1.Anatomical variations of the stylopharyngeus and superior constrictors in relation to their function

The aims of this study were to clarify the topography and variations stylopharyngeus (STP) and superior constrictor (SC) muscles, and to examine what role they play in the pharyngeal movement. Forty-four specimens (22 right and 22 left sides) from embalmed Korean adult cadavers (13 males, 9 females; age range, 46–89 years; mean age, 69.2 years) were used in this study. The accessory bundle of STP and petropharyngeus was found in 18.2% (8/44) and 25.0% (11/44) of cases, respectively. A variation of the STP, in which it ran transversely and merged with the SC muscle, was found in 2.3% (1/44) of cases while a variation of the SC muscle, in which it ran longitudinally and merged with the contralateral constrictors, was found in 11.4% (5/44). The variant muscle bundles play their own role in pharyngeal movement according to their morphology. These results provide information that will help a comprehensive understanding of the effects of pharyngeal muscles on movement.
2.Anatomic Variation of the Anterior Belly of Digastric Muscle and Positional Relationship between the Posterior Belly of Digastric and Stylohyoid Muscle.

The digastric muscle is located in the suprahyoid region which consists of anterior belly, intermediate tendon and posterior belly. This muscle is an important landmark when performing an operation of submental and upper neck region. Previous researches have reported about variations of the anterior and posterior belly of digastric muscle. However, there are few studies about the general morphology of the digastric muscle and the relationship with surrounding muscles. The purpose of this study was to analyze the morphology of the anterior belly of digastric muscle and confirm the topographic relationship between the digastric muscle and the stylohyoid muscle of Korean. Thirty-four cadavers (21 males, 13 females; mean age 65 years; range 24~89 years) were used in this study. The skin, subcutaneous tissues, superficial fascia and platysma were removed and a detailed dissection was performed, with extreme care being taken not to damage underlying the muscles of submental and upper neck region. After the dissections, all specimens were sketched and photographed. In 8 specimens, we observed the accessory bellies of the anterior belly of digastric muscle. We classified the accessory bellies into the crossover type (five specimens, 14.7%) that cross the mandibular raphe and unilateral type (three specimens, 8.82%). The findings resulting from observation of the anatomical relationship between the posterior belly of digastric and stylohyoid muscles, the posterior belly of digastric muscle perforated the stylohyoid muscle in 32 cases (65%) out of 49 sides. This case was subdivided into the belly of the stylohyoid muscle lean to the lateral (twenty-one specimens, 42.9%) or medial side (eleven specimens, 22.4%) of the posterior belly of digastric muscle. In 17 specimens (35%), the stylohyoid muscle existed on the medial side of the posterior belly of digastric muscle.

4.A Study on Nutritional Status, Maternal Factors, and Lifestyles according to BMD in Rural Postmenopausal Women.

Osteoporosis and other related conditions pose a growing public health problem, especially in postmenopausal women. The main purpose of the study was to investigate the correlations among BMD, maternal factors, and life styles, and intake of nutrients in postmenopausal women. One hundred participants in Kyungge-do were divided into three groups according to their BMD measurements measured by DXA. Dietary analysis, anthropometric measurements, and questionares were administered to these women. The percentage of the osteoporosis, osteopenia(Osteopinia), and normal groups were 32%, 48%, and 20% respectively. The average age was significantly the highest in the osteoporosis group. The average age at menopause was 47.2. Osteoporosis group's age at menopause was significantly the lowest. The sleeping hours of the osteoporosis and osteopenia group were significantly longer than the normal group. The intake of vitamin B2 was positively correlated with the BMD of femoral neck. The BMD of these two sites was positively correlated with weight, BMI, waist, and hip size and negatively correlated with the length of the menstrual cycle, duration after menopause, the age at the last delivery, and sleeping hours. Spinal BMD positively correlated with hours of outdoor activity. Therefore, maternal factors, lifestyles, and intake of nutrients contribute to BMD.

5.Reference line-pair values of panoramic radiographs using an arch-form phantom stand to assess clinical image quality.

