2.Clinical significance of hepatic artery variation in hepatic portal lymphadenectomy

Objective:To investigate the recognition and injury prevention strategies of hepatic artery variations during hepatic portal lymphadenectomy. Methods:A retrospective analysis was performed, and 12 patients of hepatic arterial variation among 62 pa-tients with hepatic portal lymphadenectomy were the subjects. The study was conducted in the First Affiliated Hospital of Bengbu Medi-cal College between January 2013 and July 2014. The intraoperative treatment and postoperative complications were recorded. Results:Among 12 cases of hepatic artery variation, we found the following cases:3 cases (25.0%) of Michels' Type III, 2 cases (16.7%) of Mi-chels' Type VI, 1 case (8.3%) of Michels' Type IX, 1 case (8.3%) of Hiatt's Type 6, 2 cases (16.7%) of spatial location variation between right hepatic artery and hepatic duct, 2 cases (16.7%) of left and right hepatic artery originating from a common hepatic artery, and 1 case (8.3%) of right hepatic artery originating from the gastroduodenal artery. No injury of hepatic artery occurred. Two cases had post-operative complications, including 1 case of pancreatic leakage and 1 case of incision infection;postoperative hemorrhage, bile leakage, hepatic abscess did not occur in these two cases. Patients recovered well in general. Conclusion:Hepatic arterial injury can be signifi-cantly reduced by the following:increased familiarity with the various types of hepatic artery variations;complete imaging examina-tions for inspection and evaluation before surgery;and careful and meticulous operations in surgery.
3.Ecology survey of Microtus fortis in natural foci of plague in Dingbian town Shaanxi province in 2010

Objective To investigate the constitution,density changes and carrier rate about Yersinia pestis of rodents in plague foci,and to provide the scientific evidence for plague prevention.Methods According to the program of national monitoring plague,two survey procedures,namely quadrat of single-ha for 24 h and 5 m mouse jam,were used to monitor the host animals; culture and identification of Yersinia pestis in liver or spleen of the experimental animals was carried out by using self-made medium in the north of Beiyuanzi village in Dingbian town Shaanxi province.Results One hundred twelve rodents were captured using the first procedures and the rodent average density was 8.62 ind./hm2 and six species of rodents were found namely Meriones unguiculatus ( 100 individuals),Microtusfortis(5 individuals),Ochotona daurica(3 individuals),Meriones meridianus (2 individuals),Mus musculus Linnaeus (1 individual) and Cricetulus barabensis (1 individual).One hundred seventy-three field mouses were captured using the second procedures including Mus musculus Linnaeus (136 individuals),Cricetulus barabensis (36 individuals),and Microtus fortis ( 1 individual ).Among them,Microtus fortis was found in the salt marshes in the southern edge of Ordos Plateau steppe in plague area of Dingbian county.Yersinia pestis was not identified in all animals.Conclusions Microtus fortis is found in natural foci of plague in Shaanxi province for the first time,and a new geographic region was found.Its epidemiological significance needs further study.
4.Clinical study on adverse reactions of peginterferon alfa-2a in treatment of chronic hepatitis in adults and children

