Objective To identify the potential correlation of serum 25-hydroxyvitamin D3 [25 (OH)D3]and C-reactive protein (CRP) with coronary artery disease.Methods We measured the serum 25 (OH)D3 and CRP levels in 416 suspected coronary heart disease patients who underwent coronary angiography.Gensini scores were used to assess the severity of coronary stenosis.Based on the results of coronary angiography,the 416 patients were divided into normal group (n =246),stable angina group (n =110),and acute coronary syndrome (ACS) group (n =60).Results The serum levels of 25 (OH) D3 in the normal group and stable angina group were significantly higher than that in the ACS gropu [(33.15 ± 20.95) nmol/L vs.(15.55 ±5.7) nmol/L,(28.93 ± 16.45) nmol/Lvs.(15.55 ± 5.7) nmol/L,bothP＜0.001].TheCRPlevelin the normal group was significantly lower than those in the stable angina group and the ACS group [(4.33 ± 0.12) rng/Lvs.(5.68 ± 0.25) mg/L,(4.33 ± 0.12) mg/Lvs.(5.73 ± 0.31) mg/L,bothP＜0.001].The serum 25 (OH)D3 was not correlated with CRP or Gensini score in the normal group.In the ACS group,the serum 25 (OH) D3 was negatively correlated with CRP level and Gensini score (r =-0.026,P =0.045 ; r =-0.256,P =0.048),while CRP level was positively correlated with Gensini score (r =0.459,P ＜0.001).Inthe stable angina group,the serum 25 (OH)D3 was also negatively correlated with CRP level and Gensini score (r =-0.211,P =0.027; r =-0.208,P =0.029),and the latter two were positively correlated (r =0.574,P ＜ 0.001).Conclusions Low serum 25 (OH) D3 levels are associated with coronary artery stenosis.Combining with CRP,it may be involved in the pathogenesis of coronary heart disease through inflammatory mechanism.

Objective To investigate the clinical effects of Kangfuxin Solution used together with Baofukangshuan on chronic cervicitis after having been processed cervical LEEP circumcision.Methods Retrospective analysis was made among 176 out-patients diagnosed of chronic cervicitis from November 2007 to January 2009.Of all these patients,90 patients were treated with Kangfuxin Solution and Baofukangshuan after LEEP circumcision(the treatment group),and 86 patients were treated with simple cervical LEEP circumcision(the control group).The wound healing,vaginal discharge,bleeding volume,and side effects were observed in both groups.Results The method in the treatment group significantly reduced the amount of vaginal discharge and the duration,decreased the amount of bleeding and the duration;excluded wound infection and adverse reactions,and facilitated cervical wound recovery.The curative rate(98.89%)in the treatment group was significantly higher as compared to the control group(86.05%)after 8 weeks of the treatment.Conclusion Cervical LEEP circumcision surgery supplemented by Kangfuxin solution and Baofukangshuan therapy has better efficiency than treated by simple cervical LEEP circumcision.

OBJECTIVE:To provide reference for storage management of injection drugs protected from light in inpatient phar-macy of hospital. METHODS:From Jan. to Mar. 2016,the storage items of injection drugs protected from light in inpatient phar-macy of our hospital and the current storage situation were surveyed,the quality management tool PDCA circulation was adopted to improve storage situation and evaluate the effect of PDCA. RESULTS:For improper storage of injection drugs protected from light caused by employees lacking knowledge in photophobic knowledge,unfamiliar to the list of injection drugs protected from light, no packaging in returned drugs,after developing and carrying out corresponding countermeasures,including reinforcing staff train-ing,studying characteristics of the new-coming drug timely,renewal photophobic knowledge and the list of injection drugs protect-ed from light,and ameliorating working procedure,the light storage pass rate was increased from 96.5% to 100% in 310 injection drugs protected from light in inpatient pharmacy. CONCLUSIONS:Appling PDCA can effectively improve the storage management of injection drugs protected from light and provide reliable protection for the drugs'safe and effective application.

