1.Application of Quality Risk Management in Drug Withdrawal Links of Hospital Pharmacy

OBJECTIVE:To control the drug quality risk in drug withdrawal links of hospital pharmacy,and provide reference for the application of quality risk management(QRM)in hospital pharmacy. METHODS:Drug QRM was developed in drug with-drawal links through risk identification,risk assessment,risk control,risk assessment and other steps. Using the incidence of risk factors and reusable rate of withdrawal drug as indexes,related data of before(Jul.-Dec. 2015)and after(Jan.-Jun. 2016)devel-oping QRM in our hospital were compared to evaluate the effect of drug QRM. RESULTS:Determining whether specific storage drugs kept the required storage conditions,whether the numbers of withdrawal drugs counted to minimal packaging and other 5 fac-tors were high-risk factors(the risk score of each factor>4 points);control measures were taken separately for high risk factors, then high-risk factors were reduced to acceptable levels(risk score<4 points). After developing QRM,the incidence of risk factors were reduced than before(reduce 1.35%-6.19%),reusable rate of withdrawal drug was increased(98.64% vs. 86.32%)(all P<0.05). CONCLUSIONS:Developing QRM in drug withdrawal links of hospital pharmacy can reduce the drug quality risk.
2.Effect Analysis of the Optimization of Initial Drug Placement in the Automated Drug Dispensing Machine by Minimum Time Algorithm

OBJECTIVE:To optimize the drug placement in the automated drug dispensing machine to improve work efficien-cy. METHODS:Based on the principle of the minimum time algorithm,the drug which would be used at a high frequency was placed in the drug storage tank nearest to the drug outlet. Meanwhile,the rule of drug use was drawn from the information on a large number of prescriptions,based on which the drugs correlated with each other were placed in the drug storage tanks that were adjacent. With daily time it takes to add drugs,average time it takes to make up a prescription and the maximum number of drugs stored as the evaluated indexes,the initial drug placement in the automated drug dispensing machine was optimized. The changes in the indexes within 3 months before and after the above-mentioned optimization were statically analyzed. RESULTS:After calculat-ing the three-dimensional sizes of the packages of drugs and the dispensing frequency data of the previous year,354 drugs were se-lected and placed in the nearer or farther storage tanks in the automated dispensing machine according to the dispensing frequency and the correlation among them. After the optimization of the placement,daily time it takes to add drugs reduced by 54 min(218 vs. 165 min)on average,average time it takes to make up a prescription reduced by 8 s(24 vs.16 s)and the maximum number of drugs stored increased by 1 333 boxes(13 113 vs. 14 446 boxes)on average. There was statistical significance in differences(P<0.05). CONCLUSIONS:The initial drug placement in the automated drug dispensing machine that was optimized by minimum time algorithm has reduced daily time it takes to add drugs and average time it takes to make up a prescription and increased the maximum number of drugs stored and thus improved work efficiency.
3.Clinical application of serum GFAP,NDKA and PARK7 in patients with ischemic stroke

