2.The diagnostic value of neutrophil-to-lymphocyte rate (NLR) in acute pancreatitis related acute kidney injury patients

Objective To investigate the diagnostic and prognostic value of neutrophil-to-lymphocyte rate ( NLR) in acute pancreatitis related acute kidney injury patients .Methods Peripheral blood specimens and clinical information of 98 acute pancreatitis patients in Wenzhou Center Hospital were collected .The WBC, neutrophils , lymphocytes were detected and NLR were calculated when they were admitted .The patients were divided into two groups by their NLR and whether they were with AKI respectively .Besides, ALT, Hct, TC, TG, blood calcium concentration , serum creatine and urea nitrogen , C reactive protein were detected and patients′APACHEⅡ score were also recorded to analyze the difference between the two groups .Results There is no significance in the age , BMI, Hct, TG, TC, ALT and blood calcium between AP patients with and without AKI.The blood creatinine, BUN, CRP, APACHEⅡscore were (395 ±122)μmol/L, (28.2 ±5.2) mmol/L, (34.0 ±8.2)mg/L, (11.5 ±3.8) score, respectively in AP patients with AKI, and which were (79 ±17 )μmol/L, ( 7.3 ±2.0 ) mmol/L, ( 14.8 ±2.9 ) mg/L, ( 6.9 ±2.4 ) score, respectively in AP patients without AKI.The blood ALT, blood creatinine, BUN, CRP, APACHEⅡscore were (257 ±76)U/L, (159 ±62)μmol/L, (20.5 ±6.6)mmol/L, (24.8 ±5.5)mg/L and (12.4 ±4.6) score in the patients with higher NLR respectively , and which were ( 165 ±30 ) U/L, ( 98 ±23 )μmol/L, ( 14.3 ±5.2 ) mmol/L, (19.5 ±3.0)mg/L and (5.4 ±2.1) score in the patients with lower NLR respectively .NLR was 4.97 ±0.19 in AP patients with AKI, and was 9.62 ±0.81 in AP patients without AKI.The difference between the two groups was significant(P=0.0001).The area under ROC curve of diagnosing AP by NLR was 0.895 (95%CI 5.75).the sensitivity was 89.5%and the specificity was 77.2% when using 5.75 as the cut-off value to diagnose AP related AKI with NLR .Conclusions NLR can be a potential predictive index of the severity and relate to renal function in acute pancreatitis related acute kidney injury patients .

Objective To compare and quantify the determinants in quantitative electroencephalogram(q-EEG) and heart rate variability power spectrum analysis(HRV-PSA) of ketamme(KTM) anesthesia for preschoolers. Methods Seventy four cases were selected and assigned into 3 groups named A(4-5 years), B(5-6 years), C(6-7 years), 22,28,24 cases in every group respectively. All cases were induced with KTM 5 mg /kg intramuscularly and changes of determinants were recorded continuously. If body movement happened, KTM would be injected with 1 mg/kg. 4.The analysis of outcome of modified Manipal tricuspid annuloplasty

Objective To analyze comparatively the outcome of modified Manipal and DeVega tricuspid annuloplaaty.Methods From Oct.2001 to Aug.2004,the consecutive 123 patients operated with modified Manipal tricuspid annuloplasty for tricuspid re- gurgitation at the time of left cardiac valve replacement(group A)were elected in this study.The other 174 patients operated with De Vega tricuspid annuloplasty at the time of left cardiac valve replacement were elected randomly for control(group B).There were no significant differences of the patient data before surgery between two groups.Results There were overall 11 early deaths(4 in group A,7 in group B),The overall in-hospital mortality rate was 3.7 %.215 of the 286 surviving patients were followed;the overall follow up was 75.2 %.Mean follow-up was(28.4?9.1)months(range from 13 to 49 months).There were ten late deaths and the late mortality rate was 4.7%.There was no significant differences in the patients with 3+~4+ tricuspid regurgitation at follow-up 18,30 and 42 months in group A;but there was an increase of the patients with 3+~4+ tricuspid regurgitation with an incremental follow up term in group B(P
5.Diagnosis and surgical management of carotid body tumor as well as blood vessel prosthesis' role.

OBJECTIVETo analyse the diagnostic and therapeutic aspects of carotid body tumor (CBT).
METHODSSeven patients with CBT had been hospitalized between 2003 and 2006. The clinical data was analyzed retrospectively. The preoperative evaluation included angiography in 7 patients. Most of them had an asymptomatic cervical lateral mass. Only one patient had the hoarseness and buckling and was given radiation therapy alone. Six of seven patients with carotid body tumour underwent surgery. Simple tumor excision was accomplished in 4. Carotid artery resection with the tumor was required in 2 patients and in the both, interposition of a 7 mm polytetrafluoroethylene graft was performed . During the resection, temporary carotid shunt was required in the two patients.
RESULTSAll tumors by surgery were identified as carotid paragangliomas without evidence of malignancy. There was no mortality and no hemiplegia. After surgery, temporary cranial nerve dysfunction was noted in one case. In the follow-up period of 2 months to 2 years, no recurrent disease occurred. The patient's tumor who accepted radiotherapy was in the stable stage under the half year follow up, and the follow up would be further continued.
CONCLUSIONSWith non-invasive investigation and arteriography it was possible to obtain an early and precise diagnosis. The surgical management was the major treatment of these tumors. The pattern of operation should be chosen according to the relation of tumor and carotid. The decision to perform simple tumor excision or additional arterial resection was based on diagnostic preoperative and after the arterial resection the polytetrafluoroethylene graft would be used for carotid reconstruction.