Objective To investigate the clinical charactors and treatment of IgM nephropathy with nephrotic syndrome(NS) in children.Methods Thirty-six IgM nephropathy patients hospitalized in Hunan Children's Hospital as research group(group A),from June 2005 to June 2012.One hundred and six patients with minimal change disease (MCD) as control group (group B),followed up for 1 ～ 8 years.Results (1) Hematuria at presentation of the two groups respectly 3.8％ vs 30.6％ (x2 =20.403,P ＜ 0.05).(2) Renal pathology revealed that there were 26 (72.2％)patients with minimal change disease,9 cases (72.2 ％)with moderate membranoproliferative glomerulonephritis,1 cases with focal segmental glomerulosclerosis.(3) According to renal pathology,group A patients were divided into two sub groups:mild group and moderate group.To compared with group B,the steroid-resistant incidence of the 3 group were respectly 12.3％,19.2％,77.8％ (x2 =24.369,P ＜ 0.05).There was no significant difference between control group and mild group.(4)The remission rate of proteinuria in steroid-resistant patients who combined to use mycophenolate mofetil (MMF) with the two groups were respectively 50％ and 85.7 ％ (x2 =3.60,P ＞ 0.05).Conclusion Incidence of hematuria is higher in IgM nephropathy patients and patients with renal pathology for moderate lesions have a high steroid-resistant,and need use immunosuppressive early.MMF may be a good immunosuppressive for theses patients.

Objective To study the effect of the health education path on health education in patients with diabetes.Methods According to random number table method,100 type 2 diabetes mellitus(T2DM)patients were divided into control group and health education path group.Health education path group received health education through the health education path and the control group by traditional methods.The rate of diabetes-related knowledge mastery and the control of blood glucose in two groups were compared. Result The rate of diabetes-related knowledge mastery and the control of blood glucose in the health education path group were significantly better than those of the control group(P<0.05,for both).Conclusion Health education according to health education path to the patients with diabetes can increase the degree of knowledge and the control effect of blood sugar.

Objective To compare the curative effects and safety of mirtazapine and fluoxetine in treatment of elderly depressive patients with insomnia.Methods 92 cases of elderly depressive patients with insomnia were randomly divided into two groups: the mirtazapine treatment group and the fluoxetine treatment group. The period of treatment was six weeks. The antidepressant effect was assessed with the drop rate of Hamilton Depression Scale (HAMD),the effect of improving sleep was assessed with HAMD sleep disturbance factor and the Pittsburgh Sleep Index (PSQI), and the administration of security was assessed with vice Treatment Emergent Symptom Scale (TESS).Results After treatment, the effective rate of the mirtazapine group was 90.9% (40/44), that of the fluoxetine group was 84.1% (37/44). In the end of 1st, 2nd, and 4th weeks after treatment, the difference between the two groups were significant (P<0.001). At the end of 6th week, the difference was not significant (P>0.05) in HAMD; the difference was significant in HAMD sleep disturbance factor and PSQI score (P<0.05).Conclusion The clinical effects of mirtazapine in treatment of depression are similar to that of fluoxetine, but mirtazapine takes effects fast, can improve the symptoms of insomnia, has higher safety and better compliance, and is more appropriate for the depressive elderly patients with insomnia than fluoxetine.

OBJECTIVE:To investigate the effects of Astragalus injection combined with Salmeterol propionate powder inhala-tion on pulmonary function,cytokines and blood rheology indexes of Chronic obsrnctine pulmonary disease (COPD) patients. METHODS:104 COPD patients were divided into observation group and control group according to the randorn number table meth-od,with 52 cases in each group. Control group was given routine symptomatic treatment+Salmeterol propionate powder inhalation 1 dose,bid;observation group was additionally given Astragalus injection 30 ml added into 5% Glucose injection 250 ml,ivgtt, qd,on the basis of control group. Treatment course of 2 groups lasted for 2 weeks. Clinical efficacy,pulmonary function indexes [forced expiratory volume in one second(FEV1),forced vital capacity(FVC),FEV1/FVC],CRP,cytokine [IL-6,IL-8,TNF-α] levels,blood rheology indexes (whole blood high-shear viscosity,whole blood low-shear viscosity,fibrinogen,hematokrit and plasma viscosity),and the occurrence of ADR were observed in 2 groups. RESULTS:The total effective rate of observation group (94.23%)was significantly higher than that of control group(75.00%);compared with before treatment,FEV1,FVC and FEV1/FVC of 2 groups were significantly increased after treatment,and those of observation group were significantly higher than those of control group;CRP,IL-6,IL-8 and TNF-α of 2 groups were significantly decreased,and those of observation group were signifi-cantly lower than those of control group. The whole blood high-shear viscosity,whole blood low-shear viscosity,fibrinogen,hema-tocrit and plasma viscosity of observation group were decreased significantly after treatment,and the observation group was signifi-cantly lower than the control group,with statistical significance(P<0.05). No significant ADR was found in 2 groups. CONCLU-SIONS:Astragalus injection combined with Salmeterol propionate powder inhalation is significantly effective for COPD,improves pulmonary function of patients,improves micro inflammatory state and decreases blood rheology indexes with good safety.