Objective To explore the relationship of GFAP, NDKA and PARK7 serum concentrations of patients with IS, and their diagnose and prognosis value in IS. MethodsThe serum concentrations of GFAP, NDKA and PARK7 were detected in 37 IS patients, 28 ICH patients and and 30 healthy persons by ELISA. These indexes of patients were detected in 12 hours, 3 d and 14th day after onset of ischemic stroke. Their neurological injury status were also evaluated by MESSS at corresponding time points, and their activities of daily living were evaluated by BI at 14 d discharge from hospitaL At the same time, the diagnostic efficiency was analysed for IS using the three biomarkers and the combined detection. ResultsIn IS group, the serum concentrations of GFAP in 12 hours, 3rd and 14th day after onset were (5. 49 ±2. 25 )μg,/L, (5. 17 ± 2. 29) μg/L and (5. 96 ± 2.39 ) μg/L, respectively. The serum concentrations of NDKA were 9. 15(6.28 -12.79) μg/L, 9. 13(6.31 - 12.23) μg/L, 9.31(6.40 - 11.83) μg/L respectively,and the serum concentrations of PARK7 were (32. 71 ±6. 34 ) μg/L, (31.23 ±6. 04) μg/L, (32. 79 ±6. 94) μg/L respectively. The serum levels of GFAP, NDKA and PARK were respectively (4. 62 ± 1. 56)μg/L, 4. 24(3. 30 -5. 61 ) μg/L, ( 14. 25 +2. 65) μg/L in healthy control group. The levels in IS groups were remarkably increased compared with the healthy control group except the level of GFAP in the 3rd day (t = 1. 129, P>0. 05). The levels in other time points were significantly different between patients group and healthy control. t value of GFAP were respectively 2. 642, 1. 870,P<0. 05; Z value of NDKA were 6. 173, 6.100, 6.278,P <0. 01; t value of PARK7 were 14.964, 15.367,16.060, P <0. 01. The specificity and sensitivity of the individual detection for diagnosis of IS was 46. 7% (14/30) and 81.1%( 30/37 ) for GFAP, 90. 0% ( 27/30 ) and 78.4% ( 29/37 ) for NDKA, 96. 7% (29/30) and 97.3% ( 36/37 )for PARK7. The specificity and sensitivity for combined detection of 3 biomarkers was 96. 7% (29/30) and 100% (37/37). The combined detection achieved better specificity and sensitivity. Moreover, the risk of IS with higher level GFAP was 1. 3 times that of the controls ( OR = 1. 300, P = 0. 044 ). The risk of higher NDKA was 1.7 times higher( OR = 1. 668, P = 0. 036 ). The risk of higher PARK7 was 1.8 times higher (OR = 1. 809, P =0. 005 ). The serum levels of GFAP were significantly different between IS and ICH in 12 h(t= 4.097, P=0.000). The serum concentrations of GFAP, NDKA and PARK7 were positively correlated with MESSS score at different time points. In IS, r value were 0. 534, 0. 482, 0. 357 , P < 0. 05at less than 12 h; r value were 0.433, 0.487, 0. 299,P value were 0. 007, 0. 002, 0.073 at 3 d;r value were 0. 394, 0. 200, 0. 084,P value were 0.016, 0. 236, 0.620 at 14 d. And the serum levels of GFAP,NDKA and PARK7 were negatively correlated with BI score at 14th day, r value were -0. 430, -0. 321,-0.076,P value were 0.044,0.050,0.657. Conclusions The concentrations of GFAP, NDKA and PARK7 in serum are closely related with IS. The increased seruro levels of these indexes are risk factors in IS. The detection of these indexes could be helpful for the early diagnosis, timely treatment and prognosis assessment for IS.
4.Transfection of pemt-2-cDNA inhibits the expression of cell cycle related proteins in rat CBRH-7919 hepatoma cells.

OBJECTIVESTo unravel the molecular mechanism of proliferation inhibition induced by transfection of pemt2-cDNA into rat CBRH-7919 hepatoma cells.
METHODSWe started with the highly expressed PEMT2 clone. Cell culture and Western blotting techniques were used to examine the expression of cyclinD1/CDK4, cyclinE/CDK2, phospho-Rb, caspase-3, c-jun and caveolins.
RESULTSOur results showed that CDK4, CDK2, phospho-Rb and c-jun were down regulated in the pemt2 highly expressed cell clone. The high expression clone of pemt2-transfected cells also showed over expression of caspase-3.
CONCLUSIONThe reductions of proliferation and apoptosis of pemt2 transfected cells could be related to the G1 phase arrest induced by down-regulation of the cell cycle-associated proteins.