Objective To investigate the scope and degree of short-term adverse reactions of peginterferon alfa-2a in treatment of chronic hepatitis in adults and children to provide basis for anti-viral treatment in clinical practice. Methods A prospective study was conducted in adults and children with chronic hepatitis treated with pegintefferen aifa-2a. Meanwhile, the reactions in the patients were recorded with a table designed by ourselves and statistically analyzed. Results The short-term adverse reactions included increase in body temperature and aching pain in joints and muscles. The increase in body temperature was the major reaction and accounted for 54.11%. The increase in body temperature began to appear in 47.6% of the patients. The body temperature was 37.3℃-38.9℃ in most of the patients and mediate and low increase was found in 85.4% of the patients, which was decreased to 70% in the 4th week. However, the percentage of patients with high temperature was increased from 14.5% in the 1st week to 30% in the 4th week. The increase of body temperature began to appear in 9-12 h and 3-5 h after injection of peginterferon alfa-2a in the 1st and later, respectively. The duration of fever was 3-4 h in most of the patients. It appeared once in 1 week after the rejection in most of the patients. For management of fever, coohng with medication was conducted in 45.5% of the patients. Conclusion The short-term adverse reactions in patients with chronic hepatitis treated with pegintefferon alfa-2a include the increase in body temperature etc. The severity of the adverse reactions gradually reduces with continuation of the treatment. Of the adverse reactions, the increase in body temperature is the major (47.6%) and others only account for 1%- 16.9%. The increase in body temperature is mainly transient and no management is needed in 50% of the patients. Since the "ladder-type" dose-adding method is used for administration of peginterferon alfa-2a in group of patients, the adverse reactions is low in number and mild in degree.
5.Clinical control study on the effect of health education in improving the social function of schizophrenia patients

Objective To explore the effect of health education in improving the social function of schizophrenia patients.Methods 100 schizophrenia inpatients who met the demand were randomly divided into the experiment group and the control group.The two groups accepted the routine therapy of antipsyehotic drags and the nursing.In addition,the experiment group accepted the health education and the exercise of social function for 8 months.The scores of social disability screening schedule(SDSS) and brief psychiatric rating scale(BPRS)were compared among the schizophrenia inpatients.Results After 8 months of therapy and nursing.the total score and the scores of social function shrink,self care,interest to outside environment,sense of responsibility and planning in the experiment group were lower than those in the control group.The differences had statistical meanings(P<0.05 or P<0.01).Conclusions Giving continuous and normative health education and exercise of social function to schizophrenia patients can improve the social function.retard the mental deterioration and reduce the rate of recurrence.
6.Pulmonary neuroendocrine cell hyperplasia and tumorlets in bronchiectasis: a clinicopathologic study of 22 cases with review of literature.

OBJECTIVETo study the clinical and pathological features of pulmonary neuroendocrine cell hyperplasia and tumorlets with bronchiectasis.
METHODSBoth the clinicopathologic changes and immunohistochemical findings were examined with microscopy and EnVision method in 22 cases of pulmonary neuroendocrine cell hyperplasia and tumorlets.
RESULTSThe average age of the 22 patients was 53 years, with a male to female ratio of 9:13. On macroscopic examination the lungs showed bronchiectasis; one case was accompanied by gray-white, soft nodules (diameter < 5 mm). Microscopy of the HE sections showed the basic pathologic change was bronchiectasis, accompanied by neuroendocrine cell hyperplasia and tumorlet formation in the pulmonary parenchyma surrounding the bronchioles, presenting as single nodule (10 patients), or multifocal nodules (12 patients), with average size of 1.6 mm in diameter. No tumor cells were identified in the lymph nodes. Sixteen of 22 patients were disease-free after an average follow-up period of 58 months (17 - 117 months); one patient died suddenly after surgery; and five were loss of follow up. Immunohistologically, the tumor cells were positive for CgA (18/18), Syn (16/16), AE1/AE3 (16/16) , TTF-1 (14/15), and CD56 (14/14), and Ki-67 index was < 2% in 12 cases.
CONCLUSIONSImmunohistological staining for CgA, Syn, CD56, TTF-1 and AE1/AE3 can confirm the diagnosis. Early detection, pulmonary resection and follow-up help prevent the progression of these diseases.

