1.An experimental study on half-dose immunosuppressive agents multi-target therapy for lupus nephritis

Objective To investigate the efficacy and adverse reactions of half-dose glucocorticosteroid and cytoxan,combined with leflunomide for the treatment of lupus nephritis (LN) of MRL/lpr mice,and provide experimental evidences for LN therapy.Methods Twenty-eight 10-week-old MRL/lpr mice were randomly divided into four groups:Group A,blank control group; Group B,classical control group; Group C,full-dose control group; Group D,half-dose treatment group,with 7 mice in each group.The therapeutic efficacy and side reactions in the four groups were observed and compared before and 12 weeks after treatment.Statistical analysis was conducted with one-way ANOVA,q test and Pearson's correlation analysis.Results The serum anti-double stranded DNA (anti-dsDNA) antibody titers (0.43±0.16,0.32±0.09,0.44± 0.18,1.95±0.19) U/ml,serum creatinine level (1.63±0.63,0.40±0.23,0.82±0.21,10.86±2.17) mg,24-hour urine protein excretion level (71±8,60±5,68±3,121±10) μmol/L and renal pathological changes in group B,C,D were significantly improved than those of the group A (P<0.05) after 12 weeks treatment.There was no significant difference in the efficacy between group B,C,and D (P>0.05).The incidence of adverse reactions in group D was significantly lower than that in other groups (P<0.05).Conclusion Multi-target therapy,such as half-dose prednisone and CTX,combined with leflunomide can effectively control lupus disease activity with less side effects.This regimen is cheap,safe and effective for the treatment of LN in MRUL/lpr mice.This study has provided animal evidences for this multi-target therapy for LN.

3.Effectiveness of volume therapy guided by response of CVP and global end-diastolic volume index in septic shock patients

Objective To evaluate the effectiveness of volume therapy guided by the response of CVP and global end-diastolic volume index (GEDVI) in septic shock patients.Methods Twenty-three patients of both sexes aged 18-64 yr who had been in septic shock for < 6 h were randomly divided into 2 groups:control group (group Ⅰ,n =12) and study group (group Ⅱ,n =11 ).Tracheal intubation was performed and the patients were mechanically ventilated in both groups.Right internal jugular vein or subclavian vein was cannulated for CVP monitoring and fluid administration.PiCCO catheter was inserted into femoral artery.Normal saline,6% hydroxyethyl starch 200/0.5,albumin and plasma were infused via CVP line.CVP was maintained ≥8 mm Hg in both groups while in Ⅱ group GEDVI was maintained at 600-750 ml/m2 during resuscitation.Blood samples were taken from artery and CVP line before (baseline) and at 6 h of volume therapy (T2) for determination of blood lactate concentration and central venous oxygen saturation ( ScVO2 ).The changing rate of lactate ( (baseline lactate concentration-lactate concentration at 6 h of volume therapy) ÷ baseline value × 100% ) and ScvO2 ( ( ScvO2 at 6 h of volume therapy - baseline ScvO2 ) ÷ baseline value × 100% ) ) was calculated.Results The changing rate of lactate was significantly higher in Ⅱ group than in Ⅰ group.There was no significant difference in the changing rate of ScvO2 between the 2 groups.Conclusion Volume therapy guided by CVP and GEDVI can provide better tissue perfusion than by CVP alone in septic shock patients.
4.Effects of tumor necrosis factor-?and rosiglitazone on the expression of protein tyrosine phosphatase 1B during the differentiation of 3T3-L1 preadipocytes

Objective To investigate the protein level of protein tyrosine phosphatase 1B (PTP1 B) during the differentiation of 3T3-L1 preadipocytes and the effects of tumor necrosis factor-?(TNF-?) and rosiglitazone on the expression of PTP1B during the differentiation,and to explore the relationship between PTP1B and adipecyte differentiation.Methods 3T3-LI preadipecytes were cultured in vitro and differentiated by three groups of inducers: basic inducers only (3-isobutyl-1-methylxanthine+dexamethasone+insulin,group C),20?g/L TNF-?+basic inducers (group CT) and 10~(-5) mol/L rosiglitazone+basic inducers (group CR).Protein level of PTP-1B in adipocytes during differentiation was detected by Western blot.Results Each group showed the relatively high level of PTP1B in 3T3-L1 preadipecytes,going down with the differentiation of adipecytes,and reaching the lowest level in fully-matured adipecytes.Comparing the late period of differentiation in these three groups,CT group was sluggishly differentiated with more PTP1B protein,and CR group showed active differentiation with the lowest level of PTP1B.Conclusion PTP1B decreases with the differentiation of adipoeytes.The effects of TNF-?and rosiglitazone on insulin sensitivity perhaps partly via their influences on PTP1B level in adipecytes.
5.Changes and significance of induced sputum and interleukin - 6, interleukin 8 in children with acute asthma

Objective To investigate the characteristic and clinical significance of airway inflammation in children with acute asth-ma. Methods Underwent sputum induction and sputum induction in children (n=34) with acute asthma was repented in recovered children ( n = 24).Induced sputum were also taken from 15 healthy children as controls.Total and differential cell counts were per-formed. Interleukin(IL)-8、IL-6 were measured,The relationship between inflammatory cells and IL-8、 IL-6、peak expiratory (PEF) were analyzed.Results The inflammatory cell infiltrate was mixed including eosmophilic granulocytes, neutrophils, and macrophages. They decreased significantly, but eosinophilic granulocyte remained a higher percentage compared with healthy subjects. There was low-er percentage of lymphocytes at acute exacerhation.Eosinophilic granulocytes were correlated with the degree of airflow obstruction. Levels of IL-8、IL-6 were elevated during the acute exacerbation and decreased at resolution.IL-8 was correlated significantly with neutrophils at acute exacerhation and resolution.IL-6 was correlated signifficantly with eosinophilic granulocytes at acute exacerbation. Conclusions Airway inflammation is chataeterized by infiltration of eosinophils, neutrophils and macrophages.IL-8、IL-6 possibly is the important cytokines of airway inflammation in children with acute asthma. Increased eosinophils in induced sputum correlates with asthma severity. Therapy to the cytokines may have potential values.J Appl Clin pediatr,2004,19(12): 1023-1025

