1.Case-control study on locking titanium plate in treating comminuted proximal humeral fracture in elderly.

OBJECTIVETo observe therapeutic effects of locking titanium plate for the treatment of comminuted proximal humeral fracture in elderly.
METHODSFrom June 2011 to May 2013, 72 elderly patients with comminuted proximal humeral- fractures were divided into locking titanium plate group and anatomical plate group, 36 cases in each group. In locking titanium plate group, there were 16 males and 20 females aged from 60 to 79 years old with an average of (69.55±5.62) years old; 10 cases were type Neer II, 18 were type Neer III and 8 cases were type Neer IV in accordance with Neer classification; treated with locking titanium plate. In anatomical plate group, there were 15 males and 21 females aged from 60 to 81 years old with an average of (69.76±5.70) years old; 9 cases were type Neer II, 20 were type Neer III and 7 cases were type Neer IV; and treated with anatomical plate. Clinical effects, preoperative and postoperative Neer scoring, operative time, bone healing time and incidence of complications between two groups were compared.
RESULTSAll patients were followed up from 1 to 3 years with an average of 15 months. The excellent and good rate of locking titanium plate group (91.7%) was significantly higher than anatomical plate group (75.0%). Postoperative Neer score of two groups were improved obviously, but locking titanium plate group (92.51±7.85) was higher than anatomical plate group (83.64±8.56); there was no significant differences between two groups in operative time (P>0.05); bone healing time in locking titanium plate was (18.6±3.4) weeks, and shorter than anatomical plate group (24.3±3.9) weeks; incidence of complications in locking titanium plate was (5.6%) shorter than anatomical plate group (22.2%), and had obviously differences between two groups.
CONCLUSIONLocking titanium plate for the treatment of comminuted proximal humeral fracture in elderly plays an important role in good rate, bone healing time and Neer score. It has advantages of early rehabilitation exercise, less shoulder pain, rapid recovery of shoulder joint, less complications, safe and effective, and be worthy of clinical application, especially for senile osteoporosis patients.

2.Therapy progression in surgery of inflammatory bowel diseases.

The inflammatory bowel diseases (IBDs), consisting of ulcerative colitis (UC) and Crohn's disease (CD), are characterized by idiopathic, chronic inflammation of the gastrointestinal tract. The overall incidence of IBDs is constantly increasing in eastern countries. In comparison with the data from western nations, in China, the incidence of male IBDs is relatively higher, the onset age is older. The severity of most cases is mild to moderate. The occurrence of fistula and peri-anal involvement are rare. Although significant improvements of IBDs therapy have been achieved in recent years, there are still over 30% UC and 70% CD cases need at least one surgery throughout their life span. Here we review the literatures published in recent years about the surgical management of IBDs.

3.Studies on chemical constituents in roots of Heracleum rapula.

OBJECTIVETo study the chemical constituents in roots of Heracleum rapula.
METHODThe constituents were isolated by column chromatography on silica gel and ODS, and identified by spectroscopic methods.
RESULTEight compounds, xanthotoxol (I), 8-geranyloxypsoralen (II), (+)-marmesin (III), beta-sitosterol (IV), stigmasterol (V), oleanolic acid (VI), ferulic (VII), scopoletin (VIII) were isolated and identified.
CONCLUSIONCompounds IV, V, VI, VII, VIII were isolated from this plant for the first time.

4.Mental disorder and suicide among youths in rural China: a case control study based on consecutive samples from Hunan, Liaoning and Shandong provinces

Objective To study the prevalence of mental disorders among the Chinese youths aged 15-34 years,in rural areas and to identify risk factors related to suicide.Methods A consecutive sampling strategy was used for suicidal cases in 16 randomly selected counties in Hunan,Liaoning,and Shandong provinces.Between 2005 and 2008,a total of 392 suicide cases were recruited with 416 community controls at the same age range,selected from the same areas one family member together with one close friend of each suicidal case were interviewed,using the psychological autopsy (PA) method.The same method with structured instruments was performed on the two informants for each control in the same community.SCID was used for the diagnosis of mental disease.Results 48.0% of the suicides were diagnosed as having at least one mental disorder episode,in comparison with only 3.8% among the controls.It was found that mental disorder was the most important risk factor for the Chinese young suicide cases in the rural areas.Conclusion As seen in the Western countries,mental disorder had also been the number one correlate on suicidal cases in China,with the difference as other social and psychological factors might have played relatively more important roles in China.
5.aa155-171 Motif Deletion of MyD88 Attenuates Expression of Co-stimulatory Molecules and Cytokines in Immune Associated-cells

