1.Energy metabolism and related mechanism in patients with malignant tumors

Energy metabolism refers to the metabolism of three major nutrients associated with the process of energy production and usage.The metabolism in patients with tumors manifests specifically, as a result of tumor-bearing state, host's response to tumor and anti-neoplastic therapy.It is believed that resting energy expenditure increases in patients with newly diagnosed tumors, which is related to C-reactive protein and release of inflammatory cytokines. After eflfectjve therapy,resting energy expenditure decreases in some degree.In addition,the stage and differentiation of tumors also affect metabolism.However,there are some conflicts among research findings.The characteristics of energy metabolism and influential factors in patients with tumors are reviewed in this paper.
2.Study on application of informational support to reduce disease uncertainty in liver cancer patients undergoing interventional therapy

Objective To investigate the effect of informational support on disease uncertainty in liver cancer patients undergoing interventional therapy. Methods Sixty patients of liver cancer patients undergoing interventional therapy were randomly divided into the intervention group and the control group with 30 in each.The control group was treated with general treatment and care. The patients in the intervention group were given general treatment and care with a series of informational support interventions. Patients in the two groups were measured by uncertainty in illness scale (MUIS) at the day of admission,before operations and discharged from hospital. The measurement results were statistically analyzed. Results There was significant decrease about disease uncertainty scores both on the day before operation and the day before discharge in the intervention group compared to the day of admission. The decreasing score of uncertainty in illness in the intervention group was significantly greater than the control group on the day before operation and the day before discharge. Conclusions A series of informational support interventions could significantly reduce the diseases uncertainty in liver cancer patients undergoing interventional therapy.
3.Investigation and evaluation on antimicrobial prophylaxis in cleaning operation

Objective To investigate antimicrobial prophylaxis in clean operation,and provide evidence for formu-lating antimicrobial management measures.Methods 12 types of clean operation cases who discharged from a tertia-ry first-class hospital in May of 2011 -2014 were selected,120 cases were selected each year,patients were divided into group I and group II according to the types of operation,survey forms were designed,assessment criteria for the rationality of antimicrobial use was formulated, antimicrobial use in different years was compared. Results Prophylactic use of antimicrobial agents for clean operation decreased from 93.33% in 2011 to 35.00% in 2013 and 2014;rates of correct medication time increased from 43.75% in 2011 to 97.62% in 2013 and 92.86% in 2014 respectively;rates of medication according to indications,rational choice of antimicrobial agents,and rational treatment course increased from 42.86%,60.71 %,and 21 .43% in 2011 to 100.00% in 2014 (all P < 0.05 ). Conclusion Special rectification activities of antimicrobial use effectively promoted the standard application of anti-microbial agents for clean operation.
4.Comparison of ECToolbox Formulae for the Assessment of Left Ventricular Ejection Fraction

Purpose To evaluate the correlation and consistency of left ventricular ejection fraction (LVEF) obtained by ECG-gated myocardial perfusion SPECT (GMPS) using four formulae (R0-R3) of ECToolbox software and findings on equilibrium radionuclide angiography (ERNA), and to determine the optimal diagnostic thresholds by using the four formulae. Materials and Methods A total of 38 patients with myocardial infarction and 65 patients with suspected coronary heart disease underwent both 99Tcm-MIBI rest GMPS and 99Tcm-RBC ERNA within a week. The LVEF values calculated by ECToolbox R0, R1, R2 and R3 were compared with those obtained by ERNA, and compared with ERNA results, the optimal diagnostic thresholds of the four formulae (R0-R3) were assessed by receiver operating characteristic (ROC) curves. Results The results calculated by the formulae (R0-R3) presented a significant positive correlation with that obtained by ERNA [mean LVEF value by ERNA (54.6±17.5)%, mean LVEF value by formulae R0-R3 (64.1±15.7)%, (56.3±15.1)%, (69.9±17.9)% and (56.7±13.6)%, respectively, r=0.899, 0.898, 0.890, 0.895; P<0.01]. All mean LVEF values calculated by the four formulae were higher than that by ERNA, and the difference was significant (P<0.05). LVEF≥50%obtained by ERNA was considered as normal diagnostic value, the optimal diagnostic thresholds of R0-R3 were 56.5%, 51.5%, 64.5% and 52.5%, respectively. Conclusion The results calculated by R0, R1, R2 and R3 in the ECToolbox software and that by ERNA show significant correlation and difference for the assessment of LVEF. Thus it is advisable to stick to one formula in the follow-up of each patient and select correspondent threshold in the clinical diagnosis.
5.Polymorphisms in The 5' flanking Region of IGF-Ⅰ Gene are Associated With Cashmere Fibre Traits in Liaoning Cashmere Goats

