1.Detection and clinical significance of urinary monocyte chemoattractant protein-1 and interleukin-18 in children with primary nephrotic syndrome

Objective To investigate the concentration of monocyte chemoattractant protein-1 (MCP-1) and interleukin-18 (IL-18) in the urinary of children with primary nephrotic syndrome(PNS) at different time points,and to explore their correlation with occurrence,development,progression,and prognosis of PNS.Methods A total of 65 pediatric cases from our hospital was enrolled in this study,and was divided into three groups based on the retrospective the follow-up results including steroid-sensitive NS (SSNS) (n =35),steroid-resistant NS (SRNS) (n =15),and frequent-relapse NS (FRNS) (n =15) groups.Another 20 healthy children served as normal controls.Peripheral blood samples and urine specimen were collected at three time points (without glucocorticoids,treatment after 8 weeks,and treatment after 16 weeks or recurrence).The levels of MCP-1 and IL-18 in the urine were assayed by enzyme-linked immunosorbent assay (ELISA).The levels of blood urea nitrogen,creatinine,and 24-hour urinary protein excretion were assayed by full-automatic biochemical study appliance.Results (1)In SSNS group,the levels of urinary IL-18 before treatment and treated for 8 weeks were higher than the normal control group [before treatment:(160.30 ±27.29) pg/ml; treated for 8 weeks:(157.62 ±26.85) pg/ml; normal control group:(70.88 ± 14.43) pg/ul].However,after treated for 16 weeks,the levels of urinary MCP-1 and IL-18 were markedly decreased compared with that of control group and those before treatment and treated for 8 weeks [treated for 16 weeks:(20.98 ±4.53) pg/ml,and (79.09 ±7.23) pg/ml,P <0.05].(2)In SRNS group,the levels of urinary MCP-1 and IL-18 before treatment were remarkably higher than that of control group and that of SSNS group before treatment[SSNS group before treatment:(76.84 ± 5.58) pg/ml,and (252.37 ± 25.34)pg/ml,P <0.05],but no significant difference was found when it was compared with that treated for 8 weeks [treated for 8 weeks:(72.32 ±4.30) pg/ml,and (243.70 ±35.43) pg/ml,P >0.05] ; However,its level was markedly decreased after treated with immunosuppressants of CTX for 16 weeks when it was compared with those before treatment and treated for 8 weeks[treated for 16 weeks:(34.03 ± 2.56) pg/ ml,and (114.42 ± 21.33)pg/ml,P < 0.05].(3)In FRNS group,the levels of urinary MCP-1 and IL-18 were no remarkable difference between control and SSNS groups [before treatment:(30.43 ± 4.61) pg/ml,and (156.65 ± 34.39)pg/ml; treated for 8 weeks:(29.41 ± 4.76) pg/ml,and (152.21 ± 34.73) pg/ml,P > 0.05],but its level was markedly lower than SRNS group (P < 0.05).When it was recurred,the levels of urinary MCP-1 and IL-18 were significantly increased compared with before treatment and treated for 8 weeks[recurred:(72.92 ±3.01)pg/ml,and (224.33 ±26.07)pg/ml,P <0.05].(4)No correlation was found between the levels of MCP-1 and IL-18 and the levels of blood urea nitrogen and creatinine (P >0.05).Positive correlation was found between the levels of MCP-1 and IL-18 and the 24-hour urinary protein excretion (r =0.706,0.556,P <0.01).There's a correlation between urinary MCP-1 and IL-18 (r =0.811,P < 0.01).Conclusions For children with PNS,the detection of urinary MCP-1 and IL-1 8 contributed to the early prediction of children'sensitivity on glucocorticoid.The elevated levels of urinary MCP-1 and IL-18 in combination with clinical symptoms and proteinuria can be used as an important noninvasive marker to monitor PNS recurrence.
2.Bone loss (osteoporosis)induced by retinoic acid in mice and its dose-dependent effects

