1.Design and application of round going process of nursing group leader in Orthopedic Department

Objective To evaluate the effect of round going process of nursing group leader in Orthopedic Department.Methods The round going process of nursing group leader in orthopedic department had been implemented since January 2016 in the 309th Hospital of PLA. It was compared, before ( March 2015) and after ( March 2016) the implementation, of the patients′ awareness of health education, qualified rate of basic nursing quality, accuracy of nursing assessment and high risk assessment, patients′satisfaction, accuracy rate of nursing marks and implementation of checking and signature. Results Before and after the implementation, of patients′health education awareness, rates of awareness of specialty drugs were 92.50% and 98.33% and rates of perioperative health education were 94. 17% and 99. 17%; of qualified rates of basic nursing, rates of cleanliness were 76.30% and 91.11% and rates of comfort were 81.11% and 92.22%; of nursing assessment accuracy, rates of admission assessment were 93. 33% and 98. 52% and rates of high risk assessment were 91.67% and 98.33%; of accuracy of nursing marks, rates of nursing grade were 85. 88% and 96. 68%, rates of tube marking were 80.72% and 92.64%, and rates of high risk marking were 95.74% and 98.89%; of implementation of checking and signature, rates of patrol card were 83. 17% and 95. 26%, rates of nursing measure table were 84.95% and 97.67%, and rates of turning over card were 89.24% and 95.63% ( P<0.05) . Conclusions Process quality control, health education level and basic nursing quality can all be improved by round going process of nursing group leader in orthopedic department.
2.Problems existing in nursing assessment of duty nurses in orthopedic department and training strategie

Objective To comprehend status quo of nursing assessment of duty nurses in orthopedic department and to develop corresponding strategies, so as to provide references for clinical teaching of specialty nursing.Methods In March 2016, 48 duty nurses in the Orthopedic Center in The 309th Hospital of Chinese PLA , were selected by convenience sampling method and examined about their nursing assessment, using the self-designed nursing assessment process and criteria. Comprehensive assessment was done by rank-sum ratio in CHISS 2011, and corresponding strategies were make accordingly.Results Analysis result of the rank-sum ratio showed that, 32 items could be divided into three levels according to times of mark deduction, with 7 items of the first level, 18 of the second, and 7 of the third. The first level included specialty physical examination, inquisition of the illness history, inquisition of current symptoms, assessment method of health education effect, induction of positive data, making nursing goals and putting forward nursing diagnosis. Conclusions In response the above mentioned problems, Nursing handbook of specialty physical examination in orthopedic department, Nursing inquisition guidelines of duty nurses in orthopedic department, and Standard operating procedure on nursing health education in orthopedic department were made. Layered training examinations were conducted accordingly, so as to improve nursing assessment quality of duty nurses.
3.Current situation of duty performance and management countermeasures among orthopedics specialized nurses

Objective? To understand the current situation of deployment among orthopedics specialized nurses so as to provide a basis for specialized nurses management. Methods? From August 2018 to September 2018, the self-designed orthopedics specialized nurses work status questionnaire was used to investigate a total of 70 orthopedics specialized nurses with the specialized nurse certification before November 2017 in 15 provinces and cities by convenience sampling. The investigation content included 26 items, such as the respondents' general information, duty performance and feeling of working state of specialized nurses. The Wenjuanxing small program in WeChat was used to push information. CHISS 2010 was used to the statistical analysis. Results? Among those respondents, a total of 85.71% of them were from Class Ⅲ Grade A hospitals, 98.57% of them from public hospitals and military hospitals. There were 21 (30.00%) nurses worked for 3-5 years, 22 (31.43%) nurses worked for 6-10 years, 11 (15.71%) nurses worked for 11-15 years, 16 (22.86%) nurses worked more than 15 years. Those respondents included 3 (4.29%) associate chief nurses, 40 (57.14%) supervisor nurses and 27 (38.57%) nurse practitioners. After gaining the certification, there were 6 (8.57%) nurses not satisfied with working status, 23 (32.86%) nurses not satisfied much, 20 (28.57%) nurses quite satisfied, 19 (27.14%) nurses satisfied and 2 (2.86%) nurses very satisfied. Multiple regression analysis showed that the influencing factors of satisfaction with working status among orthopedics specialized nurses included five independent variables involving the difference with job content of other nurses, change of job content compared with that before training, having an opportunity to play a key role, getting supports form higher level and taking charge of nursing round. Conclusions? Training for orthopedics specialized nurses can promote the development of special nursing. Implementing the position management of orthopedics specialized nurses and building diversified working mode can inspire the career development motivation of specialized nurses.
4.Construction and effects of orthopedic specialized nurse studio

Objective? To explore the construction method and the effects of the orthopedic specialized nurse studio. Methods? The orthopedic specialized nurse studio included 4 working groups, which were intravenous therapy group, wound care group, chronic diseases management group and perioperative venous thromboembolism (VTE) management group. Each group were assigned 1 team leader and 10 to 12 team members. Nursing training, management and research were carried out. The professional competency of orthopedic nursing staff and the nursing quality were compared before (January to December 2016) and after (January to December 2017) the construction of the studio. Results? The scores of theoretical assessment, specialized nurses' performance and nursing quality in 2017 were (97.76±4.09), (82.25±6.60) and (99.06±0.91) respectively, which were higher than those of the same period in 2016. The differences were statistically significant (t=2.169, 3.327, 2.129; P< 0.05). Conclusions? The establish of the orthopedic specialized nurse studio can enhance the growth of specialized nursing team, improve the nursing quality and promote the development of multi-disciplinary integration.
5.The method and application to construct experience recommendation platform of acupuncture ancient books based on data mining technology.