7.Study on the compliance and medication satisfaction in patients receiving antiviral therapy

Objective To investigate the compliance and medication satisfaction in patients who received antiviral therapy.Methods Chronic hepatitis B and hepatitis C patients who had antiviral therapy from January 2008 to May 2011 in Center of Diagnosis,Treatment and Research for Teenagers Liver disease,302 Mihtary Hospital of China were chosen,and their compliance,medication satisfaction and reasons of bad treatment compliance werc investigated.Results Among chronic hepatitis B and hepatitis C patients who received antiviral therapy,82.80% had good compliance,in which,91.86% of hepatitis C patients and 80.57% of hepatitis B had good compliance.Hepatitis C patients were better than hepatitis B patients in compliance,and the difference was statistically significant (x2 =6.177 3,P < 0.05).The medication satisfaction of hepatitis B patients who had good compliance was lower than that of hepatitis C patients [(58.38 ±9.17) vs (59.62 ± 9.81)],and the difference was statistically significant (t =-1.050 1,P < 0.05).Conclusions Treatment compliance and medication satisfaction degree are not as good as expected in patients who receive antiviral therapy,which are due to side effects of drugs,low therapeutic efficacy,lacking of medication monitoring and mistaken understanding of the diseases.
8.A prospective randomized controlled study of the application effect of hydrogel dressings on deep partial-thickness burn wounds after dermabrasion and tangential excision

Objective:To investigate the clinical effect of applying hydrogel dressings in deep partial-thickness burn wounds after dermabrasion and tangential excision.Methods:A prospective randomized controlled study was conducted. From November 2015 to August 2019, 168 patients with deep partial-thickness burns hospitalized in Zibo Sixth People's Hospital met the inclusion criteria. According to the random number table, the patients were divided into hydrogel dressing+vaseline gauze group (84 cases, 67 males and 17 females) and vaseline gauze only group (84 cases, 65 males and 19 females) who were aged (31±16) and (35±17) years, respectively, and were given corresponding treatment after dermabrasion and tangential excision. The dressings were changed every 3-5 days. Autologous skin grafting was performed with split-thickness or medium-thickness skin grafts taken from trunk or thigh if the wounds failed to heal over 21 days or the wounds were less than 21 days but located in the joints, which might affect the functional activities at later stage. After operation, the general condition of the wounds was observed continuously and dynamically till the wounds were healed. The degree of dressing adhesion was evaluated during the first 4 dressing changes after operation, and the degree of pain was evaluated using Numerical Rating Scale (NRS). The wound healing rate on post operation day (POD) 3, 6, and 15 was calculated. The positive proportion of bacterial culture of wound exudates/cleanout fluid on admission and at dressing change on POD 3, 6, and 15 was calculated. The number of dressing changes were recorded, the rate of skin grafting operation was calculated, and the time of complete wound healing was recorded. Vancouver Scar Scale (VSS) was used in 6 months of follow-up to evaluate the hyperplasia of scar. Data were statistically analyzed with analysis of variance for repeated measurement, independent sample t test, Mann-Whitney U test, chi-square test or Fisher's exact probability test, and Bonferroni correction. Results:During the observation period after operation, the wound was moist with less exudates and the wound healed much faster in patients of hydrogel dressing+vaseline gauze group, with the inner dressing being easier to remove with mild pain, while the wounds showed more exudates and slower healing in patients of vaseline gauze only group with the obvious adhesions of inner dressing, stronger pain, and bleeding in the wounds. Compared with those in vaseline gauze only group, the degree of dressing adhesion and pain NRS score of patients in hydrogel dressing+vaseline gauze group decreased significantly during the first 4 dressing changes after operation ( χ 2=52.625, Z=-10.854, P<0.01), the wound healing rate increased significantly at dressing change on POD 3, 6, and 15 ( t=10.347, 41.150, 167.627, P<0.01), the positive proportion of wound exudates/cleanout fluid bacterial culture did not change significantly on admission or at dressing change on POD 3 ( P>0.05) but decreased significantly at dressing change on POD 6 and 15 ( χ 2=15.616, 15.226, P<0.01), the dressing change times was significantly reduced ( t=-11.986, P<0.01), and the rate of skin grafting operation was significantly decreased ( χ 2=35.850, P<0.01). The complete wound healing time of patients in hydrogel dressing+vaseline gauze group was (17.6±2.8) d, significantly shorter than (27.1±3.0) d in vaseline gauze only group ( t=-21.288, P<0.01). During the follow-up of 6 months, the VSS score of scar hyperplasia of patients in hydrogel dressing+vaseline gauze group was significantly lower than that in vaseline gauze only group ( Z=-11.287, P<0.01). Conclusions:Compared with the use of vaseline gauze only, the application of vaseline gauze coated with hydrogel dressing in deep partial-thickness burn wounds after dermabrasion and tangential excision is more effective in moisturizing, significantly reducing the degree of dressing adhesion and patients' pain during dressing change, increasing wound healing rate, decreasing wound infection rate and the rate of skin grafting operation, and reducing the number of dressing changes. It shortens the time for complete wound healing, and effectively relieves scar hyperplasia.
9.Analysis of measles immunity level in persistent populations in Beijing, 2012.

