2.Continuous Electroencephalographic Monitoring in Acute Cerebrovascular Diseases

Continuous electroencephalographic monitoring (CEEG) has been used for clinical monitoring of acute cerebrovascular diseases for nearly 3 decades,but it has not been used as a conventional monitoring approach in clinical practice.In recent 15 years,CEEG technology has made considerable progress,especially in the field of real-time monitoring of acute cerebral ischemia and epilepsy,which has provided a new possible direction for the diagnosis and treatment of acute cerebrovascular diseases.The article briefly comments the application of CEEG in acute cerebrovascular diseases.

The validity of fasting plasma glucose(FPG)combined with HbA_1c in diagnosing diabetes was assessed in patients with coronary heart disease.The results showed that the paired determination of FPG and HbA_1c helped to identify potentially diabetic subjects in patients with coronary heart disease.
4.DDA and BCG polysaccharide nucleic acid improved the immunogenicity and protective efficacy of tuberculosis subunit vaccine against Mycobacterium tuberculosis infection in mice

Objective To investigate the adjuvant effect of dimo-thylidioctyl ammonium bromide (DDA) and/or DDA-BCG polysaccharide nucleic acid( BCG-PSN), which was combined with a Mycobacterium tuberculosis fusion protein AMM ( Ag 8 5 B - MPT64190-198 - Mtb8.4 ) to boost BCG primed immunization. Methods DDA with or without BCG PSN was mixed with the fusion protein AMM to construct the boosting vaccine. Mice were immunized with BCG and then boosted twice with AMM formulated with the adjuvant DDA with or without BCG-PSN. PBS or BCG vaccination without boosting was used as control. The humoral and cell-mediated immune responses were analyzed by ELISA and ELISPOT. Moreover, the protective efficacy of BCG prime-AMM subunit vaccine boosting against Mycobacterium tuberculosis infection was analyzed. Results With in vitro stimulation of Ag85B and PPD( purified protein derivative) antigen, the number of IFN-γ secreting cells from the mice boosted twice by AMM/DDA/BCG-PSN and AMM/DDA were higher than BCG and PBS group (P <0.05). The CFU in lungs of mice boosted with AMM/DDA/BCG-PSN was less than that of PBS group(P <0.05), while the CFU of AMM/DDA-boosted mice was less than that of BCG and PBS group(P < 0.05).However, fewer lesions were seen in lungs of mice immunized with BCG alone or BCG-prime-AMM/DDA/BCG-PSN boosting than the other groups. Conclusion DDA is an idea adjuvant for tuberculosis subunit vaccine;BCG-PSN might play a role in alleviating the immunity-mediated pathology.
5.Low ankle brachial index and risk of death: a meta-analysis

Objective To evaluate the association between low ankle brachial index and risk of death. Methods Related cohort studies in the past 12 years were collected. Heterogeneity was analyzed with χ2 and I2. Pooled hazard ratio (HR) as the effect indicator and 95% confidence interval (CI) were used to estimate the association of low ankle brachial index and risk of death by fixed effect model ( inverse variance method). Results A total of 8 studies were included with sum sample size of 30 029. In heterogeneity analysis, χ2 was 5.46, I2 was 0. 0%, and P was 0. 604. Meta-analysis resulted in pooled HR of 1.69 (95%CI:1.55 - 1.84). Conclusion Low ankle brachial index is associated with increased risk of death.
6.Efficacy observation after pylorus-preserving gastrectomy for early gastric cancer.

OBJECTIVETo evaluate the outcomes after pylorus-preserving gastrectomy (PPG) for early gastric cancer(EGC).
METHODSClinicopathologic data of 52 patients with EGC undergoing PPG between August 1995 and December 2005 were analyzed retrospectively. A total of 159 patients of EGC who underwent distal gastrectomy with lymph node dissection(control group) were compared with those who received PPG.
RESULTSThe lymph node metastasis rate of EGC was 9.6% in PPG group, including 9.6% in No.3, 3.9% in No.4, 3.9% in No.6, and 3.9% in No.7. In the control group, the lymph node metastasis rate was 17.0%, including N1(14.5%) and N2(2.5%). There were no significant differences between the PPG group and the control group (P>0.05). In the PPG group, D1 dissection was 25%, D1+α was 25%, D1+β was 34.6%, and D2 was 15.3%. In control group, 121 patients(76.1%) had less than D2 dissection, while there were 33(20.7%) D2, and 5(3.1%) D3, and the difference was not statistically significant(P>0.05). There were no significant differences between the two groups in overall 5-year survival rate(92.3% vs. 93.1%, P=0.881). The 5-year survival rate in the PPG group was 100% for D1, 92.3% for D1+α, 88.9% for D1+β, and 85.7% for D2, while the 5-year survival rate in the control group was 92.3% for D1, 93.3% for D1+α, 91.7% for D1+β, and 93.9% for D2. The difference was not statistically significant(P>0.05). The recurrence rate was comparable (5.7% vs. 5.6%, P>0.05).
CONCLUSIONPylorus-preserving gastrectomy may provide long-term survival benefits for patients with early gastric cancer.