6.Effects of xiongshao capsule combined with ischemic postconditioning on monocyte chemoattractant protein-1 and tumor necrosis factor-alpha in rat myocardium with ischemic reperfusion injury.

OBJECTIVETo investigate effect of Xiongshao Capsule (XSC) combined with ischemic postconditioning (IPoC) on tumor necrosis factor-alpha (TNF-alpha) and monocyte chemoattractant protein-1 (MCP-1) contents as well as inflammatory cell infiltration (ICI) in myocardium of rat with ischemic reperfusion (I/R) injury.
METHODSSeventy-five Sprague-Dawley rats were equally randomized into 5 groups, the sham-operated group (A), the I/R group (B), the IPoC group (C), the fosinopril sodium plus IPoC group (D), and the XSC plus IPoC group (E). Excepting rats in Group A, all animals received I/R injury through a 30-min occlusion of left anterior descending artery followed by 1-h reperfusion. Additionally, IPoC (3 cycles of 10 s reperfusion/10 s of ischemia) was applied on rats in Group C before 1 h of reperfusion; while rats in Groups D and E were pretreated for 14 days with 0.9 mg/kg fosinopril sodium and 0.135 g/kg XSC respectively via gastrogavage, and the I/R injury with IPoC applied 2 h after the final gavage. Serum creatine kinase-MB (CK-MB) and cardiac troponin T (cTnT) levels were detected by colorimetric method, myocardial infarction size was measured by nitro blue tetrazolium chloride (NBT) staining, MCP-1 and TNF-alpha contents in myocardial tissue were examined by enzyme-linked immunosorbent assay (ELISA), and ICI was detected by HE staining.
RESULTSCompared with Group B, myocardial enzymes and infarction size were significantly decreased (P<0.01), contents of MCP-1, TNF-alpha and ICI in myocardial tissue were significantly decreased (P<0.05, P<0.01) in Group C. Compared with Group C, further reduced infarction size and release of myocardial enzyme CK-MB (P<0.01) were seen in Group E, and contents of MCP-1 and TNF-alpha as well as ICI in myocardial tissue in Group E were also significantly lower (P<0.05, P<0.01).
CONCLUSIONXSC could enhance the protective effect of IPoC on rat with myocardial I/R injury, and the mechanism may be related to its inhibition on MCP-1 and TNF-alpha expressions as well as ICI suppression.

7.Anatomic reduction of midfacial sag after intraoral reduction malarplasty.

OBJECTIVETo study the technical refinements of intraoral reduction malarplasty to prevent postoperative drooping of the cheek.
METHODSTwenty-four patients with prominent zygomatic complex underwent reduction malarplasty from January 2005 to January 2006. In all these cases, the osteotomized malar complex was repositioned superioposteriorly, and the perioral muscles and periosteum were redraped accordingly and fixed on deep temporal fascia.
RESULTSSymmetry between the left and right complexer were achieved, with a decreased midfacial width of 10.22-/+1.97 mm. During postoperative follow-up of 4 to 6 months, all the patients had satisfactory operative results, and reported no postoperative drooping of cheek.
CONCLUSIONThis method of superioposterior reduction of the perioral muscles and periosteum is simple and effective, but the long-term result needs to be observed.

8.Changes of subthalamic nucleus and cortex activity in rat during exhausting exercise.

OBJECTIVETo observe the modulatory effect of subthalamic nucleus (STN) on activity of motor cortex during exhausting exercise.
METHODSElectrocorticogram (ECoG) and local field potentials (LFPs) recording techniques were applied simultaneously to observe the dynamic changes of oscillations in sensorimotor area and STN of rat during exhausting exercise.
RESULTSRats ran well initiatively with treadmill at the beginning of the exercise, about 45 min (45 +/- 11.5) later, movement capacity reduced. Corresponding electrical property showed that STN activity increased significantly while activity of cortex decreased significantly. Subsequently rats continued exercise with minor external stimulation utill exhaustion. Activity of ECoG reached the minimum under exhausting stations (P < 0.01), while the activity of LFPs changed insignificantly (P > 0.05).
CONCLUSIONDuring the exhausting exercise, the cortex activity was extensively depressed with the development of fatigue, while the activity of STN increased significantly at the early stage of fatigue, STN took part in the modulation of central fatigue through negative induction. And the increase of STN activity may be one of the key measures accounting for protective inhibition.

10.Prevention of injury in the frontal branch of facial nerve with improved zygomatic reduction

Objective To study the improved zygomatic reduction in the prevention of injury in the frontal branch of facial nerve.Methods Sixty-two patients with protrusion zygomatic complex,received treatment in our hospital from March 2008 to April 2010,were chosen in our study; 32 were given improved zygomatic reduction,and the other 30 were performed traditional zygomatic reduction.The clinical data of these patients,injury of the frontal branch of facial nerve and incidence of complication in these patients were compared between the 2 groups.Results No one suffered from injury of the facial nerve in the 32 patients given improved zygomatic reduction,but 3 suffered from nerve injury (mid injury in 2 and heavy injury in 1) in patients received traditional zygomatic reduction; 3 patients could not move the eyebow,2 suffered from sensory disability,and 1 had no wrinkle of forehead in patients received traditional zygomatic reduction.Patients given improved zygomatic reduction did not have the above complications.Conclusion Improved zygomatic reduction can effectively prevent injury in the frontal branch of facial nerve.
