1.Clinicopathological Anlalysis of Isolated Hematuria in 23 Children

Objective To study the kidney histological categories of isolated hematuria in children.Methods Twenty-three children with isolated hematuria were performed renal biopsy under real time ultrasound guidance utilizing menghini style negative pressure biopsy device after local anesthesia or general anesthesia.The renopuncture tissue was directly sent by the mail-boxes to the remote pathologic service.All of the biopsies were examined by light microscopy,electron microscopy and immunohistochemistry.Results Biopsies were classified as measangial proliferative glomerulonephritis(MsPGN)(8 cases),minimal change nephropathy(MCN)(5 cases),IgA nephropathy(IgAN)(4 cases),thin basement membrane nephropathy(TMN)(3 cases),Alport′s syndrome(AS)(1 case),focal segmental glomerulosclerosis(FSGS)(1 case)and IgM nephropathy(IgMN)(1 case).Conclusions In this series,MsPGN,MGA,IgAN are the most common biopsy diagnosis.TMN and Alport′s are account for some proportion.A few IgMN and FSGS may also present as isolated hematuria.
2.Analysis of clinical effects of cervical artificial disc replacement or anterior cervical decompression and fusion for the treatment of single cervical disc herniation.

OBJECTIVETo explore the clinical effects of Mobi-C cervical artificial disc replacement (CADR) and anterior cervical decompression and fusion (ACDF) in treating single cervical disc herniation.
METHODSFrom June 2009 to June 2012, the clinical data of 27 patients with single cervical disc herniation were retrospectively analyzed. There were 18 males and 9 females, aged from 30 to 62 years old with an average of 46.7 years. Of them, 12 patients were treated with CADR (CADR group) and 15 patients with ACDF (ACDF group). All patients had pain and numbness in neck, shoulder and upper limbs, and courses of disease was from 1 to 13 months with an average of 2.4 months. The data of clinical evaluation and questionnaire survey about quality of life were collected before operation, postoperative at 1 week and final follow-up. Odom criterion was used to evaluate postoperative effect. Visual analogue scale (VAS) was used to record pain levels. Neck disability index (NDI) and health questionnaire SF-36 were used to assess the quality of life.
RESULTSNo complications about nerve and blood vessel were found and the patients were followed up from 6 to 30 months, with an average of 16 months. One week after operation, 10 cases got excellent results and 2 good in CADR group; 5 cases got excellent results and 10 good in ACDF group; there was significant difference between two groups (P<0.05). At final follow-up, 10 cases got excellent results and 2 good in CADR group; 12 cases got excellent results and 3 good in ACDF group; there was no significant difference between two groups (P> 0.05). Pain of upper limbs had obviously relieved between two groups at 1 week after operation and final follow-up (P<0.05). VAS of neck and NDI in CADR group had decreased respectively from preoperative 3.58±0.79, 23.42±6.36 to 0.58±0.51, 5.42±1.68 at 1 week after operation (P<0.05); but the index in ACDF group was no obvious at 1 week after operation. At final follow-up, VAS of neck and NDI and SF-36 score were obviously improved than preoperation (P<0.05) between two groups.
CONCLUSIONMobi-C CADR retains the movement unit in the decompression segment and can quickly recover normal action for patients. Using CADR method has a good curative effect in the early phase, and the clinical effect is reliable, may improve the quality of life.

3.Clinical observation on the relation between the confident degree of the patient with pain in acupuncture and moxibustion with the therapeutic effect.

OBJECTIVETo understand preliminary relation between patient's confident degree in acupuncture and moxibustion with the therapeutic effect on pain.
METHODSFifteen cases with pain were investigated. The confident degree for acupuncture and moxibustion and pain degree were evaluated by the patient with visual analogue scale (VAS), and the pain VAS scores before and after acupuncture were recorded as the indexes for assessment of the therapeutic effect.
RESULTSAfter acupuncture treatment, the average pain VAS score was decreased by 3.3 +/- 2.8 (P< 0 . 5). There was no significant correlativity between the patient's confident degree VAS and the pain VAS change rate before and after the treatment (R = 0.62, P = 0.83).
CONCLUSIONThere is no significant correlativity between the confident degree of the patient of pain in acupuncture and moxibustion and clinical therapeutic effect.

