1.Myocardial protective effects of“integrated strategy”with Na~+/H~+ exchange inhibitor (HOE-642) plus modified K/Mg cold blood cardioplegia on canine heart grafts preserved

Objective To investigate the myocardial protective effects of“integrated strategy”with HOE-642 plus modified K/Mg on canine heart grafts.Methods Sixteen donors-recipient pairs of adult canines were divided into the“integrated strategy”group,i.e.,donor hearts were protected with HOE-642 plus K/Mg(IS,n=8)and the routine method(RM,n=8)group.Both groups un- derwent hypothermia stored for 4 h,followed by orthotopic transplantation.Cardiac heamodynamics was measured and myocardial ultrastructure and myocardial water content were evaluated.Results One of 8 animal died of aortic anastomoses leakage in each group,and the rest could wean from car- diopulmonary bypass successfully.Preservation of left ventricular contractility,as well as left ventric- ular compliance was significantly better in the IS group than in the RM group(P<0.05),also,per- centage of myocardial water content was significantly lower in the IS group than in the RM group(P<0.05).Myocardial ultrastructures were not damaged significantly in both groups,however,better myocardial preservation were seen in the IS group.Conclusion Both the clinically used“routine meth- od”and“integrated strategy”might provide safe donor heart preservation for 4 h,but the later scheme conferred more optimal effects with alleviating the myocardial swelling,preserving myocardial ultra- structure integrity and enhancing postischemic-reperfusion cardiac functional recovery.
2.Locating the displacement of the steel wire implantation with the stereotactic mammography

Objective To analyze the manifestation,reason,the processing method of the steel wire implantation with the sereotactic mammography to improve the accuracy of the preoperative positioning.Methods Seventy-nine cases which got the stereotactic steel wire implantation.In 96 lesions, 13 had steel wire displacement.Among them,5 cases got steel wire displacement during the sereotactic process,5 cases got steel wire displacement after the stereotactic process,2 cases got steel wire displacement during the operation,one case did not show the calcification on the postoperative radiography.Results The steel wire displacement occurred in 5 cases during the stereotactic process came from the patients and doctors respectively and the repositioning was needed.The steel wire displacement after the stereoscopic positioning was attributed to the overdose injection of local anesthesia,which led to the mismatch between the depth of Z axis of the mammary gland and the actual depth the computer given,the incorrect method for needle placement,and,neglecting whether the steel wire have got the lesion anchored when pulling out the needle set of steel wire hood,besides,these three kinds of instances above were all exaggerated by the accordion effect.For the displacement within 2 cm,the lesion can be excised toward the pathological change direction according to the position that steel wire prompted and re-place the second steel wire,putting the J-shaped steel wire into the needle hood and taking it out of the body.After repositioning,2 cases had the steel wire prolapse during operation,which resulted from the over-lifting of the steel wire.After placing the steel wire, the radiologist should give an accurate description on the depth and direction to the surgeon and the notch should be taken for incision from the steel wire head end which is proximate to skin.The postoperative specimen from one case had no calcification,which might be related to the condition that the calcification was located in the gland body,which got destruction from the surgical electrical electrotome.The excisionscope should be extended and the short term reexamination is recommended to make sure the complete excision of the calcification.Conclusion It is the gold standard method that implanting the steel wire with the stereotactic mammography to guide the surgical dissecting technique to diagnose non-palpable breast lesion(NPBL).Thorough understanding of the displacement manifestation of implanting steel wire with stereotactic technique and the treatment methods will be helpful in the surgical dissecting guidance.
3.Efficacy of Chinese Herbs for Supplementing Qi and Activating Blood Circulation on Patients with Acute Coronary Syndrome and Type 2 Diabetes Mellitus after Percutaneous Coronary Intervention: a Clinical Observation.

