1.The effects of domestic irbesartan on blood pressure and renal function in the aged with primary hypertension

Objective To evaluate the effects of domestic irbesartan on blood pressure and renal function in the aged with primary hypertension.Methods Renal function,plasma and urine β2 microglob ulin WBS measured and blood pressure was investigated by 24-hour ambulatory blood monitoring in 42 aged with primary hypertension who received domestic irbesartan(1 50 mg/d,followed up every 1-2 weeks.If the effect was not ideal,the dose could be added to 300 mg/d)in 8-week's treatment.Results After the treatment.24 hours recall systolic blood pressure,24-hour mean diastolic blood pressure,mean day systolic blood pressure,mean day diastolic blood pressure,mean night systolic blood pressure,mean night diastolic blood pressure were significantly reduced.At the same time,domestic irbesartan induced a remarkable reduction of plasma and urine β2 microglobulin [(5.9±3.3)μg/L vs(2.6±2.6)μg/L, (811.2±97.2) mmol/L vs(457.6±69.8)mmol/L,respectively].Conclusion The antihypertensive effects of domestic irbesartan can persist for 24-hour and Can reduce urinary protein.
2.Clinical value of carbon nanoparticle in gastrectomy of stomach neoplasms

Objective To evaluate the clinical application value of local injections of carbon nanoparticle in gastrectomy of stomach neoplasms.Methods Forty cases of stomach neoplasms with local injections of carbon nanoparticle in gastrectomy of stomach neoplasms were as experimental group,and 40 cases of stomach neoplasms with conventional gastrectomy of stomach neoplasms were as control group.The number of removed tiny lymph node and transferred tiny lymph node was compared.Results There was significant difference in the average number of removed tiny lymph node N1,N2,N3 between experimental group and control group (5.120 ± 0.455 vs.2.900 ± 0.245,3.890 ± 0.367 vs.1.750 ± 0.256,1.790 ± 0.224 vs.0.590 ± 0.054)(P < 0.01).There was significant difference in the average number of transferred tiny lymph node N1,N2,N3 between experimental group and control group (1.090 ±0.087 vs.0.430 ± 0.044,0.550 ± 0.052 vs.0.340 ± 0.027,0.410 ± 0.044 vs.0.130 ± 0.013)(P < 0.05).There was no significant difference in postoperative complications,mortality and length of hospital stay(P> 0.05).Conclusion Carbon nanoparticle in gastrectomy of stomach neoplasms is a simple,safe,easy method,and it has certain supplementary role in conventional gastrectomy of stomach neoplasms.
3.Autogenous orbital septum fat redistrbution in correction of sunken upper eyelid

Objective To study the effect and methods of orbital septum fat redistribution or autogenous orbital septum granular fat transplantation methods on upper eyelid depression.Methods Blepharoplasty incision was used to cut open musculus orbicularis oculi and septum orbitale horizontally,exposuring orbital fat,the conjunction was relieved between orbit fat envelope and septum orbitale,displaced orbital fat was extended and replaced to the depression region of upper eyelid,and the wound was routinely sutured.For orbital septum fat volume depletion,appropriate amount of granular fat was injected into depression region and put fat backward upto supraorbital margin.Then,resetting orbital septum and closing incision were conducted with suture technique of blepharoplasty.Results Of 48 cases (96 eyes),32 cases was followed-up for 6 to 42 months with mean of 11 months.All cases got ideal outcomes,with smooth and symmetry lid folds,depression of upper eyelid were fixed and no complication occurred.Conclusions Orbital septum fat redistribution or autogenous orbital septum granular fat transplantation can anatomically reduce the fat distribution of upper eyelid.This method is easy and effective to improve the appearance of eyelid.
4.Vulnerability of Atherosclerotic Plaque Through Contrast-enhanced Ultrasonography

