1.Associated factors of poor vision after cataract surgery

AIM: To analyze the associated factors of poor vision after phacoemulsification operation.
METHODS:Clinical data of cases with poor vision after cataract phacoemulsification combined with IOL implantation from 341 patients(418 eyes) during Jan. 2015 to Jan. 2016 in our hospital were collected and logistic correlation analyzed.
RESULTS: The incidences of postoperative low vision were 10. 3% for one month after operation. Forty-three eyes were reported low vision, and 41 of 43 the patients with postoperative low vision had fundus oculi disease before operation, which included 20 eyes ( 48. 8%) with age-related macular degeneration, 16 eyes(39. 0%) with diabetic retinopathy, 3 eyes ( 7. 3%) with glaucomatous optic neuropathy, 1 eye(2. 4%) with pathologic myopia of retinopathy and 1 eye ( 2. 4%) with branch retinal vein occlusion.
CONCLUSION: The associated factors of poor vision after phacoemulsification operation are preoperative eye diseases and the nuclear hardness, and preoperative eye diseases may be the main causes of postoperative low vision. Comprehensive examination should be attached importance to, and be careful in the operation to decrease the harm for visual performance.
2.Analysis of clinical effects of cervical artificial disc replacement or anterior cervical decompression and fusion for the treatment of single cervical disc herniation.

OBJECTIVETo explore the clinical effects of Mobi-C cervical artificial disc replacement (CADR) and anterior cervical decompression and fusion (ACDF) in treating single cervical disc herniation.
METHODSFrom June 2009 to June 2012, the clinical data of 27 patients with single cervical disc herniation were retrospectively analyzed. There were 18 males and 9 females, aged from 30 to 62 years old with an average of 46.7 years. Of them, 12 patients were treated with CADR (CADR group) and 15 patients with ACDF (ACDF group). All patients had pain and numbness in neck, shoulder and upper limbs, and courses of disease was from 1 to 13 months with an average of 2.4 months. The data of clinical evaluation and questionnaire survey about quality of life were collected before operation, postoperative at 1 week and final follow-up. Odom criterion was used to evaluate postoperative effect. Visual analogue scale (VAS) was used to record pain levels. Neck disability index (NDI) and health questionnaire SF-36 were used to assess the quality of life.
RESULTSNo complications about nerve and blood vessel were found and the patients were followed up from 6 to 30 months, with an average of 16 months. One week after operation, 10 cases got excellent results and 2 good in CADR group; 5 cases got excellent results and 10 good in ACDF group; there was significant difference between two groups (P<0.05). At final follow-up, 10 cases got excellent results and 2 good in CADR group; 12 cases got excellent results and 3 good in ACDF group; there was no significant difference between two groups (P> 0.05). Pain of upper limbs had obviously relieved between two groups at 1 week after operation and final follow-up (P<0.05). VAS of neck and NDI in CADR group had decreased respectively from preoperative 3.58±0.79, 23.42±6.36 to 0.58±0.51, 5.42±1.68 at 1 week after operation (P<0.05); but the index in ACDF group was no obvious at 1 week after operation. At final follow-up, VAS of neck and NDI and SF-36 score were obviously improved than preoperation (P<0.05) between two groups.
CONCLUSIONMobi-C CADR retains the movement unit in the decompression segment and can quickly recover normal action for patients. Using CADR method has a good curative effect in the early phase, and the clinical effect is reliable, may improve the quality of life.

Objective To explore the association of Toll-like receptor 4 TLR4 and lipopolysaccharide receptor CD14 gene polymorphisms with Kawasaki disease (KD) susceptibility.Methods Three-color fluorescent staining flow-cytometry was used to detect the expression of TLR4 in peripheral blood white blood cell of 76 KD children and 118 healthy control group.The gene of TLR4 (-896A/G), (-1196C/T) and CD14 (-260C/T) polymorphisms was identified by polymerase chain reaction-restriction fragment length polymorphisms; and the relationship between genotype and KD was analyzed.Results 1.The values of mean fluorescence intensity (MFI) of TLR4 in peripheral blood white blood cell of the KD groups and the healthy control groups were 2.87?0.96, 10.55?4.87, 23.36?8.28 and 3.26?0.65, 7.55?1.21, 25.41?6.97, respectively; There was a gradual increase of these values on lymphocyte, neutrophilic leukocyte and mononuclear cell in both groups.2.(-896A/G), (-1196C/T) polymorphisms of TLR4 gene were not found in both groups.3.The frequency of each genotype of CD14 gene (-260C/T) was 35.5%CC, 30.3%CT, 34.2%TT in KD group and 38.1%CC, 47.5%CT, 14.4%TT in healthy control group.The frequency of each genotype was significantly different in 2 groups(?2=11.62 P
4.Evaluation of enzymatic method for determination of serum beta-hydroxybutyrate and its clinical application.