AIM To study the dose dependent effects on osteoporosis induced by retinoic acid(RA) and to analysis the metabolic characteristics of bone. METHODS Three dosages of retinoic acid were given to the mice for 2 weeks while the body weight(BW) were measured once a week. At the end, femoral dry weight, Bone hydroxyproline, calcium content, Phosphor, Magnesium, Zinc, Manganese and Sulphonium of femora were measured. RESULTS Compared with normal control mice, three dosages of retinoic acid can induce the decrease of femoral dry weight and the short transverse diameter of femoral, Bone hydroxyproline content decreased remarkably, and so did the bone mineral. The Bone HOP and Bone Ca content was coincident with the femoral dry weight, but the lessening of Bone P, Bone Mg, Bone Zn and Bone Mn became inverse ratio with the femoral dry weight, particularly that of the Bone Zn. CONCLUSION Three dosages of retinoic acid in mice can induce osteoporosis with the lessening of bone mass, which characteristics were the losing of Bone HOP and the Bone Ca content in geometric proportion. The levels of Bone P, Bone Mg, and Bone Zn had lost remarkably in the early time, the Bone Zn may play importance function in osteoporosis model induced by retinoic acid.
3.Application of flow-through hybrization and gene chipon human papiliomavious subtype detection in Chongqing

Objective To evaluate the application of flow-through rapid hybridization technique and gene chip (HybriMax) on human papiUomavions (HPV) subtype in Chongqing.Methods Cervical tissue samples were taken under the colpnscope form 473 females who had cervical lesion for pathological analysis.The predictive value of HybriMax in cervical abnormality was compared with pathological results,which were used as golden standard.Resuits 13 different subtypes were found and total HPV positive rate was 63.0% (284/473) Among the 17 different subtypes ,the higher positive rote HPV subtypes were HPV16 (23.7%,112/473),HPV58 (12.7% ,60/473),HPV53(7.4% ,35/473).The HPV infection rates were higher with the worse d cervical lesion(X2=77.06,P<0.01).Conclusions The most frequent subtypes of HPV infection in Chongqing cervical lesion were HPV 16,58.HybriMax was an effective method to detect HPV subtype in clinical.
4.Trend analysis of endemic arsenism in the monitored areas of the disease in 2010 - 2012 in Inner Mongolia

Objectives To monitor the residents prevalence of endemic arsenism in the disease affected areas in Inner Mongolia, so as to provide feasible suggestions for control of arsenism in the future. Methods Monitoring data were obtained from the Project of Endemic Disease Prevention Granted by Central Government in 2010 - 2012, and the conditions of arsenism patients from 38 endemic arsenic villages were analyzed among different year, age and gender. Results The detection rate of arsenism of the 38 surveillance villages was 7.38%(517/7 004) in 2010, 7.10%(482/6 784) in 2011 and 6.62% (431/6 514) in 2012. The arsenism patients of mild;moderate and severe cases from 2010 to 2012, accounted for 74.47% (385/517), 74.27% (358/482), 75.17% (324/431); 16.83% (87/517), 16.60% (80/482), 15.78% (68/431) and 8.7% (45/517), 9.13% (44/482), 9.05% (39/431), respectively. For skin lesions, the detection rates of keratosis, pigmentation and depigmentation from 2010 to 2012, were 8.08%(566/7 004), 7.90%(536/6 784), 7.77%(506/6 514);3.27%(229/7 004), 3.29%(223/6 784), 2.87%(187/6 514) and 6.68% (468/7 004), 6.63% (450/6 784), 5.82% (379/6 514), respectively, showed a declining trend. It also showed a declining trend with age, and the patients were mainly 40 years old people and older, and the highest detection rate was in the 60- 70 years old group[15.54%(143/920)、14.72%(135/917)、13.36%(136/1 018)]. For gender distribution, the detection rate of the three years was higher in male than female [male 8.24%(300/3 639), 7.99%(283/3 542), 7.71%(260/3 372);female 6.45%(217/3 365), 6.14%(199/3 242), 5.44%(171/3 142),χ2=8.24, 8.77, 13.54, all P〈0.01]. Conclusion There is no big change of arsenism conditions in 2010-2012, with a slight declining trend.
5.Analysis of 36 patients treated with fine needle aspiration on spleen by ultrasonic guidance.