Myeloid differentiation factor MyD88 is a critical adaptor molecule that integrates and transduces intracellular signals in inducing the differentiation of dendritic cells (DCs).The domain regions within MyD88 was searched,it could potentially affect the function of dendritic cells and found that MyD88 aa155-171 motif not only regulate the activity of transcription factor NF-?B, but also control the production of cytokines and expression of costimulatory molecules. Indeed, aa155-171 motif deleted type MyD88 (MyD88155-171) transfected RAW264.7 cells exhibited the reduced NF-?B and AP-1 activity and interrupted the expression of CD86 and B7H1. Meanwhile, lower level expression of cytokines such as IL-12,IFN-? were also observed by means of cytokine array in MyD88-/-DC trasfected with MyD88155-171 as compared to the MyD88 transfected cells. Thus, aa155-171 motif inside MyD88 could affect the expression of costimulatory molecules, production of cytokines and transduction of Toll like receptor signal pathway, suggesting that this motif may play an important role in regulating responses of innate immune system.
6.Surveillance of antimicrobial resistance in Enterobacteriaceae isolated from first class hospitals in China of 2012

Objective To investigate the antimicrobial resistance of Enterobacteriaceae isolated from patients of first class hospitals in China in 2012.Methods All the clinical isolates of Enterobacteriaceae were collected and the susceptibility tests of the isolates were performed by 557 first class hospitals around China from 1st Jan.2012 to 31st Dec.2012.The susceptibilities of isolates to antimicrobial agents were determined by using CLSI (2010) guideline and the susceptibility data were processed with Whonet 5.6 software.Results A total of 372052 clinical isolates were collected.Among all the strains Escherichia coli ( 161211starins, 43.3%) outnumbered the other bacteria, followed by Klebsiella pneu-moniae (112600 strains, 30.3%), Enterobacter clacaer (31887strains, 8.6 %).The most effective antimicrobials were carbapenems.The re-sistant rates of Escherichia coli to imipenem and meropenem were both be-low 12.5%, and the rates in other Enterobacteriaceae were below 10%.Besides this, piperacillin/tazobactam, cefoperazone/sulbactam and ami-kacin demonstrated good antibacterial activity against Enterobacteriaceae in vitro.Overall, little difference was observed in the resistant rates of Enterobacteriaceae isolated from first class hospi-tals in different parts of China.Conclusion Enterobacteriaceae from firs class hospital were of different degree resis-tance to various antimicrobial agents.Carbapenems remained very high activity against Enterobacteriaceae,but the car-bapenem resistance strains has appeared and the issue should be attracted attention.
7.Surveillance of antimicrobial resistance in nonfermenting gram-negative bacteria in China of 2012

Objective To investigate the antimicrobial resistance of nonfermenting gram-negative bacteria in China in 2012.Methods All the clinical isolates of nonfermenting gram -negative bacteria were collected from 557 tertiary hospitals from 1st January 2012 to 31st December 2012 in China.The antimicrobial susceptibilities of the isolates were de-termined by using various methods such as broth microdilution, disk diffusion, or E-test which according to CLSI 2012 guideline.The sus-ceptibility data were processed with WHONET 5.6 software.Results A total of 198334 nonfermenting gram-negative bacteria susceptibility data were obtained.Among all the isolates, the top 3 isolated nonfermenting gram-negative strains were Pseudomonas aeruginosa ( 80964 strains, 40.8%) , followed by Acinetobacter baumannii(67368 strains, 34.0%) , and Stenotrophomonas maltophilia ( 17520 strains, 8.8%).Against Pseudomonas aeruginosa, polymyxin B, tobramycin, ceftazidime, ciprofloxacin, levofloxacin, cefepime, gentamicin and cefoperazone/sulbactam ( CSL) were the only agents to which >70%of isolates were susceptible.No antibiotics except polymyxin B ( 96.9%) , minocycline ( 61.4%) , amikacin (58.7%), CSL (54.3%) and tobramycin (51.3%) were active more than 50% against Acinetobacter baumannii.Susceptibility rates of carbapenem against Acinetobacter baumannii were around 49%, about 9 percentage points higher than 2011 results.There are some differences among the monitoring results from various districts in China.The suscep-tibilities of Pseudomonas aeruginosa and Acinetobacter baumannii to the tested antimicrobial agents in Central China were generally low, whereas the antimicrobial susceptibilities of Acinetobacter baumannii from North China were genera-lly high.Conclusion The multidrug resistance of nonfermenting gram -negative bacteria, especially Acinetobacter baumannii, has become a worldwide problem.There are some differences among the susceptibilities of isolates from various districts.Polymyxin B has the most effective in vitro antimicrobial activity against Pseudomonas aeruginosa and Acinetobacter baumannii isolates, the emerging resistant isolates should cause enough attention.Further surveillance needed to be strengthened to prevent the further development and spread of resistant bacteria.
8.Antimicrobial susceptibility pattern and epidemiology of bacteria isolated from bone and joint in China, 2012

