1.Effects of Jinzhuyalong Ershiwuwei coral capsule on PLA2 in degenarated cervical intervertebral discs

Objective To study the anti-inflammation mechanism of Jinzhuyalong Ershiwuwei coral capsule on cervical spondylosis by investigating Jinzhuyalong Ershiwuwei coral capsule on PLA2 in degenerated cervical intervertebral discs.Methods 60SD rats (SPF) were divided into a control group (15),a medicine model control group (15),a medicine modle 1 group (15),and a medicine model 2 group (15) by random lottery; Medicine model 1,2 groups were given Jinzhuyalong Ershiwuwei coral capsule powder 0.33 × 5 g/kg 0.66×5 g/kg,and added 1 ml distilled water dilution lavage,l/d,90 d,The unbalance of power cervical spondylosis animal models were chosen and C4~5 neck intervertebral discs were taken after 90 days.Millipore colorimetry methods were used to measure PLA2 activity in C4~5 cervical intervertebral discs.Results The PLA2 activity of the cervical intervertebral disc (12.37 ± 4.21) IU/mg · min-1 in the model group was significantly higher than the control group(2.56± 1.19)IU/mg· min-1 (P<0.05).The cervical intervertebral disc PLA2 activity of medicine model group 1 (8.56± 2.13)IU/mg · min-1 and medicine model group 2 (5.58 ±2.39)IU/mg· min-1 was lower than the model control group(12.37±4.21)IU/mg· min-1,with significant differences (both P<0.01).Conclusion Jinzhuyalong Ershiwuwei coral capsule could down regulate PLA2 activity in degenerated cervical intervertebral discs,and also decrease the production of some inflammaroty mediators.
2.Application of serun soluble inerleukin-2 receptor and soluble tumor necrosis factor receptor Ⅱ levels in extrapulmonary organ involvement sarcoidosis

Objective To investigate the application of serum soluble interleukin-2 receptor(sIL-2R)and soluble tumor necrosis factor receptor Ⅱ (sTNFR Ⅱ) in extrapulmonary organ involvement sarcoidosis.Methods Of 28 cases with sarcoidosis which were in active period,12 cases were in extrapulmonary organ involvement subgroup,16 cases were in pure pulmonary lesions subgroup.Serum sIL-2R and sTNFR Ⅱ of the two groups were detected by enzyme linked immunosorbent assay and compared.Results The expression level of sIL-2R in extrapulmonary organ involvement subgroup was (1.60 ± 0.80) μg/L,significantly higher than that of pure pulmonary lesions group,and there was significant difference((1.00±0.29)μg/L,t =2.764,P<0.05);and the expression level of sTNFR Ⅱ in extrapulmonary organ involvement subgroup was (1.09 ± 0.24) μg/L,significantly higher than that of pure pulmonary lesions group,and the difference between the two groups was statistically significant((0.85±0.29) μg/L,t =2.183,P<0.05).Conclusion The expression level of peripheral blood serum sIL-2R and sTNFR Ⅱ may serve as the important indexes to judge the extrapulmonary organ involvement sarcoidosis.
3.Advance in MicroRNAs and EGFR-TKIs Secondary Resistance Research in Non-small Cell Lung Cancer

In recent years, in non-small cell lung cancer (NSCLC) targeted therapy, especially in patients with epidermal growth factor receptor (EGFR) mutations, EGFR-tyrosine kinase inhibitors (TKI) more and more come into the clinical treatment, but EGFR-TKI resistance not only inlfuence the drug sensitivity, appear even disease progression, become the main bottleneck of its curative effect. MicroRNAs (miRNAs) is a non coding RNA and protein involved in regulating gene expression in the transcription level. Recent studies found that miRNAs involved in EGFR-TKIs resistance, which affect the sensitivity of tumor cells to treatment. In this paper, we reviewed brielfy advance in miRNAs and EGFR-TKIs secondary resis-tance research in NSCLC.
4.Efficacy analysis of endovascular graft exclusion for the treatment of 24 patients with infra-kidney abdominal aortic aneurysms

Objective To investigate the efficacy and the indication and the management of perioperative complications in treatment of infra- kidney abdominal aortic aneurysm (AAA) by using endovascular graft exclusion (EVGE). Methods From April 2006 to September 2008, 24 patients with infra- kidney abdominal aortic aneurysms were diagnosed by contrast-enhanced CT or MRI scan. Vascular access was obtained through the bilateral femoral artery after arteriotomy and stent-graft was deployed into AAA of below the renal artery to occlude the left over cavity of AAA. The stent- graft was extended and anchored to the both side wall of AAA, the blood flow enter into the arteria iliaca communis through the sten't.Results Stent-graft deployment was successfully performed in all the patients. Immediate aortography after the procedure showed no leakage in 20 patients and the type Ⅰ minor leakage in 4 patients. No stent movement or organ and both lower extremities ischemia was found at the early post operative stage in all the patients. Six months after the operation, in all the 24 patients, contrast-enhanced CT scan showed the disappearance of the aneurysm and thrombosis at the level of the stent. Conclusions EVGE is simple,minimally invasive,less complication and quick recovery after operation. Thus it becomes first choice for the treatment of AAA for the elder patients.
5.Assessment of referral cases for root canal therapy.