Objective To investigate the clinical application of the fine needle aspiration on spleen by ultrasonic guidance.Methods The fine needle aspiration by ultrasonic guidance was performed on 36 patients with spleen diseases.Among them,29 patients underwent diagnostic puncture,and the other 7 patients underwent remedial puncture.The definite diagnosis rate of the diagnostic puncture group was calculated by organism pathology and cytology result,as well as final diagnosis after the operation,while the curative effect of the remedial puncture group was appraised by re-examination after the operation.Results The definite diagnosis rate of the diagnostic puncture was higher,and the definite diagnosis rate of histology biopsy group(85.7%)was higher than that of cytology aspiration group(62.5%).All the patients who took remedial puncture were cured.Conclusion The fine needle aspiration on spleen by ultrasonic guidance has very important practical value in identifying the diagnosis of spleen disorders and treating fluidity processes of spleen.
6.Inhibitory effects of triptolide on cell proliferation and metastasis in Raji cells in vitro

AIM:To investigate the inhibitory effects of triptolide on cell proliferation and metastasis in Burkitt's lymphoma cell line Raji cells.METHODS: The effects of triptolide on the growth of Raji cells were studied by 3-(4,5-Dimethyl-2-thiazolyl)-2,5-diphenyl-2H-tetrazolium(MTT) assay.The effects of triptolide on the cell apoptosis of Raji cells were detected by using Annexin Ⅴ/PI double-labled cytometry.The effects of triptolide on CXCR4 expression on Raji cells were studied by flow cytometric analysis.Chemotaxis assays were performed to observe the effects of triptolide on migration of Raji cells towards recombinant human SDF-1?(rhSDF-1?)in vitro.RESULTS: Triptolide inhibited the proliferation of Raji cells in a dose-and time-dependent way with a 24 h IC50 value of 43.06 nmol/L and a 36 h IC50 value of 25.08 nmol/L.Following the treatment of triptolide,the cell apoptosis rate was increased as the treatment concentration increased and the culture time extended.The effects were dose-and time-dependent.Triptolide could downregulate the expression of CXCR4 on Raji cells in a dose-dependent manner.Moreover,chemotaxis assay suggested that triptolide could block the migration of Raji cells to rhSDF-1? in vitro,and the inhibition was dose-dependent.CONCLUSION: Triptolide could inhibit the cell proliferation and induce the cell apoptosis of Raji cells.Furthermore,it could block the cell metastasis of Raji cells in vitro and the underlying mechanism might be related to the inhibition of the SDF-1/CXCR4 axis.
7.Clinical application of transcatheter closure of secundum atrial septum defect with Amplatzer occluder in patients over 50 years of age

