4.Repair finger with the second digit lateral shank flap through free transplantation of lateral defect

Objective To introduce an ideal method of repairing of finger volar skin defect. Methods Thirty-five patients whose finger was repaired by second digit lateral shank flap through free transplantation of lateral defect were selected. Twenty-four cases were male and 11 cases were female, with age of 21 to 45 years old. Cutting injury was in 7 cases, crush injury was in 16 cases, degloving injury was in 9 cases, and firearm injury was in 3 cases. Skin defect area was 2.0 cm × 1.5 cm-4.0 cm ×2.5 cm. The course was 1 h to 5 d. Donor area was repaired by thick skin flap. Results The postoperative wound of 35 cases all healed in stage Ⅰ, and skin flap transplantation and the donor skin grafts survived. Twenty-nine cases received follow-up for 5-26 (14.5±5.0) months. The skin flap had good appearance and good texture, with two points discrimination of 5-8 (6.7±1.1) mm. According to the Chinese medical association upper part of the function evaluation trial standard: excellent was in 19 fingers, good was in 8 fingers, and the excellent and good rate was 93.1% (27/29). Walk, run, jump functions of donor foot were normal. Conclusions Repairing finger with the second digit lateral shank flap through free transplantation of lateral defect has small area damage, and damage to the area around the organization is small. The effect of repairing is good, and can achieve the aim of the palm side repairing.
5.Recent advances on treatment of primary central nervous system lymphoma

Primary CNS lymphoma (PCNSL) is a rare malignancy with peculiar clinical and biologic features,aggressive course,and unsatisfactory outcome.the particular microenvironment of this malignancy,and sanctuary sites where tumor cells grow undisturbed, strongly affects treatment efficacy. The execution of prospective trials is also difficult because of the rarity of the tumor and the impaired general condition and poor performance status of patients. Chemotherapy is indispensable for the treatment of PCNSL. High-dose methotrexate is the most effctive drug.PCNSL is sensitive to irradiation,but responses to radiotherapy alone are short-lived. MTX-based chemotherapy followed by whole-brain radiotherapy prolonged survival but is associated with delayed neurotoxicity especially in patients older than 60 years. A new approach is attempted to treat PCNSL with chemotherapy alone with defered radiotherapy in old patients with similar survival rencenfly. High-dose chemotherapy with autologous stem cell transplantation HDC/ASCT yielded promising results for recurent PCNSL.Clinical investigation with Temozolomide or rituximab have been reported.Further studies with these new drugs are needed.
6.The expression differences and their signiifcance of the relevant receptors in breast cancer

ER, PR, HER-2 are signiifcant biological markers in guiding the choice of treatment and evaluating clinical prognosis. As the progression of tumor cells, the expressions of these receptors in recurrent tumors may be different from primary tumors. Nowadays clinicians have realized the necessity of receptor detection in the metastatic lesions. This article summarized the current research status on the above issues.
7.The Design and Application of Self-learning and Self-check System of Medical Immunology Based on ASP Online

The self-learning and self-check system of medical immunology has been established by online Active Server Pages(ASP) technique.The students’learning results can be examined and evaluated automatically by the system.The students can freely choose the parameters of grouping up test-papers,according to their different situation,which is more rational and the test result is closer to students’true achievement.After the test is finished,the system can check the answers automatically and show the test-score.Through the humanized test design and the feedback encouraging teaching mode,the system affords fine conditions for independent learning of medical students and solves the problems of the combination between teachers’teaching and learners’ practicing.
8.Platelet recovery in patients with chronic idiopathic thrombocytopenic purpura after eradication of helicobacter pylori

Objective Clinical research was made to observe the relationship between idiopathic thrombocytopenic purpura(ITP)and Helicobacter pylori(ITP)and to evaluate the effect of HP eradication in the treatment of chronic ITP.Methods ~(14)C urea breath test were made in 17 patients with chronic idiopathic thrombocytopenic purpura,including 6 males and 11 females,aging from 15-48,with course of disease from 4 months to 8 years.Omeprazole,clarithromycin and amoxicillin were used for Helicobacter pylori eradication,and the effect was evaluated by platelet count.Results Thirteen out of 17 patients were HP positive,and eleven turned negative after HP eradication,eight of whom were observed platelet recovery.Conclusion Platelet count can be raised in some patients with chronic ITP after eradication of Helicobacter pylori.

10.Expression of ?-Smooth Muscle Actin in Rats with Tubulointerstitial Fibrosis

Objective To observe the expression of ?-smooth muscle actin(?-SMA)in rats with tubulointerstitial fibrosis and explore the possible mechanism of renal-protecting of ?-SMA.Methods Sixty male Sprague-Dawley(SD)rats,aged 3 months,were randomly divided into 2 groups:control group(n=30)and model group(n=30).The renal tubulointerstitial fibrosis model was established by gavage with 150 mg/(kg?d)adenine solved by a solution of 20 g/L starch and the same volume starch was given to the rats from control group.At the 7th,12th,17th week,10 rats of every group were killed,the urinary protein,N-acetyl-?-D-glucosaminidase(NAG)in the urine and renal profile including BUN and serum Cr were examined.The histological changes of kidney were observed by light microscopy,and the expression of transforming growth factor-beta 1(TGF-?1)and ?-SMA were examined by immunohistochemistry staining.Results At the 7th,12th and 17th week,the value of urinary protein,urinary NAG,serum BUN and Cr of rats from model group were higher than those of controls(Pa