OBJECTIVETo assess referral cases for root canal therapy to provide clinical evidence with establishment of referral system.
METHODSA questionnaire was designed to collect 312 cases (355 teeth) referred to Department of Conservative Dentistry and Endodontics, West China School of Stomatology, Sichuan University for root canal therapy from April to June in 2011. The questionnaire consisted of questions such as age, gender, occupation, general disease and medical histories of patients, sources. Difficulty of treatment before therapy and reasons for referral were evaluated.
RESULTSIn 355 referral teeth, the most were mandibular first molars (17%), the greatest percentage of reasons were calcified root canals (30%), the second was root canal retreatment (24%). 72% of teeth belong to the complicated difficulty (Class III), while 7% were listed as the uncomplicated difficulty (Class I). No significantly difference from different sources was found (P = 0.9).
CONCLUSIONEstablishment of referral system needs cooperation with every dentist. Difficulty of cases for root canal therapy before treatment should be first evaluated to treat or to refer. Referral cases for root canal therapy should be treated by endodontist of corresponding level according to the difficulty.

6. Repeated transcranial magnetic stimulation combined with Donepezil can improve the cognition of cognitively impaired stroke survivors

Objective:
To investigate the effect of repetitive transcranial magnetic stimulation (rTMS) combined with donepezil on the cognition of persons with post-stroke cognitive impairment (PSCI) and their ability to perform activities of daily living (ADL).
Methods:
A total of 106 PSCI patients were randomly divided into an observation group and a control group using a random number table. Those in the observation group received 10Hz rTMS (5 seconds on and 25 seconds off for 20 minutes daily) and donepezil daily, 5 days per week for 4 weeks, while those in the control group were provided with donepezil but only sham rTMS on the same schedule. Before and after 4 weeks of treatment, the Montreal cognitive assessment scale (MoCA), the Rivermead behavior memory test (RBMT) and the modified Barthel index (MBI) were used to evaluate the subjects′ cognitive functioning, memory capacity and ADL ability. The latency and amplitude of auditory event-related potential P300 were also assessed using a myoelectric evoked potential apparatus.
Results:
After the treatment, improvement was observed in all the measurements of both groups. After the treatment, the average MoCA, RBMT and MBI scores, as well as the latency and amplitude of P300 in the observation group were all significantly better than among the control group.
Conclusions
rTMS can supplement donepezil′s ability to improve the cognition and ADL ability of persons with PSCI. Such therapy is worthy of clinical promotion and application.
7.Research Status and Progress on Immunotherapy for Advanced Esophageal Squamous Cell Carcinoma in Hebei Province

Esophageal squamous cell carcinoma (ESCC) is one of the most prevalent gastrointestinal cancers in Hebei province. Most patients are diagnosed at an advanced stage due to untypical early symptoms, making surgical treatment often unfeasible. As a result, most of them are treated with comprehensive medical treatment, which tends to have limited effectiveness and poor prognosis. However, the recent introduction of immunotherapy has considerably advanced treatment options for patients with advanced ESCC, notably improving their prognosis. Uncertainties regarding immune resistance, the selection of predictive indicators of efficacy, the identification of dominant population, and the choice of combined treatment strategies are still evident. Therefore, based on the high incidence and poor prognosis of ESCC in Hebei Province, this article will briefly review the research progress in the treatment of advanced ESCC, aiming to provide guidance for treatment strategies for advanced esophageal cancer.
8.Practice and effect evaluation of self-nursing on the stent-tract in the discharged patients with orthopedic external fixation support

Objective To investigate the effect evaluation of self-nursing on the stent-tract in the discharged patients with orthopedic external fixation support. Methods A total of 112 patients with orthopedic external fixation support were selected, and designed the self-nursing material. Before discharging guidance, make sure that the patient or patients′ family mastered the operation methods and nursing key points. After discharge, the continuing nursing support was provided. Results The incidence of infection was 4. 46%. All the 112 patients, the times of return visits were 432, and the times of changing the dressing were 3 940, with the mean times of 35. 2;The reducing the costs were 43 340, with the mean reducing of 387. Conclusions The self-nursing on the stent-tract in the discharged patients with orthopedic external fixation support can save the human resource, time and medical costs of the patients and patients′ family, it can also help the doctors with discharged treatment and reduce the work of nurses.
9.Apolipoproteins as Differentiating and Predictive Markers for Assessing Clinical Outcomes in Patients with Small Cell Lung Cancer.