Objective To elucidate the efficacy and safety of transcatheter closure of secondary atrial septal defect (ASD) with Amplatzer septal occluder (ASO) in people over 50 years of age. Methods Retrospective comparison of 41 patients (14 males, 27 females) with ASD over 50 years of age ranging from 50 to 75(mean age 55.5?7.6, elder group) was conducted with 132 patients younger than 50 years(control group). After diagnosis of ASDs and evaluation of pulmonary artery systolic pressure and right atrial and ventricular dimensions by transthoracic echocardiography (TTE), all patients underwent transesophageal echocardiography (TEE) for complete assessment of ASD size, margins and anatomic relationship of the defect before closure of ASD. Each case was treated with ASO through the percutaneous transcatheter procedure under fluoroscopy and TTE or TEE. Early follow up(3 months) by echo was taken after the intervention. Results There was no difference of mean defect diameter measured by TEE and the balloon-stretched defect diameter of the ASDs between 2 groups. Objective To observe the influence of Epstein-Barr virus(EBV) infection on T cell subsets in children with infectious mononucleosis(IM).Methods Eighty children patients with IM in the Hanzhong Municipal People's Hospital from January 2013 to January 2016 were chosen as the study subjects and received the anti-virus therapy.The negative conversion rate of EBV-DNA after treatment was observed.The distributions of T cell subsets werecompared between the children patients with EBV-DNA positive and patients with EBV-DNA negative.The related factors of EBV-DNA non-negative conversion were analyzed.Results After treatment,67 children cases (83.75%) were EBV-DNA negative conversion.The proportions of CD3+ and CD8+ after treatment in the EBV-DNA negative group were lower than those in the EBV-DNA positive group,and the proportion of CD4+ and CD4+/CD8+ were higher than those in the EBV DNA positive group,the difference had statistical significance (P<0.05).In the analysis on the related factors in the children patients with EBV DNA non-negative conversion,the gender,WBC count and lymphocyte count had no significant correlation with EBV-DNA negative conversion(P> 0.05),while interferon use,age and initial Ct demonstrated significant correlation with EBV-DNA non-negative conversion(P<0.05).Conclusion EBV infection can lead to disturbance of T cell subsets in children with IM,so clinic should adopt an active anti-virus treatment.
9.Efficacy of precise hyperthermic intraperitoneal perfusion chemotherapy in the treatment of advanced pancreatic cancer

Objective To investigate the efficacy of precise hyperthermic intraperitoneal perfusion chemotherapy (HIPEC) in the treatment of advanced pancreatic cancer.Methods Thirty-six patients with advanced pancreatic cancer were analyzed retrospectively.Eighteen patients who received HIPEC combined with radiotherapy and chemotherapy were assigned as the treatment group and the other 18 patients who received chemotherapy and radiotherapy were assigned as the control group.Recent curative efficacy,Karnofsky Performance Status (KPS) score,postoperative complications and survivals between the two groups were analyzed,respectively.Results Significant differences were found between two groups in total short-term effective rate (P < 0.05).The total short-term effective rate of treatment group and control group were 66.67% (12/18) and 27.78% (5/18),respectively.The increment of KPS score of treatment group was significantly higher than that of control group (P < 0.05).There was no significant difference between two groups in postoperative complications (P > 0.05).The median overall survival time (OS) of treatment group was 11 months (7 ~ 31 months),and the median OS of control group was 7 months (4 ~ 18 months).The survival of the treatment group was longer than the control group (P < 0.05).Conclusions HIPEC treatment improved significantly the survival and life quality of advanced pancreatic cancer patients.With acceptable morbidity and mortality rates,HIPEC regime was an effective treatment modality for patients with advanced pancreatic cancer.
10.Estimating clinical competence of general practitioner by DxR Clinician software

Clinical competence was estimated for grades 2011-2013 general practitioners (GPs)trained by the DxR Clinician software.Clinical cases were solved on computer.And the abilities of diagnosis,thinking and handling were assessed.The average score of all GPs was 61.68 ± 10.69.The average score of diagnostic ability was the highest at 71.82 ± 15.47 (weighted score of 25.1).The average score of thinking ability was 62.36 ± 10.40 (weighted score of 28.1).And the average score of handling ability was the least at 42.27 ± 18.22 (weighted score of 8.5).Significant differences in average scores existed between grades 2011 and 2013 (P < 0.05).Significant differences in score of clinical thinking ability existed between grades 2011,2012 and 2013 (P < 0.01) respectively.None of them passed the part of handling.The longer period of training,the higher scores the residents obtained.Since the ability of clinical handling fared the worst,targeted training should be strengthened.
