OBJECTIVE:To investigate the clinical efficacy and safety of amoxicillin sodium and clavulanate potassium com-bined with cranial mild-moderate hypothermia in the treatment of neonatal hypoxic-ischemic encephalopathy (HIE) complicating with pulmonary infection. METHODS:80 children with HIE complicating with pulmonary infection were randomly divided into ob-servation group(42 cases)and control group(38 cases). Control group received routine treatment under normal body temperature as lowering intracranial pressure, correcting acid-base balance, giving nutritional support and anticonvulsive treatment, and then was given amoxicillin sodium and clavulanate potassium 30 mg/kg intravenously, tid, more than 30 min each time, for consecutive 3 d. On the basis of control group, observation group was additionally given cranial mild-moderate hypothermia therapy:wearing cooling cap,at 10 ℃;keeping pharynx nasalis temperature at 34 ℃;keeping rectal temperature at 33-37 ℃;decreasing at rate of 1 ℃/h. They continued treatment for 3 d till body temperature decreased to 34.5 ℃;and then cooling cap was taken off to recover normal body temperature. Therapeutic efficacy of HIE and pulmonary infection were observed in 2 groups. NBNA score of 2 groups were recorded 1st,2nd and 4th week after birth, and the occurrence of ADR was observed. RESULTS:Total effective rate of HIE therapy (95.24% vs. 78.95%),cure rate of pulmonary infection (52.38% vs. 26.32%) and total effective rate (85.71% vs. 65.79%) in observation group were significantly higher than control group, with statistical significance (P<0.05). There was no statistical significance in NBNA score between 2 groups 1st week after birth(P>0.05). Compared to 1st week after birth,NBNA score were significantly improved 2nd and 4th week after birth, and the observation group were significantly higher than the control group, with statistical significance(P<0.05). There was no statistical significance in the incidence of ADR between 2 groups(P>0.05). CONCLUSIONS:Amoxicillin sodium and clavulanate potassium combined with cranial mild-moderate hypothermia is effec-tive for HIE complicating with pulmonary infection, and can effectively improve the prognosis of children with good safety.

Objective To observe and compare the anti-angiogenic effect of sorafenib, unfractionated heparin(UFH)and low molecular weight heparin (LMWH), singly used or in combination, with chick chorioallantoic membrane assay. Methods Sixty 8d-developed chick embryos were randomly divided into 6 groups (10 each): control group, sorafenib group, UFH group, LMWH group, sorafenib+UFH group, and sorafenib+LMWH group. Drugs were given respectively and incubation continued. The dosage of drugs for each embryo was 20 nmol of sorafenib, 5U of UFH and 5U of LMWH, respectively, and H-DMEM was applied to the embryo in control group in the same volume (2?l). After continuing incubation for another 72h, the anti-angiogenic effect in each group was observed and the vascular number in the chick chorioallantoic membrane was counted. Results Vascular number in single drug groups (sorafenib group: 8.9?1.9; UFH group: 7.6?2.3; LMWH group: 6.9?1.3) was significantly smaller than that in the control group (26.8?2.3, P

Chinese Journal of Anesthesiology 2011;31(4):497-499
Objective To investigate the effects of electroacupuncture (EA) of different intensities on cerebral energy metabolism in a rat model of global cerebral ischemia-reperfusion (I/R) . Methods Forty male SD rats weighing 200-230 g were randomized into 5 groups ( n = 8 each) : group A sham operation; group B global cerebral I/R and C, D, E groups cerebral I/R+ 5, 3, 1 mA EA. Global cerebral I/R was induced by 4-vessel occlusion technique. Bilateral vertebral arteries were permanently occluded by cauterization.Bilateral common carotid arteries were clamped. When the bilateral pupils were completely dilated, the arteries were unclamped. Baihui,Mingmen and Zusanli were electrically stimulated with 5,3,1 mA (30-50 Hz) for 20 min at 1 h of reperfusion in C, D, E groups. The EA was repeated at 12 h of reperfusion. The animals were sacrificed at 24 h of reperfusion. The activities of Na+ -K+ -ATPase, succinodehydrogenase (SDH) and lactic dehydrogenase(LDH) in brain tissue were measured. Results Cerebral I/R significantly increased LDH activity and decreased Na+ -K+ -ATPase activity in group B as compared with group A. EA with 5 mA significantly decreased LDH activity and increased SDH and Na+ -K+ -ATPase activities in group C compared with group B. Conclusion EA can improve the cerebral energy metabolism in a rat model of global cerebral I/R and it is related to the intensity, which may be the mechanism by which EA reduces the global cerebral I/R injury.

Objective We reported previously that single nucleotide polymorphisms SNP) + 11G > A in intron 3 of the human urate transporter 1 (hURAT1) gene are associated with hyperuncaemia in Han Chinese. The aim of the present study was to evaluate the effect of the variants on hURAT1 function. Methods The wild-type,mutant-type hURAT1 and exon 5-null hURAT1 were constructed, and respectively microinjected into the zebrafish embryo yolks. The subcellular localization of different genotypes of hURAT1 was detected by confocal laser scanning microscope. Results Compared with wild type, the mutant recombinant plasmid transcribed two types of mRNA spliceosome, the wild type and the exon 5-null type. The hURAT1 wild type protein was prominent localized on cell membrane, while the mutant type and exon 5-null hURAT1 proteins were distributed uniform in the cytoplasm but not on the cell membrane. Conclusion The hURAT1 variant + 11 G > A resulted in an alternative splicing of hURAT1 mRNA-exon 5-null type. Its protein product exhibited a different subcellular localization compared with that of wild type.

AIM: To investigate the relationship between the protective effect of sodium oxybate on neuronal damage induced by hypoxia reoxygenation and GABA A receptor in primary cultured rat cortical neurons. METHODS: The primary cultured rat cortical neurons were used to make the hypoxia reoxygenation damage model. The morphology of cell was observed. The lactate dehydrogenase (LDH) effluxed into the media as an indicator of neuronal injury was detected after 6 h of the reoxygenation injuries. The malonyldialdehyde (MDA) contents, superoxide dismutase (SOD) and glutathione peroxidase (GPX) activities were determined at the same time. RESULTS: The hypoxia reoxygenation caused neuronal swelling and widespread neuronal degeneration, increased LDH efflux and MDA contents, and decreased SOD and GPX activities. Sodium oxybate assuaged neuron damage, decreased LDH efflux and MDA contents (P