PURPOSE: The present study aimed to investigate the value of apolipoproteins, including ApoA-1, ApoC-III, and ApoE, in patients with small cell lung cancer (SCLC) as potential biomarkers for diagnosis, prognosis, and cancer progression. MATERIALS AND METHODS: Lung samples were collected from 89 patients with SCLC. Nineteen lung samples from non-small cell lung cancer (NSCLC) patients and 12 normal lung tissues were used as controls. Expression profiles of ApoA-1, ApoC-III, and ApoE in different samples were examined using immunohistochemical methods, and the expression levels were correlated with cancer types, treatment, and outcomes using chi-square and Mann-Whitney tests. RESULTS: Expression of ApoA-1 and ApoC-III in SCLC was significantly different, compared with that in NSCLC and normal lung tissues, and was correlated with recurrence of SCLC. Patients undergoing neoadjuvant chemotherapy before surgery showed significantly reduced expression of ApoA-1 and increased expression of ApoC-III and ApoE. Nevertheless, the expression levels of ApoA-1, ApoC-III, and ApoE were not correlated with SCLC staging. CONCLUSION: ApoA-1 and ApoC-III may be used as differentiating and predictive markers for SCLC. ApoA-1, ApoC-III, and ApoE may be used to monitor the efficacy of chemotherapy.

10.Research on the effect of emergency rescue ability training for nurses in operating room based on video tracking method

Objective:To explore the effect of emergency rescue ability training of operating room nurses based on video tracking method, and provide reference for improving emergency ability of operating room nurses.Methods:This study was a quasi-experimental study. In March 2021, 85 nurses working in the Department of Anesthesiology and Surgery of the Provincial Hospital Affiliated to Shandong First Medical University were selected by cluster sampling method. We implemented a training program of rescue emergency ability based on video tracking and adopted the inspection list of rescue emergency ability assessment and doctors′ satisfaction questionnaire of operating room nurses to evaluate the scores of each item in the nurses' rescue emergency ability list and doctors' satisfaction of rescue cooperation of operating room nurses.Results:The scores of the dimensions of recognition of fatal arrhythmias, fatal arrhythmia, several situations that can directly call help, writing of emergency state nursing records and handover of medical records, use of defibrillator, correct use and maintenance of negative pressure attraction, use and management of rescue vehicle and situation disposal in the inspection list of rescue ability after training were 9.80 ± 1.61, 26.06 ± 2.20, 17.34 ± 1.29, 13.00 ± 1.57, 7.35 ± 0.74, 10.24 ± 1.14, 33.89 ± 2.73, which were higher than before training 9.24 ± 1.18, 24.92 ± 2.15, 15.69 ± 1.92, 12.21 ± 1.66, 6.55 ± 0.92, 8.94 ± 1.32, 32.94 ± 2.20. The differences were statistically significant ( t values were -6.83 to -2.51, all P<0.05); after the training, in the questionnaire of doctors' satisfaction with nurses, surgical materials and instruments preparation, first aid skill operating level, attention to surgical progress, active and correct delivery, orderly and busy, coordination and communication ability, professional knowledge, evaluation ability and foresight, ability to deal with emergencies, clear division of labor and good cooperation, and responsibility scores were 4.22 ± 0.58, 4.52 ± 0.54, 4.53 ± 0.47, 4.43 ± 0.58, 4.44 ± 0.44, 4.37 ± 0. 59, 4.45 ± 0.51, 4.51 ± 0.53, 4.51 ± 0.57, 4.17 ± 0.63, which were higher than the pre-training 4.05 ± 0.58, 4.38 ± 0.56, 4.26 ± 0.76, 4.04 ± 0.67, 4.25 ± 0.62, 4.19 ± 0.74, 4.25 ± 0.74, 4.34 ± 0.67, 4.21 ± 0.84 and 3.56 ± 0.58. All differences were statistically significant ( t values were -8.22 to -2.10, all P<0.05). Conclusions:The training method based on video tracking method improved the emergency rescue ability of operating room nurses and the doctor's satisfaction with rescue cooperation, and provided a reference for the training of operating room nurses.
