1.Effect of N-acetylglucosamine on osteoblastic activity and expression of bone morphogenetic protein in fracture healing

BACKGROUND: Chitin and its derivatives possess many biological activities such as immune modulation and promoting wound healing and hemostasis. As one of the derivatives of chitin, the effect of N-acetylglu cosamine on fracture healing has not yet been acknowledged.OBJECTIVE: To observe the changes in osteoblastic activity and the expression of bone morphogenetic protein (BMP) in response to the intervention with N-acetylglucosamine during the fracture healing. DESIGN: A randomized controlled experiment SETTING: Department of Anatomy, Medical College of Qingdao University.MATERIALS: This experiment was carried out in the Laboratory of Neuroanatomy, Medical Colloge of Qingdao University from March to August 2002. Seventy healthy New Zealand rabbits were randomized into four groups, namely saline group (n=23), Jiegupian group (n=23) and N-acetyl glucosamine group (n=24).METHODS: The rabbits were subjected to operations to induce 3-mm bone defect in the middle segment of the right radius with subsequent treatments with saline (1.00 mL/kg), Jiegupian (1.00 mL/kg) and N-acetyl glucosamine (0.28 g/kg) via intragastric administration on a daily basis till the time of tissue sampling at postoperative days 9, 17, 30 and 42, respectively. X-ray examination and HE staining were performed to observe the healing process, and the expression of BMP was assessed immunohistochemically.MAIN OUTCOME MEASURES :① General condition of the rabbits after operation; ② Fracture healing process revealed by X-ray at the specified time points; ③ Expression of BMP between the fracture ends at each time point; ④ Results of histological observation.RESULTS: According to intent-to-treat analysis, 70 rabbits entered the data analysis. X-ray revealed better fracture healing in N-acetylglucosamine group than in the ither two groups at postoperative days 9, 17, 30 and 42(P < 0.05). Compared with saline group, the expression of BMP protein in Jiegupian group and N-acetylglucosamine group was increasing at postoperative 9 and 17 days, particularly in the latter group (P < 0.05), but decreased at postoperative 30 and 42 days. Histologically, the gap between the fracture ends reduced in N-acetylglucosamine group at postoperative 9 days with the appearance of ()bmogenous bone trabecula an orderly arrangement of osteoblasts; in saline group, the gap was greater and contained less or even no bone trabecula, with much less, if any, osteoblasts.On postoperative day 30, the bone defect were completely filled by bone trabecula in N-acetylglucosamine group, with the bone trabecula almost communicating with the bone marrow cavity, and the osteoblasts surrounding the bone trabecula were arranged orderly with osteoclasts seldom observed; in saline group, the defect was substituted mostly by bone trabecula with breaches, and the chondrocytes could be observed scarcely. On postoperative day 42, the bone plate woven bone formed primarily at the fracture ends in N-acetylglucosamine group, with completely communicating marrow cavity and numerous active osteoclasts, and osseous lacuna can be observed; in saline group, the defect was filled with bone trabecula but clear fusion lines were not seen, and communication of the bone marrow cavity was nearly achieved with orderly arranged osteoblasts and presence of the osteoclasts.CONCLUSION: N-acetylglucosamine can accelerate fracture healing possibly by promoting the osteoblastic activity and increase the expression of BMP during the early period of fracture healing.
2.Surgical treatment of coronary artery lesions caused by Kawasaki disease

Kawasaki disease is an acute self-limiting systemic vasculitis syndrome and usually occurs in children.The small and medium arteries of the whole body are mainly invaded, and marked with coronary arteries.Coronary artery dilation and aneurysm formation mainly occur in the early period.Thrombi, intimal hyperplasia, and calcification could be formed during the later period.Consequently, sometimes, it develops intochronicischemic cardiopathy or myocardial infarction.In that, Kawasaki disease has become the leading cause of acquired heart disease for children in developed countries.Presently, the treatment of coronary artery lesions caused by Kawasaki disease includes drug therapy, and interventional and surgical treatment.However, medications usually fail to solve severe coronary conditions, and only interventional and surgical treatment can we choose.Therefore, the development and indications of interventional and surgical treatment of coronary artery lesions caused by Kawasaki disease were reviewed in this article.
3.Rural environment, pesticide exposure and the risk of amyotrophic lateral sclerosis: a meta analysis