OBJECTIVETo construct a knowledge platform of acupuncture ancient books based on data mining technology, and to provide retrieval service for users.
METHODSThe Oracle 10 g database was applied and JAVA was selected as development language; based on the standard library and ancient books database established by manual entry, a variety of data mining technologies, including word segmentation, speech tagging, dependency analysis, rule extraction, similarity calculation, ambiguity analysis, supervised classification technology were applied to achieve text automatic extraction of ancient books; in the last, through association mining and decision analysis, the comprehensive and intelligent analysis of disease and symptom, meridians, acupoints, rules of acupuncture and moxibustion in acupuncture ancient books were realized, and retrieval service was provided for users through structure of browser/server (B/S).
RESULTSThe platform realized full-text retrieval, word frequency analysis and association analysis; when diseases or acupoints were searched, the frequencies of meridian, acupoints (diseases) and techniques were presented from high to low, meanwhile the support degree and confidence coefficient between disease and acupoints (special acupoint), acupoints and acupoints in prescription, disease or acupoints and technique were presented.
CONCLUSIONSThe experience platform of acupuncture ancient books based on data mining technology could be used as a reference for selection of disease, meridian and acupoint in clinical treatment and education of acupuncture and moxibustion.
6.Resveratrol improves cognitive function in severely burned rats by inhibiting hippocampal NF-κB/JNK pathway.

Objective To investigate the protective effect of resveratrol (RSV) on improving cognitive function in severely burned rats and its possible mechanism. Methods 18 male SD rats aged 18-20 months were randomly divided into 3 groups: control group, model group and RSV group, with 6 rats in each group. After successful modeling, the rats in RSV group were gavaged once daily with RSV (20 mg/kg). Meanwhile, the rats in control group and model group were gavaged once daily with an equal volume of sodium chloride solution. After 4 weeks, the cognitive function of all rats was estimated by Step-down Test. The concentration of tumor necrosis factor α (TNF-α) and interleukin 6 (IL-6) protein in serum of rats were detected by ELISA. The expression of IL-6, TNF-α mRNA and protein were estimated by real-time PCR and Western blotting. The apoptosis of hippocampal neurons was tested by terminal deoxynuclectidyl transferase-mediated dUTP-biotin nick end labeling assay (TUNEL). The expression of nuclear transcription factor-κB (NF-κB)/c-Jun N-terminal kinase (JNK) pathway-related proteins in hippocampus were assessed by Western blotting. Results Compared with the rats in model group, rats in RSV group exhibited improved cognitive function. Consistently, the rats in RSV group had a reduced concentration of TNF-α and IL-6 in serum, decreased mRNA and protein expressions of TNF-α and IL-6 in hippocampus, and decreased apoptosis rate and relative expression of p-NF-κB p65/NF-κB p65 and p-JNK/JNK in hippocampal neurons. Conclusion RSV alleviates inflammatory response and hippocampal neuronal apoptosis by inhibiting NF-κB/JNK pathway, thereby improving cognitive function in severely burned rats.

7.Establishment of Specific Chromatogram and Thin-layer Identification Method for Kaixinsan

ObjectiveTo establish the specific chromatogram and thin layer chromatography(TLC) identification method of Kaixinsan(KXS) samples, in order to clarify the key quality attributes and provide reference for the quality evaluation of KXS. MethodHigh performance liquid chromatography(HPLC) specific chromatogram of KXS was developed with YMC Hydrosphere C18 column(4.6 mm×250 mm, 5 μm), the mobile phase was acetonitrile(A)-0.2% formic acid aqueous solution(B) for gradient elution(0-15 min, 2%-20%A; 15-25 min, 20%-25%A; 25-30 min, 25%-30%A; 30-45 min, 30%-31%A; 45-50 min, 31%-44%A; 50-65 min, 44%-45%A; 65-73 min, 45%-75%A; 73-95 min, 75%-100%A; 95-105 min, 100%A; 105-105.1 min, 100%-2%A; 105.1-120 min, 2%A), the detection wavelength was 320 nm. Ultra high performance liquid chromatography-linear ion trap-electrostatic field orbitrap mass spectrometry(UHPLC-LTQ-Orbitrap MS) was used to identify the chemical components of KXS with electrospray ionization(ESI), negative ion mode and scanning range of m/z 50-2 000. TLC identification methods for Poria and Ginseng Radix et Rhizoma in KXS were established. ResultThere were 11 common peaks in the specific chromatogram of KXS, attributed to Polygalae Radix, Poria and Acori Tatarinowii Rhizoma. Taking peak 9(α-asarone) as the reference peak, the relative standard deviations of the retention times of 15 batches of KXS samples were<0.2%. A total of 34 compounds were identified by UHPLC-LTQ-Orbitrap MS, including terpenoids, phenylpropanoids, oligosaccharides and ketones. The established TLC had good separation and was rapid, reliable, simple, feasible, suitable for the identification of Poria and Ginseng Radix et Rhizoma in KXS. ConclusionThe specific chromatogram and TLC of KXS are stable and reproducible. The material basis of KXS is basically clarified by MS, which can provide a reference for the development and quality control of KXS.
