Objective To discuss the intervention effect of four oral care model on compliance of atomizing inhalation and mouth and throat complications in outpatient infants and young children patients. Methods A total of 480 cases of children patients using budesonide inhalation for the treatment of respiratory tract infection were divided into the observation group(n=260)and the control group (n=220) according to the time sequence. The control group received conventional nursing, the observation group used quaternity oral care model in nursing care. The compliance and occurrence of complications in the mouth and throat were observed between the two groups. Results In the observation group with nebulae inhalation compliance was 79.2% (206/260), evidently higher than that in the control group the compliance of 58.6%(129/220) (χ2=23.97,P<0.01). The incidence of oral herpes and ulcers in the control group was 2.73%(6/220), and 0.77%(2/260) in the observation group, but no significant difference was seen (χ2=2.79, P>0.05). The incidence of thrushes was 3.64%(8/220) in the control group, and none occurred in the observation group, which showed significant difference (χ2=9.62, P<0.01). Conclusions Using quaternity oral care model in corticosteroid nebulae inhalation patientscan increase compliance and decreasecomplications of oral and throat of outpatient children.

Objective To learn the levels of serum 25-(OH)D in 0 12 years old chindren in Kunming.Methods Serum 25-(OH)D levels of children (4 498) were measured by using Roche cobas 8 000 / E602 automatic electrochemical luminescence immunoassay.Results As the increase of age,the levels of serum 25-(OH)D were gradually decreased,different age groups had significant difference (P＜0.05).The levels of 25-(OH)D between boys and girls had no significant difference (P＞0.05);The average level of sernm 25-(OH)D was 83.48 nmol/L,the level of serum 25-(OH)D was distributed in 0 ～ 1 years old group,and most concentrated in the 6 ～ 9 years old group.Conclusion The levels of 25-(OH)D in 0 ～ 12 years old chindren are in a good status,but with the increase of age,the level of 25-(OH)D is decreasing gradually,especially the levels of 25-(OH)D decreased significantly after the age of three,in shortage or lack of status,this should cause everybody's attention.

AIM: To evaluate the mobilization of endothelial progenitor cells (EPCs) in mice peripheral blood during hindlimb ischemia alone or in combination with granulocyte colony stimulating factor (G-CSF). METHODS: Hindlimb ischemia was established in mice by surgical excision of both femoral arteries. Fluorescence-activated cell sorting (FACS) was used to detect the expression of cell-surface CD34 and vascular endothelial growth factor recepter-2 (VEGFR2) antigens. The ratio of double-positive cells for CD34 and VEGFR2 was regarded as the level of EPCs in peripheral blood. In G-CSF administration in combination with hindlimb ischemia group, the percentage of double-positive cells was also detected. RESULTS: As compared with control group, hindlimb ischemia increased the percentage of EPCs in mice peripheral blood. The hindlimb ischemia combined with G-CSF administration significantly enhanced the percentage of EPCs. CONCLUSION: Ischemia increases the number of EPC in peripheral blood. It may induce the migration of EPC from barrow to peripheral blood. By mobilizing barrow, G-CSF enhances this effect.

Objective:To investigate the predicting value of European system for cardiac operative risk evaluation ( EuroSCORE ) and sino system for coronary operative risk evaluation ( SinoSCORE ) in early quality of life of patients after coronary artery bypass surgery (CABG).Methods:A total of 218 consecu-tive patients who underwent CABG from March 2010 to January 2013 were evaluated with both systems before operation .Health related quality of life ( QoL) was estimated by using 36-item short form health survey ( SF-36) preoperatively and postoperatively in order to evaluate the predicting value of the two sys -tems in early post-operative QoL.Calibration was evaluated by Hosmer-l,emeshow goodness-of-fit test. Discrimination was tested by determining the area under the receiver operating characteristic ( ROC ) curve .Results:There was no significant difference between the accumulation of the EuroSCORE and SinoSCORE in the all patients (t=-0.904, P=0.368), When using Wilcoxon test on life quality in the preoperative and postoperative patients respectively ,the data showed that the quality of life improved significantly in various dimensions of the postoperative patients (Z=-2.886, P<0.001).Except for bodily pain (BP) and mental health (MH), statistically significant correlation was found between the preoperative risk evaluation scores and the postoperative QoL scores (r:-0.203 to -0.493, P<0.05). Logistic regression analyses indicated that both the scores emerged as the independent predictor for a relatively worse QoL ( OR>1 , P<0 .05 ) .Furthermore , the EuroSCORE predicted the outcome with a higher OR.For SinoSCORE the Hosmer-Lemeshow test was significant (P=0.628) and the area under ROC curve was 0.754.For the EuroSCORE the Hosmer-Lemeshow test was significant (P=0.538) and the area under ROC curve was 0.854.Conclusion:Both EuroSCORE and SinoSCORE could be viewed as a predictor for several aspects of postoperative QoL , while EuroSCORE might have a greater predicting value.