Objective To explore the relationship between the risk of amyotrophic lateral sclerosis (ALS) and exposure to rural environments and pesticide.Methods Studies relevant to rural residence,farmer occupation,pesticide exposure and ALS were identified from the databases including Embase,Ovid Medline,Pubmed,Cochrane Library,Wanfang data,Chinese BioMedical Literature Database,Chinese National Knowledge Infrastructure,China Science and Technology Journal Database up to March 2015.Quality of studies was assessed according to the Newcastle-Ottawa Scale (NOS).Analysis of data and publication bias was performed with software Revman 5.3.Results A total of 24 case-control studies and 3 cohort studies were included into the analysis.The NOS scores of all studies were ≥6.The risk of ALS was associated with pesticide exposure (OR =1.41,95% CI 1.28-1.56) and farmer occupation (OR =1.42,95% CI 1.29-1.57),but not associated with rural residence (OR =1.21,95% CI 0.97-1.51).Subgroup analysis of pesticide exposure and ALS revealed that males (OR =1.75,95% CI 1.39-2.21) had a higher risk than females (OR =1.53,95% CI 1.13-2.08),and the risk estimate was higher in studies using E1 Escorial standard (OR =1.68,95% CI 1.45-1.95) than studies not (OR =1.23,95% CI 1.08-1.40).The meta analysis had a slight publication bias.Conclusions Our findings support pesticide exposure might increase the risk of ALS.Given that farmers always have high levels of pesticide exposure in their work,they should decrease their exposure level or take proper precautions to lower the risk of ALS.
4.Rho kinase and ischemic cerebrovascular disease

Ischemic cerebrovascular disease may result in severe neurological deficits, and even death. Rho kinase is an important downstream substrate of Rho protein. It involves in multiple biological effects, including vascular smooth muscle contraction, cell migration, inflammatory cell infiltration and vascular endothelial dysfunction. It plays important roles in the processes of the occurrence, development and expansion of infarct volume in ischemic cerebrovascular disease. Rho kinase inhibitor can effectively relax vascular spasm, reduce local inflammatory response, protect endothelial function, increase blood flow in ischemic region, reduce infarct volume, and improve neurological function.
5.Clinical applied value of computed tomography perfusion imaging in early stage of ischemic stroke

Objective To study the clinical applied value of computed tomography perfusion imaging(CTPI) in early stage of ischemic stroke.Methods Routine CT scan and CTPI were performed in 45 patients with early stage of ischemic stroke(onset within 24 hours).CTPI yields a set of perfusion related parameters of relatine cerebral blood flow(rCBF), cerebral blood volume(rCBV),mean transmit time(rMTT),time to peak(rTTP) and positive enhancement integral (rPE).Results In all 45 patients,21 cases showed concerning lesions in the 29 cases with cerebral ischemia results of routine CT scan.16 cases presented normal results. 24 cases were not showed concerning lesions.The sensitivity of diagnose for early stage of ischemic stroke by CT was 46.67% (21/45).The specificity was 72.41% (21/29).44 cases of CTPI with abnormal perfusion changes were all showed concerning lesions. Only one case of CTPI presented negative result,which was acute brainstem infarction confirmed by follow-up magnetic resonance imaging (MRI).The sensitivity of diagnose for early stage of ischemic stroke by CTPI was 97.78% (44/45).The specificity was 100% (44/44).There was significant difference between CTPI and routine CT scan(P
6.An outbreak of extended-spectrum beta-lactamase-producing Klebsiella oxytoca in a surgical ward

