1.Value and measuring methods of elasticity contrast index in the diagnosis of thyroid nodules

Objective:To explore the value of the elasticity contrast index (ECI) in differential diagnosis of thyroid nodules, and contrast the effect of 4 different methods on measuring ECI.Methods:A total of 122 patients with 131 thyroid nodules in Binzhou Medical University Hospital from May 2018 to May 2019 were enrolled, and elastography was performed in 4 different ways such as in axial plane for internal (AI), axial plane for periintranodular (AP), longitudinal plane for internal (LI) and longitudinal plane for periintranodular (LP). The cut-off values for predicting malignant nodules in 4 different ways were determined separately using receiver operating characteristic (ROC) curve analysis.Results:There were 54 benign and 77 malignant ones in 131 nodules. The ECI in AI, AP, LI and LP in benign thyroid nodules was significantly lower than that in malignant ones: 2.10 (1.48, 2.34) vs. 3.07 (2.73, 3.87), 1.91 (1.64, 2.18) vs. 2.62 (2.24, 3.07), 2.19 (1.59, 2.39) vs. 3.00 (2.72, 3.63) and 1.89 (1.71, 2.16) vs. 2.66 (2.21, 3.10), and there was statistical difference ( P<0.05). Among the 4 different ways, the largest area under curve was achieved in AI with 0.925, and the corresponding optimal diagnostic threshold was 2.43 (with 90.9% sensitivity, 87.0% specificity, 88.5% accuracy, 89.7% positive predictive value and 86.8% negative predictive value). Conclusions:ECI is helpful for conventional ultrasound to diagnose thyroid nodules as malignant or benign. The best value is obtained in AI.
2.Prescription Review and Drug Use Rationality Analysis for Pregnant Patients in Outpatient Department of Our Hospital

Objective:To analyze the situation and problems of prescription medication for pregnant patients in outpatient depart-ment of our hospital in order to improve and ensure the safe and rational drug use. Methods: The prescriptions concerning pregnancy except normal pregnancy monitoring in our outpatient department in 2014 were selected and analyzed and reviewed according to classifi-cation, related regulation and medicine instruction issued by FDA. Results:Totally 882 prescriptions were selected with average varie-ties of (1. 3 ± 0. 1) and sum of (77. 7 ± 0. 2) yuan. Among them, 31 ones were irrational prescriptions accounting for 3. 5%. Con-clusion:In order to promote the rational and safe drug use in pregnant patients, it is necessary to strengthen relevant knowledge educa-tion and prescription review of clinical pharmacist.
3.Relationship between attachment behavior and social adaptation among left-behind middle school students: mediation effect of sense of responsibility

Objective To explore the relationship between responsibility and social adaptation of the left-behind middle school students and to provide evidence for mental health education of left-behind children.Methods 290 left-behind middle school students were selected by cluster-random-sampling.All of them were assessed by the middle school students' social adaptability scale (SAS-SSS),the inventory of parent and peer attachment (IPPA) and the scale of high school students' responsibility.Results The correlation between responsibility,attachment behavior and social adaptation of left-behind middle school students were significant(r=0.39,r=0.56,P<0.01).There were significant differences between gender on the responsibility(male(202.64±26.78) vs female(212.67±23.24)).Regression analysis showed that mother attachment,overall responsibility,general responsibility and special fault responsibility could predict the social adaptation(P<0.05).Responsibility and its factors played a mediating role between attachment and adaptation,taking 52.1%,20.1%,30.8%,9.1%,47.9%,40.7% proportion of the total effect.Conclusion Responsibility is an important factor affecting the social adaptation of left-behind middle school students.
4.Development of web-based literature database for Yersinia pestis

Objective To build a literature database that systematically sorts out and integrates related knowledge of Yersinia pestis researches, which will improve the efficiency of future researches on Y.pestis.Methods EndNote X5 software was used as a literature collection tool in this research.Computer languages, including Perl and PHP, were employed to standardize the text data, establish the entity relationship model, code the webpage and build the basic literature database.The tool combination of Apache+PHP+MySQL that is usually used in development of small or medium-scale websites was selected to build a dynamic website.Results and Conclusion The Y.pestis literature database(YPKD) was constructed (accessed by http://101.201.51.148/ypkd/.), from which users can quickly search or browse the required literature, download the required full text, and obtain the latest online information on Y.pestis.The database can serve as a model for establishing literature databases of other crucial pathogens.
5.Impaction bone grafting with morselized bone in total hip revision for acetabular deficiency

BACKGROUND:Bone graft for acetabular reconstruction includes morselized bone graft, structural bone graft and hybrid bone graft, and the morselized bone has been widely used because of the advantages of simple
production and short healing time.
OBJECTIVE:To explore the key technologies and clinical effect of impaction bone grafting with morselized bone in total hip revision for AAOS Ⅲ acetabular deficiency.
METHODS:Sixteen cases of AAOS Ⅲ acetabular deficiency were treated with impaction bone grafting with morselized bone combined with metal devicesor constructive bone grafting. The hip Harris scores and
radiographic data were compared before and after treatment. The effect of impaction bone grafting with morselized bone on acetabular deficiency was assessed.
RESULTS AND CONCLUSION:Al the patients were fol owed-up at 3, 6, 12 months after surgery and every half a year successively. The pain of hip joints after operation was relieved significantly and the walking function was
restored. The hip Harris score was improved from 48.00 points before surgery to 84.94 points after surgery (P<0.01). Five cases were graded as excel ent, eight cases as good, two cases as average, and one case as poor. The excel ent and good rate was 81%, and the satisfying rate of the patients was 94%. The post-operative X-ray films of al the 16 patients showed that the acetabular rotation centers were recovered (near) to normal and the acetabular cups were covered wel by bone. The grafting bone particles got radiological osseointegration and the
acetabular cup prosthesis did not displaced, and no displacement and breakage happened to the metal devices.
Impaction bone grafting with morselized bone in total hip revision for AAOS Ⅲ acetabular deficiency can effectively reconstruct the acetabular bone structure, retain and restore the acetabular bone mass, and it has good technical advantages and good clinical effects.
7.Applications of various machine learning algorithms in lung cancer research