Purpose To investigate the feasibility to diagnose and evaluate the vulnerability of the atherosclerotic plaque through contrast-enhanced ultrasonography. Materials and Methods Thirty-four patients with carotid atherosclerosis were enrolled in the study to take contrast-enhanced ultrasonography and were observed on whether the plaque was enhanced and the features of its enhancement. The peak intensity (PI), time to peak (TTP), and density echo (DE) were calculated according to the time-intensity curve with QLAB software. Results The ratio of enhanced malacoplakia and the mixed plaque showed no difference (χ2=0.847, P>0.05). The percentages of enhanced plaque distributed on the base, tail and the shoulder were 70.0%, 23.3% and 6.7%, respectively with significant difference (χ2=29.100, P<0.001); the distribution ratio of enhanced plaque from high to low were as follows: the plaque on the shoulder > the plaque on the base> the plaque on the tail. It was positively correlated between enhanced plaque and its distribution (r=0.404, P<0.01). The TTP of the ROI between the malacoplakias and the mixed plaques showed no difference (t=0.479, P>0.05). The PI and DE of the ROI in the malacoplakia and the mixed plaques were analyzed by the time-intensity curve and the differences proved to be statistically significant (t=7.497 and 12.224, P<0.05). Conclusion Contrast-enhanced ultrasonography could present the neovessels in the atherosclerotic plaque, which is helpful in evaluating the vulnerability of atherosclerotic plaque.
5.Mutations of the phenylalanine hydroxylase gene in phenylketonuria patients from Shaanxi.

OBJECTIVETo investigate the feature of phenylalanine hydroxylase (PAH) gene mutations and provide guidance for genetic and prenatal diagnosis of patients with phenylketonuria from Shaanxi.
METHODSFor 55 patients whose blood Phe concentration was over 2.0 mg/dL, potential mutations in 13 exons and flanking sequences of the PAH gene were detected by PCR and DNA sequencing.
RESULTSA total of 98 mutations were detected in 110 PAH alleles, with the detection rate being 89.10%. Nine mutations have been identified in exon 7, which accounted for 33.67% of all. Exon 12 (14.29%) and exon 3 (12.24%) have followed. Thirty eight mutations, locating in exon2-exon12 and the flanking sequence, were detected in the 55 PKU patients. p.R243Q (24.49%) was the commonest mutation, whilstp.A47E, p.I65S and p.A259T were first discovered in China. After querying international databases including PAHdb and HGMD, the p.C334X was verified as the novel PAH gene mutation.
CONCLUSIONThe mutation spectrum of the PAH gene in Shaanxi has been identified. And a novel mutation has been identified. This may facilitate the diagnosis of PKU in the future.

6.TIPE2 governs the phenotypic switch of adipose tissue macrophages via the TLR4 /IκBα/NF-κB pathway

Objective:To investigate the effect and molecular mechanism of tumor necrosis factor-α-inducible protein 8-like 2(TIPE2)on lipopolysaccharide(LPS)or interleukin-4(IL-4)-induced phenotypic switch of adipose tissue macrophages(ATM).Methods:The expression levels of TIPE2, inducible nitric oxide synthase(iNOS), monocyte chemoattractant protein 1(MCP-1), CD206, and arginase 1(Arg-1)in the visceral adipose tissue of obese mice, TIPE2-knockout(KO)mice, and control mice were detected by immunohistochemistry, Western blotting, and real-time PCR(RT-qPCR). Peritoneal macrophages isolated from KO and wild-type mice and RAW 264.7 mouse macrophage cell line were cultured, and then stimulated with LPS(100 ng/mL)or IL-4(20 ng/mL)for 6 hours. The expression levels of TIPE2, iNOS, MCP-1, CD206, and Arg-1 were detected by Western blotting and RT-qPCR.Results:Obese mice showed down-regulated TIPE2 expression, up-regulated pro-inflammatory markers iNOS and MCP-1 expressions, and down-regulated anti-inflammatory markers CD206 and Arg-1 expressions. LPS decreased the expression of TIPE2 in RAW 264.7 cells and peritoneal macrophages from mice, increased the expression of the classically activated macrophages(M1 phenotype)markers iNOS and MCP-1, and decreased the expression of the substituting activated macrophages(M2 phenotype)markers CD206 and Arg-1. IL-4 increased the expression of TIPE2 in RAW 264.7 cells and peritoneal macrophages, decreased the expression of iNOS and MCP-1, and increased the expression of CD206 and Arg-1. During the M1 polarization of macrophages, LPS increased toll-like receptor(TLR4)expression as well as nuclear transcription factor κBα suppressor protein(IκBα) and NF-κB phosphorylations in macrophages. Knockout of TIPE2 further increased the expression of the TLR4/IκBα/NF-κB signaling pathway and M1 macrophage markers, and further reduced the expression of the M2 macrophage markers.Conclusion:TIPE2 regulates ATM phenotypic transformation through inhibition of the TLR4/IκBα/NF-κB signaling pathway, which ameliorates adipose tissue inflammation in obese states.