OBJECTIVETo analyze the measles immunity level of persistent population in Beijing.
METHODSA total of 2125 objects from 10 age groups, who had been living in Beijing for over 6 months, were selected from urban and rural areas in Beijing in 2012. Demographic characteristics, history of measles and vaccine immunization were investigated by questionnaire. 5 ml blood sample of each subject was collected, and the Measles IgG antibody was measured by ELISA assay.
RESULTSPositive rate of measles antibody was 84.71% (1800/2125) and standardized positive rate was 88.07% . Median of antibody was 960.46 IU/L. Positive rate and median of measles antibody were significantly different between population from different age groups (χ(2) = 341.60, P < 0.01; H = 216.27, P < 0.01). Antibody positive rate and median were lowest in the <1 year age group, which were separately 43.06% (90/209) and 185.80 IU/L; and highest in the 1-4 (97.31% (181/186) and 2448.81 IU/L) and 5-9 years age group (96.46% (218/226) and 1910.72 IU/L). The range of antibody positive rate and median in adults of ≥ 15 years were 81.98%-90.14% and 744.38-1474.84 IU/L. Antibody positive rate and median in persistent population, which were separately 82.45% (883/1071) and 899.82 IU/L, were lower than those in migrant population, which were 87.00% (917/1054) and 166.19 IU/L, respectively (χ(2) = 8.51, P < 0.01;U = 538 704.00, P < 0.01). Antibody positive rate and median in population with vaccination history, which were separately 91.95% (891/969) and 1443.11 IU/L, were higher than those population without vaccination history and people whose history unknown (32.95% (57/173) , 127.33 IU/L; 86.67% (852/983) , 923.73 IU/L). The difference showed statistical significance (χ(2) = 399.92, P < 0.01; H = 202.11, P < 0.01).
CONCLUSIONAmong the persistent population in China, measles antibody level among the children aging 1-9 years old was high enough to prevent outbreak and epidemic of measles. However, we should try our best to strengthen the measles antibody level among the babies younger than 1 year old and the migrant population aging between 15 and 40 years old.

10.Effect of nursing intervention of hospital-community integration on the social function of schizophrenia patients

Objective To explone the rehabilitation nursing intervention of hospital-community integration on the social function of patients with schizophrenia. Methods 240 schizophrenia patients were randomly divided into observation group ( 120 cases) and control group ( 120 cases). Both groups received routine treatment and care. Other than this, observation group received early intervention and rehabilitation care. Social Disability Screening Scale (SDSS) and the Morning Side rehabilitation status scale (MRSS) were used. Recurrence rate and medication compliance of two groups after 1 year were compared. Results Better improvement of the social function and medication compliance, as well as lower recurrence rate was found in the observation group. There was significant difference between the two groups ( P < 0.01 or P < 0.05 ).Conclusions The rehabilitation nursing intervention of hospital-community integration can improve the social function of schizophrenia patients and contributes to their quality of life.