PURPOSE: This study was performed to suggest reference line-pair values of panoramic images with clinically desirable qualities using an arch-form phantom stand. MATERIALS AND METHODS: The line-pair test phantom was chosen. A real skull model was selected for setting the arch-form model of the phantom stand. The phantom stand had slits in four regions (incisor, premolar, molar, TMJ). Four raw images of the test phantom in each region and one raw image of the real skull were converted into 50 test phantom images and 50 skull phantom images with various line-pair values. 50 post-processed real skull phantom images were divided into 4 groups and were randomly submitted to 14 evaluators. Image quality was graded on a 4 point scale (1. good, 2. normal, 3. poor but interpretable, and 4. not interpretable). The reference line pair was determined as the first line-pair value scored less than 2 points. RESULT: The mean scores tended to decrease as the line-pair values increased. The reference line-pair values were 3.19 LP/mm in the incisor, 2.32 LP/mm in the premolar and TMJ, and 1.88 LP/mm in the molar region. CONCLUSION: Image quality evaluation methods and criteria should be able to assess various regions considering the characteristics of panoramic systems. This study suggested overall and regional reference line-pair values and established a set of standard values for them.

6.Vitrectomy and Ahmed Valve Implantation in Neovascular Glaucoma Patients with Vitreous Hemorrhage.

PURPOSE: To compare the surgical outcomes between sequential -and simultaneous combined vitrectomy and Ahmed valve implantation (AVI) in neovascular glaucoma (NVG) patients with vitreous hemorrhage. METHODS: The medical records of 22 eyes of 22 patients, who had NVG with vitreous hemorrhage treated with vitrectomy and AVI, were retrospectively reviewed. Surgical success was defined as 6 mm Hg < or = IOP < or = 21 mm Hg, with or without the use of antiglaucoma medications and failure was defined as cases that had no light perception during the study period and which required additional surgery. The authors of the present study evaluated the surgical success rates and factors affecting surgical success between sequential and simultaneous combined vitrectomy and AVI. RESULTS: The cumulative surgical success rate by the Kaplan-Meier survival analysis was 45% in sequential combined vitrectomy and AVI (group1), and 18% in combined vitrectomy and AVI (group 2), at 1 year, a significant difference. Preoperative panretinal photocoagulation was related to surgical success rate by Cox's regression model analysis. CONCLUSIONS: Simultaneous combined vitrectomy and AVI is considered as a primary procedure in a patient who has NVG with vitreous hemorrhage. Panretinal photocoagulation in the preoperative periods, is thought to increase the surgical success rate.

7.A Evaluation Study on Nutrient Intake Status and Diet Quality of Middle and Old Aged Vegetarian Women in Korea.

The purpose of this study was to evaluate on nutrient intake status and diet quality of middle and old aged vegetarian women in Korea. The research group was composed of vegetarian women (n = 91), all of them were Seven Day Adventists, who had been on vegetarian diets, over 20 yrs. Their anthropometric measurements, dietary intakes, and diet quality indices were compared to omnivores (n = 122). The average age of vegetarians and omnivores were 61.8 yrs and 60.3 yrs, respectively. The mean daily energy intakes of vegetarians and omnivores were 1428.8 kcal and 1424.5 kcal, respectively and there was no significant difference. The vegetarians consumed significantly lower intakes of protein (p<0.05), zinc (p<0.001) and vitamin B2 (p<0.05) compared to omnivores. In the diet quality, zinc and vitamin B2 nutrient adequacy ratio (NAR) of vegetarians were significantly lower than those of omnivores. Mean adequacy ratio (MAR) of vegetarians and omnivores were 0.80 and 0.77, respectively and there was no significant difference. The KDDS (Korean's Dietary Diversity Score) of vegetarians and omnivores were 3.7 and 4.0, respectively, and there was significant difference (p<0.01). The KDQI (Korean's Diet Quality Index) of vegetarians (1.5) was significantly lower than that (2.1) of omnivores (p<0.001). In conclusion, vegetarian women have low intake status of protein, zinc and vitamin B2, and partly low diet quality. Therefore it was needed that well planed diets to replace the nutrients supplied from excluded food groups in middle and old aged vegetarian women.

8.Internet Addiction in Adolescents and its Relation to Sleep and Depression.

