1.Research progress of chimeric antigen receptor natural killer cells in hematological malignancies

Chimeric antigen receptor natural killer cell (CAR-NK) is a new promising immunotherapy. NK cell can derive from peripheral blood cells, cord blood cells, NK-92 cell line and iPSC cells. CAR-NK cells can recognize target antigens non-specifically and are not restricted by human leukocyte antigens. Allogeneic infusion of CAR-NK cells does not cause graft-versus-host disease. It has a strong effect on primary cells of leukemia, lymphoma and multiple myeloma, thus making animal models have strong anti-tumor effect. Therefore, as a "off-the-shelf" product, CAR-NK cells have a wide range of clinical applications. In this paper, the construction of CAR-NK vector and its preliminary clinical application in hematological tumors and the challenges were reviewed.
2.The value of MRI parameters combined with serum MIF and miR-1203 in the early diagnosis of hepatocellular carcinoma and its relationship with efficacy

Objective:To explore the value of magnetic resonance imaging (MRI) parameters combined with serum macrophage migration inhibitory factor (MIF) and microRNA-1203 (miR-1203) in the early diagnosis of hepatocellular carcinoma (HCC) and its relationship with efficacy.Methods:From October 2017 to October 2019, 100 patients with HCC in Hunan Provincial Hospital of Traditional Chinese Medicine were selected as the observation group. 92 patients with benign liver tumor and 102 healthy people were randomly selected as the control group and normal control group. The clinicopathological features, MRI parameters [hepatic artery perfusion index (HPI), volume transfer constant (K trans)], serum MIF and miR-1203 levels were compared among the three groups; the value of MRI parameters, serum MIF and miR-1203 in single and combined diagnosis of HCC was explored; the relationship between each index and curative effect of HCC patients was analyzed. Results:The levels of HPI, K trans, serum MIF and miR-1203 in observation group were higher than those in control group and normal control group ( P<0.05); There were significant differences in HPI, K trans, serum MIF and miR-1203 levels among patients with different tumor length, differentiation degree, Child Pugh grade and distant metastasis in the observation group ( P<0.05). Among HPI, K trans and serum MIF, miR-1203, the specificity of HPI and K trans for diagnosis of HCC were the largest (94.57%), and the sensitivity of K trans for diagnosis of HCC was the largest (75.00%); the area under the curve (AUC) of the combined diagnosis of HCC was 0.879, which was greater than the AUC of the single diagnosis of HPI, K trans and serum MIF, miR-1203 (0.753, 0.793, 0.792, 0.809); the optimal sensitivity and specificity of the combined diagnosis were 79.00% and 86.96%, respectively; the levels of HPI, K trans, serum MIF, and miR-1203 in effective patients in the observation group were lower than those in the ineffective patients after treatment ( P<0.05); the clinical efficacy of HCC patients were significantly correlated with the levels of HPI, K trans, serum MIF and miR-1203 ( P<0.05). Conclusions:The MRI parameters HPI, K trans and serum MIF, miR-1203 levels in patients with HCC increased significantly, which has high application value in assisting clinical diagnosis of HCC, and is closely related to the clinical efficacy of patients, and has great development potential in efficacy evaluation.
3.Investigation on the current state of training system for master of clinical medicine (general practice)

Objective To gain a better knowledge of the education for masters in clinical medicine (general practice) and provide a reference for the better exploration of the training mode of GP students.Methods 9 colleges and universities enrolling students for this program were selected with stratified sampling.A questionnaire was conducted in terms of admission,curriculum setting,clinical training,guidance and evaluation through officials in charge of graduate affairs from February to July in 2014.Descriptive analysis was performed to the investigation results,using SPSS 20.0 statistical software.Results A full-time undergraduate degree in clinical medicine was explicitly required in 88.9% (8/9) of colleges and universities.The average total credits of the course were 23.7 and the average clinical training duration was 32.4 months.Tutor group was adopted in all colleges and universities.The assessment and evaluation of the GP masters was mainly composed of multiple forms of process assessment and final evaluation.Conclusion We suggest attaching more importance to the improvements of enrollment policy,curriculum system,clinical training,standardized training base construction,teaching staff,and evaluation system in order to explore GP training mode in accord with the law of general practice and suitable for China's national conditions.