OBJECTIVETo observe the efficacy of Chinese herbs for supplementing qi and activating blood circulation (CHSQABC) on patients with acute coronary syndrome (ACS) and type 2 diabetes mellitus (DM) after successful percutaneous coronary intervention (PCI).
METHODSIn this ChiCTR-TRC-00000021, a total of 281 ACS patients complicated with type 2 DM after successful PCI were randomly assigned to the Western medicine treatment group (the control group, treated by routine Western medicine treatment) and the combined treatment group (the treatment group, treated by CHSQABC + routine Western medicine treatment). Patients in the combined treatment group took Xinyue Capsule (2 pills each time, 3 times per day) and Compound Chuanxiong Capsule (2 pills each time, 3 times per day for half a year and 1-year follow-ups). Primary endpoints covered incidence of death, nonfatal myocardial infarction (MI), ischemia-driven revascularization, and secondary endpoints included stroke, heart failure, and rehospitalization for ACS. At the same time scores for blood stasis syndrome (BSS) and the incidence of angina pectoris were evaluated before treatment, at month 1, 3, 6, 9, and 12 after treatment.
RESULTSThe incidence of ischemia-driven revascularization was obviously less in the treatment group than in the control group (P < 0.05). No patient had nonfatal MI in the treatment group, while 5 patients in the control group had it. The incidence of non-fatal MI showed an obvious lowering tendency in the treatment group, but with no statistical difference when compared with that in the control group (P > 0.05). Four patients readmitted to hospital in the treatment group, while 12 patients readmitted. There existed obvious tendency in the treatment group, but with no statistical difference when compared with that in the control group (P > 0.05). The incidence of angina was significantly lower in the treatment group at month 6, 9, and 12 than that at month 1 , but it was lower in the control group at 9 months (P < 0.05). The incidence of angina was 15. 4% in the treatment group, obviously lower than that in the control group (26.2%, P < 0.05). Compared with before treatment, scores for BSS were obviously lowered in the treatment group at 1, 3, 6, 9, and 12 months of treatment and in the control group at 3, 6, 9, and 12 months of treatment (P < 0.05). It was obviously lower in the treatment group than in the control group at 3, 6, 9, and 12 months of treatment (P < 0.01).
CONCLUSIONAdministration of CHSQABC combined routine Western medicine treatment could reduce the event of revascularization and post-PCI recurrent angina, and improve scores for BSS of ACS patients complicated with DM after PCI.

4.Genotypic and environmental variation in cadmium, chromium, arsenic, nickel, and lead concentrations in rice grains.

Genotypic and environmental variation in Cd, Cr, As, Ni and Pb concentrations of grains, and the relationships between these heavy metals and Fe, Zn were investigated using 9 rice genotypes grown in 6 locations for two successive years. Significant genotypic variation was detected in the five heavy metal concentrations in grains, indicating the possibility to reduce the concentration of these heavy metals in grains through breeding approach. The environmental effect varied with metal, with Pb and Ni having greater variation than the other three metals. There was significant genotype-environment (location) interaction of the concentrations of all five heavy metals in grains, suggesting the importance of cultivar choice in producing rice with low heavy metal concentrations in grains for a given location. Correlation analysis showed that Cd and As, Cr and Ni, and As and Pb concentrations in rice grains were closely associated, and that Ni concentration in grains was negatively correlated with Zn concentration.

5.Repair of finger soft tissue defects using axial pattern flaps.

OBJECTIVETo investigate the feasibility and effect of different types of axial pattern flaps in repairing soft tissue defects of the fingers.
METHODSFive types of axial pattern flaps were used to repair soft tissue defects of the fingers of 30 patients admitted to the Second Hospital of Shaoxing Municipality from 2005 to 2010, including distally-based dorsal thumb neurocutaneous vascular flaps in 4 cases, free flaps from the fibular side of the great toe in 6 cases, modified retrograde dorsal metacarpal artery flaps in 8 cases, free flaps based on the radial dorsal septum muscular perforator of the posterior interosseous artery in 6 cases, and free posterior interosseous artery flaps carrying a long segment of the posterior interosseous artery in 6 cases. The flap size ranged from 2.5 cm × 2.0 cm to 8.0 cm × 3.0 cm. The donor sites were sutured directly or covered with skin graft. The condition of flaps was observed. The local feeling, function, and appearance of affected hands were followed up.
RESULTSTwenty-seven flaps grew well without infection. Partial necrosis at the distal portion in two flaps and venous congestion in 1 flap were observed after surgery, and they were healed with suitable treatment. Eighteen patients were followed up for 1 to 12 months. The flaps were found to have good color, texture, and thickness. Satisfactory appearance and function both in the donor sites and in the recipient areas were found in most patients. The sense of two-point discrimination of repaired fingers ranged from 5 to 9 mm.
CONCLUSIONSOptimizing the repair method for soft tissue defects in the fingers by taking both the characteristics of various axial pattern flaps and the specific conditions of soft tissue defects into account can achieve expected effect.

6.Effects of guanxin II on cardiac protein expression after acute myocardial ischemia on rats.

OBJECTIVEGuanxin II is a famous modern formula of traditional Chinese medicine. Guanxin II after myocardial infarction (MI) has been shown to have beneficial effects on cardiac anatomy and function. The purpose of this study was to examine the effects of Guanxin 1I on cardiac protein expression after MI.
METHODRats were randomized into 3 groups, sham, model and treating groups. Model and treating groups were fed with Guanxin II and sham group was fed with water for 10 days before MI. MI operation is to ligate left coronary artery. 24 hours after MI, myocardial protein expression of junctional zone was assessed with 2D gel electrophoresis and mass spectra analysis.
RESULTGuanxin II was found to be able to improve myocardial protein expression, especially 11 proteins. These proteins are mainly involved in suppressing changes of cell shape and structure and energy metabolism.
CONCLUSIONGuanxin II after MI affected myocardial protein expression. Further experiments of larger research extent should be done to receive more results.