Insulin-like growth factorⅠ(IGF-Ⅰ) is essential for the growth and differentiation of hair follicles which is an important part of wool and cashmere.But there is no report on polymorphisms of the IGF-Ⅰ gene in cashmere goat,and also few candidate genes for cashmere production traits have been reported in cashmere goats.The objectives of this work were to detect the single nucleotide polymorphism(SNP) in the 5' flanking region of IGF-Ⅰ gene and to determine their association with fibre traits in Liaoning cashmere goats.The fibre traits data investigated in the experiment were combed cashmere weight,cashmere fibre length and cashmere fibre diameter.A few individuals of the Liaoning cashmere goats,selected according to phenotypic character,were used for SNPs detection in the 5'flanking region of IGF-Ⅰ gene,and four point mutation G→C(388 bp),A→G(668 bp),A→ C(719 bp),G→A(752 bp) were identified,which result in a CdxA bonding-site lack,and score increase of C/EBP site in the 5' flanking region compared with the wild type.Then different genotypes were detected in 520 Liaoning cashmere goats using create restriction site(CRS) and polymerase chain reaction-restriction fragment length polymorphism(PCR-RFLP),three genotypes,AA,AB and BB were observed coded for by two different alleles A and B for each SNP,and 13 diplotypes were identified in the groups of the four SNPs.The relationships between the genotypes and combed cashmere weight,cashmere fibre length and cashmere fibre diameter were analyzed.The statistical analysis showed that animals with the genotype AA of SNP2 in Liaoning cashmere goats had the thinnest cashmere fiber diameter compared with those with the AB and BB genotype(P
6.Influence factors of energy metabolism and nutritional risk screening in patients with newly diagnosed esophageal cancer

Objective To evaluate the possible factors that may influence the resting energy expenditure (REE) in patients with newly diagnosed esophageal cancer. Methods Totally, 40 patients with newly diagnosed esophageal cancer were prospectively collected from November 2008 to June 2009 in Xinhua Hospital. Nutritional risk screening 2002 (NRS 2002) was performed. REE and body composition were measured using indirect calorimetry and bioeletrical impedence method, respectively. Results Twenty-seven (67.5%) patients were found with nutrition risk, and NRS score was negatively correlated with prealbumin ( r = - 0.444, P = 0.004) and albumin levels (r = - 0.386, P = 0.014). Measured REE and predicted REE values were ( 6770 ± 1360) and (6021 ± 841 ) kJ/d, respectively (P < 0.001 ). Among all 40 patients, 57.5% of them were hypermetabolic,30.0% were normal, and 12.5% were hypometabolic. Stepwise linear regression analysis showed that fat free mass was the only significant determinant variable for REE (P < 0.001 ). Conclusion Fat free mass is a factor than can influence the energy metabolism in patients with newly diagnosed esophageal cancer.
7.Expression of FBXW7 in a murine model of T-cell acute lymphoblastic leukemia

Objective To investigate the expression of FBXW7 during the development of Notch1induced murine leukemia.Methods Notch1 over-expressing murine model of T-cell acute lymphoblastic leukemia was used to study the expression of FBXW7 by real-time PCR methods.Bone marrow mononuclear cells (BMNC) were isolated on different days after transplantation and CD45.2+ GFP+ leukemia cells were sorted by flow cytometry at late stage.The expression changes of FBXW7 were tested by real-time PCR.Results The mouse bone marrow cells both from leukemia and control groups expressed FBXW7.Different expression patterns of FBXW7 were observed during the development of leukemia. The expression of FBXW7 was gradually increased in control group, whereas the expression level of FBXW7 in leukemia group was decreased steadily and reached one-sixth of that in control group on 12th day.Furthermore,lower expression level of FBXW7 was observed in CD45+.2 GFP+ leukemia cells.Conclusion Decreased expression of FBXW7 is observed in Notch1-induced mouse leukemia model,suggesting that the abnormal ubiquitin degradation pathway mediated by FBXW7 might contribute to the leukemogenesis in Notch1-induced murine leukemia model.
8.Interaction of Flightless I with Nup88 and Importin β.

High expression of Fightless I (FLII) is associated to multiple tumors. Based on our previous study that FLII might be involved in the nuclear export, we assessed the possible interaction of FLII with the nuclear envelop associating proteins Importin β and Nup88. We first constructed GST-FLII, GST-LRR recombinant plasmids and transformed them into the Rosetta strain to produce GST-FLII, GST-LRR fusion protein. After purification of these proteins, GST-pull down, as well as co-immunoprecipitation, were used to test the interaction of FLII with Importin β and Nup88. FLII interacted with Importin β and Nup88, and FLII LRR domain is responsible for these interactions. Thus, FLII may play a role in nuclear export through interaction with Importin β and Nup88.

