1.Effects of intrathecal 2-PMPA on chronic inflammatory pain in rats

Objective To evaluate the effects of intrathecal 2-PMPA on chronic inflammatory pain in rats . Methods Twenty four male Sprague-Dawley rats ,aged 4-6 months ,weighing 200-250 g ,were randomly divided into 3 groups (n=8 each) using a random number table :normal saline (NS) group ,complete Freund′s adjuvant (CFA ) group and N-acetylaspartylglutamate peptidase inhibitor 2-PMPA group (group 2-PMPA ) . Chronic inflammatory pain was induced by injecting 100μl of CFA into the plantar surface of the left hindpaw .Immediately after injection of CFA ,2-PMPA 100 μg was injected intrathecally once a day for 3 consecutive days in group 2-PMPA ,while the equal volume of NS was given instead of 2-PMPA in NS and CFA groups .The paw withdrawal latency to thermal nociceptive stimulus (TWL ) and paw withdrawal threshold (PWT ) to von Frey filament stimulation were measured before injection of CFA (baseline ,T1 ) and after the last injection of CFA (T2 ) .Then the rats were sacrificed and the L4 ,5 segments of the spinal cord were removed for determination of NR2B expression by Western blot .Results Compared with group NS ,TWL and PWT were significantly decreased at T2 and the expression of NR2B was up-regulated in CFA and 2-PMPA groups ( P<0.05 ) .Compared with group CFA ,TWL and PWT were significantly increased at T2 and the expression of NR2B was down-regulated in group 2-PMPA ( P<0.05) .Conclusion Intrathecal 2-PMPA can alleviate CFA-induced chronic inflammatory pain in rats ,and inhibition of NR2B expression in the spinal cord is involved in the mechanism .
2.Analysis of Prescribing Patterns in Outpatients based on the Latest Chinese Guideline for the Management of Hypertension

Objective To investigate the pattern of antihypertensive medication prescribing in outpatients from the Second Hospital of Tianjin Medical University, and analyze the shortcoming and deficiency compared with 2010 Chinese guidelines for the management of hypertension. Methods A total of 154 262 electronic prescribing for outpatients with hy-pertension, from January-December 2012 in a Grade 3A hospital in Tianjin, were enrolled in this retrospective survey. Data of commonly used antihypertensive medication and combination therapy in patients were analyzed. The patient data collected were divided into different groups according to age, gender, high blood pressure level and the onset of the season. Results (1)The list of the drugs commonly used for treating hypertension in outpatients were calcium antagonist (52.3%), angiotensin receptor blockers (34.0%),βblockers (25.9%), angiotensin-converting enzyme inhibitors (12.1%), fixed-dose combination (11.0%) and diuretics (1.4%).(2)The fewer combination therapy was found in outpatients than that of monotherapy (43.9%vs 56.1%). Some prescriptions were not routinely recommended by the Guideline (4.6%).(3)The combination therapy used in patients with stage 3 hypertension was higher than that of patients with stage 1or stage 2 hypertension (44.5%vs 37.7%vs 37.7%, P<0.01). The rate of combination therapy was significantly higher in cardiology department than that of other clini-cal departments (P<0.01). The combination therapy tended to be used in the elderly patients than that of non-elderly pa-tients (P<0.01). The number of prescriptions was lower in summer than that of other seasons,but the rate of combination therapy was higher in summer than that of spring, autumn and winter (P<0.01). Conclusion The prescriptions of combina-tion therapy and diuretic were inadequate in outpatients with hypertension. These findings indicate the difference between clinical prescription and the guideline for the management of hypertension.