7.Helical CT presentation of focal nodular hyperplasia of the liver.

OBJECTIVETo improve the diagnostic accuracy of focal nodular hyperplasia (FNH) of the liver by analyzing its findings by helical CT multiphase scanning.
METHODSA retrospective analysis of the plain scanning and dynamic enhanced scanning helical CT findings was conducted in 20 pathologically verified FNH cases.
RESULTSOn plain CT scans, the FNH lesions appeared as heterogeneous or homogeneous hypodense areas. In the arterial phase, all lesions were vigorously and homogeneously enhanced except for the central scar and lesions in 1 case. In 6 cases, tortuous and dilated arteries were seen in the center or peripheral of the lesion in 16-slice spiral CT. In the portal venous phase and delayed phase, the lesions were slightly hyperdense, isodense or slightly hypodense. Central scar was found in 13 cases, which showed late enhancement. Atypical findings included multinodular FNH in 2 cases, pseudocapsular enhancement in 2 cases, calcification in 1 case, and necrosis in 1 case.
CONCLUSIONMultiphase helical CT scanning can fully display the pathological and blood supply characteristics of FNH and improve the differentiation of FNH from other malignant hypervascular tumors.

8.Over-expression of hypoxia-inducible factor-1alpha induces epithelial-mesenchymal transition in LNCaP cells in vitro and in vivo.

OBJECTIVETo investigate the effects of the over-expression of hypoxia-inducible factor-1alpha (HIF-1alpha) on the epithelial-mesenchymal transition (EMT) and invasive potency of LNCaP cells in vitro and in vivo.
METHODSWe cultured LNCaP cells stably expressing HIF-1alpha (LNCaP/HIF-1alpha) and LNCaP cells, identified the over-expression of HIF-1alpha, determined the proliferation of the two cell lines by MTT assay and the level of PSA in the supernatant of culture medium, and detected the anchorage independent growth by soft-agar colony formation assay. A subcutaneous tumor model was established in nude mice by injecting LNCaP/HIF-1alpha and LNCaP cells followed by observation of the tumor growth. Tumor specimens were obtained for immunohistochemistry.
RESULTSThe over-expression of HIF-1alpha was confirmed in the LNCaP/HIF-1alpha cells by immunofluorescence staining and Western blotting. The level of PSA was obviously decreased in LNCaP/HIF-1alpha as compared with that in LNCaP cells. MTT assay identified the increased proliferation of LNCaP/HIF-1alpha cells. The cell colony forming ability of the LNCaP cells was significantly lower than that of the LNCaP/HIF-1alpha cells. The rate of tumorigenesis was increased and its time shortened in the LNCaP/HIF-1alpha group. Immunohistochemistry revealed an up-regulated expression of vimentin and a down-regulated expression of E-cadherin in the tumor specimens.
CONCLUSIONThe overexpression of HIF-1alpha can up-regulate the expression of vimentin and down-regulate the expression of E-cadherin, which may enhance the invasive potency of LNCaP cells by inducing EMT.

9.Synthesis and alpha-glucosidase inhibitory activity of N-(1,5-diaryl-3-pentone-1-yl)-4-aminobenzoic acid.

In order to find highly active antidiabetic lead compound, sixteen 4-aminobenzoic acid derivatives were designed and synthesized directly through Mannich reaction in the solution of ethanol at 15-35 degrees C with facile method, mild reaction condition and high yield (45%-90%). Fifteen of them are new compounds. Their structures were confirmed by 1H NMR, 13C NMR, IR, ESI-MS and HR-MS. Alpha-glucosidase inhibitory activity of these compounds indicated that most of these compounds possess the activity with the order: 2c > 2b > 2h > 1a > 1f. The structure-activity relationship of these 4-aminobenzoic acid derivatives was also discussed.

10.Inhibitory effect of Biejiajianwan pills on tumor growth and proliferating cell nuclear antigen expression in mice bearing H22 cell-induced tumor.

OBJECTIVETo study the anti-tumor effect of Biejiajianwan pills and explore its mechanism.
METHODSMice bearing neoplasm induced by H22 cell inoculation were randomized into 4 groups, namely the negative control group, positive control group (with cyclophosphamide treatment), and high- and low-dose Biejiajianwan pill group. The tumor-bearing mice received corresponding treatments for 15 consecutive days, after which their thymuses and spleens were isolated and weighed to calculate the thymus and spleen indices. The tumor mass was also weighed and the tumor inhibition rate calculated. Immunohistochemistry was performed to evaluate the expression intensity of proliferating cell nuclear antigen (PCNA) in the tumor tissue.
RESULTSThe pills significantly inhibited the growth of the implanted tumor and reduced the expression of PCNA in the tumor.
CONCLUSIONOne of the possible mechanisms of the anti-tumor effects of Biejiajian pills might be associated with the inhibition of PCNA expression in the tumor.