4.Quality Study on Cinnabar-analysis of Dyeing Status for Cinnabar

OBJECTIVE:To analyze the dyeing status of cinnabar and its pieces,and provide reference for its quality clinical safetey appicaton. METHODS:TLC was used for the qualitative identification of amaranth,carmine,erythrosine,acid red 73, 808 udan and indirubin. HPLC-MS was used to detect the 808 udan :HPLC conditions were as follows,column was Acquity UPLC BEH C18 with mobile phase of cetonitrile-0.1% formic acid(70∶30,V/V)at a flow rate of 0.3 ml/min,the detection wave-length was 520 nm;MS conditions were as follows,ion source was electrospray ionization source,scanning mode was positive ion scanning with full scanning tandem mass spectrometry,nebulizer pressure was 30 psi,drying gas was nitrogen,ion spray voltage was 4 000 V,collision energy was 30 V,and the injection volume was 5 μl. The volumetric method was used for content determi-nation of HgS. RESULTS:TLC spots of amaranth,carmine,erythrosine,acid red 73,808 udan and indirubin were clear and well-separated. 4 batches of 808 udan dyeing were included in the 18 batches of samples,6 batches had non-compliance contents (including 3 batches of 808 udan dyeing). CONCLUSIONS:Dyeing-doped and other quality problems exist in the cinnabaris in markets,which should be noticed.
5.Significance of urinary protein in evaluating the damage degree of diseased kidneys in children with hydronephrosis

Objective To evaluate the damage degree of diseased kidneys in children with hydrone- phrosis using urinary?2-microglobulin(?2-MG),albumin(Alb),T-H glycoprotein(THP)and immuno- globulin(IgG).Methods The?2-MG,Alb,THP and IgG in pelvis urine in diseased kidneys and in healthy kidneys(as controls)were detected in 78 children with congenital hydronephrosis by radioimmuno- assay;at the same time urinary creatinine(Cr)was detected in 36 cases of them.Pathologic changes of the diseased kidneys were graded;and the correlations of pathologic grades with?2-MG,Alb,THP,IgG and?2-MG/Cr,Alb/Cr,THP/Cr,IgG/Cr were analyzed by Spearman's test.Results In healthy and hydro- nephrotic pelvises,the relevant parameters were as follows.Urine?2-MG was(130.06?22.02)ng/ml vs(386.18?42.15)ng/ml;Alb was(80.52?9.32)ng/ml vs(275.54?69.46)ng/ml,respectively (both P<0.05).Urine?2-MG/Cr was(0.16?0.04)ng/?mol vs(1.57?0.39)ng/?mol;Alb/Cr was (0.11?0.03)ng/?mol vs(1.07?0.56)ng/?mol(both P<0.01).THP was(25.80?9.02)ng/ml vs(31.43?10.56)ng/ml;and IgG was(15.21?3.56)ng/ml vs(20.52?10.85)ng/ml(P>0.05). THP/Cr was(0.05?0.01)ng/?mol vs(0.14?0.06)ng/?mol;and IgG/Cr was(0.04?0.01)ng/?mol vs(0.12?0.05)ng/?mol(P<0.05).There was significant correlation between urine?2-MG,Alb lev- els and pathologic grades(P<0.05),also between?2-MG/Cr,Alb/Cr levels and pathologic grades(P<0.01).There was positive correlation between THP,IgG and pathologic grades but with no significance(P>0.05),also between THP/Cr,IgG/Cr and pathologic grades with significance(P<0.05).Conclu- sions For hydronephrotic kidneys,urine?2-MG/Cr can evaluate the damage degree of nephric tubules, whereas urine Alb/Cr can evaluate that of glomeruli.Increase in THP/Cr and IgG/Cr respectively indicate the impaired nephric tubules and glomeruli in children with congenital hydronephrosis.

8.A clinical observation of combined application of magnetic attachments and extracoronal attachments.