This study conducted questionnaire survey to graduate medical students in Medicine (M.D.) and Stomatological Medicine (S.M.D) enrolled at Peking University Health Science Center in the year 2013,analyzed on the understanding,period,forms,mentoring and effect of research training.Suggestions are proposed to strengthen the research training,for example,focusing on clinical training as well as research training,aiming at clinical issues,emphasize of clinical scientific research,permeating the concept oftransformation medicine,promotion of intense full-time research,strengthen the mentoring of professors.
5.Dynamic changes of the expression of HIF-1,VEGF and miRNA-210 in the kidney after unilateral ureteral occlusion

Objective To observe the expression of inducible factor 1(HIF‐1) ,vascular endothelial growth factor(VEGF) and miRNA‐210 and explore the regulatory mechanism after unilateral ureteral occlusion (UUO) .Methods UUO models are made by ligation unilateral ureteral with silk ,and they were divided into sham group and UUO group .All the mice were sacrificed by cer‐vical dislocation after 2 ,5 ,9 and 14 d of UUO .The kidney samples were examined for HIF‐1 mRNA ,VEGF mRNA and miRNA‐210 by quantitative real time reverse transcription polymerase chain reaction (RT‐PCR) ,and HIF‐1 protein by Western blot . Results The expression level of HIF‐1 mRNA was gradually raised after UUO (P<0 .05) ,and compared with the sham group ,the expression level of VEGF mRNA and miRNA‐210 were up regulated after 2 d of UUO and down regulated after 5 ,9 and 14 d of UUO compared with the sham group (P<0 .05);the expression level of HIF‐1 protein increased after UUO compared to the sham group .Conclusion The results of the level of HIF‐1 protein increased and VEGF mRNA and miRNA‐210 were up regulated ,which might be related to the adaptive response of kidney hypoxia and ischemia after UUO .
6.Treatment of 40 Cases of Stubborn Constipation by Acupuncture

We treated 40 cases of stubborn constipation by puncturing Zhongwan (CV 12),Qihai (CV 6), Tianshu (ST 25, bilateral), Daheng (SP 15, bilateral). After 20 times of treatment, 16 cases were cured(accounting for 45%) ,18 cases were significantly improved(accounting for 45%),4 case were improved(accounting for 10%),2 cases were ineffective(accounting for 5%).The total effective rate was 95%.

A female patient of 56 years old had hoarseness that seems worse after talking excessively,which occasionally associated with slightly sore throat and pharyngeal foreign body sensation. The symptoms are not associated with sore throat, fever, night sweats, not drinking cough, breathing and swallowing difficulties, but no cough, bloody sputum. Neck ultrasound can be showed: goiter and real echo uneven thickening, increased blood supply pan; the right thyroid lobe multiple cysts pan. Enhanced CT shows occupying lesions were found out in the right side of the supraglottic larynx gap and the right lobe of the thyroid, nature to be determined. Full thyroid function showed: thyroid microsomal antibodies 278.2 u/ml, the rest of the indicators in the normal range. Other routine preoperative examinations were normal. Immunohistochemistry: CD45(++) CD68(+) CD99(++) EMA(-) CK(-) Sclc(-) TTF-1(--) CgA(-) SY(-) NSE(--) S-100(-) ESA(-). Supported by immunohistochemistry, hyperplasia organizations was diffuse lymphoid tissue. Through expert consultation by superior hospital the pathology showed: Hashimoto's thyroiditis with thyroid papillary carcinoma (lesions of the right thyroid); Lesion on the right side of the throat gene rearrangement results show: B lymphocyte clonal consider mucosa-associated extranodal marginal zone B-cell lymphoma the gene rearrangement that the right side of the throat disease is: B lymphocyte clonal, be considerd mucosa-associated extranodal marginal zone B-cell lymphoma.