OBJECTIVETo evaluate an enzymatic method for determining serum beta-hydroxybutyrate (beta-HB) with the National Committee for Clinical Laboratory Standards (NCCLS) projects, and to discuss its clinical values in diabetic ketoacidosis (DKA).
METHODSThe precision, accuracy, specificity, linearity and interference of the enzymatic method were analyzed. This method was used to determine serum beta-HB in 60 cases of normals, 50 cases of diabetes, and 34 cases of DKA by autochemistry analyzer.
RESULTSEnzymatic beta-HB assay was precise (within-run CV, day-to-day CV, and total CV < 5%). The linearity studies showed the method was linear up to 4 mmol/L. Recovery rate was 98.5%-104.1%. Hemolysis (Hemoglobin up to 18.2 g/L), icteric samples with total bilirubin up to 224 mumol/L, and lipemia up to triglyceride concentration of 2.28 mmol/L did not interfere with the beta-HB results in this method. Serum beta-HB levels were significantly elevated in DKA patients compared with DM patients and controls (P < 0.01). Positive rate of serum beta-HB in DKA patients was significantly higher than that of urinary ketone (P < 0.05).
CONCLUSIONSEnzymatic method is convenient and reliable, allows full automation, and is rapid enough to be used for both routine and urgent determinations of serum beta-HB. It can be used in diagnosing and monitoring treatment of DKA.

5.A new alkaloid from Opuntia vulgaris.

AIMTo study the chemical constituents of the stems of Opuntia vulgaris Mill(Cactaceae).
METHODSThe compounds of Opuntia vulgaris were isolated by chromatography of Amberlite Dowex 50 and silica gel, and identified by means of UV, IR, MS, 1D and 2D NMR.
RESULTSThree compounds were isolated and identified as: opuntin B(I), 4-hydroxyproline(II) and tyrosine(III).
CONCLUSIONCompound I is a new alkaloid.

6.Effect of esculentoside A on cellular adhesion.

AIMTo investigate the anti-inflammatory mechanism of esculentoside A (EsA) and to observe the effects of EsA on cellular adhesion between human umbilical vein endothelial cell (VEC304) and human neutrophil and to further observe the mRNA expression of intercellular adhesion molecule-1 (ICAM-1) and cluster of differentiation 18(CD18).
METHODSThe hemocyte counting method was used for assaying the adhesion rate between VEC304 and neutrophil. The RT-PCR method was used for measuring the mRNA expression of ICAM-1 and CD18.
RESULTSThe adhesion rate between VEC304 and neutrophil was increased with treatment of lipopolysaccharide(LPS). EsA (3 - 12 x 10(-6) mumol.L-1) was shown to inhibit the high cellular adhesion induced by LPS. A further investigation of adhesion molecules mRNA expression was undertaken using semi-quantitative reverse transcribed polymerase chain reaction (RT-PCR). The results of RT-PCR from VEC304 and human neutrophil treating with LPS showed that ICAM-1 and CD18 mRNA expressions were higher than those of normal cells, while this increased expression of ICAM-1 and CD18 mRNA was remarkably attenuated by the addition of EsA.
CONCLUSIONEsA was found to inhibit the increased adhesion rate induced by LPS. Moreover, LPS induced high expression of ICAM-1 and CD18 was inhibited with treatment of EsA. It might be involved in the mechanisms of anti-inflammation of EsA.

8.Effects of indoor air pollution on asthma and asthma-related symptoms among children in Shenyang city.