8.Correlation between soil environment and yield and quality of Sharen (Amomi Fructus) under different planting patterns

【Objective】 To study the effects of soil environment on the growth, yield, and quality of Sharen
(Amomi Fructus) under different planting patterns.
【Methods】 Soil physical and chemical indices and enzyme activities in four periods including early flowering (March), full flowering (June), fruit ripening (September), and late fruit picking (December), were measured under three planting patterns including natural forest, greenhouse, and rubber forest in Xishuangbanna, China. The changes in soil indices during the growth periods of Sharen (Amomi Fructus) under different planting patterns were analyzed, and the differences in plant growth, yield, and quality under different planting patterns were explored. Pearson correlation analysis was used to analyze the relationship between soil indices and Sharen (Amomi Fructus) growth, yield, and quality. Principal component analysis was used to investigate the effects of soil environment under different planting patterns on Sharen (Amomi Fructus) growth, yield, and quality.
【Results】 The soil moisture, available potassium content, and urease activity of the three planting patterns of Sharen (Amomi Fructus) increased initially and decreased afterwards throughout the year; pH and organic matter content showed little change in the whole year. Exchangeable manganese content and acid phosphatase activity gradually increased throughout the year. Hydrolyzed nitrogen content, exchangeable calcium content, available zinc content, protease activity, and sucrase activity decreased initially and increased afterwards throughout the year. Exchangeable magnesium content, available iron content, and catalase activity decreased annually. Total nitrogen content, total phosphorus content, and available phosphorus content fluctuated throughout the year. The total potassium content under natural forest and greenhouse planting decreased throughout the year, while the total potassium content under rubber forest showed an upward trend all year round. The organic
matter content, total nitrogen content, total potassium content, available potassium content, available zinc content, urease activity, acid phosphatase activity, and catalase activity under greenhouse were significantly lower than those under natural and rubber forests (P < 0.05). Correlation analysis showed that plant growth, yield, and quality of Sharen (Amomi Fructus) were significantly correlated with soil organic matter, total nitrogen, hydrolyzed nitrogen, total phosphorus, available phosphorus, total potassium, available potassium, exchangeable manganese, exchangeable magnesium, exchangeable calcium, available zinc, urease, acid phosphatase, and invertase (P < 0.05). The results of the principal component analysis indicated that the soil environment of Sharen (Amomi Fructus) under natural forest was the best, followed by rubber forest and greenhouse. The order of its advantages and disadvantages is consistent with the growth index of Sharen (Amomi Fructus), but contrary to the yield
of Sharen (Amomi Fructus), indicating that the soil environment directly affects the growth index and nutritional components of plants.
【Conclusion】 Different planting patterns of Sharen (Amomi Fructus) have different soil nutrient content, and the change rules in the growths period are not similar, with some differences. Soil indices have impacts on plant growth, yield, and quality of Sharen (Amomi Fructus). Soil ecological environment is positively correlated with the growth characteristics of Sharen (Amomi Fructus) plants, but has no direct correlation with yield and quality.