OBJECTIVES: "Internet-addiction" came into common use not only in clinical setting but also in everyday life. But, pathophysiology and diagnostic criteria of the internet addiction remain unknown. Because adolescents are in developing period, they might be vulnerable to the internet addiction, depression and sleep-related problem. The objectives of this study were to investigate the characteristics of internet addiction and its association with sleep pattern and depression in Korean adolescence. METHODS: Subjects were 799 middle and high school students in Seoul, Korea. We administered a self-reported questionnaire including socio-demographic data, Korean versions of Young's Internet Addiction Scale (YIAS), Pittsburgh Sleep Quality Index (PSQI), the Center for Epidemiologic Studies for Depression Scale (CES-D) and questions about internet using patterns. Data of 696 subjects were included in analysis. Chi-square tests were used to analyze proportional differences, and ANOVA with post-hoc tests were used to analyze differences among groups. Partial correlation analyses were performed to analyze the correlation of internet addiction with other variables (two-tailed, p<0.05). RESULTS: Of the 696 participants (grade 2 of middle school;M2 135 vs. grade 1 of high school;H1 238 vs. grade 2 of high school;H2 323), 2.0% (n=14) were internet-addicted (IA), 27.7% (n=193) were over-using (OU) and 70.3% (n=489) were not-addicted (NA). The mean scores of YIAS, PSQI and CES-D scores were 35.24+/-12.78, 5.53+/-3.04 and 16.72+/-8.69, respectively. In higher grade students, average total sleep time was shorter (M2 426.20+/-67.68 min. vs. H1 380.47+/-62.57 min. vs. H2 354.67+/-73.37 min., F=51.909, p<0.001), and PSQI (4.69+/-3.14 vs. 5.42+/-3.15 vs. 5.97+/-2.83, F=8.871, p<0.001) CES-D (13.53+/-8.37 vs. 16.96+/-8.24 vs. 17.87+/-8.84, F=12.373, p<0.001) scores were higher than those of lower grade students. Comparing variables among IA, OU and NA groups, computer using time not for study (96.36+/-63.31 min. vs. 134.92+/-86.79 min. vs. 213.57+/-136.87 min., F=34.287, p<0.001) and portable device using time not for study (84.22+/-79.11 min. vs. 96.97+/-91.89 min. vs. 152.31+/- 93.64 min., F=5.400, p=0.005) were different among groups. PSQI (5.26+/-2.97 vs. 6.08+/-2.97 vs. 7.50+/-4.41, F=8.218, p<0.001) and CES-D scores (15.40+/-8.08 vs. 19.05+/-8.42 vs. 30.43+/-13.69, F=32.692, p<0.001) were also different among groups. YIAS score were correlated with computer using time not for study (r=0.356, p<0.001) and portable device using time not for study (r=0.136, p<0.001). PSQI score (r=0.237, p<0.001) and CES-D score (r=0.332, p<0.001). YIAS score and PSQI score (r=0.131, p=0.001), YIAS and CES-D score (r=0.265, p<0.001), PSQI score and CES-D score (r=0.357, p<0.001) were correlated each other. CONCLUSION: These results suggested that adolescents' internet-addiction was correlated with not only computer and portable device using time not for study but also depression and sleep-related problems. We should pay attention to depression and sleep-related problems, when evaluating internet-addiction in adolescents.

9.The Effects of Dexmedetomidine on Hemodynamics and Plasma Catecholamine Concentrations during Coronary Artery Bypass Graft Surgery.

BACKGROUND: Alpha-2 adrenergic agonists decrease the sympathetic tones of hemodynamic and endocrine responses to surgical stimuli. The goal of this study was to evaluate the hemodynamic and endocrinologic effects of intraoperatively administered dexmedetomidine, a highly selective alpha-2 adrenergic agonist, in patients undergoing coronary artery bypass graft surgery. METHODS: Fifty coronary artery bypass graft patients, in a double-blind, randomized manner, received a continuous infusion of normal saline placebo or dexmedetomidine, 10 ng/kg/min for 1 hr from 5 minutes after sternotomy. All patients received standardized anesthesia. Plasma catecholamine was measured at 5 minutes after sternotomy (T1), 30 minutes after drug infusion (T2), and 30 minutes after drug cessation (T3). Heart rate, blood pressure, pulmonary artery pressure and cardiac output were monitored. RESULTS: Compared with the placebo group, plasma norepinephrine concentrations decreased in the dexmedetomidine group Dexmedetomidine attenuated increased blood pressure on surgical stimuli and induced more hypotension. CONCLUSIONS: The intraoperative infusion of dexmedetomidine to patients undergoing coronary artery bypass graft surgery reduced sympathetic tone and attenuated hemodynamic responses to surgical stimuli.

10.Primary Central Nervous System Involvement in Peripheral T-Cell Lymphoma:A Case Report

Primary central nervous system T-cell lymphoma (PCNSTL) is an extremely rare type of brain tumor. There are only few reports on the imaging findings of patients with PCNSTL. Herein, we report the imaging findings of a patient with peripheral T-cell lymphoma-not otherwise specified that presented with numerous small nodular and patchy strongly enhancing lesions on MRI.