Objective To observe the efficacy and safety of Shenqiwuweizikeli for treating chronic insomnia.Methods One hundred and ninety-six cases of subjects were randomly divided into Shenqiwuweizikeli group (n =98) and Estazolam tablets group (n =98).The pittsburg sleep quality index (PSQI) was adopted to evaluate the clinical effects and records of adverse reactions during the study period.Also the lab routine inspection(blood routine,urine routine,liver and kidney function, electrocardiogram were conducted to evaluate safety.Results The Shenqiwuweizikeli and Estazolam tablets all had significant effects for chronic insomnia.The total effective rate of Shenqiwuweizikeli group was 92.86％ (91/98), of Estazolam tablets group was 93.88％(92/98) ,and there was no significant difference (P＞0.05).There were no abnormalities in terms of each routine inspection index.After stopping take the medicine, The adverse reactions including bounce sex insomnia(60 cases), daytime sleepiness/drowsiness (55 cases), dizziness with lacking of power and light headedness(23 cases) in Estazolam tablets group were all more than Shenqiwuweizikeli group with significant difference(P＜0.01).Conclusion The Shenqiwuweizikeli has definite efficacy and safety for treated with chronic insomnia without withdrawal of recoil and dependence.

ObjectiveTo report the preliminary results and surgical technique of Stoppa approach used in treatment of acetabular fracture.MethodsFrom May 2005 to May 2010,23 patients with acetabular fracture were treated using modified Stoppa approach.The mean age was 37 years old(range,21-71years old).According to Letournel classification,there were 7 cases in anterior column fracture,6 cases in anterior column with posterior hemitransverse fracture,6 cases in transverse fracture,4 cases in associated with both column fractures.All patients in the study were operated under general anesthesia on a radiolucent table in a supine position and underwent Stoppa approach.Fracture reduction was evaluated by Matta's score system.The clinical outcome was judged by modified Merle d' Aubigne and Postel score system.Four cases of associated both column fractures,4 cases of anterior column with posterior hemitransverse fracture required the use of the lateral window.ResultsThe mean surgical time was 166 min (range,110-320 min).The mean blood lose was 647 ml (range,300-2500 ml).Blood transfusion was 3.3 unites (0-12 unites).All the factures healed in 3 months.The fracture reduction was evaluated according to the Matta's score system:an excellent reduction was obtained in 15 patients(65％),a good reduction was obtained in 5 patients (22％),and three(13％) were considered poor.Clinical results were excellent in 11 patients(48％),good in 7 patients (30％),fair in 3 patient (13％),and poor in 2 patients (9％).Superficial wound infection happened in 1 case.It healed after debridment.One patient noted to have significant weakness of the hip adductors (obturator nerve palsy) after surgery.It reached grade Ⅳone year later.No sciatic nerve,femoral nerve and vascular injury happened in this study.ConclusionFor the selected acetabualar fracture,Stoppa approach can get satisfied exposure and fixation of the fracture,especially in the patient with central dislocation of the femoral head as well as medial displacement of quadrilateral plate.

Objective To investigate the value of Q-analysis real-time elasticity in differentiating between benign and malignant thyroid nodules. Methods Eighty-six thyroid nodules in 62 patients with pathologic diagnosis were included in this study and were examined using Q-analysis real-time elasticity. The real-time elasticity features were observed and the quantitative index including the whole elasticity rate and the local elasticity rate were compared between benign and malignant nodules. Results There were 51 benign and 35 malignant nodules according to histopathological examination. The Q-analysis curve of real-time elasticity of benign nodules was smoother and with lower peak, compared with that of malignant nodules. The whole elasticity rate of malignant nodules were significantly higher than that of benign nodules (3.59±0.84 vs 2.32±0.56, P=0.000). And the local elasticity rate of malignant nodules were significantly higher than that of benign nodules (3.96±1.32 vs 2.39±0.58, P=0.000). The cutoff point of whole elasticity rate for the differential diagnosis was 3.25 with sensitivity, specificity and diagnostic accuracy as 71.4%, 96.1% and 86.0% respectively. The cutoff point of local elasticity rate for the differential diagnosis was 3.45 with sensitivity, specificity and diagnostic accuracy as 68.6%, 96.1% and 84.9% respectively. The diagnostic efficiency of whole elasticity rate and local elasticity rate had no significant difference (P=0.591).Conclusions Q-analysis real-time elasticity could provide the real-time elasticity features of thyroid nodules. The whole and local elasticity rate as the quantitative index contributed to the differential diagnosis of benign and malignant thyroid nodules.

Objective To explore atechnical modification bytransperineal Da-Wanorectoplasty(Da-WARP) ontreatinglow,intermedi-ate anorectal anomalies(AA) andto evaluate the results of long-termfollowup.Methods Forty-six cases of AAunderwent a Da-WARP,whichinvolved preservation of the rectal end of the fistula andinvagination of the perineal skinflapthroughthe neoanus.The clinical records werereviewed and analyzedretrospectively.Results The medianfollow-up period was 7 years and 2 months.All of themachieved a goodfecal conti-nence exception of 1 patient withintermediate AAand associated anomaly,hadfair result.Conclusions The Da-WARPis relativelysimple andpractical methods for surgical treatment of low,intermediate AA,with goodlong-termresults.

Objective To explore a technical modification by transperineal Da-W anorectoplasty(Da-W ARP) on treating low,intermediate anorectal anomalies(AA) and to evaluate the results of long-term follow up.Methods Forty-six cases of AA underwent a Da-W ARP,which involved preservation of the rectal end of the fistula and invagination of the perineal skin flap through the neoanus.The clinical records were reviewed and analyzed retrospectively.Results The median follow-up period was 7 years and 2 months.All of them achieved a good fecal continence exception of 1 patient with intermediate AA and associated anomaly,had fair result.Conclusions The Da-W ARP is relatively simple and practical methods for surgical treatment of low,intermediate AA,with good long-term results.
