with autoblood on chronic urticaria.

OBJECTIVETo compare the effect of acupoint injection desensitization with autoblood and routine combined therapy for treatment of chronic urticaria.
METHODSTwo hundred patients with chronic urticaria were randomly divided into an acupoint injection with autoblood (AJA) group and a medicine group, 100 cases in each group. The AJA group was treated by acupoint injection desensitization with autoblood and Dazhui (GV 14), Fengfu (GV 16), Feishu (BL 13), Neiguan (PC 6) and etc. were selected, 3-5 acupoints each time, once every three days, 30 days for a course. The therapeutic effect was assessed after one course. The medicine group was treated with external application of Dexamethasone Acetate cream, twice a day, and oral administration of Setastine Hydrochloride, twice a day, 1 mg each time and the treatment duration was the same as that in the AJA group.
RESULTSThe clinical cured rate was 66.0% (66/100) in the AJA group, which was superior to that of 0 (0/100) in the medicine group (P < 0.05).
CONCLUSIONThe acupoint injection desensitization with autoblood has obvious therapeutic effect on chronic urticaria with no apparent dependence and rebound problem.