8.Effects of caffeine and ryanodine on K_(Ca) in porcine coronary artery smooth muscle cells

AIM: To study the effect of caffeine on the large conductance calcium activated potassium (K_(Ca)) channels by patch-clamp technique on smooth muscle cells enzymatically isolated from the porcine coronary artery (PCASMC),and to investigate the effect of ryanodine on K_(Ca) being activated by caffeine.METHODS: Using the single channel patch-clamp technique,single PCASMC was isolated by collagenase,the activity of single K_(Ca) channel was recorded in porcine coronary artery smooth muscle cells.RESULTS: Caffeine (0.1-10 mmol/L) enhanced the open probability (Po) of K_(Ca) channels in a dose-dependent manner in the intracellular side of inside-out patches and its effect was almost completely abolished by washout. Caffeine decreased the mean close time markedly,but had no effect on the amplitude of K_(Ca) channels. However,ryanodine (10-40 μmol/L) decreased Po of K_(Ca) channels activated by caffeine in a dose-dependent manner in cell-attached patches. The mean open time also decreased.CONCLUSION: Caffeine directly activates K_(Ca) channels of porcine coronary artery smooth muscle cells in inside-out patches,the activity of single K_(Ca) channel is inhibited by ryanodine indirectly in cell-attached patches.
9.Clinical research on feeding effect of semi-demand feeding and abdominal massage in preterm infants

Objective To evaluate the feeding effect of semi-demand feeding and abdominal massage on the feeding process, feeding performances and clinical safety (including the time of returning to birth weight, feeding tolerance, apnea). Methods A total of 120 preterm infants that met inclusion criteria of the study were randomly assigned to three groups, and each group had 40 cases. Control group:received standard feeding care. Semi-demand feeding group: received 5-10 minutes of oral motor intervention every time before feeding(including oral stimulation and non nutritional sucking), then decided feeding methods according to its consciousness behavior state. Semi-demand feeding combined abdominal massage group: received both semi-demand feeding and abdominal massage before feeding. Observed and recorded the three groups′ transition time, whether there were statistically significant differences in feeding performance and clinical safety. Results The feeding transition time was (11.55± 4.69), (10.98±3.85), (8.50±3.87)d in control group, semi-demand feeding group and semi-demand feeding combined abdominal massage group, and there were significant differences (F=6.088, P=0.003). In terms of feeding performance, the feeding efficiency were (7.27 ± 2.65), (9.74 ± 2.63), (9.52 ± 2.13) ml/min, the feeding proficiency were (66.31 ± 14.51)%, (76.64 ± 12.46)%, (82.26 ± 10.65)%, the volume transfer were (63.99 ± 11.02)%, (80.16 ± 14.41)%, (84.97 ± 11.16)% in control group, semi-demand feeding group and semi-demand feeding combined abdominal massage group, and there were significant differences (F=12.189, 16.383, 31.959, all P < 0.01).Among the clinical safety, the time of restoration of birth weight were (10.55 ± 1.99), (9.28 ± 1.77), (10.05 ± 2.46) d in control group, semi-demand feeding group and semi-demand feeding combined abdominal massage group, and there were significant differences(F=3.759, P=0.026). The incidence of feeding intolerance were 50.0%(20/40), 40.0%(16/40), 22.5%(9/40), and there were significant differences(χ2=6.613, P=0.037). The incidence of apnea were 37.5%(15/40), 20.0%(8/40), 15.0%(6/40), and there were significant differences(χ2=6.093, P=0.048). Conclusions The combined intervention methods could accelerate the feeding process, improve feeding performance as well as reduce the incidence of feeding intolerance and apnea. Hence, semi-demand feeding combined abdominal massage is a recommended choice for clinical use.
10.Expression of CXC chemokine receptor 5 in skin lesions of bullous pemphigoid and its significance

Objective To study the role of CXC chemokine receptor 5 (CXCR5) in the pathogenesis of bullous pemphigoid (BP) by detecting the expression of CXCR5 on follicular helper T (Tfh) cells in skin lesions of BP.Methods An immunohistochemical procedure was used to detect CXCR5 expression in biopsy specimens from skin lesions of 16 patients with BP and normal skin of 10 healthy individuals.Results CXCR5 was mainly expressed in the cytoplasm and membrane of Tfh cells in the basal cell layer of normal skin as well as in both basal cell layer and prickle cell layer of BP lesions.The number of CXCR5-positive ceils per high power field (× 400) was 11.16 ± 4.47 in normal skin specimens,significantly lower than that in lesional skin specimens (35.70 ± 12.20,t =6.07,P < 0.01).Conclusion CXCR5 may be involved in the pathogenesis of BP.