A new teaching method was developed in the curriculum of Harmful Microorganisms Control Technology.It is characterized by students’ self-learning followed by student’s instruction.Both students and teacher have succeeded in this model after four stages of practice,in which a pleasant learning atmosphere was created in the classroom.An effective interaction between teacher and students was achieved.Students are viewed as main objects in the classroom and they are encouraged to ask questions,to formulate their own ideas,or to find things out for themselves.Thus,students’ abilities including presentation,communication,competition,and cooperation were enhanced.By adapting their role to the new teaching method,teachers have also improved their teaching skill and strategies.

Objective To investigate the feasibility and accuracy of magnetic resonance microneurog- raphy of sciatic nerve fascicles in rabbil by correlation with the gross anatomy.Methods The 3D T_2-weigh- ted imaging(3D-T_2 MI),3D T_2-weighted imaging plus spectral presaturation with inversion recovety(SPIR), T_1-weighted imaging(T_1 WI)of the sciatic nerve in 10 rabbits were performed on a 1.5 Tesla magnetic reso- nance system.The radiological ananomy and imaging features of sciaticnerve fascicles were observed and the anterior-posterior diameter was measured on 3D T_2-weighted imagingThe imaging evaluation was correlated with the gross anatomy.The T_1 and T_2 relaxation time were determined by multiple echo spin echo and mix se- quecerespectively.Results The libial nerve and peroneal nerve in the main trunk of sciaticnerve in all 10 rabbits could be clearly displayed on the 3D T_2 WI,3DT_2WI plus SPIRand T_1WI.Strikingly,the 3D T_2 WI could delineate the fine branches of the sural nerve and posterior femural cutaneous nervesThe T_1 and T_2 relaxation time were 915 ms40 msrespectivelyGrosslythe anterior-posterior diameter of sciatic nerve trunk was3.17?0.21)mmwhile was(3.15?0.19)on 3D T_2 WI.There was no statistically significant difference(t=0.768,P=0.462).Conclusion With 1.5 Telsa MR system the microneurography of the sci- atie nerve could be achievable and the individual fascicles of sciatic nerve trunk could be clearly and accurately discriminated.
9.Morphological evaluation of lumbar dorsal root ganglion on three-dimensional magnetic resonance imaging

Objective To investigate the morphological features of normal lumbar dorsal root ganglia using a three-dimensional(3D)coronal MR imaging.Methods One hundred and fifteen volunteers were included.Ages ranged from 15 to 75 years,with a mean of 40 years.Coronal 3D fast field echo(FFE) with water selective excitation(Proset)MR examination of 1150 dorsal root gangha were underwent at nerve root levels from L1 to L5.The source coronal images were further reconstructed into a series of rotational alignment coronal images with an interval angel of 12 degree using maximum intensity projection(MIP) technique.All DRGs of bilateral spinal nerve from L1 to L5 were morphologically analyzed on the original and MIP images including qualitative evaluation of the location,signal intensity,architecture and quantitative dimensional measurement.Results There were 225,225,219,210 and 160 foraminal ganglia from L1 to L5 level,respectively.The incidence of intraspinal ganglia from L3 to L5 gradually increased with a maximum at L5 level of 29.1%(X~2=188.371,P
10.Fermentation transformed ginsenoside by Lactobacillus plantarum.

OBJECTIVETo explore ginseng fermentation process by Lactobacillus plantarum, and to make part of total saponins transformed into more reactive ginsenoside Rd.
METHODMicrobial fermentation was carried out by still dark culture. Total saponins were extracted by Soxhlet extraction, and determined by UV visible spectrophotometry with colours reaction by vanillin-sulfuric acid. Ginsenoside Rd was determined by HPLC method.
RESULTThe fermentation process was: MRS medium, 35 degrees C, pH 5.0, cultured for 2 days. The content of total saponins was inhance 32%, and the content of ginsenoside Rd was increased 4.864 mg x g(-1).
CONCLUSIONThe fermentation system's process was reasonable, and it's suitable for mass production, important significance for ginsenoside microbial transformation.