7.Clinical Observation on Treatment of Pediatric Intractable Nephropathy with Modified Taohong Siwu Decoction(桃红四物汤)

Objective: To observe the clinical effect of Modified Taohong Siwu Decoction (MTSD) in treating pediatric intractable nephropathy (PIN). Methods: Ninety-five patients with PIN were divided into 2 groups at random. The 60 patients in the treated group were treated with MTSD and the 35 patients in the control group were treated with heparin. The clinical therapeutic effect and levels of thromboxane B2 (TXB2) and 6-keto-prostacyclin F1α (6-keto-PGF1α) before and after treatment were observed. Results: The total effective rate in the treated group was 81.7%, which was similar to that in the control group (80.0%, P>0.05). The levels of TXB2 and TXB2/6-keto-PGF1α ratio were higher in both groups of the patients as compared with those of the healthy control (P<0.01). After treatment, the two criteria were significantly improved in the two treated groups, as compared with those before treatment, the difference was significant (P<0.01), while in comparison between the MTSD treated group and the control group, no significant difference was found (P>0.05). Conclusion: MTSD has good effect in treating PIN, it could improve the metabolic unbalance of thromboxane and prostacyclin.

9.Investigate on rational lymph-node dissection for gastric cardia cancer.

OBJECTIVETo investigate the rule of lymph-node metastasis in gastric cardia cancer and the rational extent of lymph node dissection.
METHODSClinicopathological data of 77 patients with gastric cardia cancer were reviewed and the relationship between extent of lymph-node dissection and prognosis was analyzed retrospectively.
RESULTS(1) The lymph node metastasis rates were 64.9% for N(1), 14.3% for N(2) and 10.4% for N(3). (2) No lymph node metastasis was detected in T(1) stage tumor and maximum diameter of less than 2.0 cm. The lymph node metastasis rates were 20% for T(2), 68.2% for T(3) and 82.8% for T(4) respectively. (3) Lymph node No.1, 3, 2 were often involved in the metastasis of lymph node group 1, and No.7, 8, 10, 9 in Group 2. In lymph node group 3, lymph node metastasis rates were 6.5% for No.5, 1.3% for No.6, 1.3% for No.16 and 2.6% for No.107-110. (4) The five-year survival rates were 36.5% for D(3), 31.3% for D(2), and 22.7% for D(1) lymphadenectomy respectively. The survival rates of patients undergone D(2) and D(3) lymphadenectomy were significantly higher than that undergone D(1) dissection (P<0.05).
CONCLUSIOND(2) or more than D(2) lymphadenectomy associated with enlargement of esophageal hiatus via laparotomy, lower partial esophagectomy and total gastrectomy is able to achieve surgical resectability and improve the survival rate of gastric cardia cancer patients.

10.Immunohistochemical study with depletion of cardiac troponin T in rats on early myocardial ischaemia.

OBJECTIVE:
To study the depletion of cardiac troponin T in Rats of early myocardial ischaemia.
METHODS:
Immunohistochemical method (LSAB) was used and the depletion area of CTnT in myocardial ischaemia were measured by the computer image analysis.
RESULTS:
There is obviously depletion of CTnT after 15 min in myocardial ischaemia of rats, the mean of CTnT depletion of myocardial ischaemic group is significant difference compared with that of the control group (P < 0.01), and the depletion area increased with the prolongagtion of ischaemic time.
CONCLUSION
The depletion of CTnT is one of sensitive signs of early myocardial ischaemia.
