4.Insulin resistance of islet ?-cells
Chinese Journal of Endocrinology and Metabolism 1985;0(02):-
It has been proved recently that insulin signal transduction pathway and insulin resistance exist in islet?-cells.Defects in insulin signaling of the?-cells decrease glucose-stimulated first-phase insulin release, inhibit proliferation of?-cells and promote apoptosis of?-cells.Obesity accelerates the development of insulin resistance of?-cells,which may play a central role in the pathogenesis of type 2 diabetes mellitus.

OBJECTIVETo investigate the effect of compound Danshen Dripping Pill (DSP) on carotid arterial intima-media thickness (IMT) in patients with type 2 diabetes mellitus (T2DM).
METHODSOne hundred and thirty T2DM patients were assigned to four groups, 32 in the Group A, the control group treated with blood glucose (BG) and blood pressure (BP) controlling; 32 in the Group B, with BG, BP and blood lipid (BL) controlling, 32 in Group C with BG, BP, BL controlling and vitamin E administration, and 34 in Group D with BG, BP, BL controlling and DSP administration. Patients in Group D were subdivided by Chinese medicine syndrome differentiation into four types, 8 of Yin-deficiency with flourishing heat type (YDFH), 5 of both qi-yin deficient type (BQYD), 8 of both yin-yang deficient type (BYYD) and 13 of blood-stasis and qi-stagnant type (BSQS). Fasting blood glucose (FBG), BP and BL in patients were observed periodically, and IMT in them were measured by ultrasonography before treatment, as well as at the end of the 1st, 3rd, and 5th year of treatment to dynamically observe the changes of IMT and condition of plaque formation, and analyze the relation between them with FBG, BP and BL.
RESULTSThe 5-year follow-up was performed in 105 patients. In the observation period, level of total cholesterol (TC) showed a decreasing trend and level of high density cholesterol (HDL-C) showed an increasing trend in all the 4 groups, the improvements in Group C and D were slightly better than those in Group B, while significantly superior to those in Group A; the changes of FBG and glycosylated hemoglobin (HbAlc) were insignificant in the 4 groups. IMT and numbers of atheroma plaque increased gradually in all groups in the observation period, however, the changes in Group D were lesser than those in other groups, showing significant difference (P < 0.01). It was showed that the increasing of cervical carotid IMT in T2DM patients was correlated with levels of HbAlc, HDL-C, LDL-C, triglyceride and TC, especially in Group D.
CONCLUSIONDSP might delay the occurrence and development of diabetic macro-vascular disease.

Objective To evaluate the clinical value of 6 h 99mTc-diethyl iminodiacetic acid (99mTc-EHIDA) planar hepatobiliary scintigraphy (HBS),6 h tomographic HBS and 24 h planar HBS in diagnosis on biliary atresia(BA).Methods Seventy cases(32 male,38 female) with continuous jaundice received planar and tomographic HBS in Beijing Friendship Hospital from Jan.2005 to Dec.2007.The mean age was 48.7 d (29 d-4 months).According to final diagnosis, all cases were divided into BA group (45 cases) and non-BA group (25 cases). All cases fasted at least 4 hours before HBS.The equipment was 3 head IRIX from Philips company with low energy high resolution collimator. The tracer was 99mTc-EHIDA and the radiochemistry purity was more than 95 percent. The dosage was 7.4 MBq/kg.All diagnosis demonstrated by operation pathology and clinical follow-up.All cases received HBS at 5,10,15,20,30 min and 1,6 h after tracer injection.HBS would ended if radioactivity appeared in gallbladder or intestine. These cases would received tomographic HBS and 24 h HBS if radioactivity did not appear in gallbladder or intestine at 6 h post injection. All these images were analyzed by 2 or more nuclear medicine physicians. Results There were not radioactivity appearing in gallbladder and intestine on planar and tomographic HBS of 27 cases, which suggested the BA.There were radioactivity appearing in gallbladder and intestine on planar and tomographic HBS of 30 cases, which suggested the non-BA.Positive rate of 6 h tomographic HBS was significantly higher than that of 6 h planar HBS and there was significantly difference between the 2 methods.Positive rate of 6 h tomographic HBS was significantly higher than that of 24 h planar HBS and there was significant difference between the 2 methods. Conclusions 99mTc-EHIDA HBS is a noninvasive, safety, valuable examing method and has definitely clinical value in the diagnosis on BA.The clinical value of 6 h tomographic HBS is significantly higher than that of 6 h planar HBS and 24 h planar HBS.

Objective To study topiramate′s neuroprotection on primary cultured hippocampal neurons which were injuried by glutamate and its mechanism. Methods The primary cultured hippocampal neurons were made as the research object, and excitotoxicity model was exected with glutamate.Hippocampal neuron survival was assessed by MTT method and hippocampal neuron mitochondrial membrance potential(MMP) was evaluated by fluorescence microscope and flow cytometry. Results Hippocampal neuron survival of normal control was(98.4?0.8)%,and the survival of glutamate model group was(59.6?3.2)%,at the same time, two topiramate′s groups′ cell survivals was(74.1?0.5)% and(79.2?3.4)%,and topiramate with two levels all could obviously increase hippocampal neuron survival((P
9.A new procedure for repairing coronal or subcoronal hypospadias with severe chordee.
Chinese Journal of Plastic Surgery 2005;21(2):109-111
OBJECTIVETo introduce a new procedure for repairing coronal or subcoronal hypospadias with severe chordee.
METHODSA modified technique of elongating the anterior urethra, combined with the MAGPI procedure, was used to repair the coronal or subcoronal hypospadias in 19 severe chordee patients (aged 5 to 12 years). The fibrous bands on the ventral aspect of the penis were excised to correct the chordee. If the penile curvature still remained, the corpora cavernosa dissection and elongation could be applied.
RESULTSAll of the patients were successfully treated by this procedure in one stage.
CONCLUSIONSThe modified technique of the anterior urethra elongation, combined with MAGPI procedure, might be one of the simple and effective method for repairing coronal or subcoronal hypospadias with severe chordee.

National Journal of Andrology 2014;20(4):334-337
OBJECTIVETo determine the exact location of the opening of the ejaculatory duct in men and provide some basic anatomical evidence for seminal vesiculoscopy and the treatment of ejaculatory duct obstruction.
METHODSWe performed ureterocystoscopy for 21 male patients aged 26 - 47 years with hematuria (n = 12), hematospermia (n = 2), glandular cystitis (n = 6), and anejaculation after radical resection of rectal carcinoma (n = 1), and meanwhile, with the consent of the patients, massaged the prostate and ejaculatory duct and observed the outlet of the expelled fluid. Under the microscope, we described the fluid samples with sperm as the expulsion from the ejaculatory duct.
RESULTSUreterocystoscopy showed that the exact anatomical sites of the expulsion of prostatic fluid and semen in the patients were the side and lower side of the prostatic utricle opening above the verumontanum and the ventral side of the verumontanum. Quantities of sperm were found in the expulsion fluid of 13 of the patients, and no expulsion, including semen, was seen from the prostatic utricle opening.
CONCLUSIONAnatomically, the ejaculatory duct openings of males are located at the two sides of the verumontanum adjacent to the opening of the prostatic utricle, rather than in the prostatic utricle above the verumontanum.