Objective To investigate the bacterial distribution and resist-ance to antibiotic of bone and joint infection in China, 2012.Methods The bacteria isolated from bone and joint in the tertiary hospitals were routinely identified.Disc diffusion test, minimum inhibitory concentra-tion ( MIC) test and E-test were used to detect the antimicrobial sensi-tivity.Results All the clinical stains isolated from 557 tertiary hospi-tals.2636 pathogenic strains were collected from bone and joint speci-men, which included Klebsiella pneumonia 370 strains (14.0%), Esche-richia coli 348 strains ( 13.2%) , Acinetobacter baumannii 319 strains (12.1%), Staphylococcus aureus 252 strains (9.6%) and Pseudomonas aeruginosa 167 strains ( 6.3%).The resistant rate of Staphylococcus aureus strains to oxacillin were 36.0%, no vancomycin and linezolid re-sistant isolate were found.The resistant rate of Escherichia coli, Klebsiella pneumonia and Enterobacter cloaecae strains to ceftriaxone were 64.6%, 33.0%and 37.4%, respectively, the resistant rate to imipenem were<2%.The resistant rate of Pseudomonas aeruginosa to carbapenem, ceftazidime and cefepime were <20%, the resistant rate to quinolone were <10%.The resistant of Acinetobacter buamannii to cabarpenem were <40%, the resistant rate to amikacin were 18.8%.The resistant rate of Acinetobacter lwoffii to cabarpenem were <1%, and the resistant rate to pipercillin and cefepime were <10%.Conclusion Gram negative bacteria were predominant organism in bone and joint infections in China, it may be related to the initial em-piric therapy always cover Staphylococcus.There were some difference in bacterial resistance to different antibiotics.These data present information for the proper treatment of bone and joint infections.
9.Establishment of a nasopharyngeal carcinoma cell line CNE1 stably overexpressing ebv-miR-BART7 mediated by lentivirus.

OBJECTIVETo establish a nasopharyngeal carcinoma (NPC) cell line CNE1-pLVTHM/BART7 with stable ebv-miR-BART7 overexpression.
METHODSThe recombinant lentivirus pLVTHM/BART7 expression plasmid was packaged into mature lentivirus by 293FT cells and used to infect CNE1 cells. Flow cytometry was employed for sorting the GFP(+) cells. The efficiency of ebv-miR-BART7 overexpression was determined using qRT-PCR.
RESULTSThe recombinant lentivirus plasmid pLVTHM/BART7 was successfully constructed and verified by PCR and sequencing. The expression of ebv-miR-BART7 in CNE1 cells infected with the lentivirus pLVTHM/BART7 was significantly increased as compared with the negative control and the blank control cells.
CONCLUSIONThe recombinant lentivirus vector pLVTHM/BART7 results in high and stable expression of ebv-miR-BART7 in infected CNE1 cells, which provides a useful cell model for further studies of the role of ebv-miR-BART7 in nasopharyngeal carcinoma.

10.Antimicrobial susceptibility pattern and epidemiology of bacteria isolated from peritoneal fluid in China in 2012

Objective To investigate the bacterial distribution and resis-tance to antibiotic isolated from peritoneal fluid in China, 2012.Methods The bacteria isolated from peritoneal fluid in the tertiary hospitals were routinely identified.Disc diffusion test, minimun ihibitory concentration ( MIC) test and E-test were used to detect the antimicro-bial sensitivi-ty.Results Total of 10252 pathogenic strains were collec-ted from peri-toneal fluid specimens from 557 tertiary hospitals, which included Esche-richia coli 3215 strains ( 31.4%) , Klebsiella pneumoniae 860 strains ( 8.4%) , Enterococcus faecium 730 strains ( 6.9%) , Pseu-domonas aeruginosa 548 strains ( 5.3%) and Staphylococcus epidermidis 531 strains (5.2%).The resistance rate of Staphylococcus aureus strains to oxacillin were 47.4%, no vancomycin and linezolide resistant isolate were found.The rates of susceptibility of Escherichia coli, Klebsiella pneumoniae and Enterobacter cloacaeisolates to ceftriaxone were 28.2%, 50.8%and 41.0%, respectively;to meropenem were 96.7%, 88.8%and 96.5%, respectively.The rates of susceptibility of Pseudomonas aeruginosa to carbapenem, ceftazidime and cefepime were >60%, to quinolone was >75%.The rates of susceptibility of Acinetobacter baumannii isolates to cabarpenem were <40%, to amikacin were 50.8%.Conclusions The two most common species from peritoneal fluid in tertiary hospital were Escherichia coli and Klebsiella pneumonia.There were some difference in bacterial resistance to different antibiotics.