5. Experimental Study of the Antioxidant Effect of Taurine Against Radiation-induced Intestinal Injuries

Background: Oxidative stress is crucial for the development of ionizing radiation-induced intestinal injuries and inflammation. Taurine is a sulfur-containing amino acid distributed in tissues and organs throughout the body. It possesses several important physiological functions, including anti-oxidative activity and anti-inflammatory effects. Aims: To investigate the therapeutic effect and underlying mechanism of taurine against radiation-induced intestinal injuries. Methods: CCK-8 assay and DCFH-DA, a fluorescence probe of intracellular reactive oxygen species (ROS) were applied to determine the effects of taurine on radiation-induced inhibition of cell viability and ROS accumulation in human intestinal epithelial cell line HIEC-6. The modulatory effects of taurine on expressions of oxidative stress-related genes in intestinal cells after exposure to radiation were measured by real-time PCR in in vitro and in vivo studies. The roles of taurine in maintenance of intestinal morphology and suppression of cell apoptosis in mice receiving whole body radiation were assessed by HE staining and TUNEL staining. Results: In in vitro study, taurine improved the radiation-induced inhibition of cell viability and reduced intracellular ROS accumulation in HIEC-6 cells. The expression levels of catalase (CAT) and glutathione peroxidase 1 (GPx1) in HIEC-6 cells were up-regulated by taurine treatment. In vivo study showed that taurine activated the nuclear factor erythroid 2-related factor 2/heme oxygenase-1 (Nrf2/HO-1) signaling pathway, alleviated intestinal villi disorganization and loss of crypt cells, and suppressed cell apoptosis in mice after radiation. Conclusions: Taurine can reduce the intracellular ROS accumulation via activating Nrf2/HO-1 antioxidant signaling pathway, and thereby, exerts protective effect against radiation-induced intestinal injuries. It might be a candidate for treatment of intestinal injuries in patients undergoing radiotherapy.
6.Effect of different serum sodium levels on heart function and prognosis in very elderly patients with heart failure

Objective To investigate the effect of different serum sodium levels on heart function and prognosis in very elderly patients with heart failure. Methods Totally 152 elderly patients(aged ≥ 85 years)with hyponatremia were enrolled.Based on the serum sodium level ,they were divided into mild(125 mmol/L ≤ Na+ < 135 mmol/L) ,moderate(125 mmol/L ≤ Na+ < 135 mmol/L) ,and severe hyponatremia(Na+ < 115 mmol/L)groups ,and 76 patients with normal blood sodium level(Na+ ≥ 135 mmol/L)were selected as control group. The general data ,levels of serum creatinine and N-terminal pro-brain natriuretic peptide (NTproBNP)and prognosis were compared among the above groups. Results Compared with the control group ,patients with severe hyponatremia were much older ,with more diuretics administration ,higher level of serum creatinine , lowerlevelofeGFRandleftventricularejectionfractions[(38.1±3.9)% vs.(45.2±9.7)% ].There were higher incidences of coronary atherosclerotic heart disease [72.4% (n=21)vs. 56.6% (n=43)] ,diabetes[41.4% (n=12)vs. 23.7% (n=18)] ,chronic renal insufficiency [22.6% (n=8)vs. 11.8% (n=9)] ,stroke[20.6% (n=6)vs. 9.2% (n=7)]in patients with severe hyponatremia compared with other groups. The severe hyponatremia group showed significantly higher levels of NTproBNP [(4823. 9 ± 588. 1 )ng/L v s. (1124. 4 ± 349. 1 )ng/L ,(1836. 2 ± 369. 3 )ng/L ,(2894. 1 ± 687. 3 )ng/L ]and higher rates of mortality [27.6% (n=8) vs.5.3% (n=4) ,7.6% (n=6) ,13.6% (n=6)]as compared to the control group ,mild and moderate hyponatremia groups. Furthermore ,length of stay in hospital were markedly longer in patients with severe hyponatremia than those with mild and moderate hyponatremia[(11.1 ± 7.1)d vs. (19.6 ± 5.7)d ,(16.1 ± 4.2)d] . Conclusions Hyponatremia is a common type of electrolyte disorder in very elderly patients with heart failure ,and severe hyponatremia may predictand increased risk of death.
7. Application of mesh elastic gloves in the treatment of zinc oxide paste in patients with subacute eczema in the hands

Objective:
To introduce the method of making mesh elastic gloves, and to explore its application effect in the treatment of zinc oxide paste in patients with subacute eczema in the hands.
Methods:
This study was a kind of experimental study. A total of 96 patients with subacute eczema in the hands in the Dermatology Ward of the First Affiliated Hospital of Yangtze University were selected as the study subjects.They were divided into the control group (admitted from June to December in 2017) and the experimental group (admitted from June to December in 2018) according to the time of admission, 48 cases in each group. After the two procedures(apply medicine and cover with sterile gauze) of the conventional zinc oxide paste coating, the control group was fixed with "gauze bandage+ medical tape", and the experimental group was fixed with "mesh elastic gloves". The external fixed cost and time, the disassembly time, the dressing fixation, the comfort condition, the grip strength, the daily activity of the hand, and the healing effect of the skin lesions were evaluated.
Results:
The external fixed cost and time, the disassembly time, grip strength and hand daily activities ability of the patients in the experimental group were (0.37±0.05) yuan, (26.06±2.53) s, (13.70±1.28) s, (25.96±5.83) kg, and (65.04±7.10) points, significantly better than the control group (1.63±0.21) yuan, (160.98±22.82) s, (22.36±3.50) s, (20.01±3.56) kg, (61.83±6.31) points, the difference between the two groups was statistically significant (
8.Content Analysis of 5 Kinds of Heavy Metal in Yougui Pills