Aim To investigate the role of mitochondrial aldehyde dehydrogenase 2 ( ALDH2) in the cardio-protection of ischemic postconditioning in isolated rat hearts. Methods Hearts isolated from male Sprague-Dawley rats were perfused on a langendorff apparatus and subjected to 30 min of regional ischemia( occlusion of left anterior descending artery) followed by 120 min reperfusion. Ischemic postconditioning was achieved by 6 cycles of 10 s reperfusion/10 s global ischemia starting at the beginning of reperfusion. The ventricular hemodynamic parameters and lactate dehydrogenase ( LDH) release during reperfusion were measured. The infarct size was measured by TTC staining method. The expressions of ALDH2,Bcl-2 and Bax at mRNA level of left anterior myocardium were detected by RT-PCR analysis. Results In contrast to ischemia and reperfusion,ischemic postconditioning improved the recovery of left ventricular developed pressure,rate pressure product during reperfusion,and reduced LDH release and infarct size. The expressions of ALDH2 mRNA level and the ratio of Bcl-2 /Bax were increased. Adminis-tration of ALDH2 antagonist cyanamide at the beginning of reperfusion attentuated the role of ischemic postconditioning. Conclusion Ischemic postconditioning plays a role in the cardioprotection partially through increasing mitochondrial ALDH2 mRNA expression.

Objective To construct recombinant plasmids containing HPV18E7 gene ,and explore the optimization condition of its expression in Escherichia coli .Methods The genomic DNA extracted from HeLa cell line which served as a template to the HPV18 E7 gene was amplified using PCR method ;and the amplified product of HPV18E7 gene was connected to the pET-32a(+ ) vector ,which composed the pET-32a(+ )-HPV18E7 recombinant plasmid ;the positive recombinant plasmids were transformed into BL21-DE3-pLysS competent cells and the optimized expression condition was explored in order to obtain a large amount of HPV18E7 oncogenic protein .Results The fragment length of PCR products of HeLa cell genomic DNA was consistent with that of HPV18 E7 gene .In LB medium ,the expression level of the target protein was not high under such conditions as different concentra-tion of IPTG and lactose ,different temperatures and different induction starting amount .Therefore the ZYM-5052 auto-induction medium was tried in this experiment ,and the expression amount of the fusion protein was much higher than that induced with IPTG and lactose .Conclusion The amount of HPV18E7 fusion protein in ZYM-5052 automatic induction medium is much higher than that induced with IPTG and lactose .

Objective To summarize the key points of nursing care in reservation of catheters in 11 cases of nasopharyngeal carcinoma patients who received concurrent chemo-radiotherapy with symptomatic thrombosis after ultrasound-guided implantation of double-lumen PICCs(PowerPICC).Methods From January,2014 to December,2015,totally 11 cases with symptomatic venous thromboembolism were identified among 109 cases of nasopharyngeal carcinoma patients receiving concurrent radiotherapy and chemotherapy,and observation and nursing care were provided at the early stage of thrombosis and during thrombosis.Results All double-lumen PICCs were reserved,and no recurrence or aggravation of thrombosis was recorded to the end of the treatment.The duration of carrying PICCs was 67～89(77.45±6.65) days.Conclusion With careful treatment and nursing,PICC catheter-related complications can be reduced and the duration of carrying catheters can be prolonged,which leads to accomplishment of the treatment plan for patients.