9.Expression of the transmembrane emp24 domain-containing protein 4 in liver tissues of patients with hepatocellular carcinoma and its effects on biological behavior of hepatocellular carcinoma cells

Objective:To examine the expression of transmembrane emp24 domain-containing protein 4(TMED4) in liver tissue of patients with hepatocellular carcinoma, and to investigate the effects of TMED4 gene on the proliferation and migration of hepatocellular carcinoma cells and related molecular mechanisms. Methods:The expression of TMED4 at protein level in liver cancer tissue and paracancerous tissue of patients with hepatocellular carcinoma were detected by Western blotting and immunohistochemical stainning, and the correlation between its expression and clinicopathological features was analyzed. The effects of TMED4 overexpression or knockdown on proliferation, migration and healing ability of hepatocellular carcinoma cells in vitro and in vivo were determined by cell proliferation test, Transwell test, wound healing test and subcutaneous tumor formation in nude mice. The molecular mechanism of TMED4 in regulating the biological behavior of hepatocellular carcinoma cells was preliminarily explored by pathway analysis. Independent sample t test, Mann-Whitney U test and chi-square test were used for statistical analysis. Results:The results of Western blotting showed that the expression of TMED4 at protein level in hepatocellular carcinoma tissue was lower than that in paracancerous tissue(0.52±0.29 vs. 0.83±0.22), and the difference was statistically significant( t=2.54, P=0.022). The results of immunohistochemical examination indicated that the expression of TMED4 at protein level in liver cancer tissue was lower than that in paracancerous tissue(5.46±3.37 vs. 7.58±3.08), and the difference was statistically significant( t=3.49, P<0.001). The expression of TMED4 at protein level was significantly correlated with vascular invasion and Barcelona clinic liver cancer stage( χ2=6.83 and 4.20, P=0.009 and 0.040). The results of cell proliferation assay showed that the absorbance value of SMMC-7721 cells in TMED4 overexpression group was lower than that in control group(1.38±0.05 vs. 2.37±0.08), while the optical density value of HepG2 in TMED4 knockdown group was higher than that in control group(0.76±0.04 vs. 0.54±0.01), and the differences were statistically significant( t=18.23 and 8.85, both P<0.001). The results of Transwell test showed that the number of migrated SMMC-7721 cells in TMED4 overexpression group was less than that in control group(286.30±13.01 vs. 439.70±12.34), while the number of migrated HepG2 cells in TMED4 knockdown group was higher than that in control group(249.00±6.00 vs. 160.00±6.56), and the differences were statistically significant( t=14.81 and 17.34, both P<0.001). The wound healing test showed that the healing rate of SMMC-7721 cells in TMED4 overexpression group was lower than that in control group((0.21±0.01)% vs.(0.45±0.01)%), the healing rate of HepG2 cells in TMED4 knockdown group was higher than that in control group((0.46±0.01)% vs.(0.20±0.01)%), and the differences were statistically significant( t=200.10 and 30.46, both P<0.001). The results of subcutaneous tumor formation assay in nude mice showed that the growth rate of cells in TMED4 overexpression group was slower than that in control group. After cell inoculation for 6 weeks, the subcutaneous tumor volume of mice in TMED4 overexpression group was smaller than that in control group(27.36 mm 3(138.70 mm 3) vs. 1 741.62 mm 3(1 783.39 mm 3)), the tumor weight was lower than that in control group(0.06 g(0.14 g) vs. 1.46 g(1.09 g)), and the differences were statistically significant(both Z=-2.31, both P<0.001). The results of Western blotting showed that the expression of Snail at protein level in SMMC-7721 cells of the TMED4 overexpression group was lower than that of the control group(0.32±0.01 vs. 0.90±0.03), the protein level of Snail in HepG2 cells of TMED4 knockdown group was higher than that of control group(1.03±0.01 vs. 0.97±0.01), and the differences were statistically significant( t=28.49 and 12.31, both P<0.001). The results of real time fluorescent quantitative polymerase chain reaction showed that the expression of Snail at mRNA level in SMMC-7721 cells of TMED4 overexpression group was lower than that of control group(0.13±0.05 vs. 1.00±0.15), the expression of Snail at mRNA level in HepG2 cells of TMED4 knockdown group was higher than that of control group(1.25±0.32 vs. 0.21±0.14), and the differences were statistically significant( t=9.62 and 5.10, P<0.001 and P=0.007). Conclusion:TMED4 may affect the proliferation and migration of hepatocarcinoma cells by regulating the expression of Snail, and which is expected to become a potentially therapeutic target for hepatocellular carcinoma.
10.Effects of homogeneity management in elderly chronic pain nursing

Objective To explore the effects of homogeneity management in elderly chronic pain nursing.Methods In Mindong Hospital, Affiliated to Fujian Medical University, we selected 84 elderly patients with chronic pain by convenience sampling as observation group from June 2016 to May 2017 carrying out chronic pain nursing homogeneity management. And we selected 84 patients as control group from January 2015 to May 2016 implementing conventional nursing. The Visual Analogue Scale (VAS), satisfaction questionnaire and Medical Outcomes Study 36-item Short-Form Health Survey (SF-36) were used to compare the pain, satisfaction with nursing and life quality of patients between two groups.Results The score of VAS of patients in observation group and control group was (2.09±0.55) and (3.47±0.53) respectively 30 days after intervention with a significant difference (t=16.559,P<0.01). The satisfaction with nursing of observation group and control group was 92.86% and 67.86% respectively with a significant difference (χ2=16.630, P<0.01). The dimensions scores of SF-36 of patients in observation group were significantly higher than those in control group three months after intervention (P<0.05).Conclusions Application of homogeneity management has an ideal effect in elderly chronic pain nursing. It can effectively ease patients' pain and improve patients' satisfaction with nursing as well as life quality.