OBJECTIVETo explore the clinical effect of combined application of magnetic attachments and extracoronal attachments in prosthodontics.
METHODSTwenty-two cases of dentition defect with isolated residual root or residual crown were selected. All the cases accepted the restorative treatment combined magnetic attachments with extracoronal attachments. The clinical effect of dentures and the condition of abutment teeth were evaluated by chief complaint of patients and clinical examination. The follow-up time ranged from 2 years to 4 years.
RESULTSSatisfactory functional, stable and esthetic results of dentures were achieved for all the cases. And there was no abutment loosening and no secondary caries. Alveolar bone loss around abutment occurred in 1 case, gingivitis of abutments appeared in 5 cases. Chewing pain occurred in 1 case and unstable state of dentures appeared in 5 cases after the use of denture for 2 years. The dentures of above-mentioned cases were used normally after symptomatic treatment.
CONCLUSIONThe combined application of magnetic attachments and extracoronal attachments in prosthodontics is an effective treatment option for cases of dentition defect with isolated residual root or residual crown.

9.Application of continuous blood purification combined with hemoperfusion in the treatment of hemolytic-uremic syndrome in children.

OBJECTIVETo observe the therapeutic effect of continuous blood purification (CBP) combined with hemoperfusion (HP) in children with hemolytic-uremic syndrome (HUS) and to investigate its possible mechanism.
METHODSEight children with HUS received CBP combined with HP on the basis of internal medicine treatment in the acute stage. Before and after treatment, serum levels of interleukin-6 (IL-6), IL-8 and tumor necrosis factor-α (TNF-α) were measured by chemiluminescence method, and levels of blood urea nitrogen (BUN), serum creatinine (SCr), alanine aminotransferase (ALT), aspartate aminotransferase (AST), creatine kinase (CK), creatine kinase MB (CKMB), hemoglobin (Hb), platelet (PLT) and C-reactive protein (CRP) were measured. Eight healthy children undergoing physical examination were used as controls.
RESULTSThe 8 children with HUS all survived after CBP combined with HP and showed improved conditions. They had increased Hb and PLT levels and decreased serum levels of IL-6, IL-8, TNF-α, BUN, SCr, ALT, CK and CRP after treatment (P<0.05).
CONCLUSIONSCBP combined with HP can quickly remove pathogenic factors, continually eliminate inflammatory mediators and toxins, and reverse multiple organ dysfunction, and is one of effective methods for treating HUS in children.

10.Repair of finger soft tissue defects using axial pattern flaps.

OBJECTIVETo investigate the feasibility and effect of different types of axial pattern flaps in repairing soft tissue defects of the fingers.
METHODSFive types of axial pattern flaps were used to repair soft tissue defects of the fingers of 30 patients admitted to the Second Hospital of Shaoxing Municipality from 2005 to 2010, including distally-based dorsal thumb neurocutaneous vascular flaps in 4 cases, free flaps from the fibular side of the great toe in 6 cases, modified retrograde dorsal metacarpal artery flaps in 8 cases, free flaps based on the radial dorsal septum muscular perforator of the posterior interosseous artery in 6 cases, and free posterior interosseous artery flaps carrying a long segment of the posterior interosseous artery in 6 cases. The flap size ranged from 2.5 cm × 2.0 cm to 8.0 cm × 3.0 cm. The donor sites were sutured directly or covered with skin graft. The condition of flaps was observed. The local feeling, function, and appearance of affected hands were followed up.
RESULTSTwenty-seven flaps grew well without infection. Partial necrosis at the distal portion in two flaps and venous congestion in 1 flap were observed after surgery, and they were healed with suitable treatment. Eighteen patients were followed up for 1 to 12 months. The flaps were found to have good color, texture, and thickness. Satisfactory appearance and function both in the donor sites and in the recipient areas were found in most patients. The sense of two-point discrimination of repaired fingers ranged from 5 to 9 mm.
CONCLUSIONSOptimizing the repair method for soft tissue defects in the fingers by taking both the characteristics of various axial pattern flaps and the specific conditions of soft tissue defects into account can achieve expected effect.
