Objective To investigate the life satisfaction and its related factors in primary caregivers of breast cancer patients. Methods The Life Satisfactory Index A (LSIA) and self-administrated questionnaire were used. The life satisfaction between breast cancer patients'primary caregivers and breast benign patients' primary caregivers were compared. The factors of the life satisfaction in primary caregivers of breast cancer patients were analyzed. Results The life satisfaction was poorer in the breast cancer patients' primary caregivers than in the breast benign patients' primary caregivers (P<0.01). The life satisfaction in the breast cancer patients' primary caregivers was correlated with the stage of breast cancer, course of disease, primary caregivers' age and family monthly income (P<0.05). Conclusion The life satisfaction in breast cancer patients' primary caregivers is worse and is related with stage of breast cancer, course of disease, primary caregivers' age and family monthly income.

Objective To study the profile of peripheral blood cortisol concentration in patients with hepatitis B virus (HBV)-related acute on chronic liver failure (ACLF) and its association with disease severity and predictive value on prognosis. Methods Forty-five patients with HBV-related ACLF,including 15 patients in early-stage,15 in medium stage and 15 in end-stage; and 15 patients with severe chronic hepatitis B (CHB) were also enrolled.Peripheral blood cortisol concentration was tested by chemiluminesence immunoassay.SPSS version 18.0 was used to compare peripheral blood cortisol concentration among different groups and analyze its correlation with prothrombin activity (PTA),total bilirubin (TBil),albumin (Alb),alanine aminotransferase (ALT),aspartate aminotransferase (AST),HBV DNA,international normalized ratio (INR) and model for end-stage liver disease (MELD) scores. The independent risk factors of prognosis in patients with HBV-related ACLF were determined by performing Logistic regression analysis. Results The concentrations of peripheral blood cortisol in severe CHB group, early stage ACLF group, medium stage ACLF group and end-stage ACLF group were (595.6±114.0),(496.0±87.2),(303.9±81.1),and (183.8±71.8) nmol/L,respectively.A decreasing trend was observed and the differences among groups were statistically significant (F =63.93,P < 0.01).Peripheral blood cortisol concentration were significantly different among subgroups of patients with MELD<30,followed by 30-40 and >40(F 9.01,P<0.01).Peripheral blood cortisol concentration in patients with ACLF was positively correlated with PTA level (r=0.83,P<0.01),and inversely correlated with TBil (r =-0.34,P<0.05),MELDscore (r =-0.60,P<0.01),AST/ALT (r =-0.35,P<0.05),and INR (r=-0.59,P<0.01).Association with Alb,ALT,AST and HBV DNA was not observed.According to multivariate Logistic regression analysis,MELD score, cortisol level,Alb and total cholesterol were independent risk factors of prognosis for patients with ACLF.Serum cortisol concentration in survival group of HBV-related ACLF was higher than that in death group, while the death group exhibited a gradually decreasing trend. Conclusions The peripheral blood cortisol concentration decreases in patients with HBV-related ACLF,which is related to the degree of liver dysfunction, disease severity and prognosis of patients. Moreover, the lower level of cortisol concentration indicates poor short-term prognosis in patients with HBV related ACLF.

Objective To explore the value of echocardiography in evaluation of pulmonary venous in total anomalous pulmonary venous connection (TAPVC).Methods Fifty-five children with TAPVC were enrolled in the study. The data of echocardiography and CT angiography were retrospectively analyzed and compared with intraoperative findings. Results Totally 55 patients with TAPVC were classified into supra-cardiac type (n=24),cardiac type (n=20),infra-cardiac type (n=7) and mixed type (n=4) according to the sites of drainage of pulmonary venous in echocardiography. In 15 patients with obstruction of pulmonary vertical vein, the sites of obstruction in the supra-cardiac type mostly presented between vertical vein and superior vena cava or innominate vein, and the sites of obstruction in the infra-cardiac presented all between vertical vein and hepatic or portal vein.In 4 patients with pulmonary vein stenosis,3 cases with local pulmonary vein stenosis were all cardiac type, which presented between individual pulmonary vein and common confluence or right atrium;1 patient with diffuse pulmonary vein stenosis was infra-cardiac type.In 9 patients of abnormal individual pulmonary vein,8 cases were not detected by echocardiography, but all were detected by CT angiography. Conclusion Echocardiography is able to make more comprehensive evaluation for the pulmonary venous drainage, obstruction, and proximal stenosis of individual pulmonary vein in TAPVC.CT angiography is superior in evaluation of abnormalities of connection and amount of individual pulmonary vein, and imaging of distal pulmonary vein.

OBJECTIVETo deepen the understanding of clinical manifestations and treatment of patients with positive lupus anticoagulant (LAC).
METHODSThe clinical data of 2 patients were analyzed and related literature were reviewed.
RESULTSCase 1, a 31-year-old female, diagnosed as lupus anticoagulant positive, secondary to undifferentiated connective tissue disease, was presented with menorrhagia and thrombocytopenia. Anti-nuclear antibody (ANA) was positive 1:1000 (homogeneous type) with anti-double stranded DNA positive, and dRVVT LA1/LA2 was 3.4. Coagulation function was alleviated after treatment with glucocorticoid and total glucosides of paeony. Case 2, a 59-year-old female was presented with gingival bleeding, hematuria with the level of F II:C 13%. dRVVT LA1/LA2 was 2.0. Anti-nuclear antibody (ANA) was positive 1:1000 (type of cytoplasmic granule), anti-double stranded DNA was positive. The patient was diagnosed as hypoprothrombinemia-lupus anticoagulant syndrome (LAHS) and acquired coagulation factor deficiency. The signs of hemorrhage were alleviated after treatment with methylprednisolone 40 mg/day and cyclophosphamide, while the level of F II:C was below normal.
CONCLUSIONSymptoms of patients with positive LAC are variable. The diagnosis relies on history of disease and laboratory test. Currently, there is no standardized treatment. Cases of LAHS should be thoroughly investigated for any known causes and related disorder.