OBJECTIVETo study the effects of indoor air pollution and individual susceptible factors on prevalence of children's asthma and asthma-related symptoms in Shenyang city.
METHODSOn April, 2007, 8733 Han children who were under age of 12 and lived for more than 2 years in Shenyang city, were selected from five administrative areas (one primary school and two kindergartens for each area) through cluster random sampling method. Information on children's general condition, asthma and related symptoms (including stridor, stridor symptoms, persistent cough, persistent phlegm), indoor air pollution, and susceptibility history were obtained by a standard questionnaire from the American Thoracic Society. The effects of indoor air pollution on asthma and asthma-related symptoms was analyzed through χ(2) test. Logistic regression was used to research the effects of risk factors on the prevalence of asthma and asthma-related symptoms of both susceptible and non-susceptible children.
RESULTSAmong the 8733 subjects, 4420 (50.6%) were boy and 4313 (49.4%) were girl, with the age of (8.08 ± 2.88) years old. The prevalence of asthma, current asthma, cough, persistent phlegm, stridor and stridor symptom were 6.4% (559 cases), 2.5% (215 cases), 9.6% (836 cases), 4.4% (386 cases), 17.5% (1524 cases) and 2.6% (229 cases) respectively. The prevalence of asthma the boys and girls were among 7.1% (313 cases) and 5.7% (246 cases) (χ(2) = 6.916, P < 0.05); and stridor symptom for them were 19.2% (850 cases), 15.6% (674 cases) (χ(2) = 19.678, P < 0.05), respectively. Passive smoking before two years old, house decoration and pet were related to asthma of children, and there was significant difference between the two groups. The prevalence of asthma of exposed children were 7.7% (312 cases), 9.5% (159 cases), 8.0% (270 cases), 9.0% (114 cases), respectively. Compared with the non-exposed children who had asthma, the prevalence of asthma were 5.7% (400 cases), 5.4% (289 cases), 6.0% (445 cases), the value of χ(2) were 33.646, 23.944 and 16.527 respectively (all P values < 0.05). Children who had family history of asthma, family history of allergy and allergy history were also related with asthma, the prevalence of asthma were 17.3% (106 cases), 13.1% (85 cases), 22.0% (147 cases), compared with the non-exposed children who had asthma, the prevalence of asthma were 5.5% (453), 5.9% (474), 5.1% (412), and there was significant difference between the two groups, the value of χ(2) were 130.522, 59.929 and 293.997, respectively (all P values < 0.05). Logistic regression analysis showed that passive smoking (OR = 1.7, 95%CI: 1.2 - 2.4), house decoration (OR = 1.5, 95%CI: 1.1 - 1.9) and pet (OR = 1.6, 95%CI: 1.1 - 2.3) were statistically significant to asthma in non-susceptible children. While passive smoking (OR = 1.3, 95%CI: 1.0 - 1.7) and house decoration (OR = 1.4, 95%CI: 1.1 - 1.7) were increased the risk of asthma.
CONCLUSIONIndoor air pollution is a risk factor of children' s asthma. Family history of asthma and physical susceptible children are high risk to asthma, and susceptible children are easily influenced by other risk factors.

9.Perioperative depression and anxiety in patients undergoing acoustic neuroma resection

Objective To investigate the prevalence ofperioperative depression and anxiety in patients who underwent acoustic neuroma resection (ANR) and to analyze their associated factors.Methods This survey recruited 62 patients who accepted ANR from June 2010 through June 2011 in our department. Participants were interviewed one day preoperation and one day pre-discharge with 2questionnaires:the Zung self-rating depression scale (SDS) and the Zung self-rating anxiety scale (SAS).Personal information and clinical data were also documented. Facial nerve outcomes were reported according to the House-Brackmann scale.Statistical analysis was performed with the SPSS,version 13.0,using two-sample t-test,paired t-test,pearson Chi-square test and Binary classification unconditional logistic regression analysis.Significance was accepted at P<0.05. Results Thirteen patients (20.9%)had depression and/or anxiety preoperation while 24 patients (38.7%) had postoperation.Patients with a long preoperative hospital stay,young in age (Wald=5.958,OR=0.910,P=0.015) and associated with chronic diseases (Wald=6.578,OR=14.591,P=0.010) were prone to preoperative depression and/or anxiety.Female patients (Wald=4.258,OR =4.572,P=0.039),young patients (Wald=8.152,OR=0.913,P=0.004) and patients with facial paralysis of H-B grade Ⅳ-Ⅵ (Wald=6.310,OR=7.630,P=0.012) had a higher incidence of postoperative depression and/or anxiety.Occupation,educational level,economic status,hearing loss or balance dysfunction of the patients showed no statistically significant impact on the incidence of the condition. Conclusions Patients undergoing ANR tend to have a high incidence of perioperative depression and/or anxiety.Surgeons should pay special attention to and take effective measures for female,young patients and those with facial dysfunction because they,in particular,are prone to postoperative depression and/or anxiety.
10.The difference between the positive rate of anti-HCV in the patients with severe viral hepatitis detected by the first generation or second generation enzyme linked-immunosorbent assay.

OBJECTIVETo compare the positive rate of antibody to hepatitis C virus (anti-HCV) in sera of patients with severe viral hepatitis between 1984-1990 year and 1997-2003 year.
METHODSSerum anti-HCV was detected by enzyme linked-immunosorbent assay (ELISA). It was detected by the first generation (1st) ELISA (Ortho Co. USA) in 79 cases of severe viral hepatitis during 1984-1990 year, and it was detected by the second generation (2nd) ELISA (Xiamen Xingchuang Co. China) in 251 cases of severe viral hepatitis during 1997-2003 year.
RESULTSThe positive rate of serum anti-HCV was 51.9% detected by the 1st ELISA in 79 cases of severe viral hepatitis during 1984-1990 year, and it was 1.2% detected by the 2nd ELISA in 251 cases of severe hepatitis during 1997-2003 year (chi2 = 133.68, P = 0.001).
CONCLUSIONTo compare with the positive rate of serum antibody to hepatitis C virus in severe viral hepatitis detected by the 1st ELISA, it was lower that detected by the 2nd ELISA