7.Effect of intravitreal Ranibizumab treatment in polypoidal choroidal vasculopathy with different types

AIM: To investigate the efficiency of intravitreal ranibizumab therapy ( IVR ) for polypoidal choroidal vasculopathy ( PCV) in single or multiple polyps.
METHODS: A total 63 patients diagnosed with PCV in Shaoxing City People's Hospital from May 2013 to May 2015 were enrolled and divided into single polyp group and multiple polyps group. All patients received intravitreal ranibizumab 3 monthly and were followed up for 12mo. Observe the changes of best corrected visual acuity ( BCVA ) and central retinal thickness ( CRT ) at different time points.
RESULTS: The single polyps group exhibited a better BCVA, shorter greatest linear dimension, and lower prevalence of fibro - vascular pigment epithelial detachment compared with the multiple polyp group before treatment (P<0. 05). Significant difference of BCVA were observed at 3, 6 and 12mo between the two groups (P<0. 05). BCVA at 3, 6, 12mo was significantly better than that at baseline in single polyps group. The single polyp group exhibited a significantly thinner CRT at 6 and 12mo compared with multiple polyps group (P<0. 05). The single polyp group showed improvement in CRT over the followed up period(P<0. 05). The CRT in multiple polyps group at 3 and 6mo significantly decreased compared with preoperative (P<0. 05).
CONCLUSION: IVR meet better result in PCV patients with multiple polyp and polyp numbers may be valuable to prognosis.
8.Pathogenic surveillance of scarlet fever in Xicheng District of Beijing in 2014

Objective To perform pathogenic surveillance of scarlet fever in Xicheng District of Beijing in 2014 ,and provide sci‐entific data for developing reasonable prevention policy for the disease .Methods The throat swab specimens from 212 patients who were diagnosed with streptococcal infection ,tonsillitis and angina in surveillance hospital were collected ,and from which the patho‐gens were isolated and identified .Results In the 212 samples ,the positive rate of A group hemolytic streptococcus were 21 .2%(45/212) .The patients were mainly 4-15 years old ,especially 6- <8 years old .The positive rate was highest in 6- <7 years old children .The peak of disease incidence was observed in May and June .Conclusion Preschools and schools were the key sites for scarlet fever prevention and control ,therefore ,the surveillance and prevention should be further strengthened to prevent the out‐break .
9.Comparison of Latanoprost and Brimonidine in the treatment of open angle glaucoma

AlM:To compare the efficacy and safety of latanoprost and brimonidine in the treatment of open angle glaucoma, and provide reference for rational drug use.METHODS:A total of 121 cases ( 136 eyes ) who were diagnosed as primary open angle glaucoma were selected in this study, and they were randomly divided into experimental group (62 cases, 70 eyes) and control group ( 59 cases, 66 eyes) according to different drug treatment. Patients in the control group received brimonidine eye drops twice a day, while patients in the experimental group received latanoprost eye drops once a day. The intraocular pressure, visual acuity and adverse reactions were checked of the two groups in the following 3mo.RESULTS:The intraocular pressure of patients in the control group was 18. 1 ± 1. 3mmHg, while the experimental group was 17. 0 ± 0. 9mmHg after 12wk of treatment, which were both lower than before (P<0. 05). The fluctuation of intraocular pressure in the experimental group was significantly lower than that of the control group. There was no significant difference in the LogMAR visual acuity between before and after treatment in the control group, while the LogMAR visual acuity of the experimental group was significantly improved. The control group had hyperemia, burning sensation, tearing, eyelid edema and other adverse side effects, and the experimental group had little adverse reactions. CONCLUSlON: Latanoprost can significantly reduce intraocular pressure in glaucoma patients with in the follow- up time, and reduce the impact of elevated intraocular pressure in the vision of glaucoma patients, with little adverse reaction, worthy of clinical application.
10.Expression changes of protein tyrosine phosphatase 1B during differentiation of 3T3-L1 preadipocytes

Objective:To observe the expression changes of protein tyrosine phosphatase 1B(PTP1B)mRNA and protein during the differentiation of 3T3-L1 preadipocytes,so as to explore the relationship between PTP1B and adipocytes differentiation.Methods:3T3-L1 preadipocytes were cultured in vitro and were induced to differentiate into mature adipocytes; the differentiation of adipocytes was assessed through detecting expression of peroxisome proliferator-activated receptor-gamma 2(PPAR?2)mRNA by RT-PCR and oil red O staining.Expression of PTP1B in adipocytes was detected by RT-PCR and Western blot during differentiation.Results:With the progression of 3T3-L1 cell differentiation,oil red O staining showed that the lipid droplets increased gradually to 90% of the vision field;meanwhile,the expression of PPAR?2 also increased gradually, suggesting the proliferation and maturation of the preadipocytes.The expression of PTP1B mRNA and protein decreased in 3T3-L1 preadipocytes with their differentiation and maturation;the expression reached the lowest level in mature adipocytes. Conclusion:Expression of PTP1B mRNA and protein decreases during the differentiation of preadipocytes,indicating a role for PTP1B in the maturation of adipocytes.