OBJECTIVE:To investigate the contents of 5 kinds of heavy metal as copper (Cu),arsenic(As),cadmium (Cd),mercury(Hg)and lead (Pb)in Yougui pills ,and to evaluate its safety risk. METHODS :Using yttrium (89Y),indium (115In)and bismuth (209bi)as internal standard ,the contents of each element were determined by ICP-MS. ICP-MS condition included that atomization gas flow rate was 0.95 L/min,auxiliary gas flow rate was 1.2 L/min,plasma gas (argon)flow rate was 18 L/min,pump speed was 30 r/min. RF power of inductively coupled plasma was 1 200 W,the voltage in simulation stage was 1 750 V,the voltage in pulse stage was 1 300 V,the voltage of deflection device was -12 V,and the detector was in analog and pulse dual-mode. The determination methods of various elements were investigated ,and 45 batches of marketed Yougui pills were determined. Hazard index (HI)was used to analyze the non-carcinogenic risk of each element and calculate the maximum residual limit(MRL)of each element. RESULTS :The linear range of Cu ,As,Cd,Hg and Pb ranged from 10-200,1-50,0.4-30,0.2-6 and 2-100 μg/L(all r>0.999 0),respectively. The limits of quantitation were 0.67,0.23,0.20,0.07,0.27 μg/L. The limits of detection were 0.20,0.07,0.06,0.02,0.08 μg/L. RSDs of precision,stability and reproducibility tests were all lower than 3.5% (n=6 or n=5). Average recoveries were 92.96%-100.89%(RSD=2.23%-3.62%,n=3). Average contents of Cu ,As,Cd,Hg and Pb in 45 batches of Yougui pills were 2.72,0.28,0.07,0.05,0.62 mg/kg,and superimposed HI of each element was less than 1. The contents of 5 kinds of heavy metals in Yougui pills were lower than the proposed MRL (MRL of Cu ,As,Cd,Hg and Pb were 20,2,1,0.2,5 mg/kg or 111.11,4.44,2.22,1.48,8.89 mg/kg respectively ). CONCLUSIONS :Established method can be used for the determination of content of 5 kinds of heavy metal in Yougui pills ;the heavy metal pollution rate of marketed Yougui pills is low and the safety risk is small.
9.Clinical and immune response characteristics among vaccinated persons infected with SARS-CoV-2 delta variant: a retrospective study.

OBJECTIVES:
This study aimed to observe the clinical and immune response characteristics of vaccinated persons infected with the delta variant of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) in Yangzhou, China.
METHODS:
We extracted the medical data of 129 patients with delta-variant infection who were admitted to Northern Jiangsu People's Hospital (Yangzhou, China) between August and September, 2021. The patients were grouped according to the number of vaccine doses received into an unvaccinated group: a one-dose group and a two-dose group. The vaccine used was SARS-CoV-2-inactivated vaccine developed by Sinovac. We retrospectively analyzed the patients' epidemiological, clinical, laboratory, and imaging data.
RESULTS:
Almost all patients with delta-variant infection in Yangzhou were elderly, and patients with severe/critical illness were over 70 years of age. The rates of severe/critical illness (P=0.006), fever (P=0.025), and dyspnea (P=0.045) were lower in the two-dose group than in the unvaccinated group. Compared to the unvaccinated group, the two-dose group showed significantly higher lymphocyte counts and significantly lower levels of C-reactive protein (CRP), interleukin-6 (IL-6), and D-dimer during hospitalization and a significantly higher positive rate of immunoglobulin G (IgG) antibodies at admission (all P<0.05). The cumulative probabilities of hospital discharge and negative virus conversion were also higher in the two-dose group than in the unvaccinated group (P<0.05).
CONCLUSIONS
Two doses of the SARS-CoV-2-inactivated vaccine were highly effective at limiting symptomatic disease and reducing immune response, while a single dose did not seem to be effective.