Objective To study 8 isolates of multiple antibiotic-resistant Klebsiella oxytoca in 8 different patients from one surgical ward. Methods Susceptibility test, extended-spectrum ?-lactamases (ESBLs) screening methods and some molecular biological methods were performed. Results 8 isolates of Klebsiella oxytoca were verified as closely related homologous CTX-M-3-producing strains. Conclusions The little outbreak was caused by 8 closely related homologous strains in one ward.
7.International comparative study of ADR compensation system and its implications in China

In recent years , increasing use of drugs has given rise to more and more Adverse Drug Reaction ( ADR) cases in China.However, due to the absence of ADR compensation legislation in China , these victim's rights cannot be restituted.By analyzing experiences from Germany , Sweden, Japan, USA, and Taiwan, as well as the current situation and difficulties of ADR compensation in China , this paper provides suggestions for ADR compensa-tion policy in China , which is to constitute an ADR compensation system based on funds .
8.Regulatory Effect of Acupuncture and Moxibustion on the Hematopoiesis of Leukopenia Model in Rats

Objective: To investigate the effect of acupuncture or moxibustion at acupoint Geshu (BL 17) on raising leukocyte and promoting marrow proliferation. Methods: Wister rats were selected to establish the leukopenia and immuno-suppression model by intraperitoneal injection of cytoxan (CTX). After they were grouped and treated, leukocyte and marrow karyocyte were counted in different phases to evaluate the effect of acupuncture or moxibustion at acupoint Geshu (BL 17) on raising leukocyte and promoting marrow hematopoiesis. Results: Acupuncture or moxibustion at acupoint Geshu (BL 17) and routine western medicine could all significantly raise leukocyte and increase marrow karyocyte. The curative effect was better in moxibustion group than in acupuncture group and in routine western medicine group (P<0.05). Conclusion: Moxibustion at acupoint Geshu (BL 17) can markedly raise peripheral blood leukocyte and marrow karyocyte of the leucopenia model in rats.
9.Limited incision plus autologous tendon graft improve knee joint functions in the treatment of knee ligament injuries

BACKGROUND:Early diagnosis and accurate treatment of ligament injuries are the premise of avoiding chronic instability of knee joint and promoting functional recovery of knee joint.
OBJECTIVE:To investigate the clinical efficacy of limited incision with autologous tendon graft for knee ligament injury.
METHODS:Eighty patients with knee ligament damage were randomly divided into two groups. The cases of control group were given orthosis treatment, and the cases of observation group were treated with the limited incision plus autologous tendon graft.
RESULTS AND CONCLUSION:At 6 months post-treatment, the excellent efficacy rate and postoperative quality of life score were increased in the observation group, compared with the control group (P<0.05). The active and passive ranges of motion of knee joint in the observation group were significantly lower than that in the control group (P<0.05). At 3 months post-treatment, Lysholmp scores of the observation group was significantly lower than that in the control group (P<0.05). Limited incision plus autologous tendon graft treatment can significantly improve knee function after knee ligament injury.
10.Study on the Complication of Multiple Organ Failure in Acute Episode of Chronic Cor Pulmonale: A Clinical Analysis of 154 Cases

in the period from Lanuary, 1983 to June, 1990, a total of 980 cases of cor pulmonale were hospitaluzed in this institute. Among the 980, 154 cases were complicated with multiple organ failure (MOF), and 114 cases out of the 154 died with a mortality rate of 74%. Besides the heart and the lungs, the organs involved were the brain, the liver, the gastrointestinal tract, the kidneys and the blood system. It was found that MOF more often occurred in elderly persons. The older the patient is, the more the organs will be involved and the higher the mortality rate will be. Infection especially that of gram negative bacilli was the main factor to induce MOF. The combined administration of penicillin and the antibiotics of cephalosporin group was quite helpful to control the infections, which is the key step in the management of such cases, when the antibiotics of aminoglycoside group were used, special attention must be paid to the prevention of their toxic effects on the kidneys High-frequency jet ventilation is not suitable to treat cor pulmonale complicated with MOF.