8.Studies on Specific Diagnostic Antigens in Excretory-secretory Products from Trichinella spiralis Muscle Larvae

Objective To find out the specific diagnostic antigens in excretory-secretory (ES) products from muscle larvae of Trichinella spiralis. Methods The ES antigens (ESA) of Trichinella spiralis muscle larvae cultured in vitro at 18 h and 30 h were analyzed by SDS-PAGE and Western blotting. Results At different times after cultivation, the protein components of ESA of T. spiralis muscle larvae were similar. SDS-PAGE revealed that the molecular weight(MW) of the major bands of 2 ES antigens were 112, 110, 108, 97, 53, 49, 45, 42, 35, 23 and 16 kDa. Western blotting showed that the protein bands with 102, 97, 95 and 53 kDa in 18 h ESA and the protein bands with 53, 49, 45 and 43 kDa in 30 h ESA cross-reacted with sera from the patients with paragonimiasis, clonorchiasis, schistosomiasis, and cysticercosis, respectively. The protein component with 23 kDa in ESA only reacted with sera from the rats and mice infected with T. spiralis and the patients with trichinellosis, but not reacted with sera from animals and patients infected with other parasites, and sera from normal rats, mice and persons. Conclusion The protein component with 23 kDa in T. spiralis ESA is the specific antigen of T. spiralis muscle larvae and it could be applied to the serodiagnosis and seroepidemiological survey of trichinellosis.
9.IDENTIFICATION OF SPECIFIC ANTIGEN OF TRICHINELLA SPIRALIS MUSCLE LARVAE

Objective To find out the specific antigens for immunodiagnosis of trichinellosis. Methods The soluble antigens of Trichinella spiralis muscle larvae were analyzed by SDS-PAGE and Western blot. Results SDS-PAGE revealed that the soluble antigens of T. spiralis muscle larvae had 29 protein bands with molecular weight (MW) from 112 kDa to 12 kDa, among them the protein bands with MW 65,43,42,31,30,20,17,16 kDa were the major bands. Western blot results showed that the protein bands with 112,110,108, 102,97,95,65,63,58,55,53,49,45,43,42 kDa in T.spiralis muscle larval soluble antigens were cross-reacted with sera from rats and patients with paragonimiasis, sera from patients with clonorchiasis, schistosomiasis, and cys-ticercosis. The protein components with 24 - 20 kDa were only reacted with sera from rats, mice infected with T.spiralis and patients with trichinellosis, and not reacted with sera from animals and patients infected with other parasites,and sera from normal rats, mice and healthy persons. Conclusion The protein components with 24-20 kDa in T.spiralis muscle larval soluble antigens are the specific antigen for T.spiralis muscle larvae, it could be applied to the immunodiagnosis and seroepidemiological investigation on trichinellosis.
10.Effect of Zinc Toxicity on Peripheral Blood T-lymphocyte in the Broiler

The experiment was conducted to examine the effect of zinc toxicity on the peripheral blood T-lymphocyte by the method of flow cytometry(FCM) and ANAE.200 one-day-old Avian broilers were divided into four groups,and fed with the diets as follows:①control group(Zn 100 mg/kg diet) and ② zinc toxic groups(Zn 1 500 mg/kg diet,zinc toxic group Ⅰ;Zn 2 000 mg/kg diet,zinc toxic group Ⅱ;Zn 2 500 mg/kg diet,zinc toxic group Ⅲ) for seven weeks.The ANAE positive ratios of the peripheral blood T-lymphocytes were much lower in the three zinc toxic groups than that in control group at 7 weeks of age.CD4+ T cell numbers were reduced from 2 to 7 weekds of age in zinc toxic group Ⅲ and from 6 to 7 weeks of age in zinc toxic group Ⅱ,as compared with that of control group.The numbers of CD8+ T cell decreased at 2 and 4 weeks of age in zinc toxic groups Ⅱ and Ⅲ,and at 7 weeks of age in zinc grups Ⅰ and Ⅲ.CD4+/CD8+ ratio was higher at 2 and 4 weeks age,and lower at 6 and 7 weeks of age in zinc toxic groups Ⅰ,Ⅱ and Ⅲ than in control group.The results showed that zinc toxicity would suppress the development of T-lymphocytes and reduce the peripheral blood T-lymphocyte populations.Potential mechanism underlying these observations are also discussed.
